Clinical utility of rifabutin
1 (RBT), a potent antibiotic used in multidrug regimens for tuberculosis (TB) as well as for infections caused by
Mycobacterium avium complex (MAC), has been hampered due to dose-limiting toxicity. RBT analogs
2–11 were synthesized and evaluated against
M. avium 1581 and
Mycobacterium tuberculosis susceptible and resistant strains in vitro. A selection of candidates were also assayed against non-replicating persistent (NRP)
M. tuberculosis. Subsequent in vivo studies with the best preclinical candidate drugs
5 and
8, in a model of progressive pulmonary tuberculosis of Balb/C mice infected either with H
37Rv drug–sensible strain or with multidrug resistant (MDR) clinical isolates, resistant to all primary antibiotics including rifampicin, were performed. The results disclosed here suggest that
5 and
8 have potential for clinical application.
Objetivos: Analisar a mortalidade materna no Rio Grande do Norte, na série histórica 2003- 2008, a partir de dados do Sistema de Informação sobre Mortalidade (SIM) e do Comitê de Mortalidade Materna ...(CMM) do estado. Métodos: Estudo descritivo, de base populacional, com dados secundários sobre mortalidade materna coletados no SIM estadual através do programa Tabwin, versão 3.2, e no CMM-RN, através das fichas e relatórios do arquivo. Resultados: Observou-se que o percentual de óbitos maternos investigados e confirmados pelo Comitê em relação aos registrados no SIM variou de 3% em 2003, decrescendo para 1% em 2007, o que é muito pouco se comparado ao total de mortes de mulheres em idade fértil ocorridas no período estudado. Conclusões: É possível perceber fragilidades da vigilância epidemiológica no rastreamento das mortes maternas e uma atuação do Comitê na investigação dessas mortes muito aquém daquelas registradas no SIM, mostrando subregistro, incompletude dos dados e falta de consistência no fluxo de informações entre esses bancos, o que impossibilitou a comparação das informações existentes. Sugere-se que as Portarias nº 1172/2004 e nº 1119/2008 do Ministério da Saúde sejam cumpridas, que a vigilância fiscalize o registro da morte materna como evento de notificação compulsória e que um efeito punitivo para os municípios que descumprirem esse protocolo seja estabelecido. Essas medidas poderão contribuir para a redução da mortalidade materna.

We present the case of an 11-year-old girl with a 2-month history of ...recurrent postprandial vomiting and weight loss. On physical examination, the patient presented with abdominal distension. Upper gastrointestinal endoscopy revealed a very small pyloric orifice through which the endoscope could not be advanced. Abdominal ultrasonography and a computed tomography confirmed pylorus thickening. She underwent Heineke-Mikulicz pyloroplasty with symptom resolution.
Primary hyperparathyroidism is a rare occurrence in pregnancy with significant risks to the mother and the foetus, witch is related to the level of serum calcium. A 41-year-old women, gravida 2, para ...1, presented at 22 weeks gestation with nausea, vomiting and mild cognitive dysfunction associated with hypercalcemic crisis. The hypercalcemia was observed to be related to parathyroid hyperplasia that was surgically removed. Complete resolution of her symptomatology and hypercalcemia occurred postoperatively. The pregnancy was complicated with transient hypertension. A small for gestational age healthy male infant was delivered at term with no neonatal complications related with this pathology.
