Les inhibiteurs des checkpoints ont révolutionné le traitement de certains cancers et permis d’améliorer de façon spectaculaire le pronostic des patients. Contrairement aux chimiothérapies ...cytotoxiques classiques, ces traitements entraînent une immunostimulation en restaurant l’activité antitumorale des lymphocytes T et peuvent se compliquer de toxicités immuno-induites. Ces toxicités peuvent toucher tous les organes, à des niveaux de gravité différents et, parfois, mettre en jeu le pronostic vital. Le délai de survenue est très variable, mais se situe généralement dans les premiers mois qui suivent l’initiation du traitement. Ces traitements et leurs effets indésirables sont méconnus des urgentistes. Pourtant, l’utilisation croissante de ces molécules, dans des indications chaque fois plus larges, exposent de plus en plus de patients à ces toxicités et placent les urgentistes en première ligne lorsque celles-ci surviennent. L’enjeu pour les urgentistes est de reconnaître cette famille de molécules et d’évoquer une éventuelle toxicité devant des symptômes compatibles, ne pas banaliser certains symptômes non spécifiques en les attribuant trop facilement à la progression du cancer, et évoquer les diagnostics différentiels et notamment l’infection. Ces diagnostics de toxicité étant parfois longs et complexes, la collaboration rapprochée avec les oncologues et/ou spécialistes d’organes est primordiale et nécessaire avant d’initier le traitement qui repose, le plus souvent, sur les corticoïdes et, parfois, sur les immunosuppresseurs. Nous détaillons ici les toxicités liées aux immunothérapies par inhibiteurs des checkpoints les plus fréquentes et/ou sévères (digestive, endocrinienne, cardiaque, pulmonaire, neuromusculaire et hépatique), avec une approche pragmatique destinée aux urgentistes.
Checkpoint inhibitors have revolutionized cancer treatments and dramatically improved the patients’ prognosis. Unlike conventional cytotoxic chemotherapies, these treatments enhance immunity by restoring the anti-tumor activity of T lymphocytes and can generate immune-related adverse events. These adverse events can affect all organs, with varying degrees of severity, and sometimes, be lifethreatening. The onset time can vary greatly but usually occurs in the first few months following the start of immunotherapy. These treatments and their toxicities are not well known to emergency physicians. However, the increasing use of these molecules, in extended broad indications, exposes more and more patients to these toxicities and places emergency physicians on the front line when they occur. The challenge for emergency physicians is to recognize this family of molecules and to consider possible toxicity in the presence of compatible symptoms, not to trivialize certain non-specific symptoms by too easily attributing them to cancer progression, and to consider differential diagnoses, especially infection. Since the diagnosis of toxicity can sometimes be long and complex, close collaboration with oncologists and/or organ specialists is crucial and necessary before starting treatment, which most often relies on corticosteroids and, sometimes, on immunosuppressors. We detail here the immune-related adverse events related to checkpoint inhibitors that are most common and/or severe (gastrointestinal, endocrine, cardiac, pulmonary, neuromuscular, and hepatic), with a pragmatic approach intended for emergency physicians.
Guided by Self-Determination Theory, we offer an integrative and fine-grained analysis of teachers' classroom motivating style (i.e., autonomy support, structure, control, and chaos) to resolve ...existing controversies in the literature, such as how these dimensions relate to each other and to educationally important student and teacher outcomes. Six independent samples of secondary school teachers (N = 1332; Mage = 40.9 years) and their students (N = 1735, Mage = 14.6 years) read 12 ecologically valid vignettes to rate four dimensions of teachers' motivating styles, using the Situations-in-School (SIS) questionnaire. Multidimensional scaling analyses of both the teacher and the student data indicated that motivating and demotivating teaching could best be graphically represented by a two-dimensional configuration that differed in terms of need support and directiveness. In addition, eight subareas (two subareas per motivating style) were identified along a circumplex model: participative and attuning, guiding and clarifying, demanding and domineering, and abandoning and awaiting. Correlations between these eight subareas and a variety of construct validation and outcome variables (e.g., student motivation, teacher burnout) followed an ordered sinusoid pattern. The discussion focuses on the conceptual implications and practical advantages of adopting a circumplex approach and sketches a number of important future research directions.
