Raw milk Gouda and Gouda-like cheese has been implicated in three previous Shiga toxin-producing
outbreaks in North America. It was recommended product labelling to increase consumer awareness and thermization of milk to decrease the risk of illness associated with raw milk Gouda and Gouda-like cheese.
Background: The present study had two research questions. First, what is the average waiting time between diagnosis and treatment for Alberta women with breast cancer relative to Canadian Society for ...Surgical Oncology (CSSO) recommendations? Second, does patient age, cancer stage, patient community size, and year of diagnosis have a significant relationship to waiting time? Methods: The sample consisted of all Alberta women diagnosed with breast cancer between 1997 and 2000. Waiting time was defined as number of days between definitive diagnosis and treatment initiation. Multiple regression examined the relative influence of the predictor variables on waiting time. Results: There were 6,418 cases of breast cancer between 1997 and 2000. Mean waiting time was 20.2 days (SD 21.6) and median waiting time was 17 days. Longer waiting time was significantly associated with year of diagnosis (progressively longer from 1997 to 2000), patients younger than 70, and Stage 1 cancer. Waiting time increase from 1997 to 2000 appears to be due to increased demand for services without corresponding increases in resources. Less treatment delay for women older than 70 is due to more of these women being treated the same day they received their diagnosis. Conclusion: Only 44% of women had a waiting time of 14 days or less as recommended by the CSSO. The number of women who will have to wait longer than recommended for treatment will likely increase without a significant increase in oncological resources. The basis for differences in waiting times as a function of age needs to be further investigated to ensure equitable access to care. Méthode : L'échantillon se composait de toutes les Albertaines ayant reçu un diagnostic de cancer du sein entre 1997 et 2000. Le délai d'attente a été défini comme le nombre de jours entre l'évaluation définitive et le début du traitement. Par analyse de régression multiple, nous avons examiné l'influence relative des prédicteurs du délai d'attente. Résultats : Il y a eu 6 418 cas de cancer du sein entre 1997 et 2000. Le délai d'attente moyen entre le diagnostic et le traitement était de 20,2 jours (écart-type de 21,6), et le délai d'attente médian, de 17 jours. Une attente prolongée était associée de façon significative à l'année du diagnostic (le délai s'est allongé progressivement entre 1997 et 2000), aux patientes de moins de 70 ans, et au premier stade du cancer. L'augmentation du délai d'attente entre 1997 et 2000 semble s'expliquer par la demande accrue de services sans augmentation correspondante des ressources. Le traitement plus rapide des femmes de plus de 70 ans s'explique par le fait que davantage de ces femmes ont été traitées le jour même de leur diagnostic. Conclusion : Seulement 44 % des femmes ont attendu 14 jours ou moins comme le recommande la Société canadienne pour l'oncologie chirurgicale. Ces délais d'attente élevés continueront sans doute à augmenter sans hausse significative des ressources en oncologie. Pour assurer un accès équitable aux soins, il faudrait étudier plus avant la raison des écarts dus à l'âge dans les délais d'attente.
Two outbreaks of norovirus and acute gastroenteritis took place in Canada between November 2016 and April 2017. Both outbreaks were linked to oysters from British Columbia (BC) coastal waters. This ...paper describes the multi-agency investigations to identify the source and control the outbreak. Public health officials conducted interviews to determine case exposures. Traceback was conducted by collecting oyster tags from restaurants and analyzing them to determine the most common farms. Oyster samples were collected from case homes, restaurants, and harvest sites and tested for the presence of norovirus. Potential environmental pollution sources were investigated to identify the source of the outbreak. Four hundred and 49 cases were identified as part of the two outbreak waves. The oysters were traced to various geographically dispersed farms in BC coastal waters. Twelve farms were closed as a result of the investigations. No environmental pollution sources could be identified as the cause of the outbreak. Similarities in the timeframe, genotype, and geographic distribution of identified oyster farms indicate that they may have been one continuous event. Genotype data indicate that human sewage contamination was the likely cause of the outbreak, although no pollution source was identified.

