ABSTRACT Background The clinical oral examination (COE) is the criterion standard for the initial detection of oral lesions that harbor dysplasia or oral squamous cell carcinoma (OSCC) at an early ...stage when they are most treatable. The authors conducted a systematic review to assess the effectiveness of the COE in predicting histologic diagnosis of dysplasia or OSCC. Methods The authors conducted automated searches of PubMed, Web of Knowledge and the Cochrane Library from 1966 through 2010 for randomized controlled trials and observational studies that included the terms “oral mucosal lesion screening” and “oral lesions.” They determined the quality (sensitivity, specificity, positive predictive value, negative predictive value and diagnostic odds ratio) of selected studies by using the Quality Assessment of Diagnostic Accuracy Studies tool. Results The overall diagnostic odds ratio was 6.1 (95 percent confidence interval, 2.1–17.6); therefore, the COE was considered to have poor overall performance as a diagnostic method for predicting dysplasia and OSCC. Clinical Implications On the basis of the available literature, the authors determined that a COE of mucosal lesions generally is not predictive of histologic diagnosis. The fact that OSCCs often are diagnosed at an advanced stage of disease indicates the need for improving the COE and for developing adjuncts to help detect and diagnose oral mucosal lesions.
The impact of Candida sp. in the development of oral cancer remains uncertain and requires sensitive analytical approaches for clarification. Given the invasive capabilities of these microorganisms ...in penetrating and invading host tissues through hyphal invasion, this study sought to detect the presence of five Candida sp. in oral biopsy tissue samples from non-smoker patients. Samples were obtained from patients at varying stages of oral carcinogenesis, including dysplasia, carcinoma in situ, OSCC, and histologically benign lesions, and analyzed using Real-Time PCR. Oral tissue samples from 80 patients (46 males and 34 females) were included. Significantly higher C. albicans presence was detected in the mild/moderate dysplasia group compared to the healthy (p = 0.001), carcinoma in situ (p = 0.031) and OSCC groups (p = 0.000). Similarly, C. tropicalis carriage was higher in tissues with mild/moderate dysplasia compared to healthy (p = 0.004) and carcinoma in situ (p = 0.019). Our results showed a significant increase in the presence of C. albicans and C. tropicalis within the mild/moderate dysplasia group compared to other cohorts. Coexistence of these two microorganisms was observed, suggesting a potential transition from a commensal state to an opportunistic pathogen, which could be particularly linked to the onset of oral neoplasia.
This study aimed to develop a suitable buccal mucoadhesive nanoparticle (NP) formulation containing fluconazole for the local treatment of oral candidiasis. The suitability of the prepared ...formulations was assessed by means of particle size (PS), polydispersity index, and zeta potential measurements, morphology analysis, mucoadhesion studies, drug entrapment efficiency (EE), in vitro drug release, and stability studies. Based on the optimum NP formulation, ex vivo drug diffusion and in vitro cytotoxicity studies were performed. Besides, evaluation of the antifungal effect of the optimum formulation was evaluated using agar diffusion method, fungicidal activity-related in vitro release study, and time-dependent fungicidal activity. The effect of the optimum NP formulation on the healing of oral candidiasis was investigated in an animal model, which was employed for the first time in this study. The zeta potential, mucoadhesion, and in vitro drug release studies of various NP formulations revealed that chitosan-coated NP formulation containing EUDRAGIT(®) RS 2.5% had superior properties than other formulations. Concerning the stability study of the selected formulation, the formulation was found to be stable for 6 months. During the ex vivo drug diffusion study, no drug was found in receptor phase, and this is an indication of local effect. The in vitro antifungal activity studies showed the in vitro efficacy of the NP against Candida albicans for an extended period. Also, the formulation had no cytotoxic effect at the tested concentration. For the in vivo experiments, infected rabbits were successfully treated with local administration of the optimum NP formulation once a day. This study has shown that the mucoadhesive NP formulation containing fluconazole is a promising candidate with once-a-day application for the local treatment of oral candidiasis.
