Recent reports suggest that brain-derived neurotrophic factor (BDNF) could be a biomarker for relapse, drug craving and withdrawal severity. In particular, elevated BDNF levels among former cocaine ...users have been associated with higher rates of relapse in 90 d. However, no data are available on BDNF levels at baseline and during crack cocaine withdrawal. This study evaluated BDNF among crack cocaine users during inpatient treatment, before and after withdrawal, vs. healthy controls. Clinical correlates with changes in BDNF levels were also assessed. Serum BDNF was evaluated in 49 male crack users on the first and last days of hospitalization and in 97 healthy controls. Serum BDNF was assayed using a sandwich ELISA kit. BDNF levels were significantly lower upon admission when compared to controls, even after adjustment for age, length of inpatient treatment, number of crack rocks used in the last 30 d, years of crack use and interaction between the latter two variables. At discharge, BDNF levels between patients and controls were similar. Number of crack rocks used in the last 30 d and years of crack use were inversely correlated with the outcome. Our findings show that BDNF levels increase during early crack cocaine withdrawal, at an inverse correlation with number of crack rocks used in the last 30 d and years of crack use.
To report a patient with prenatal diagnosis of portosystemic shunt; a rare condition in humans.
17-Day-old female infant admitted for investigation of suspected diagnosis of portosystemic shunt, ...presumed in obstetric ultrasound. The hypothesis was confirmed after abdominal angiography and liver Doppler. Other tests such as echocardiography and electroencephalogram were performed to investigate possible co-morbidities or associated complications, and were normal. We chose conservative shunt treatment, as there were no disease-related complications and this was intrahepatic shunt, which could close spontaneously by the age of 2 years.
Portosystemic shunt can lead to various complications such as hepatic encephalopathy, hypergalactosemia, liver tumors, and hepatopulmonary syndrome. Most diagnoses are done after one month of age, after such complications occur. The prenatal diagnosis of this patient provided greater security for the clinical picture management, as well as regular monitoring, which allows the anticipation of possible complications and perform interventional procedures when needed.

O shunt portossistêmico pode levar a diversas complicações, como encefalopatia hepática, hipergalactosemia, tumores hepáticos e síndrome hepatopulmonar. A maioria dos diagnósticos é feita a partir de um mês de vida, após tais complicações ocorrerem. O diagnóstico pré-natal dessa paciente possibilitou maior segurança para o manejo do quadro, bem como um acompanhamento periódico que permite antecipar possíveis complicações e adotar conduta intervencionista, se necessário.
Prenatal cocaine exposure (PCE) is associated with neurobehavioral problems during childhood and adolescence. Early activation of the inflammatory response may contribute to such changes. Our aim was ...to compare inflammatory markers (IL-6 and IL-10) both in umbilical cord blood and in maternal peripheral blood at delivery between newborns with history of crack/cocaine exposure in utero and non-exposed newborns.
In this cross-sectional study, 57 newborns with a history of crack/cocaine exposure in utero (EN) and 99 non-exposed newborns (NEN) were compared for IL-6 and IL-10 levels. Sociodemographic and perinatal data, maternal psychopathology, consumption of nicotine and other substances were systematically collected in cases and controls.
After adjusting for potential confounders, mean IL-6 was significantly higher in EN than in NEN (10,208.54, 95% confidence interval 95%CI 1,328.54-19,088.55 vs. 2,323.03, 95%CI 1,484.64-3,161.21; p = 0.007; generalized linear model GLM). Mean IL-10 was also significantly higher in EN than in NEN (432.22, 95%CI 51.44-812.88 vs. 75.52, 95%CI 5.64-145.39, p = 0.014; GLM). Adjusted postpartum measures of IL-6 were significantly higher in mothers with a history of crack/cocaine use (25,160.05, 95%CI 10,958.15-39,361.99 vs. 8,902.14, 95%CI 5,774.97-12,029.32; p = 0.007; GLM), with no significant differences for IL-10. There was no correlation between maternal and neonatal cytokine levels (Spearman test, p ≥ 0.28 for all measures).
IL-6 and IL-10 might be early biomarkers of PCE in newborns. These findings could help to elucidate neurobiological pathways underlying neurodevelopmental changes and broaden the range of possibilities for early intervention.

