Introducción: en nuestro país, como en el resto del mundo, los sistemas de salud están sometidos a procesos de modernización y reforma con el objetivo de adecuar sus capacidades a las necesidades de ...la población. En este contexto la estructura y gestión del sistema de salud son vehículos importantes para cumplir el objetivo. A fin de hacer buen uso de esta red, resulta crucial asegurar una buena coordinación entre los diferentes niveles de atención. Un sistema de referencia-contrarreferencia (SRC) constituye una herramienta que pone, a disposición de los médicos, criterios y procedimientos homogéneos que permiten canalizar la demanda para servicios de salud y hacer uso eficiente del sistema de atención. Objetivos: analizar las derivaciones de pacientes al Hospital Pediátrico del Centro Hospitalario Pereira Rossell (HP-CHPR) a través del Departamento de Emergencia (DEP) y de la Policlínica Pediátrica General de Referencia (PPGR). Material y método: se realizó un estudio descriptivo a partir de los registros correspondientes del triage de las interconsultas de las policlínicas de atención primaria a los servicios de neuropediatría, neumología y gastroenterología realizados en la PPGR desde agosto del año 2005 a diciembre de 2006. Se analizaron además los pases realizados al DEP del CHPR desde septiembre a diciembre del año 2006. Se realizaron fichas codificadas tanto para evaluar la interconsula a especialista, así como para evaluar el pase al DEP. Se calcularon porcentajes. Los datos se procesaron mediante Epi Info 6.04. Resultados: evaluación de los niños derivados a especialistas. El 69% de los casos procedía de Montevideo. Luego del triage, 69% del total de interconsultas fueron derivadas a la policlínica de referencia. Neuropediatría fue la especialidad más solicitada en 60% de los pases. Luego de realizado el triage del total de niños con pase a neuropediatría, 30% fue derivado a esta especialidad, a gastroenterología 0,05% y a neumología 0,03%. La calidad de los datos fue buena en 30% de los casos. La derivación fue oportuna en 89% de los casos. Evaluación de los pases al DEP. Del total de consultas en ese período, 24% concurrieron con pase. Se analizó 7,6%. El 73,2% provenía de Montevideo, 20% de Canelones. Los pacientes fueron derivados desde sistemas de emergencia móvil en 39% de casos. El 26% de los pases fue realizado por pediatra y el resto por médico general o de familia. El motivo de la derivación más frecuente fue por requerimiento de especialista, 29,6%. La calidad del pase fue buena en 64%. El 57% de los pacientes fue trasladado en ambulancia con médico; de éstos, 58,5% no requería tercer nivel de atención. Conclusiones: los resultados hallados muestran una ineficiente utilización de los recursos. Se deben establecer medidas para racionalizar el sistema de atención y promover su eficacia. Se proponen medidas para mejorar el funcionamiento del SRC y la derivación oportuna y adecuada a especialista en el HP-CHPR.
Reverse remodeling is a clinically relevant endpoint in heart failure with reduced ejection fraction (HFrEF). Rho-kinase (ROCK) signaling cascade activation correlates with cardiac remodeling and ...left ventricular (LV) systolic dysfunction in HFrEF patients. Cardiac resynchronization therapy (CRT) is effective in HFrEF, especially when there is a left bundle block, as this treatment may stimulate reverse remodeling, thereby improving quality of life and prolonging survival for patients with this severe condition. Here, we evaluate the hypothesis that ROCK activation is reduced after effective CRT in HFrEF.
ROCK activation in circulating leukocytes was evaluated in 28 HFrHF patients, using Western blot (myosin light chain phosphatase subunit 1 phosphorylation, MYPT1p/t), before and three months after initiation of CRT. LV systolic function and remodeling were assessed by echocardiography.
Three months after CRT, LV ejection fraction increased an average of 14.5% (
< 0.001) in 13 patients (responders), while no change was observed in 15 patients (non-responders). End-systolic diameter decreased 16% (
< 0.001) in responders, with no change in non-responders. ROCK activation in PBMCs decreased 66% in responders (
< 0.05) but increased 10% in non-responders (NS). LV end-diastolic diameter was also 5.2 mm larger in non-responders vs. responders (
= 0.058). LV ejection fraction, systolic diameter, and ROCK activation levels were similar in both groups at baseline.
In HFrEF patients, 3 months of effective CRT induced reverse myocardial remodeling, and ROCK activation was significantly decreased in circulating leukocytes. Thus, decreased ROCK activation in circulating leukocytes may reflect reverse cardiac remodeling in patients with heart failure.

Lonchocarpus cultratus (Vell.) A.M.G. Azevedo & H.C. Lima (Fabaceae) is a native tree species and from which flavonoids, triterpenes, alkaloid were isolated. In addition, potent antiproliferative activity of hexane fraction and chalcone, obtained from this plant, has been described in different cell lines of human tumors. However, no studies were found on the allelopathic and/or bioherbicidal action, using aerial parts of L. cultratus. Thus, the aim of the present work was to evaluate the allelopathic activity of methanol extract, fractions and compounds from aerial parts of L. cultratus. Dried aerial parts were extracted by exhausting extraction with methanol at room temperature, and the solvent was evaporated under reduced pressure providing the crude extract. The crude extract was partitioned in a liquid-liquid extraction and to obtain hexanic (HF), chloroform (CF), ethyl acetate (EAF), and hydromethanolic (HMF) fractions. The compounds were extracted from fractions through chromatographic methods. Crude extract and its fractions inhibited Lactuca sativa germination mean time (GMT) and germination speed índex (GSI), but not the germination percentage (GP) of Lactuca sativa. Crude extract and hexane fraction inhibited the hypocotyl growth of L. sativa seedlings and the other fractions inhibited the primary root of seedlings. Hexane, chloroform, and ethyl acetate fractions inhibited primary root growth of Euphorbia heterophylla. Chalcone 2’,4’-dihydroxy-5’-prenylchalcone, flavanone 8-prenylpinocembrin and alkaloid 4-hydroxy-N-methylproline were tested on lettuce early growth. Thirty-three metabolites were identified by UHPLC-MS/MS analysis, with flavonoid derivatives being the most abundant constituents.
