The aim was to develop sedentary (sitting/lying) thresholds from hip and wrist worn raw tri‐axial acceleration data from the ActiGraph and GENEActiv, and to examine the agreement between free‐living ...time spent below these thresholds with sedentary time estimated by the activPAL. Sixty children and adults wore an ActiGraph and GENEActiv on the hip and wrist while performing six structured activities, before wearing the monitors, in addition to an activPAL, for 24 h. Receiver operating characteristic (ROC) curves were used to determine sedentary thresholds based on activities in the laboratory. Agreement between developed sedentary thresholds during free‐living and activPAL were assessed by Bland‐Altman plots and by calculating sensitivity and specificity. Using laboratory data and ROC‐curves showed similar classification accuracy for wrist and hip thresholds (Area under the curve = 0.84–0.92). Greatest sensitivity (97–98%) and specificity (74–78%) were observed for the wrist thresholds, with no large differences between brands. During free‐living, Bland‐Altman plots showed large mean individual biases and 95% limits of agreement compared with activPAL, with smallest difference for the ActiGraph wrist threshold in children (+30 min, P = 0.3). Sensitivity and specificity for the developed thresholds during free‐living were low for both age groups and for wrist (Sensitivity, 68–88%, Specificity, 46–59%) and hip placements (Sensitivity, 89–97%, Specificity, 26–34%). Laboratory derived sedentary thresholds generally overestimate free‐living sedentary time compared with activPAL. Wrist thresholds appear to perform better than hip thresholds for estimating free‐living sedentary time in children and adults relative to activPAL, however, specificity for all the developed thresholds are low.

No rock at the Earth's surface escapes weathering. This process is the primary source of all the essential elements for organisms, except nitrogen and carbon. Since the onset of terrestrial life, ...weathering has been accelerated under the influence of biota. The study of biological weathering started at the end of the 19th century. Although the role of bacteria (Eubacteria, Archaea) has attracted a lot of interest, until recently the role of fungi has largely been neglected. More recently, however, fungal weathering has become an increasingly important focus of biogeochemical research.
In this review we first outline the basics of transport theory and its recent generalization to off-shell transport. We then present in some detail the main ingredients of any transport method using ...in particular the Giessen Boltzmann–Uehling–Uhlenbeck (GiBUU) implementation of this theory as an example. We discuss the potentials used, the ground state initialization and the collision term, including the in-medium modifications of the latter. The central part of this review covers applications of GiBUU to a wide class of reactions, starting from pion-induced reactions over proton and antiproton reactions on nuclei to heavy-ion collisions (up to about 30AGeV). A major part concerns also the description of photon-, electron- and neutrino-induced reactions (in the energy range from a few 100MeV to a few 100GeV). For this wide class of reactions GiBUU gives an excellent description with the same physics input and the same code being used. We argue that GiBUU is an indispensable tool for any investigation of nuclear reactions in which final-state interactions play a role. Studies of pion–nucleus interactions, nuclear fragmentation, heavy-ion reactions, hypernucleus formation, hadronization, color transparency, electron–nucleus collisions and neutrino–nucleus interactions are all possible applications of GiBUU and are discussed in this article.
The clinical significance of diaphragm weakness in critically ill patients is evident: it prolongs ventilator dependency, and increases morbidity and duration of hospital stay. To date, the nature of ...diaphragm weakness and its underlying pathophysiologic mechanisms are poorly understood.
We hypothesized that diaphragm muscle fibers of mechanically ventilated critically ill patients display atrophy and contractile weakness, and that the ubiquitin-proteasome pathway is activated in the diaphragm.
We obtained diaphragm muscle biopsies from 22 critically ill patients who received mechanical ventilation before surgery and compared these with biopsies obtained from patients during thoracic surgery for resection of a suspected early lung malignancy (control subjects). In a proof-of-concept study in a muscle-specific ring finger protein-1 (MuRF-1) knockout mouse model, we evaluated the role of the ubiquitin-proteasome pathway in the development of contractile weakness during mechanical ventilation.
Both slow- and fast-twitch diaphragm muscle fibers of critically ill patients had approximately 25% smaller cross-sectional area, and had contractile force reduced by half or more. Markers of the ubiquitin-proteasome pathway were significantly up-regulated in the diaphragm of critically ill patients. Finally, MuRF-1 knockout mice were protected against the development of diaphragm contractile weakness during mechanical ventilation.
These findings show that diaphragm muscle fibers of critically ill patients display atrophy and severe contractile weakness, and in the diaphragm of critically ill patients the ubiquitin-proteasome pathway is activated. This study provides rationale for the development of treatment strategies that target the contractility of diaphragm fibers to facilitate weaning.
Being a morning person is a behavioural indicator of a person's underlying circadian rhythm. Using genome-wide data from 697,828 UK Biobank and 23andMe participants we increase the number of genetic ...loci associated with being a morning person from 24 to 351. Using data from 85,760 individuals with activity-monitor derived measures of sleep timing we find that the chronotype loci associate with sleep timing: the mean sleep timing of the 5% of individuals carrying the most morningness alleles is 25 min earlier than the 5% carrying the fewest. The loci are enriched for genes involved in circadian regulation, cAMP, glutamate and insulin signalling pathways, and those expressed in the retina, hindbrain, hypothalamus, and pituitary. Using Mendelian Randomisation, we show that being a morning person is causally associated with better mental health but does not affect BMI or risk of Type 2 diabetes. This study offers insights into circadian biology and its links to disease in humans.

