The 2018 conservation status assessment of freshwater invertebrate taxa was the most comprehensive to date. Of the Threatened and At Risk taxa in 2018 (177 taxa), 48 were listed as Nationally ...Critical, 14 as Nationally Endangered, 16 as Nationally Vulnerable, 10 as Declining, and 89 as Naturally Uncommon. Most taxa were listed as either Not Threatened (315 taxa) or required more information for assessment (Data Deficient; 178 taxa). Analyses of temporal changes in conservation status since 2002 revealed that the majority of changes were due to greater understanding of taxa distributions, rather than gains or losses in population sizes or area of occupancy. Insufficient data on taxa distributions, along with a lack of autecological information, are two of the major knowledge gaps facing freshwater invertebrate conservation in New Zealand. Further impediments to conservation include (i) a lack of systematics research, (ii) a large number of species remain undiscovered and undescribed, and (iii) the status and trend of populations is unknown for most taxa. We also provide an analysis of the habitats that harbour many of the rarer taxa. Finally, we conclude by recommending a number of actions that should, collectively, help fill these knowledge gaps and address these issues.
Abstract From 2001 to 2005, 66 patients referred for perioperative hyperbaric oxygen therapy (HBO2 ) for debridement of necrotic tissue or prevention of radionecrosis were assessed with quality of ...life measures, before and after completion of HBO2 and surgery. The Medical Outcomes Short Form 36 (SF-36) and Hospital Anxiety and Depression Scale (HADS) showed no significant changes. The European Organisation for Research and Treatment of Cancer Core (EORTC-C30) questionnaire showed significant improvement in pain, global health, and dyspnoea ( p = 0.011; p = 0.027; p = 0.008, respectively). The Head and Neck sub-module (H&N35) identified significant improvements in teeth, dry mouth and social contact ( p = 0.002; p = 0.038; p = 0.029, respectively). The University of Washington Scale (UW), showed significant changes in relation to chewing and shoulders ( p = 0.031; p = 0.047). When sub-group analysis using ‘osteoradionecrosis’ and ‘dental extraction or implants’ was performed on the EORTC and UW data, variations in the patterns of significance were found. Adjunctive HBO2 should be considered for the treatment and prevention of some of the long-term complications of radiotherapy.
Occupancy of the T-cell antigen receptor is insufficient to induce T-cell activation optimally; a second co-stimulatory signal is required. Exposure of T-cell clones to complexes of antigen with ...major histocompatibility complex molecules in the absence of the co-stimulatory signal induces a state of clonal anergy. This requirement for two stimuli for T-cell activation could have an important role in vivo in establishing peripheral tolerance to antigens not encountered in the thymus. The receptor on T cells required for the co-stimulatory stimulus involved in the prevention of anergy has not been identified. The human T-cell antigen CD28 provides a signal that can synergize with T-cell antigen receptor stimulation in activating T cells to proliferate and secrete lymphokines. Here we report that a monoclonal antibody against the murine homologue of CD28 (ref. 7; J.A.G. et al., manuscript in preparation) can provide a co-stimulatory signal to naive CD4+ T cells and to T-cell clones. Moreover, we demonstrate that this co-stimulatory signal can block the induction of anergy in T-cell clones.
The use and variety of stemless humeral components in anatomical total shoulder arthroplasty (TSA) have proliferated since their advent in 2004. Early outcomes are reassuring but independent mid-term ...results are scarce. This independent study reports a consecutive series of 143 Eclipse stemless shoulder prostheses with a minimum five-year (5 to 10) follow-up.
Outcomes of 143 procedures undertaken for all indications in 131 patients were reviewed, with subset analysis of those for osteoarthritis (OA) (n = 99). The primary outcome was the Oxford Shoulder Score (OSS) at a minimum of five years. Secondary outcomes were ranges of motion and radiological analysis of humeral radiolucency, rotator cuff failure, and glenoid loosening.
