According to the past statistics, in such a disastrous situation, tend to increase acute myocardial infarction (AMI) patients but relation with heart failure (HF) is unknown.
Purpose
– This study aims to describe how a work team adapted to its fluctuated and severe environment by changing from “lean” to “over-lean” mode. To do this, the author investigated the relations ...among productivity, the vertical division of labor, and group leaders' behavior in a Japanese automobile assembly plant.
Design/methodology/approach
– The authors conducted field study at an assembly plant for five months. They collected three plant-level data to investigate the capability of its shop floor: transition of production volume; transition of the number of workers; and productivity. And they collected two types of workforce data: skill map and work shift. Moreover, they videotaped the behavior of group leaders on several days and analyzed them through a time study.
Findings
– The work team of this study achieved high productivity even in its tough environment. However, the authors' time study of group leaders showed that the group leaders, who usually engage in some management activities outside of the production line, did many tasks within the line. This indicates the team had a weakness toward the change of team members. Changing to this over-lean mode enabled the team to survive in a short-run, but maintaining the mode has a weakness in enhancing long-term competitiveness.
Originality/value
– This study proposes a balance between the two modes is required for organizations if they are to survive their severe and fluctuating environments.
Pig skeletal muscle-derived stem cells (SK-MSCs) were transplanted onto the common peroneal nerve with a collagen tube as a preclinical large animal experiment designed to address long nerve gaps. In ...terms of therapeutic usefulness, a human family case was simulated by adjusting the major histocompatibility complex to 50% and 100% correspondences. Swine leukocyte antigen (SLA) class I haplotypes were analyzed and clarified, as well as cell transplantation. Skeletal muscle-derived CD34+/45− (Sk-34) cells were injected into bridged tubes in two groups (50% and 100%) and with non-cell groups. Therapeutic effects were evaluated using sedentary/general behavior-based functional recovery score, muscle atrophy ratio, and immunohistochemistry. The results indicated that a two-Sk-34-cell-transplantation group showed clearly and significantly favorable functional recovery compared to a non-cell bridging-only group. Supporting functional recovery, the morphological reconstitution of the axons, endoneurium, and perineurium was predominantly evident in the transplanted groups. Thus, Sk-34 cell transplantation is effective for the regeneration of peripheral nerve gap injury. Additionally, 50% and 100% SLA correspondences were therapeutically similar and not problematic, and no adverse reaction was found in the 50% group. Therefore, the immunological response to Sk-MSCs is considered relatively low. The possibility of the Sk-MSC transplantation therapy may extend to the family members beyond the autologous transplantation.
A 47-year-old male patient without a documented past medical history was referred to Sanno Hospital because of constipation and abdominal pain, which he had had for more than 5 years. Abdominal X-ray ...and CT scan showed an enlarged ascending colon from the cecum to the transverse colon, without apparent mechanical obstruction. The patient was diagnosed with chronic idiopathic colonic pseudo-obstruction, and because his symptoms were resistant to medication, surgical treatment was required. Laparoscopic subtotal colectomy was performed without any complications. Constipation was relieved, and the patient began defecating 2-3 times a day without medication. Pathological specimens showed that Meissner's plexus and Auerbach's plexus had decreased and that there were fewer ganglion cells-findings consistent with chronic idiopathic intestinal pseudo-obstruction.
We report a case of temporomandibular joint (TMJ) ankylosis suspected to be caused by psoriatic arthritis and treated by condylectomy. The patient was a 63-year-old woman. She was referred to our ...department by a university oral surgery department because of trismus. Her medical history revealed the onset of psoriasis at the age of 14 years. She experienced pain on mouth opening in the right TMJ, and the interincisal distance was 12 mm. Panoramic radiography and computed tomography revealed narrowing of the right TMJ cavity. Blood tests indicated a mildly increased C-reactive protein level, negativity for rheumatoid factor, and presence of human leukocyte antigens A2 and B46. On the basis of a clinical diagnosis of TMJ ankylosis caused by psoriatic arthritis, condylectomy was performed with the patient under general anesthesia. During surgery, the mandibular condyle was found to adhere to the temporal bone in the right TMJ region. The lower portion of the mandibular condyle adhesions was resected, and the temporal and masseter muscles were detached bilaterally. This resulted in an intraoperative interincisal distance of 40 mm. After surgery, the patient performed the recommended mouth-opening exercise training. However, 1 year after surgery, the interincisal distance had decreased to 22 mm because of an exacerbation of systemic symptoms of psoriasis.
A 59-year-old man with inferior acute myocardial infarction underwent an urgent coronary angiography and as a result a total occlusion of the ostial right coronary artery (RCA) was observed. ...Interestingly, the thrombus in the RCA clearly shortened within 2.3 s during the contrast media injection. While we prepared for percutaneous coronary intervention (PCI) of the RCA, ventricular fibrillation occurred and pulseless electrical activity continued in spite of repeated cardioversion. Despite no stenosis in the left main trunk (LMT) in the initial coronary angiography, the LMT was totally occluded. Following aspiration therapy on the LMT with percutaneous cardiopulmonary support, PCI of the RCA was performed. A coronary angiography should be carefully performed if a lot of thrombi exist in the coronary ostium because LMT embolism may have been caused by thrombus extraction from the RCA by contrast media injection.