Mechanical ventilation reduced myosin content by approximately 50% in diaphragms of wild-type mice (P less than 0.05). In contrast, ventilation of Toll-like receptor 4 deficient mice did not significantly affect diaphragm myosin content. Likewise, mechanical ventilation significantly increased interleukin-6 and keratinocyte-derived chemokine in the diaphragm of wild-type mice, but not in ventilated Toll-like receptor 4 deficient mice. Mechanical ventilation increased diaphragmatic muscle atrophy factor box transcription in both wild-type and Toll-like receptor 4 deficient mice. Other components of the ubiquitin-proteasome pathway and caspase-3 activity were not affected by ventilation of either wild-type mice or Toll-like receptor 4 deficient mice. Mechanical ventilation induced autophagy in diaphragms of ventilated wild-type mice, but not Toll-like receptor 4 deficient mice.
Toll-like receptor 4 signaling plays an important role in the development of ventilator-induced diaphragm atrophy, most likely through increased expression of cytokines and activation of lysosomal autophagy.

a retrospectively analyzed cross-sectional study was conducted in 687 patients with asthma (60% female, 58±13 years, FEV1 76±25%pred) referred for comprehensive pulmonary rehabilitation (PR). Body composition, pulmonary function, exercise capacity, quadriceps muscle function, and quality of life were assessed. Patients were classified as presenting low ALMI according to the 10th percentiles of age-sex-body mass index (BMI)-specific reference values and as having SO according to the diagnostic procedure proposed by the 2022 ESPEN/EASO consensus. In addition, clinical outcomes between patients with normal and low ALMI or with and without SO were compared.
the frequency of patients classified as low ALMI was 19%, whereas 45% of the patients were obese. Among the obese patients, 29% had SO. In patients with normal weight, those with low ALMI were younger and had worse pulmonary function, exercise capacity and quadriceps muscle function than those with normal ALMI (all p<0.05). Overweight patients with low ALMI presented poorer pulmonary function and quadriceps muscle function (both strength and total work capacity). In obese class I patients, those with low ALMI showed lower quadriceps strength and maximal oxygen uptake acquired during cardiopulmonary exercise testing. Both male and female patients with SO showed lower quadriceps muscle function and reduced maximal exercise capacity compared to non-SO asthma patients.
Approximately one in five asthma patients presented low ALM when age-sex-BMI-specific ALMI cut-offs were applied. Obesity is common among patients with asthma referred for PR. Among the obese patients a significant proportion presented SO. Low ASM and SO were associated with worse functional outcomes.

