Background
The search for high‐risk factors in stage II colon cancer (CC) is ongoing and several high‐risk factors for stage II CC have been identified; however, the effects of tumour sidedness on ...prognosis are not clear. This study aims to determine whether tumour sidedness could be identified as another high‐risk factor for stage II CC.
Methods
We retrospectively analysed 189 patients with stage II CC and compared clinicopathological findings and long‐term outcomes between the patients with right colonic cancer (RCC) and with left colonic cancer (LCC). Prognostic factors for survival were determined using univariate and Cox proportional regression analyses.
Results
A total of 72 patients were diagnosed with RCC and 117 patients were diagnosed with LCC. Patients with RCC were significantly older (P < 0.001), and the number of harvested lymph nodes was greater in the RCC group (RCC: 25 versus LCC: 19; P = 0.003). The overall survival (OS) was worse in the RCC group than the OS in the LCC group (5‐year survival rate – RCC: 81.3% versus LCC: 90.4%; P = 0.025). Cox proportional regression analysis showed that tumour sidedness was an independent prognostic factor for both OS (hazard ratio (HR) 3.78, 95% confidence interval (CI) 1.61–8.85, P = 0.022) and DFS (HR 2.58, 95% CI 1.33–4.99, P = 0.005).
Conclusion
Patients with RCC have more negative prognostic factors and worse long‐term outcomes than those with LCC in stage II CC. Tumour sidedness is a high‐risk factor in stage II CC patients.

Hepatocyte growth factor activator inhibitor-1 (HAI-1), encoded by the serine protease inhibitor Kunitz type 1 (SPINT1) gene, is a membrane-associated proteinase inhibitor that potently inhibits a ...variety of serine proteinases, including those that are membrane bound. Although HAI-1/SPINT1 is widely expressed by epithelial cells and cancer cells, its functional role is still unclear, particularly in cancer. Here, we show that stable knockdown of HAI-1/SPINT1 in the human pancreatic cancer cell line SUIT-2 induces an elongated spindle-like morphology associated with accelerated invasion, thereby mimicking an epithelial to mesenchymal transition (EMT). We found that HAI-1/SPINT1 knockdown significantly reduced the expression of E-cadherin and was accompanied by up-regulation of Smad-interacting protein 1 (SIP1), an E-cadherin transcriptional repressor. In addition, matrix metalloproteinase-9 (MMP-9) was up-regulated. Similar results were obtained in the HLC-1 lung carcinoma cell line. Moreover, a metastatic variant of SUIT-2 (S2-CP8) that showed loss of E-cadherin expression also showed a significantly reduced level of HAI-1/SPINT1. Engineered overexpression of HAI-1/SPINT1 in S2-CP8 resulted in reversion of E-cadherin expression and SIP1 down-regulation, which accompanied reestablishment of epithelial morphology in culture. The EMT caused by HAI-1/SPINT1 knockdown seemed to be mediated, at least partly, by membrane-bound serine proteinases, matriptase/ST14 and TMPRSS4, as knockdown of matriptase/ST14 or TMPRSS4 in HAI-1/SPINT1 knockdown SUIT-2 cells and HLC-1 cells resulted in reversion of SIP1 and/or MMP-9 expression levels. We suggest that interactions between HAI-1/SPINT1 and membrane-bound serine proteinases contribute to transcriptional and functional changes involved in EMT in certain carcinoma cells.
There have been only a few reports on follow-up results of serous cystic neoplasm (SCN) of the pancreas. The frequency of malignancy and surgical indication of SCN are not determined yet.
In this ...multi-institutional study of the Japan Pancreas Society, a total of 172 patients with SCN were enrolled. The mean follow-up period was 4.5 years. Surgical resection was performed in 90 patients, whereas the remaining 82 were simply observed.
Of all patients, 20% were symptomatic. The tumor was located in the pancreatic head (39%), body (35%), and tail (22%). The mean diameter of the tumor was 4.1 cm. None of the patients showed distant or lymph node metastasis except for liver metastasis found in 2 patients (1.2%). No patient died during the follow-up. The preoperative diagnosis did not correctly identify SCN in 57 (63%) of 90 resected cases. A honeycomb appearance, which is one of the most characteristic findings of SCN, could be diagnosed better by endoscopic ultrasonography than by other imaging diagnostic modalities.
