Pre-exposure prophylaxis (PrEP) is traditionally prescribed by HIV specialist physicians. Given finite specialist resources, there is a need to scale up PrEP delivery by decentralizing services via ...other healthcare professionals. We aimed to assess the feasibility of delivering PrEP to men who have sex with men (MSM) through primary care physicians and sexual health clinic nurses. We piloted a multi-component, implementation and dissemination research program to increase provision of PrEP through primary care physicians and sexual health clinic nurses in Toronto, Canada. Community-based organizations (CBOs) provided prospective participants with information cards that contained links to an online module on engaging providers in a conversation about PrEP. In our patient-initiated continuing medical education (PICME) strategy, participants saw their family doctors and gave them the card, which also contained a link to a Continuing Medical Education module. In the nurse-led strategy, participants visited one of two participating clinics to obtain PrEP. We administered an optional online questionnaire to patients and providers at baseline and six months. CBOs distributed 3043 cards. At least 339 men accessed the online module and 196 completed baseline questionnaires. Most (55%) intended to visit nurses while 21% intended to consult their physicians. Among 45 men completing follow-up questionnaires at 6 months, 31% reported bringing cards to their physicians and obtaining PrEP through them; sexual health clinics delivered PrEP to 244 patients. Participants who went through the PICME approach reported no changes in relationships with their providers. Nurses showed fidelity to PrEP prescribing guidelines. Nurse-led PrEP and patient-initiated continuing medical education (PICME) for primary care physicians are feasible strategies to increase PrEP uptake. Nurse-led PrEP delivery was preferred by most patients.

Issue Title: The James Webb Space Telescope The James Webb Space Telescope (JWST) is a large (6.6 m), cold (<50 K), infrared (IR)-optimized space observatory that will be launched early in the next ...decade into orbit around the second Earth-Sun Lagrange point. The observatory will have four instruments: a near-IR camera, a near-IR multiobject spectrograph, and a tunable filter imager will cover the wavelength range, 0.6 < ; < 5.0 μ m, while the mid-IR instrument will do both imaging and spectroscopy from 5.0 < ; < 29 μ m. The JWST science goals are divided into four themes. The key objective of The End of the Dark Ages: First Light and Reionization theme is to identify the first luminous sources to form and to determine the ionization history of the early universe. The key objective of The Assembly of Galaxies theme is to determine how galaxies and the dark matter, gas, stars, metals, morphological structures, and active nuclei within them evolved from the epoch of reionization to the present day. The key objective of The Birth of Stars and Protoplanetary Systems theme is to unravel the birth and early evolution of stars, from infall on to dust-enshrouded protostars to the genesis of planetary systems. The key objective of the Planetary Systems and the Origins of Life theme is to determine the physical and chemical properties of planetary systems including our own, and investigate the potential for the origins of life in those systems. Within these themes and objectives, we have derived representative astronomical observations. To enable these observations, JWST consists of a telescope, an instrument package, a spacecraft, and a sunshield. The telescope consists of 18 beryllium segments, some of which are deployed. The segments will be brought into optical alignment on-orbit through a process of periodic wavefront sensing and control. The instrument package contains the four science instruments and a fine guidance sensor. The spacecraft provides pointing, orbit maintenance, and communications. The sunshield provides passive thermal control. The JWST operations plan is based on that used for previous space observatories, and the majority of JWST observing time will be allocated to the international astronomical community through annual peer-reviewed proposal opportunities.PUBLICATION ABSTRACT