Stopping nucleos(t)ide analogues (NA) after hepatitis B e antigen (HBeAg) seroconversion is associated with high relapse rates in Asian patients, but data in Caucasian cohorts are ...scarce. Clinical course, outcomes and immunological aspects of chronic hepatitis B infections differ substantially between distinct ethnicities.
Aim
The aim of this study was to determine relapse rates, factors predicting relapse and clinical outcomes after nucleos(t)ide analogue cessation in a large, predominantly Caucasian cohort of chronic hepatitis B patients with nucleos(t)ide analogue‐induced HBeAg seroconversion.
Methods
This is a nationwide observational cohort study including HBeAg positive, mono‐infected chronic hepatitis B patients with nucleos(t)ide analogue‐induced HBeAg seroconversion from 18 centres in Belgium.
Results
A total of 98 patients with nucleo(s)tide analogue‐induced HBeAg seroconversion were included in the study. Of the 62 patients who stopped treatment after a median consolidation treatment of 8 months, 30 relapsed. Higher gamma‐glutamyl transferase levels at both treatment initiation (HR 1.004; P = 0.001 per unit increment) and HBeAg seroconversion (HR 1.006; P = 0.013 per unit increment) were associated with an increased risk of clinically significant relapse in a multivariate Cox regression model. Treatment cessation led to liver‐related death in 2 patients, of whom one showed a severe flare. Of the patients who continued treatment after HBeAg seroconversion, none relapsed or developed severe hepatic outcomes.
Conclusion
Treatment withdrawal in Caucasian chronic hepatitis B patients after nucleos(t)ide analogue‐induced HBeAg seroconversion results in viral relapses in more than half of patients with potential fatal outcomes. These real‐world data further lend support to preferentially continue NA treatment after HBeAg seroconversion until HBsAg loss.

The endurance shuttle walk test (ESWT) is used to evaluate exercise tolerance in patients with chronic obstructive pulmonary disease (COPD). The recommended pre-intervention tolerated duration (Tlim) ...is between 3-8 minutes for optimal interpretation of treatment effects. However, this window may be exceeded and factors determining ESWT Tlim are not completely understood. Therefore, we aimed to determine whether pulmonary function, physical and incremental shuttle walk test (ISWT) performance measures are associated with ESWT Tlim in COPD patients.
Assessment data from patients eligible for pulmonary rehabilitation was retrospectively analyzed. Inclusion criteria were: diagnosis of COPD and complete data availability regarding ESWT and ISWT. Patients performed an ESWT at 85% of ISWT speed and were divided into three groups (ESWT Tlim: <3 minutes, 3-8 minutes, >8 minutes). Subject characteristics, severity of complaints, pulmonary function, physical capacity and activity, exercise tolerance and quadriceps muscle strength were evaluated.
245 COPD patients (FEV1 38 (29-52)% predicted) were included. Median ESWT Tlim was 6.0 (3.7-10.3) minutes, 41 (17%) patients walked <3 minutes and 80 (33%) patients walked >8 minutes. Body mass index, maximal oxygen consumption, Tlim on constant work rate cycle test, physical activity level, maximal ISWT speed, dyspnoea Borg score at rest and increase of leg fatigue Borg score during ISWT independently predicted Tlim in multivariate regression analysis (R2 = 0.297, p<0.001).
This study reported a large variability in ESWT Tlim in COPD patients. Secondly, these results demonstrated that next to maximal ISWT speed, other ISWT performance measures as well as clinical measures of pulmonary function, physical capacity and physical activity were independent determinants of ESWT Tlim. Nevertheless, as these determinants only explained ~30% of the variability, future studies are needed to establish whether additional factors can be used to better adjust individual ESWT pace in order to reduce ESWT Tlim variability.
Respiratory muscle dysfunction may develop rapidly in critically ill ventilated patients and is associated with increased morbidity, length of intensive care unit stay, costs, and mortality. This ...review briefly discusses the pathophysiology of respiratory muscle dysfunction in intensive care unit patients and then focuses on strategies that prevent the development of muscle weakness or, if weakness has developed, how respiratory muscle function may be improved. We propose a simple strategy for how these can be implemented in clinical care.