O sistema purinérgico tem sido amplamente implicado em condições médicas, entre elas, o transtorno bipolar (TB). Até onde sabemos, nenhum estudo foi realizado com o objetivo de esclarecer a possível ...contribuição do receptor P2X7 (P2X7R) na patofisiologia do TB ou mesmo com o objetivo de revisar o que já fora previamente publicado sobre este assunto. Nos presentes trabalhos, nosso objetivo foi em primeiro lugar explorar a relação entre o receptor e esse transtorno e identificar os mecanismos pelos quais o P2X7R desempenha um papel no TB e por fim revisar a literatura, focando na sinalização purinérgica no TB, com ênfase na sinalização por ATP e adenosina, destacando o potencial papel do P2X7R na modulação da inflamação e ativação microglial em pacientes bipolares. Para o primeiro objetivo, analisamos o efeito de moduladores do P2X7R (BzATP, BBG e A438079) sobre o comportamento (atividade locomotora) e sobre marcadores de neuroinflamação (IL-1 , TNF- e IL-6), excitotoxicidade (GFAP, TBARS) e neuroplasticidade (BDNF) em um modelo farmacológico de mania induzido pelo tratamento agudo e crônico com D – anfetamina (AMPH) (2mg/kg). Demonstramos na atividade locomotora uma aparente falta de capacidade de resposta à AMPH por animais com P2X7R bloqueado ou ausente (P2X7R-/-). Da mesma forma, observamos que o P2X7R participa do aumento do ambiente próinflamatório e excitotóxico induzido pela AMPH, demonstrado pelo papel de reversão do bloqueio do P2X7R ou pela perda de resposta dos animais P2X7R-/-. Em conclusão, nossos resultados suportam a hipótese de que o P2X7R possui um papel na patofisiologia do TB, como sugerido pelo nosso modelo animal, principalmente por mediar a neuroinflamação e a excitotoxicidade e finalmente levando a mudanças comportamentais. Assim, acreditamos que o P2X7R possui potencial para se tornar um alvo terapêutico do TB. Baseado neste cenário, estudos mais detalhados do papel do P2X7R no TB são necessários, especialmente devido à sua capacidade de agir sobre a microglia e modular a neuroinflamação e a excitotoxicidade.
The purinergic system has been increasingly implicated in medical conditions, among them bipolar disorder (BD). Up to date, no study has been conducted in order to clarify the possible contribution of the P2X7 receptor (P2X7R) to BD pathophysiology or even to review the previously data about this subject. In the presents works, we aim firstly to explore the association between the receptor and the disorder and identify the mechanisms by which the P2X7R plays a role in BD and lastly to review the literature focused on purinergic signaling in BD, with an emphasis on ATP and adenosine signaling, highlighting the potential role of P2X7R in modulating inflammation and microglia activation in bipolar patients. For the first aim, we analyzed the P2X7R modulatory effect (BzATP, BBG and A438079) on behavior (locomotor activity) and on markers of neuroinflammation (IL-1 , TNF- and IL-6), excitotoxicity (GFAP, TBARS), and neuroplasticity (BDNF) in a pharmacological model of mania induced by acute and chronic D-amphetamine (AMPH) treatment (2 mg/kg). We demonstrate in the locomotor activity an apparent lack of responsiveness to AMPH by animals with blocked or absent P2X7R (P2X7R-/-). Likewise, we observed that the P2X7R participates in the AMPH-induced increase of proinflammatory and excitotoxicity environment, demonstrated by the reversal role of P2X7R blockage or the lack of response by P2X7R-/- mice. In conclusion, our results provide support for the hypothesis that P2X7R plays a role in BD pathophysiology, as suggested by our animal model, mainly by mediating neuroinflammation and excitotoxicity and ultimately leading to behavioral changes. Thus, we believe that P2X7R has the potential to become a therapeutic target of BD. Based on this scenario, a more detailed study of the role of P2X7R in BD is warranted, especially due to its ability to act on microglia and modulate neuroinflammation and excitotoxicity.