Educational Impact and Implications Statement
The present study suggests that rather than categorizing secondary school teachers as either motivating or demotivating, this approach reveals that an attuning and guiding approach relate to the most adaptive pattern of teacher and student outcomes, whereas an opposite pattern is found for a domineering and abandoning approach. This greater clarity allows teachers to gain a more precise insight into their own teaching style so that they adopt a more need-supportive style that benefits their students and themselves.
Objectif Évaluer les variations temporelles des hypoglycémies chez les patients accueillis dans les services d'urgence de l'Île-de-France. Matériels et Méthodes Étude multicentrique observationnelle ...menée dans les services d'accueil des urgences de l'Île-de-France utilisant un lecteur de glycémie connecté. En collaboration avec les biologistes, l'ensemble des glycémies capillaires enregistrées chez les patients accueillis dans les services d'urgence entre le 1er janvier 2014 minuit et le 31 décembre 2014 23 h 59 ont été extraites. Une valeur inférieure à 70 mg/dl définissait l'hypoglycémie modérée et à 50 mg/dl l'hypoglycémie sévère. Six créneaux horaires ont été choisis pour l'analyse des variations intra-journalières. Résultats Six CHUs (Avicenne, Beaujon, Hôpital Européen Georges Pompidou, Lariboisière, Pitié-Salpêtrière et Saint-Louis) et 1 site d'urgence ophtalmologique (CHNO des Quinze-Vingts) ont participé à l'étude. Au total, 114 528 glycémies ont été enregistrées en 2014 dans les services d'urgence de ces 7 hôpitaux. La prévalence des hypoglycémies modérées et sévères était respectivement estimée à 3,7 % ( n = 4 416) et 0,7 % ( n = 803). Près de 6 % des valeurs ( n = 6 663) étaient supérieures à 300 mg/dl et 2 % ( n = 2 308) à 400 mg/dl. La glycémie moyenne (144 ± 81 mg/dl) subissait des variations inter-hospitalières ( p < 0,001), intra-journalières ( p < 0,001) avec une valeur minimale entre 4 et 8 h du matin, inter-journalières (meilleur contrôle le week-end qu'en semaine, p < 0,001) et saisonnières (pics en mars et octobre et nadir en juillet, p < 0,001). La plus forte prévalence d'hypoglycémies modérées (4,8 %) était enregistrée entre 4 et 8 h du matin avec une augmentation de 46 % du risque comparé à celui observé entre 20 et 24 h (OR : 1,46 ; IC à 95 % 1,32-1,62, p < 0,001). Par contre, la prévalence des hypoglycémies sévères ne différait pas selon l'établissement ( p = 0,07), l'horaire ( p = 0,38), le mois ( p = 0,79) ou le jour de semaine ( p = 0,74). Conclusions La prévalence des hypoglycémies chez les patients admis dans les services d'urgence est estimée à 3,7 % avec une incidence plus élevée en 2e partie de nuit incitant à la vigilance et justifiant d'une recherche étiologique attentive et d'une intervention thérapeutique le cas échéant.

Dark fermentation (DF) is a biological process able to produce hydrogen gas from organic waste which can play a key role as a building block in biorefineries. But the optimization of hydrodynamic conditions of DF is still needed to enhance gas-liquid mass transfer, thereby reducing the self-inhibitory effect of soluble hydrogen. Mass transfer enhancement is constrained, as hydromechanical stress on microorganisms must be limited and the economic sustainability of the process maintained.
Recent results showed that DF is enhanced in the transition region between laminar and turbulent regimes. To gain a better knowledge of 3D hydrodynamic features in this regime, a modified optical trajectography technique was developed and applied to a 2-L bioreactor equipped with a dual-impeller device. The proposed methodology aims at monitoring the trajectories of up to ten particles as tracers at the same time using three cameras but is also able to provide the real-time position of the particles in the 2D-images of each camera to minimize the post-treatment time.
The methodology, including stereoscopic camera calibration, real-time and post- treatment to reconstruct the 3D trajectories, was applied, and validated against 2D-PIV and CFD data. A good agreement was achieved, but regions neighboring wall and impeller were difficult to capture due to the particle size. The results highlighted that operating with five particles was able to reduce by a factor of 3–4 the measurement time in comparison to optical trajectography with a single particle as a tracer, while a higher number of tracers increased the occurrence of artifacts.
