Using cross-sectional data from the 2018 Health Survey for England, this study describes the types of impairment reported by people with chronic conditions and the association of chronic conditions ...and impairments with physical activity(PA).
Participants self-reported the presence of seven chronic health conditions (diabetes; stroke/ischemic heart disease; hypertension; chronic obstructive pulmonary disease (COPD); asthma; arthritis/rheumatism/fibrositis; back problems), 11 types of impairment (vision, hearing, mobility, dexterity; learning; memory; mental health; stamina; social or behavioural; other; none); and their PA using the International Physical Activity Questionnaire. Multivariable Poisson regression was used to estimate the association of a)impairment type, b)number of impairments, and c)impairment type and chronic condition (mutually adjusted) with PA.
In total, 2243 adults (55% female, 44% age > 55 yrs) reported having a chronic condition. PA volume (MET minutes per week: median (IQR)) was highest in participants with asthma (2093 (693-4479)), and lowest in those with COPD (454 (0-2079)). There was a negative association between number of impairments and levels of PA. After adjustment for age, sex, ethnicity and education, and mutually adjusting for all other conditions and impairments, diabetes (Incident rate ratio (95% confidence interval): 0.83 (0.73-0.94)), COPD (0.76 (0.59-0.99)), a mobility impairment (0.63 (0.56-0.72)), a dexterity impairment (0.86 (0.75-0.98)), or a memory impairment (0.84 (0.72-0.99)) was negatively associated with PA.
Future PA research requires consideration of the number and types of impairments that individuals experience, as well as assessing chronic conditions. This will improve understanding of the barriers to PA participation and inform interventions.
Physical activity and dietary change programmes play a central role in addressing public health priorities. Programme evaluation contributes to the evidence-base about these programmes; and helps ...justify and inform policy, programme and funding decisions. A range of evaluation frameworks have been published, but there is uncertainty about their usability and applicability to different programmes and evaluation objectives, and the extent to which they are appropriate for practitioner-led or researcher-led evaluation. This review appraises the frameworks that may be applicable to evaluation of physical activity and/or dietary change programmes, and develops a typology of the frameworks to help guide decision making by practitioners, commissioners and evaluators.
A scoping review approach was used. This included a systematic search and consultation with evaluation experts to identify evaluation frameworks and to develop a set of evaluation components to appraise them. Data related to each framework's general characteristics and components were extracted. This was used to construct a typology of the frameworks based on their intended programme type, evaluation objective and format. Each framework was then mapped against the evaluation components to generate an overview of the guidance included within each framework.
The review identified 71 frameworks. These were described variously in terms of purpose, content, or applicability to different programme contexts. The mapping of frameworks highlighted areas of overlap and strengths and limitations in the available guidance. Gaps within the frameworks which may warrant further development included guidance on participatory approaches, non-health and unanticipated outcomes, wider contextual and implementation factors, and sustainability.
Our typology and mapping signpost to frameworks where guidance on specific components can be found, where there is overlap, and where there are gaps in the guidance. Practitioners and evaluators can use these to identify, agree upon and apply appropriate frameworks. Researchers can use them to identify evaluation components where there is already guidance available and where further development may be useful. This should help focus research efforts where it is most needed and promote the uptake and use of evaluation frameworks in practice to improve the quality of evaluation and reporting.
Beat the Street aims to get children more active by encouraging them to walk and cycle in their neighbourhood using tracking technology with a reward scheme. This pilot study evaluates the impact of ...Beat the Street on active travel to school in Norwich, UK. Eighty children 8–10 yrs were recruited via an intervention and control school. They wore an accelerometer for 7 days at baseline, mid-intervention and post-intervention (+20 weeks), and completed a travel diary. Physical activity overall was not higher at follow-up amongst intervention children compared to controls. However, there was a positive association between moderate-to-vigorous physical activity (MVPA) during school commute times and the number of days on which children touched a Beat the Street sensor. This equated to 3.46min extra daily MVPA during commute times for children who touched a sensor on 14.5 days (the mean number of days), compared to those who did not engage. We also found weekly active travel increased at the intervention school (+10.0% per child) while it decreased at the control (−7.0%), p=0.056. Further work is needed to understand how improved engagement with the intervention might impact outcomes.
Abstract Objectives To understand the patterning of greenspace provision and use by area deprivation, and determine how deprivation moderates relationships with physical activity. Methods The ...responses obtained from 6821 respondents to the 2005 ‘The Quality of Life in your Neighbourhood Survey’ undertaken in Bristol, England, were combined with objective measures of access to greenspaces. Area deprivation was determined using the Index of Multiple Deprivation. Descriptive analyses examined how mean distance, perceived greenspace access and safety, visit frequency, and physical activity varied by deprivation quartile. Logistic regression models examined how relationships were moderated by deprivation. Results Respondents in more deprived areas lived closer to greenspaces, but reported poorer perceived accessibility, poorer safety, and less frequent use. Frequency of use declined with distance but only in the most affluent areas. Relationships between physical activity and perceived accessibility, safety, and visit frequency were moderated by deprivation. Conclusions The accessibility of greenspaces was better in more deprived areas but those residents had more negative perceptions and were less likely to use the greenspaces. Interventions may be most effective if they target the perceptions and needs of residents of deprived neighbourhoods.
