A six-year-old, female, spayed Labrador Retriever was evaluated for progressive lameness of one year duration, ending in non-weight-bearing of the right hindlimb. The dog had a history of severe ...coxarthrosis of both hip joints, and had a HELICA hip prosthesis implanted in the right hip 18 months before. On survey radiographs, the acetabular and femoral components appeared unstable, with a large void in the proximal femur and a lacy periosteal reaction on the trochanter. Arthrocentesis was performed to rule out septic loosening. As culture samples were negative, the dog underwent surgery. We report the successful revision of an unstable HELICA screw hip prosthesis with a Zurich cementless total hip replacement. The patient had a good clinical and radiological outcome seven months postoperatively.
Details of the in vitro fertilization and embryo transfer programme at the Department of Obstetrics and Gynaecology of the H. F. Verwoerd Hospital and the University of Pretoria are presented. From 1 ...March 1982 to 30 November 1983, 67 cycles in 47 patients were induced using clomiphene citrate and human chorionic gonadotrophin (hCG) (46 patients) and human postmenopausal gonadotrophin/hCG (1 patient). In 5 patients ovum recovery was unsuccessful owing to severe adhesions. In the remaining 42 patients 59 oocyte recoveries out of a total of 62 attempts were successful, a 95% recovery rate. One hundred and seventy-nine ova were recovered (2,9 per laparoscopy), of which 143 (80%) were viable. Forty-six per cent of fertilized ova cleaved actively. Forty-one transfers of 66 embryos (average 1,6 embryos per transfer) resulted in 5 pregnancies (12% pregnancy rate per transfer). Three preclinical pregnancies aborted spontaneously. Two pregnancies are ongoing at 30 and 32 weeks' gestation. Thus, the programme has so far led to 2 third-trimester pregnancies. Consolidation and refinement of techniques should lead to a steady increase in success at this centre.
Background A pragmatic three-tiered approach to monitor the world's largest antiretroviral treatment (ART) programme was adopted by the South African National Department of Health in 2010. With the ...rapid expansion of the programme, the limitations of the paper-based register (tier 1) were the catalyst for implementation of the stand-alone electronic register (tier 2), which offers simple digitisation of the paper-based register. This article engages with theory on implementation to identify and contextualise enabling and constraining factors for implementation of the electronic register, to describe experiences and use of the register, and to make recommendations for implementation in similar settings where standardisation of ART monitoring and evaluation has not been achieved. Methods We conducted a qualitative evaluation of the roll-out of the register. This comprised twenty in-depth interviews with a diverse sample of stakeholders at facility, sub-district, and district levels of the health system. Facility-level participants were selected across five sub-districts, including one facility per sub-district. Responses were coded and analysed using a thematic approach. An implementation science framework guided interpretation of the data. Results & Discussion We identified the following seven themes: 1) ease of implementation, 2) perceived value of an electronic M&E system, 3) importance of stakeholder engagement, 4) influence of a data champion, 5) operational and logistical factors, 6) workload and role clarity, and 7) importance of integrating the electronic register with routine facility monitoring and evaluation. Interpreting our findings through an implementation theory enabled us to construct the scaffolding for implementation across the five facility-settings. This approach illustrated that implementation was not a linear process but occurred at two nodes: at the adoption of the register for roll-out, and at implementation at facility-level. Conclusion In this study we found that relative advantage of an intervention and stakeholder engagement are critical to implementation. We suggest that without these aspects of implementation, formative and summative outcomes of implementation at both the adoption and coalface stages of implementation would be negatively affected.
Objective— To compare the efficacy of 0.3% stabilized glutaraldehyde and alcohol (SG+A), 0.3% SG and water (SG+W), and 4% chlorhexidine gluconate tincture (CG+A), as skin disinfectants in dogs ...undergoing ovariohysterectomy.
Study Design— Prospective, blinded clinical study.
Animals— One hundred and twenty‐one dogs.
Methods— Cutaneous bacterial colony forming units (CFU) from the perioperative site after skin preparation, after antisepsis, and after surgery (incisional and paramedian), were quantified. The influence of high initial bacterial counts (≥150 CFU) and surgical time on antibacterial efficacy was examined and the proportion of dogs from which Staphylococcus intermedius was cultured, determined. Perioperative skin reactions and wound infections were documented.