Ils ont ensuite été interrogés en fonction de l’exposition d’intérêt, puis analysés en fonction des valeurs du Rapport Foodbook pour la Colombie-Britannique. Une investigation a été effectuée dans l’usine laitière et des prélèvements ont été effectués dans les produits de fromage afin de déterminer la source de contamination. Le typage de séquence multilocus pour le génome entier a été effectué sur tous les isolats cliniques et alimentaires positifs pour E. coli O121 au laboratoire provincial. Résultats Quatre des sept cas avaient consommé le même fromage au lait cru de type Gouda entre août et octobre 2018. Ce fromage avait été affiné pendant une période supérieure au minimum requis de 60 jours, et aucun défaut n’a été constaté dans sa production. Un échantillon du fromage visé a cependant obtenu un résultat positif à l’analyse de dépistage de la bactérie E. coli O121. Sept isolats cliniques et un isolat de fromage ont été jugés identiques par typage de séquence multilocus pour le génome entier avec une différence de 0 à 6,5 allèles. Conclusion Le Gouda au lait cru et les fromages de type Gouda au lait cru avaient déjà été impliqués dans trois éclosions antérieures d’E. coli producteur de toxines Shiga en Amérique du Nord. This study explores ways in which healthcare organizations can improve their organizational fitness for change using Beer and Nohria's framework of Theory E (concentrating on the economic value of ...change) and Theory O (concentrating on the organization's long-term capabilities for change). Data were collected from senior leaders/medical directors from health regions in Alberta. The results show that even though there is a tendency for reliance on Theory E change strategies, the respondents demonstrated other preferred approaches to change.

A semistructured dialogue guide was used by a trained facilitator to explore participants' experiences and views related to immunization delivery in British Columbia. Verbatim transcriptions were independently coded by 2 researchers. Key themes were analyzed and identified in an iterative manner using interpretive description.
Physicians highly valued vaccine delivery. Factors facilitating physician-delivered immunizations included strong beliefs in the value of vaccines and having adequate information. Identified barriers included the large time commitment and insufficient communication about program changes, new vaccines, and the adult immunization program in general. Some physicians reported good relationships with local public health, while others reported the opposite experience, and this varied by geographic location.
These findings suggest that physicians are supportive of delivering vaccines. However, there are opportunities to improve the sustainability of physician-delivered immunizations. While compensation schemes remain under the purview of the provincial governments, local public health authorities can address the information needs of physicians.
Pertussis is increasingly being recognized as an important cause of cough illness in adolescents and adults. To evaluate the safety and immunogenicity of an adult formulation of a five-component ...(pertussis toxoid, filamentous hemagglutinin, pertactin, fimbriae 2 and 3) acellular pertussis vaccine combined with diphtheria and tetanus toxoids, we randomly allocated 749 healthy adolescents and adults from 12–54 years of age recruited from five Canadian communities to receive either tetanus–diphtheria vaccine (Td), acellular pertussis vaccine (aP) or combined diphtheria–tetanus-acellular pertussis vaccine (TdaP). Subjects and personnel were unaware of the vaccine allocation. Antibody levels were measured before and one month postimmunization; adverse events were collected at 24 and 72 h and 8 to 10 days. Adverse events were reported in similar frequency amongst the three vaccine groups. Moderate pain at the injection site was reported less frequently in the aP group than the TdaP group (10.7% compared to 19.4%; relative risk 0.6, 95% confidence interval 0.3–0.9). Chills were reported less frequently after Td (5.3%) than after TdaP (12.5%; relative risk 0.4, 95% confidence interval 0.2–0.9). There were no statistically significant differences between recipients of Td and TdaP in tetanus and diphtheria antitoxin levels achieved. Antibody response against
Bordetella pertussis antigens was vigorous in all groups although recipients of aP alone had higher levels of antibody levels against pertussis toxoid, fimbriae, and agglutinins and lower antibody levels against pertactin than did TdaP recipients. We conclude that this adult formulation 5-component acellular pertussis vaccine is safe and immunogenic in adolescents and adults and is a candidate vaccine for adolescent and adult immunization programs.

Nutrients, soluble salts, and pathogenic bacteria in feedlot‐pen manure have the potential to cause pollution of the environment. A three‐year study (1998–2000) was conducted at a beef ...cattle (Bos taurus) feedlot in southern Alberta, Canada to determine the effect of bedding material barley (Hordeum vulgare L.) straw versus wood chips and season on the chemical and bacterial properties of pen‐floor manure. Manure was sampled for chemical content (N, P, soluble salts, electrical conductivity, and pH) and populations of four groups of bacteria (Escherichia coli, total coliforms, and total aerobic heterotrophs at 27 and 39°C). More chemical parameters of manure were significantly (P ≤ 0.05) affected by season (SO4, Na, Mg, K, Ca, sodium adsorption ratio SAR, total C, NO3–N, NH4–N, total P, and available P) than by bedding (K, pH, total C, C to N ratio, NH4–N, and available P). Bedding had no significant (P > 0.05) effect on the four bacterial groups whereas season affected all four groups. Numbers of E. coli and total coliforms (TC) were significantly higher by 1.72 to 2.02 log10 units in the summer than the other three seasons, which was consistent with a strong positive correlation of E. coli and TC with air temperature. The low ratio of bedding to manure in the pens was probably the major cause of the lack of significant bedding effects. Bedding material and seasonal timing of cleaning feedlot pens and land application of manure may be a potential tool to manage nutrients, soluble salts, and pathogens in manure.