Solar geoengineering refers to a range of proposed methods for counteracting global warming by artificially reducing sunlight at Earth's surface. The most widely known solar geoengineering proposal ...is stratospheric aerosol injection (SAI), which has impacts analogous to those from volcanic eruptions. Observations following major volcanic eruptions indicate that aerosol enhancements confined to a single hemisphere effectively modulate North Atlantic tropical cyclone (TC) activity in the following years. Here we investigate the effects of both single-hemisphere and global SAI scenarios on North Atlantic TC activity using the HadGEM2-ES general circulation model and various TC identification methods. We show that a robust result from all of the methods is that SAI applied to the southern hemisphere would enhance TC frequency relative to a global SAI application, and vice versa for SAI in the northern hemisphere. Our results reemphasise concerns regarding regional geoengineering and should motivate policymakers to regulate large-scale unilateral geoengineering deployments.
Summary
Sedentary behaviors are highly prevalent in youth and may be associated with markers of physical and mental health. This systematic review and meta‐analysis aimed to quantify the age‐related ...change in sedentary behavior during childhood and adolescence. Ten electronic databases were searched. Inclusion criteria specified longitudinal observational studies or control group from an intervention; participants aged ≥5 and ≤18 years; a quantitative estimate of the duration of SB; and English language, peer‐reviewed publication. Meta‐analyses summarized weighted mean differences (WMD) in device‐assessed sedentary time and questionnaire‐assessed screen‐behaviors over 1‐, 2‐, 3‐, or more than 4‐year follow‐up. Effect modification was explored using meta‐regression. Eighty‐five studies met inclusion criteria. Device‐assessed sedentary time increased by (WMD 95% confidence interval CI) 27.9 (23.2, 32.7), 61.0 (50.7, 71.4), 63.7 (53.3, 74.0), and 140.7 (105.1, 176.4) min/day over 1‐, 2‐, 3‐, and more than 4‐year follow‐up. We observed no effect modification by gender, baseline age, study location, attrition, or quality. Questionnaire‐assessed time spent playing video games, computer use, and a composite measure of sedentary behavior increased over follow‐up duration. Evidence is consistent in showing an age‐related increase in various forms of sedentary behavior; evidence pertaining to variability across socio‐demographic subgroups and contemporary sedentary behaviors are avenues for future research.
Many adults accumulate considerable time in screen-based behaviours, some of which have been associated with negative physical and psychological health outcomes. The aims of this study were to ...characterise contemporary patterns of screen-based behaviours and describe their temporal trends by global region, age, sex and education. Data covering the period 2012–2019 were obtained in aggregated form from GWI (previously known as Global Web Index), a global market research company. Temporal trends in the duration of adults' (16–64 years) self-reported personal computer, laptop and tablet use, mobile phone use, broadcast television viewing, online television viewing and games console use were described using data from over 2 million participants from 46 countries. For each activity, participants selected from response options ranging from less than 30 min to more than 10 h. Internationally, daily screen time increased from approximately 9 h in 2012 to 11 h in 2019, with notable increases in mobile phone use (approx. 2 h), online television viewing (approx. 37 min) and games console use (approx. 26 min). Differences were seen in the duration of time spent engaging in screen-based behaviours across regions and between socio-demographic groups, with Latin America, the Middle East and Africa and younger age groups seeing greater increases in overall screen time. The findings have important implications for health behaviour surveillance and for research exploring the links between screen-based behaviours and health.
•Daily screen time increased between 2012 and 2019 particularly in Latin America.•Screen use was highest in 16–24 and 25–34 age groups across all survey years.•Traditional TV viewing has decreased while mobile phone use has increased.•Industry data on technology use can provide insights for public health research.
This paper describes the results from a project to obtain radiocarbon determinations from Early Bronze Age log coffin burials. Log coffins have been recognised as a burial tradition since antiquarian ...excavations uncovered the first examples. However, comparatively few are associated with radiocarbon determinations and many old determinations are very imprecise. To address this, seven log coffin burials were identified across England, and 11 samples from these were submitted for radiocarbon dating. The dates from the project were reviewed with previously obtained reliable determinations to reconsider the origins and development of the log coffin burial by region. The resulting study indicates that the earliest log coffins were associated with Beaker burials but that regional variations involving different rites soon developed.
Radiative forcing is a useful tool for predicting equilibrium global temperature change. However, it is not so useful for predicting global precipitation changes, as changes in precipitation strongly ...depend on the climate change mechanism and how it perturbs the atmospheric and surface energy budgets. Here a suite of climate model experiments and radiative transfer calculations are used to quantify and assess this dependency across a range of climate change mechanisms. It is shown that the precipitation response can be split into two parts: a fast atmospheric response that strongly correlates with the atmospheric component of radiative forcing, and a slower response to global surface temperature change that is independent of the climate change mechanism, ∼2‐3% per unit of global surface temperature change. We highlight the precipitation response to black carbon aerosol forcing as falling within this range despite having an equilibrium response that is of opposite sign to the radiative forcing and global temperature change.