O Transtorno Bipolar é uma doença crônica e grave que apresenta curso episódico e alto impacto sobre a funcionalidade do paciente. Possui potencial para alto grau de severidade, recorrência e ...intensidade e estima-se que sua prevalência ao longo da vida seja de 2,4% da população mundial. A fisiopatologia deste transtorno ainda não é bem compreendida e ainda o fármaco de primeira escolha é o lítio, embora tampouco seja claro seu mecanismo de ação. O sistema purinérgico principalmente devido a suas ações sobre neuroproteção, inflamação e ativação microglial se destaca como uma via promissora de novos alvos terapêuticos, especialmente o receptor purinérgico P2X7 (P2X7R). Dessa maneira, o objetivo desta tese é caracterizar o envolvimento do sistema purinérgico na fisiopatologia do transtorno bipolar. De maneira geral, avaliamos metabólitos do sistema purinérgico em soro de pacientes e controles, correlacionando com dados clínicos. Avaliamos a frequência do polimorfismo do P2RX7 1513A>C em pacientes e controles. Realizamos um modelo animal farmacológico de mania e investigamos o envolvimento do P2X7R neste modelo e por último buscamos melhor compreender a ação neuroprotetora do lítio verificando a participação purinérgica. Nossos resultados primeiramente nos mostraram uma diminuição nos níveis de adenosina em pacientes em comparação a controles e uma importante correlação inversa entre os níveis de adenosina e a piora dos sintomas. Demonstramos ainda que pacientes com transtorno bipolar apresentam diminuição na frequência do alelo 1513C e o potencial aumento na frequência do genótipo 1513AA e 1513AA/AC em relação aos controles configurando um pior prognóstico de aumento da forma mais ativa do receptor. Também mostramos que o P2X7R possui um papel importante no estabelecimento do modelo de mania, modulando principalmente a via dopaminérgica que é refletida na mudança comportamental observada pelo modelo e por último, o lítio foi capaz de prevenir a resposta de insulto nas células neuronais (linhagem PC-12) e não foi capaz de prevenir a ativação mediada pelo P2X7R em células microgliais (linhagem N9), indicando uma resposta celular diferenciada. Podemos concluir que de maneira translacional conseguimos demonstrar a participação do sistema purinérgico na fisiopatologia do transtorno bipolar, sugerindo ainda um possível biomarcador tanto para diagnóstico como para progressão da doença, os níveis séricos de adenosina, e ainda um potencial biomarcador genético de predição e diagnóstico da doença, o polimorfismo do P2X7R 1513A>C, assim como contribuímos para o entendimento do mecanismo de ação do lítio e definimos um potencial novo alvo terapêutico para o transtorno, o receptor P2X7.
Bipolar Disorder is a severe and chronic psychiatric disorder that presents an episodic course and high impact on the patient's functionality. It has potential for a high degree of severity, recurrence and intensity. The prevalence throughout life of bipolar disorder is 2.4% of the world population. The pathophysiology of this disorder is still not well understood and the drug of first choice is lithium, although its mechanism of action is not clear. The purinergic system mainly due to its actions on neuroprotection, inflammation and microglial activation stands out as a promising pathway for new therapeutic targets, especially the purinergic receptor P2X7 (P2X7R). Thus, the purpose of this thesis is to characterize the involvement of the purinergic system in the pathophysiology of bipolar disorder. In general, we evaluated purinergic system metabolites in serum from bipolar disorder patients and healthy controls, correlating with clinical data. We evaluated the frequency of P2RX7 1513A>C polymorphism in bipolar disorder patients and in healthy subjects. We performed a pharmacological animal model of mania and investigated the involvement of the P2X7 purinergic receptor in this model and finally we sought to better understand the neuroprotective action of lithium by verifying the purinergic participation. Our results primarily showed a decrease in adenosine levels in patients compared to controls and a significant inverse correlation between adenosine levels and worsening of symptoms. We also demonstrated that patients with bipolar disorder present a decrease in the frequency of the 1513C allele and the potential increase in the frequency of genotype 1513AA and 1513AA/AC in relation to controls, setting a worse prognosis of the most active form of the receptor. We also showed that P2X7R plays an important role in the establishment of the mania model, modulating mainly the dopaminergic pathway that is reflected in the behavioral change observed by the model. Finally, lithium was able to prevent the insult response in neuronal cells (PC-12 lineage) and was not capable of preventing P2X7R-mediated activation in microglial cells (N9 lineage), indicating a differentiated cellular response. We can conclude that in a translational way we could demonstrate the participation of the purinergic system in the pathophysiology of bipolar disorder. We also suggested a possible biomarker for both diagnosis and progression of the disorder, the serum adenosine levels, and a potential genetic marker for prediction and diagnosis, the polymorphism of P2X7R 1513A>C. Lastly we contributed to the understanding of lithium action mechanism and defined a potential new therapeutic target for the disorder, the P2X7 receptor.