Surgical resection should be considered only when clear distinction from other surgical diseases is difficult, when symptoms or mass effects are present, and when the tumor size is large.
Car use has been identified as sedentary behavior, although it may enhance mobility, particularly in the older population. This cross-sectional study aimed to compare the time spent in objectively ...determined sedentary behavior (SB) and physical activity (PA) between older drivers and non-drivers. Four hundred and fifty Japanese older adults (74.3 ± 2.9 years) who had valid accelerometer data were included. They were asked to respond to a questionnaire and wear an accelerometer (HJA-350IT, Omron Healthcare) on their waist for 7 consecutive days in 2015. To compare activity time between drivers and non-drivers, we calculated estimated means using analysis of covariance, adjusting for sociodemographic, physical, and psychological factors and accelerometer wear time. Compared to non-drivers, drivers engaged in more light-intensity PA (LPA) (drivers: 325.0 vs. non-drivers: 289.0 min/day) and moderate-to-vigorous PA (drivers: 37.5 vs. non-drivers: 30.0 min/day) and less SB (drivers: 493.4 vs. non-drivers: 535.9 min/day) (all
< 0.05). After stratification by age, sex, and residential area, larger effect of driving on PA time was found in older-older adults, in men, and in rural residents. Older drivers were found to be more physically active than non-drivers, suggesting more access to outdoor activities or expanding social network.
Purpose
The Japanese Society of Medical Oncology (JSMO) published a guideline (GL) on febrile neutropenia (FN) in 2017. This study aims to identify promoting factors and disincentives for complying ...with GL recommendations according to attributes of doctors providing chemotherapy.
Methods
A questionnaire survey was conducted with SurveyMonkey™ for physician members of the Japanese Association of Supportive Care in Cancer and relevant academic organizations. Each question had four options (always do, do in more than half of patients, do in less than half, do not at all) and a free description form. Responses were analyzed according to the respondents’ attributes.
Result
Seven hundred eighty-eight out of retrieved 801 responses were available for analysis. Multivariable analysis demonstrated that the percentage of GL users was higher among women and Japanese Society of Clinical Oncology members. The overall compliance rate was higher among women, JSMO members, and board-certified medical oncologists. Internists emphasized the significance of collecting blood cultures at FN onset, and surgeons stressed the importance of G-CSF prophylaxis. Hematologists were less likely to adhere to recommendations on risk assessment of FN by the Multinational Association of Supportive Care in Cancer score and administration of gammaglobulin products. However, those are acceptable due to the characteristics of their practice. Eight recommendations had no difference in compliance rates between users and non-users, some of whose statements were ambiguous and discretionary.
Conclusion
Women were more likely to use and adhere to GL. The recommendations should be developed considering the characteristics of specialty and subspecialty and avoiding ambiguity and discretionary statements.

We investigated the occurrence of non-respiratory bacterial and fungal secondary infections, causative organisms, impact on clinical outcomes, and association between the secondary pathogens and ...mortality in hospitalized patients with coronavirus disease 2019 (COVID-19).
This was a retrospective cohort study that included data from inpatients with COVID-19 from multiple centers participating in the Japan COVID-19 Taskforce (April 2020 to May 2021). We obtained demographic, epidemiological, and microbiological data throughout the course of hospitalization and analyzed the cases of COVID-19 complicated by non-respiratory bacterial infections.
Of the 1914 patients included, non-respiratory bacterial infections with COVID-19 were diagnosed in 81 patients (4.2%). Of these, 59 (3.1%) were secondary infections. Bacteremia was the most frequent bacterial infection, occurring in 33 cases (55.9%), followed by urinary tract infections in 16 cases (27.1%). Staphylococcus epidermidis was the most common causative organism of bacteremia. Patients with COVID-19 with non-respiratory secondary bacterial infections had significantly higher mortality, and a multivariate logistic regression analysis demonstrated that those with bacteremia (aOdds Ratio = 15.3 5.97–39.1) were at higher risk of death. Multivariate logistic regression analysis showed that age, male sex, use of steroids to treat COVID-19, and intensive care unit admission increased the risk for nosocomial bacteremia.