Summary Stage of disease at the diagnosis of oral cancer is thought to be a significant factor in prognosis and outcome (International Agency for Research on Cancer/World Health Organization, 2014). ...Unfortunately, we continue to diagnose almost 2/3 of these cancers at advanced stages of disease despite the ongoing research for devices/methods to aid the clinicians in detection and accurate oral mucosal lesion diagnosis. This paper explores both the nature of oral cancer and the adjuncts available for detection, and presents the current issues in diagnostic delays of oral cancer detection.
The aim of the present study was to evaluate IL-1ß, IL-6 and TNF-α expression levels of macrophage cells induced by benzydamine hydrochloride (BNZ), BNZ with chitosan, calcium hydroxide (CH) and ...chlorhexidine (CHX) medicaments. Half maximal inhibitory concentrations (IC50) were assessed on THP-1, Saos-2, and CRL-2014 cells using MTT assay. THP-1 cells were differentiated into macrophages with phorbol12-myristate13-acetate and activated with lipopolysaccharide. IL-1β, IL-6 and TNF-α levels in supernatants were determined using enzyme-linked immunosorbent assay (ELISA). The data were examined with one-way ANOVA and Tukey’s multiple comparison test (p=0.05). At the selected concentrations, the cell viability was higher than 50% for chitosan and CH, whereas CHX presented lower IC50 values than BNZ and BNZ+chitosan. According to ELISA results, the lowest IL-1β, IL-6 and TNF-α values were observed with BNZ+Chitosan 50 µg/mL and BNZ 50 µg/mL. BNZ+chitosan 50 µg/mL combination has revealed promising anti-inflammatory effects. Nevertheless, these findings need to be examined in clinical conditions.
Abstract Root canal sealers should have optimum adhesion to the root dentin, low cytotoxicity, sufficient radiopacity and high dimensional stability, which depend mainly on their chemical ...composition. The aims of this study were to evaluate the chemical composition of Syntex (Cerkamed, Stalowa Wola, Poland), Adseal (Meta Biomed, Cheongju-si, Korea), Sealapex (SybronEndo, Scafati, Italy), MTA Fillapex (Angelus, Londrina, Brazil) and GuttaFlow Bioseal (Coltene/Whaledent, Altstätten, Switzerland) and to determine their radiopacity comparatively. Stainless steel molds with an inner diameter of 5 mm and a depth of 2 mm were constructed. Tested root-canal sealers were mixed and poured into the molds. Following setting, digital periapical radiographs of the specimens (n=12) were taken under standard exposure conditions and the mean gray-values of specimens were measured using a computer graphics program. Then the specimens were examined by scanning electron microscopy, and Energy-dispersive X-ray analysis was performed. The data were analyzed using One-way ANOVA followed by Holm-Sidak multi-comparison test (p = 0.05). Sealapex showed higher calcium peak than MTA Fillapex, GuttaFlow Bioseal, Adseal and Syntex sealers. MTA Fillapex contained a highest amount of tungstate. Sealapex presented the highest radiopacity value among the tested materials (p < 0.05), whereas MTA Fillapex was the least radiopaque material (p < 0.05). Each root canal sealer contains different compositions of radiopacifiers including bismuth oxide, calcium tungstate and zirconium oxide. The tested root canal sealers had radiopacity values above the ISO-specified limit. Sealapex was the most radiopaque root canal sealer followed by GuttaFlow Bioseal, Syntex, Adseal and MTA Fillapex.
