IntroductionVasodilatory hypotension is common among intensive care unit (ICU) patients; vasopressors are considered standard of care. However, optimal mean arterial pressure (MAP) targets for ...vasopressor titration are unknown. The objective of the Optimal VAsopressor TitraTION in patients 65 years and older (OVATION-65) trial is to ascertain the effect of permissive hypotension (vasopressor titration to achieve MAP 60–65 mm Hg) versus usual care on biomarkers of organ injury in hypotensive patients aged ≥65 years.Methods and analysisOVATION-65 is an allocation-concealed randomised trial in 7 Canadian hospitals. Eligible patients are ≥65 years of age, in an ICU with vasodilatory hypotension, receiving vasopressors for ≤12 hours to maintain MAP ≥65 mm Hg during or after adequate fluid resuscitation, and expected to receive vasopressors for ≥6 additional hours. Patients are excluded for any of the following: active treatment for spinal cord or acute brain injury; vasopressors given solely for bleeding, ventricular failure or postcardiopulmonary bypass vasoplegia; withdrawal of life-sustaining treatments expected within 48 hours; death perceived as imminent; previous enrolment in OVATION-65; organ transplant within the last year; receiving extracorporeal life support or lack of physician equipoise. Patients are randomised to permissive hypotension versus usual care for up to 28 days. The primary outcome is high-sensitivity troponin T, a biomarker of cardiac injury, on day 3. Secondary outcomes include biomarkers of injury to other organs (brain, liver, intestine, skeletal muscle); lactate (a biomarker of global tissue dysoxia); resource utilisation; adverse events; mortality (90 days and 6 months) and cognitive function (6 months). Assessors of biomarkers, mortality and cognitive function are blinded to allocation.Ethics and disseminationThis protocol has been approved at all sites. Consent is obtained from the eligible patient, the substitute decision-maker if the patient is incapable, or in a deferred fashion where permitted. The Fermi Gamma-ray Space Telescope was launched on 2008 June 11, carrying the Large Area Telescope(LAT), sensitive to gamma-rays in the 20 MeV – 300 GeV energy range. The Crab Nebula had been detectedand studied in the 70 MeV – 30 GeV band using the CGRO-EGRET experiment, but no pulsar wind nebula(PWN) had ever been firmly identified in the high energy gamma-ray domain.PWNe are powered by the constant injection of a relativistic wind of electrons and positrons from theircentral pulsars. These charged particles are accelerated at the shock front forming the PWN and emit photons which can be observed along the entire electromagnetic spectrum, including the high energy gamma-raydomain.Data provided by the Fermi-LAT during the first two years of the mission have allowed the detection andthe identification of three PWNe and their associated pulsars (Crab Nebula, Vela X and MSH 15-52) as well asthe PWN HESS J1825-137 discovered by ground-based experiments sensitive to very high energy gamma-rays.Results of temporal, spectral and morphological analyses of the pulsar/PWN systems detected by Fermi-LAT, as well as results of systematic studies performed (i) around every gamma-ray pulsar detected by the LATand (ii) around every very high energy source identified as a PWN or a PWN candidate are presented in thisdissertation. These studies bring new insights and constraints on the physical properties of the sources as wellas on emitting processes in pulsar magnetospheres and in PWNe.
The supernova remnant (SNR) G150.3+4.5 was recently discovered in the radio band; it exhibits a shell-like morphology with an angular size of \(\sim 3^{\circ}\), suggesting either an old or a nearby ...SNR. Extended \(\gamma\)-ray emission spatially coincident with the SNR was reported in the Fermi Galactic Extended Source Catalog, with a power-law spectral index of \(\Gamma\) = 1.91 \(\pm\) 0.09. Studying particle acceleration in SNRs through their \(\gamma\)-ray emission is of primary concern to assess the nature of accelerated particles and the maximum energy they can reach. Using more than ten years of Fermi-LAT data, we investigate the morphological and spectral properties of the SNR G150.3+4.5 from 300 MeV to 3 TeV. We use the latest releases of the Fermi-LAT catalog, the instrument response functions and the Galactic and isotropic diffuse emissions. We use ROSAT all-sky survey data to assess any thermal and nonthermal X-ray emission, and we derive minimum and maximum distance to G150.3+4.5. We describe the \(\gamma\)-ray emission of G150.3+4.5 by an extended component which is found to be spatially coincident with the radio SNR. The spectrum is hard and the detection of photons up to hundreds of GeV points towards an emission from a dynamically young SNR. The lack of X-ray emission gives a tight constraint on the ambient density \(n_0 \leq 3.6 \times 10^{-3}\) cm\(^{-3}\). Since G150.3+4.5 is not reported as a historical SNR, we impose a lower limit on its age of \(t\) = 1 kyr. We estimate its distance to be between 0.7 and 4.5 kpc. We find that G150.3+4.5 is spectrally similar to other dynamically young and shell-type SNRs, such as RX J1713.7\(-\)3946 or Vela Junior. The broadband nonthermal emission is explained with a leptonic scenario, implying a downstream magnetic field of \(B = 5\) \(\mu\)G and acceleration of particles up to few TeV energies.
Spatial extension is an important characteristic for correctly associating {gamma}-ray-emitting sources with their counterparts at other wavelengths and for obtaining an unbiased model of their ...spectra. We present a new method for quantifying the spatial extension of sources detected by the Large Area Telescope (LAT), the primary science instrument on the Fermi Gamma-ray Space Telescope (Fermi). We perform a series of Monte Carlo simulations to validate this tool and calculate the LAT threshold for detecting the spatial extension of sources. We then test all sources in the second Fermi -LAT catalog (2FGL) for extension. We report the detection of seven new spatially extended sources.

Purpose
This study analyses the relationship between medication use and frailty by considering the quantity of medications prescribed (polypharmacy) and the quality of medication prescribing ...(according to French criteria for Potentially Inappropriate Medications—PIMs) in people aged 65 and over.
Methods
This is a cross-sectional study based on the data from a nationally representative study about health and use of healthcare resources in France (ESPS 2012). The number of frailty criteria was assessed among exhaustion, unintentional weight loss, muscle weakness, impaired mobility, and low level of physical activity. Polypharmacy and PIMs were assessed from the data of reimbursement by the National Health Insurance over the whole year 2012. PIMs were defined according to the Laroche list plus additional criteria dealing with inappropriate prolonged use of medications. The analyses used Poisson regression models, with the number of frailty criteria as dependent variable.
Results
The study population was composed of 1003 women and 887 men, of mean age 74.7 +/− 7.4 years. Polypharmacy (5 to 9 drugs) and excessive polypharmacy (≥10 drugs) were reported in 42.9 and 27.4% of the study population, respectively, while 46.7% of the study population received at least one PIM during the year 2012. Polypharmacy and PIMs were both associated with the number of frailty criteria in models adjusted for socio-demographic and health characteristics of the participants. The prescription of anticholinergic medications was the only PIM that remained significantly associated with the number of frailty criteria after adjustment for polypharmacy.
Conclusions
Polypharmacy and use of anticholinergic medications are independently associated with frailty in old people.