Secondary bacteremia is an important complication that may lead to poor prognosis in cases with COVID-19. An appropriate medical management strategy must be established, especially for patients with concomitant predisposing factors.
Heart failure (HF) occurs frequently among older individuals, and dysfunction of cardiac mitochondria is often observed. We here show the cardiac-specific downregulation of a certain mitochondrial ...component during the chronological aging of mice, which is detrimental to the heart. MitoNEET is a mitochondrial outer membrane protein, encoded by CDGSH iron sulfur domain 1 (CISD1). Expression of mitoNEET was specifically downregulated in the heart and kidney of chronologically aged mice. Mice with a constitutive cardiac-specific deletion of CISD1 on the C57BL/6J background showed cardiac dysfunction only after 12 months of age and developed HF after 16 months; whereas irregular morphology and higher levels of reactive oxygen species in their cardiac mitochondria were observed at earlier time points. Our results suggest a possible mechanism by which cardiac mitochondria may gradually lose their integrity during natural aging, and shed light on an uncharted molecular basis closely related to age-associated HF.
When railway vehicles run on sharp curves, the coefficient of friction (COF) between the outer rail and the leading-outside wheel flange of a bogie is an important value related to problems such as ...wheel/rail severe wear, squeak noise arising from wheel/rail contact, and running safety against flange-climb derailment. In general, it is difficult to grasp the actual state of COF, which changes from moment to moment during commercial operations. From the viewpoint of running safety and maintenance, it is desirable to detect curved tracks with relatively high COF from the entire service line. The monitoring bogie, which can measure wheel/rail contact forces during commercial operations, has realized a long-term observation of wheel/rail contact forces. For further applications of the monitoring bogie, the simulation-based estimation method of COF at the wheel flange have been proposed in the author’s previous paper. Furthermore, an investigation based on roller-rig tests have been conducted. However, the probability of the flange wear progress cannot be determined only by COF at the wheel flange, and other factors should be involved. The wear number is used to assess rolling contact fatigue and wheel/rail wear progress. In the present paper, on the basis of a multi-body dynamics simulation model of the roller-rig equipment, the difference of the wear number among four lubrication conditions on the bogie is clarified. The estimation method of COF is extended to estimate the wear number simultaneously. The extended method is applied to the roller-rig test and the wear number is estimated. The estimated wear number shows qualitative agreement with the amount of wear debris, which is observed in the roller-rig test conducted in previous research.
We examined the relationships of objectively-measured walkability with accelerometer-measured activity patterns including bout-specific moderate-to-vigorous physical activity (MVPA), light-intensity ...physical activity (LPA) and sedentary behavior in older Japanese adults.
This cross-sectional study included 450 men and women (mean age 74 years) who were randomly selected from a resident registry of each municipality (Bunkyo, Fuchu and Oyama city). Neighborhood walkability was assessed using geographic information system data and activity patterns by a validated accelerometer (Active style Pro HJA-350IT). MVPA was assessed as overall (all minutes), short-bout (<10 min) and long-bout (≥10 min). We used analysis of covariance to examine environmental relationships with the three MVPA measures, LPA and sedentary behavior by quartiles of walkability, adjusting for accelerometer wear time and sociodemographic, behavioral and health-related factors.
In total, 27% of participants achieved ≥150 min/week of long-bout MVPA. Walkability was not associated with overall MVPA (Q1 = 35.0, Q2 = 31.9, Q3 = 31.4, Q4 = 34.2 min/day) Older adults living in low-walkability areas significantly accumulated more short-bout MVPA (Q1 = 27.2, Q2 = 22.1, Q3 = 20.1, Q4 = 20.5 min/day) and LPA (Q1 = 346, Q2 = 311, Q3 = 289, Q4 = 284 min/day), but less sedentary behavior (Q1 = 479, Q2 = 520, Q3 = 544, Q4 = 544 min/day) than those living in high-walkability areas.
Those living in low-walkability areas accumulated more LPA and short-bout MVPA. These activities might be helpful targets for initiatives to promote physical activity among older adult residents of low-walkability areas. Geriatr Gerontol Int 2019; 19: 861-867.