Mean OSS at mean follow-up of 6.67 years (5.0 to 10.74) was 40.12 (9 to 48), with no statistically significant difference between those implanted for a non-OA indication and those for OA (p = 0.056) or time-dependent deterioration between two years and five years (p = 0.206). Ranges of motion significantly improved compared with preoperative findings and were maintained between two and five years with a mean external rotation of 38° (SD 18.1, 0 to 100) and forward elevation of 152° (SD 29.9, 90 to 180). Of those components with radiographs suitable for analysis (n = 83), 23 (28%) were found to have a least one humeral radiolucent line, which were predominantly incomplete, less than 2 mm, and in a single anatomical zone. No humeral components were loose. A radiolucent line was present around 22 (15%) of glenoid components, and 15 (10%) of components had failed. Rotator cuff failure was found in 21 (15%) components. The mean time to either glenoid or rotator cuff failure was greater than three years following implantation. Survivorship was 96.4% (95% CI 91.6 to 98.5, number at risk 128) at five years, and 94.3% (95% CI 88.2 to 97.3, number at risk 76) at seven years, both of which compare favourably with best results taken from available registries.
Functional and radiological outcomes of the Eclipse stemless TSA are excellent, with no loose humeral components at minimum five-year follow-up. The presence of radiolucent lines is of interest and requires long-term observation but does not impact on the clinical results. Of the eight revisions required, this was predominantly for glenoid and rotator cuff failure. Cite this article:

Circulatory disease mortality inequalities by country of birth (COB) have been demonstrated for some EU countries but pan-European analyses are lacking. We examine inequalities in circulatory ...mortality by geographical region/COB for six EU countries.
We obtained national death and population data from Denmark, England and Wales, France, the Netherlands, Scotland and Sweden. Mortality rate ratios (MRRs) were constructed to examine differences in circulatory, ischaemic heart disease (IHD) and cerebrovascular disease mortality by geographical region/COB in 35-74 years old men and women.
South Asians in Denmark, England and Wales and France experienced excess circulatory disease mortality (MRRs 1.37-1.91). Similar results were seen for Eastern Europeans in these countries as well as in Sweden (MRRs 1.05-1.51), for those of Middle Eastern origin in Denmark (MRR = 1.49) and France (MRR = 1.15), and for East and West sub-Saharan Africans in England and Wales (MRRs 1.28 and 1.39) and France (MRRs 1.24 and 1.22). Low ratios were observed for East Asians in France, Scotland and Sweden (MRRs 0.64-0.50). Sex-specific analyses showed results of similar direction but different effect sizes. The pattern for IHD mortality was similar to that for circulatory disease mortality. Two- to three-fold excess cerebrovascular disease mortality was found for several foreign-born groups compared with the local-born populations in some countries.
Circulatory disease mortality varies by geographical region/COB within six EU countries. Excess mortality was observed for some migrant populations, less for others. Reliable pan-European data are needed for monitoring and understanding mortality inequalities in Europe's multiethnic populations.

To assess the effectiveness of scoliosis-specific exercises (SSE) on adolescent idiopathic scoliosis (AIS) compared with other non-surgical interventions.
AIS is ...a complex deformity of the spine that develops between the age of 10years and skeletal maturity. SSE are prescribed to patients to reduce or slow curve progression, although their effectiveness is unknown.
Electronic databases were searched for relevant studies. Randomised controlled trials were eligible if they compared SSE with non-surgical interventions for individuals with AIS. Three authors independently extracted data, evaluated methodological quality and assessed the quality of evidence. Meta-analysis was performed where possible; otherwise, descriptive syntheses are reported.
Nine randomised controlled trials were included. Four had a high risk of bias, three had an unclear risk and two had a low risk. Very-low-quality evidence indicated that SSE improved some measures of spinal deformity, function, pain and overall health-related quality of life (HRQoL). Very-low-quality evidence suggested that SSE had no effect on self-image and mental health. Very-low-quality evidence showed that bracing was more effective than SSE on measures of spinal deformity. However, SSE showed greater improvements in function, HRQoL, self-image, mental health and patient satisfaction with treatment. No differences were found for pain or trunk rotation.
SSE may be effective for improving measures of spinal deformity for people with AIS, but the evidence is of very low quality. Future studies should evaluate relevant clinical measures and cost-effectiveness using rigorous methods and reporting standards.