Results— All 3 antiseptic solutions significantly and equally reduced CFU to all post‐antisepsis sampling levels irrespective of surgical duration (mean surgical times 151.6, 136.2, and 149.6 minutes for CG+A, SG+A and SG+W, respectively). Median percentage reductions in CFU ranged between 99.3% and 100%. In dogs with initial high counts and disinfected with CG+A and SG+W, the incisional samples had significantly higher counts than the post‐antisepsis samples. In the CG+A and SG+W groups, the proportion of post‐surgery samples yielding S. intermedius was significantly higher at the incisional than the paramedian sites. Eight mild cutaneous reactions were recorded in equal proportions for the 3 solutions. There were no recorded infections.
Conclusions— All 3 preparations had an equal ability to reduce and maintain low CFU counts, with minimal cutaneous reactions.
Clinical Relevance— SG solutions are safe and effective preoperative skin antiseptics for elective clean‐contaminated surgical procedures.
Objectives: To test a non‐glycosylated recombinant human bone morphogenetic protein‐2 (ngly‐rhBMP‐2)/fibrin composite, which has been shown experimentally to enhance healing of bone defects in ...rodents, in a clinical case series of dogs and cats undergoing treatment for fracture non‐unions and arthrodesis. Methods: A ngly‐rhBMP‐2/fibrin composite was applied in 41 sites in 38 dogs and cats for which a cancellous bone autograft was indicated, replacing the graft. Results: Bridging of the bone defect with functional bone healing was achieved in 90 per cent of the arthrodesis and fracture nonunions treated in this manner. Clinical Significance: This prospective clinical study demonstrates the beneficial effects of ngly‐rhBMP‐2 in a specially designed fibrin matrix on the treatment of bone defects, and validates the use of this composite as an alternative to bone autografts in dogs and cats.

Dieser Sammelband diskutiert die Ursachen und Wirkungendes italienischen Intervento von 1915 in neuer Sicht und leistet damit einen Beitrag zum wissenschaftlichen Dialog zwischen den ehemaligen Kriegsgegnern, der noch längst nicht beendet ist.
The Sculptor Galaxy (NGC 253), located in the Southern Hemisphere, far off the Galactic Plane, has a relatively high star-formation rate of about 7 M\(_{\odot}\) yr\(^{-1}\) and hosts a young and ...bright stellar population, including several super star clusters and supernova remnants. It is also the first galaxy, apart from the Milky Way Galaxy to be associated with two giant magnetar flares. As such, it is a potential host of pulsars and/or fast radio bursts in the nearby Universe. The instantaneous sensitivity and multibeam sky coverage offered by MeerKAT therefore make it a favourable target. We searched for pulsars, radio-emitting magnetars, and fast radio bursts in NGC 253 as part of the TRAPUM large survey project with MeerKAT. We did not find any pulsars during a four-hour observation, and derive a flux density limit of 4.4 \(\mu\)Jy at 1400 MHz, limiting the pseudo-luminosity of the brightest putative pulsar in this galaxy to 54 Jy kpc\(^2\). Assuming universality of pulsar populations between galaxies, we estimate that detecting a pulsar as bright as this limit requires NGC 253 to contain a pulsar population of \(\gtrsim\)20 000. We also did not detect any single pulses and our single pulse search flux density limit is 62 mJy at 1284\,MHz. Our search is sensitive enough to have detected any fast radio bursts and radio emission similar to the brighter pulses seen from the magnetar SGR J1935+2154 if they had occurred during our observation.

A retrospective clinical and radiographic study was conducted and the post-treatment gait was analysed using a force plate. Ground reaction force (GRF) data were compared with data derived from 24 healthy Labrador Retrievers (control group).
Six dogs had an excellent or good clinical result. Significant differences were found between the patients and the control group for many GRF data. Propulsive and vertical forces significantly decreased in the operated limb and increased in the contralateral pelvic and ipsilateral or both thoracic limbs. The lameness was significantly correlated to the OS postoperatively, the range-of-motion, the PSI, the length of the follow-up period, and the vertical ground force. No hyperextension in stance was recorded.
Although most dogs improved, the prognosis remains guarded. This seems to be independent of age, bilateral involvement, or preoperative OS in these dogs. No relation between medial talar OCD and tarsal hyperextension in stance was found in these dogs.
