Aim: This study aimed to examine the retinal layer before and after treatment in patients with hypothyroidism with vitamin D deficiency, since the vitamin also protects the retinal cells against ...inflammatory damage.
Material and methods: The free T3, free T4, and vitamin D levels of 104 patients with no ocular disease were first measured. Ophthalmological examinations of these patients, who were divided into three groups, were performed by specialist ophthalmologists, while retinal findings were examined using optical coherence tomography (OCT) and recorded. The first group was given vitamin D for three months, the second levothyroxine, and the third vitamin D + levothyroxine. After three months repeat OCT was performed, and the results were compared with the previous values.
Results: The thickness of the left inner nuclear cell layer of the patients in the vitamin D group increased significantly compared to pre-treatment. Post-treatment right central macular thickness, right nerve fiber layer, right outer nuclear cell layer, right pigment epithelial layer, left central macular, and left inner nuclear cell layer thicknesses were all significantly higher compared to pre-treatment in the patients in the levothyroxine + vitamin D group, while right outer retinal layer and left retinal nerve fiber thicknesses decreased (p<0.05).
Conclusion: A greater increase in cell layer thickness was observed in the group using vitamin D and levothyroxine together compared to those in which vitamin D and levothyroxine were employed alone. However, further studies on the effect of vitamin D on retinal cell development and protection against injury are now needed.
Giriş: Diyabetik makula ödemi (DMÖ), görme bozukluklarına ve körlüğe sebep olan bir hastalıktır. Kötü yönetilen diyabetin, retinopati komplikasyonu riski yüksektir. Diyetle glisemik kontrolü ...iyileştirmenin, diyabetik retinopatinin önlenmesinde etkili olduğu belirtilmiştir. Amaç: Bu çalışma, serum ileri glikasyon son ürünleri (AGEs), ileri glikasyon son ürünleri reseptörü (sRAGE) ve bireylerin diyet AGE (dAGE) alımları ile DMÖ hastalığı arasındaki ilişkiyi incelemek amacıyla yapılmıştır. Yöntem: Hacettepe Üniversitesi Tıp Fakültesi, Göz Hastalıkları Anabilim Dalı’nda takip edilen DMÖ tanısı alan ve almayan toplam 90 diyabetli bireyin katılımıyla yapılmıştır. Bireylerin serumunda toplam antioksidan seviye (TAS), toplam oksidan seviye (TOS) ve ELISA yöntemiyle AGEs (karboksimetil lizin (CML)), sRAGE düzeyleri analiz edilmiştir. Ayrıca bireylerin besin tüketim kayıtları alınarak diyet AGEs alımı ve diyet glisemik indeksi/yükü hesaplanmıştır. Bulgular: DMÖ grubunun yaş ortalaması 62.6±8.4 ve kontrol grubunun yaş ortalaması 58.7±7.8 olarak belirlenmiştir. DMÖ grubunun AGEs düzeyleri ve diyet AGEs alımları kontrol grubuna göre istatistiksel olarak anlamlı düzeyde yüksek bulunmuştur (p<0,05). DMÖ grubunun sRAGE düzeyleri daha yüksek bulunmasına karşın bu fark istatistiksel olarak önemli bulunmamıştır (p>0,05). Vaka grubunun TOS değerleri ve oksidatif stres indeksi (OSİ) ortalamaları kontrol grubuna göre istatistiksel olarak anlamlı düzeyde yüksek bulunmuştur (p<0,001). Grupların diyet glisemik indeks ve glisemik yük ortalamaları arasında anlamlı bir fark bulunmamıştır (p>0.05). Vaka grubunda serum AGE ve sRAGE düzeyleri arasında pozitif yönde orta derecede korelasyon (p<0,001) bulunurken sRAGE ile TOS ve OSİ arasında negatif düşük derecede korelasyon saptanmıştır (p<0,05). Sonuç ve Öneriler: Bu çalışmada, DMÖ ile AGEs, dAGE, TOS, OSİ arasında pozitif ilişki bulunmuştur. Bu sonuçların geçerliliği için kontrollü beslenme müdahalelerini içeren çalışmalara gereksinim vardır.
We investigated the effects of vitamin D on the ocular surface, tear functions, corneal imaging, and tear film cytokine levels.
Fifty-two patients with vitamin D levels were examined in 3 groups ...according to serum vitamin D levels; 28 in group 1 (<12 ng/ml), 10 in group 2 (12-20 ng/ml), and 14 in group 3 (>20 ng/ml). Ocular surface disease index (OSDI), tear break up time (BUT), lissamine green (LG) staining, Schirmer test, in vivo confocal microscopy (IVCM), and tear collection for cytokine analysis were performed.
The mean OSDI score was 35.2 ± 23.3, 36.2 ± 17.7, 24.4 ± 18.2 (p = .253), TBUT was 6.7 ± 2.5 sec, 9.3 ± 1.8 sec, 11.1 ± 2.8 sec (p < .001), Schirmer test was 16.7 ± 8.5 mm, 18.7 ± 7.6 mm, and 20.2 ± 7 mm (p = .254), median LG staining grade was 1 (0-3), 1 (0-2), 0 (0-1) (p = .008) in group 1, group 2, and group 3, respectively. Mean IL-1 β (82.62 ± 15.26, 85.57 ± 17.41, and 66.44 ± 11 ng/ml in group 1, 2 and 3, respectively, p = .002), IL-17 (77.80 ± 24.91, 64.46 ± 25.47, 55.42 ± 12.05 ng/ml in group 1, 2 and 3, respectively, p = .012), and IL-2 (75.7 ± 18.4, 66.13 ± 26.78, and 59.65 ± 16.04 ng/ml in group 1, 2 and 3, respectively, p = .048) levels were significantly lower in group 3, whereas, IL-13 levels were significantly higher in group 3 (16.12 ± 5.24, 19.20 ± 4.90, and 21.6 ± 5.55 ng/ml in groups 1, 2, and 3, respectively, p = .010).
Vitamin D deficiency/insufficiency is associated with ocular surface changes shown with significant TBUT, LG staining, and tear film cytokine contents. Besides, significant corneal basal epithelial, sub-basal nerve density, and structural sub-basal nerve changes were associated with lower Vitamin D levels.