Ultrasound (US) imaging is a well-recognized technique for the study of static tissues but its suitability for studying tissue dynamics depends upon accurate frame time information, which may not ...always be available to users. Here we present methods to quantify the inter-frame interval (IFI) variability, and evaluate different procedures for collecting temporal information from two US-imaging devices. The devices tested exhibited variable IFIs that could only be confirmed by direct measures of timing signals, available by means of electrical signals (triggers) and/or temporal information contained in the software used for the US data collection. Interpolating frame-by-frame measures of dynamic changes within image sequences using individual IFI values provided improved synchronization between measures of skeletal muscle movement and activation; validating US as a valuable technique for the study of musculoskeletal tissue dynamics, when correctly implemented.
Degradation of the land surface has been suggested as a cause of persistent drought in tropical north Africa. A general circulation model is used to assess the impact of degradation of five regions ...within tropical north Africa. Idealized degradation scenarios are used since existing observations are inadequate to determine the extent and severity of historical degradation. It is found that the impact of degradation varies between the regions. The greatest effects are found from degradation of the Sahel or West Africa, which result in substantial reduction of precipitation over the degraded area. Both surface evaporation and atmospheric moisture convergence are reduced. In the Sahelian case the precipitation reduction extends well to the south of the area of changed land surface. The occurrence of easterly wave disturbances is not altered by degradation, but the mean rainfall from each event is reduced. Degradation of an area in eastern north Africa results in smaller reductions of precipitation and moisture convergence. Finally, degradation of a southern area next to the Gulf of Guinea has little effect on precipitation because of a compensatory increase of moisture convergence. The simulated rainfall reduction following degradation of the Sahel is comparable to observed changes in recent decades, suggesting that degradation may have contributed to that change.
Using an animal model of drug relapse, we found that intermittent footshock stress reinstates alcohol seeking, an effect attenuated by the 5-HT reuptake blocker fluoxetine and by ...corticotropin-releasing factor (CRF) receptor antagonists. Here we studied the role of the 5-HT cell body region of the median raphe nucleus (MRN) and CRF receptors in this site in reinstatement of alcohol seeking. Rats were given alcohol in a two-bottle choice procedure (water vs alcohol) for 25 d and were then trained for 1 hr/d to press a lever for alcohol (12% w/v) for 23-30 d. Subsequently, lever pressing for alcohol was extinguished by terminating drug delivery for 5-9 d. Tests for reinstatement of alcohol seeking were then performed under extinction conditions. Intra-MRN infusions of 8-OH-DPAT 8-hydroxy-2-(di-n-propylamino)tetralin (a 5-HT1A agonist that decreases 5-HT cell firing and release) reinstated alcohol seeking. Reinstatement of alcohol seeking also was observed after intra-MRN infusions of low doses of CRF (3-10 ng), which mimicked the effect of ventricular infusions of higher doses of the peptide (300-1000 ng). Finally, intra-MRN infusions of the CRF receptor antagonist d-Phe CRF (50 ng) blocked the effect of intermittent footshock (10 min) on reinstatement. These data suggest that an interaction between CRF and 5-HT neurons within the MRN is involved in footshock stress-induced reinstatement of alcohol seeking.
The constitutive responses of three glassy thermoset polymers at impact rates of strain and slower, together with measurements of adiabatic heating, were reported earlier by the authors. The results ...are interpreted here in the context of a constitutive model proposed previously for amorphous polymers, expanded to incorporate strain-softening and the adiabatic heating deficit. In terms of the model, both features are a natural consequence of strain-induced evolution of the glass structure, as represented by Tool's “fictive temperature”—the phenomenon of structural rejuvenation. A representation is proposed for the evolution of fictive temperature with plastic strain, motivated by an approximate treatment of the kinetics of physical ageing/rejuvenation. Formulated in this manner, the model agrees reasonably well with experimental results across the wide range of strain rates of the previous experiments, 10

, and across most of the range of strain to failure in compression. At the highest strains, however, an additional adiabatic heating deficit appears that is not predicted by the model, either suggesting the onset of structural breakdown possibly associated with the appearance of cracks or reflecting a need for better physical understanding of large deformations in glassy polymers.