Resumo Os cimentos endodônticos devem conter ótima adesão à dentina radicular, baixa citotoxicidade, radiopacidade suficiente e estabilidade dimensional elevada, que dependem principalmente da sua composição química. Os objetivos deste estudo foram avaliar a composição química de Syntex (Cerkamed, Stalowa Wola, Polônia), Adseal (Meta Biomed, Cheongju-si, Coreia), Sealapex (SybronEndo, Scafati, Itália), MTA Fillapex (Angelus, Londrina, Brasil) e GuttaFlow Bioseal (Coltene/Whaledent, Altstätten, Suíça) e determinar comparativamente a sua radiopacidade. Foram construídos moldes de aço inoxidável com um diâmetro interior de 5 mm e uma profundidade de 2 mm. Os cimentos testados foram misturados e vertidos em moldes. Após a fixação, foram feitas radiografias periapicais digitais dos espécimes (n=12) em condições de exposição padrão e os valores médios de cinza dos espécimes foram medidos utilizando um programa de computação gráfica. Em seguida, as amostras foram examinadas por microscopia eletrônica de varredura, e foi realizada uma análise de raios X dispersiva de energia. Os dados foram analisados utilizando a ANOVA unidirecional seguida do teste de comparação múltipla Holm-Sidak (p = 0,05). O Sealapex mostrou um pico de cálcio mais elevado do que os cimentos MTA Fillapex, GuttaFlow Bioseal, Adseal e Syntex. O MTA Fillapex continha uma quantidade mais elevada de tungstato. O Sealapex apresentou o maior valor de radiopacidade entre os materiais testados (p < 0,05), enquanto que a MTA Fillapex foi o material menos radiopaco (p < 0,05). Cada cimento endodôntico contém diferentes composições de radiopacificadores incluindo óxido de bismuto, tungstato de cálcio e óxido de zircônio. Os cimentos testados apresentaram valores de radiopacidade acima do limite especificado pela ISO. Sealapex foi o cimento obturador mais radiopaco seguido por GuttaFlow Bioseal, Syntex, Adseal e MTA Fillapex.
This study aimed to develop fluconazole loaded buccal mucoadhesive oral strip for local treatment of oral candidiasis. Oral strip was prepared by solvent casting method with different polymer ...combinations and evaluated for weight, thickness, drug content, tensile strength, swelling behavior, in vitro disintegration time and drug release, ex vivo mucoadhesive performance, stability study, ex vivo drug diffusion and in vitro antifungal activity. Oral strips were smooth and elegant in appearance; were uniform in thickness, weight and drug content. The disintegration time was within the range of 23–28 s. The release rates of fluconazole from oral strips reached ≈96%. The optimum formulation was stable for 6 months. The in vitro antifungal activity studies showed the efficacy of the oral strip against Candida albicans for an extended period. Cytotoxicity was also studied, and no cytotoxic effect was observed at the tested concentration. The developed oral strip can be a promising delivery system in oral candidiasis.

The suitability of the ...prepared formulations for buccal application was assessed by means of rheological studies, textural profile analysis, and ex vivo drug-release studies. Plastic flows, typical gel-like spectra, and suitable mechanical properties were obtained from prepared formulations. The retention time was explored in in vivo distribution studies and the effect of the gel containing CsA SLNs on the healing of oral mucosal ulceration was investigated in an animal model. In vivo distribution studies are a very important indicator of the retention time of formulations at the application site.
Distribution studies showed that 64.76% ± 8.35% of the formulation coded "F8+SLN" remained on the buccal mucosa 6 hours after application. For the second part of the in vivo experiments, 36 rabbits were separated into three groups: the first group was treated with the gel formulation without the active agent; the second group with the gel formulation containing CsA SLNs; and the third group, used as the control group, received no treatment. Wound healing was established by scoring of the rate of wound healing on Days 3, 6, 9, and 12. Histological observations were made on the same days as the scoring studies. The bioadhesive gel formulation that included CsA SLNs increased the rate of mucosal repair significantly.
This study has shown that the bioadhesive gel formulation containing CsA SLNs reported here is a promising candidate for the topical treatment of recurrent aphthous stomatitis.
Styloid process of temporal bone is a pointed projection from the petrous part of temporal bone which ranges with an average of 25 mm in length, and any process longer than 30 mm is defined as the ..."Elongated Styloid Process”. In asymptomatic cases, diagnosis is usually made with routine radiological examination, but morphological aspects of styloid process can be evaluated with different imaging modalities. Although digital panoramic radiographs are sufficiently accurate for the diagnosis of elongated styloid process, multislice computed tomography scan with 3D reconstruction can further assist in determining the actual length and correct relationship with the surrounding adjacent anatomical structures. The aim of this case report is to investigate the morphological characteristics of an unilateral elongated styloid process (ESP) using both conventional and multidetector computed tomography (MDCT) on a 55 year old asymptomatic male patient and to review the literature.