Adhesion between plant cells is a fundamental feature of plant growth and development, and an essential part of the strategy by which growing plants achieve mechanical strength. Turgor ...pressure provides non‐woody plant tissues with mechanical rigidity and the driving force for growth, but at the same time it generates large forces tending to separate cells. These are resisted by reinforcing zones located precisely at the points of maximum stress. In dicots the reinforcing zones are occupied by networks of specific pectic polymers. The mechanisms by which these networks cohere vary and are not fully understood. In the Poaceae their place is taken by phenolic cross‐linking of arabinoxylans. Whatever the reinforcing polymers, a targeting mechanism is necessary to ensure that they become immobilized at the appropriate location, and there are secretory mutants that appear to have defects in this mechanism and hence are defective in cell adhesion. At the outer surface of most plant parts, the tendency of cells to cohere is blocked, apparently by the cuticle. Mutants with lesions in the biosynthesis of cuticular lipids show aberrant surface adhesion and other developmental abnormalities. When plant cells separate, the polymer networks that join them are locally dismantled with surgical precision. This occurs during the development of intercellular spaces; during the abscission of leaves and floral organs; during the release of seeds and pollen; during differentiation of root cap cells; and during fruit ripening. Each of these cell separation processes has its own distinctive features. Cell separation can also be induced during cooking or processing of fruit and vegetables, and the degree to which it occurs is a significant quality characteristic in potatoes, pulses, tomatoes, apples and other fruit. Control over these technological characteristics will be facilitated by understanding the role of cell adhesion and separation in the life of plants.
We investigated the direct and indirect influence of tides on net ecosystem exchange (NEE) of carbon dioxide (CO2) in a temperate brackish tidal marsh. NEE displayed a tidally driven pattern with ...obvious characteristics at the multiday scale, with greater net CO2 uptake during spring tides than neap tides. Based on the relative mutual information between NEE and biophysical variables, this was driven by a combination of higher water table depth (WTD), cooler air temperature, and lower vapor pressure deficit (VPD) during spring tides relative to neap tides, as the fortnightly tidal cycle not only influenced water levels but also strongly modulated water and air temperature and VPD. Tides also influenced NEE at shorter timescales, with a reduction in nighttime fluxes during growing season spring tides when the higher of the two semidiurnal tides caused inundation at the site. WTD significantly influenced ecosystem respiration (Reco), with lower Reco during spring tides than neap tides. While WTD did not appear to affect ecosystem photosynthesis (gross ecosystem production, GPP) directly, the impact of tides on temperature and VPD influenced GPP, with higher daily light‐use efficiency and photosynthetic activity during spring tides than neap tides when temperature and VPD were lower. The strong direct and indirect influence of tides on NEE across the diel and multiday timescales has important implications for modeling NEE in tidal wetlands and can help inform the timing and frequency of chamber measurements as annual or seasonal net CO2 uptake may be underestimated if measurements are only taken during nonflooded periods.

Gay, bisexual and other men who have sex with men (gbMSM) in Canada continue to experience high rates of incident HIV. Pre-exposure prophylaxis (PrEP, the regular use of anti-HIV medication) reduces ...HIV acquisition and could reduce incidence. However, there are too few physicians with expertise in HIV care to meet the projected demand for PrEP. To meet demand and achieve greater public health impact, PrEP delivery could be 'decentralized' by incorporating it into front-line prevention services provided by family physicians (FPs) and sexual health clinic nurses.
This PrEP decentralization project will use two strategies. The first is an innovative knowledge dissemination approach called 'Patient-Initiated CME' (PICME), which aims to empower individuals to connect their family doctors with online, evidence-based, continuing medical education (CME) on PrEP. After learning about the project through community agencies or social/sexual networking applications, gbMSM interested in PrEP will use a uniquely coded card to access an online information module that includes coaching on how to discuss their HIV risk with their FP. They can provide their physician a link to the accredited CME module using the same card. The second strategy involves a pilot implementation program, in which gbMSM who do not have a FP may bring the card to designated sexual health clinics where trained nurses can deliver PrEP under a medical directive. These approaches will be evaluated through quantitative and qualitative methods, including: questionnaires administered to patients and physicians at baseline and at six months; focus groups with patients, FPs, and sexual health clinic staff; and review of sexual health clinic charts. The primary objective is to quantify the uptake of PrEP achieved using each decentralization strategy. Secondary objectives include a) characterizing barriers and facilitators to PrEP uptake for each strategy, b) assessing fidelity to core components of PrEP delivery within each strategy, c) measuring patient-reported outcomes including satisfaction with clinician-patient relationships, and d) conducting a preliminary costing analysis.
This study will assess the feasibility of a novel strategy for disseminating knowledge about evidence-based clinical interventions, and inform future strategies for scale-up of an underutilized HIV prevention tool.
Wetlands and flooded peatlands can sequester large amounts of carbon (C) and have high greenhouse gas mitigation potential. There is growing interest in financing wetland restoration using C markets; ...however, this requires careful accounting of both CO2 and CH4 exchange at the ecosystem scale. Here we present a new model, the PEPRMT model (Peatland Ecosystem Photosynthesis Respiration and Methane Transport), which consists of a hierarchy of biogeochemical models designed to estimate CO2 and CH4 exchange in restored managed wetlands. Empirical models using temperature and/or photosynthesis to predict respiration and CH4 production were contrasted with a more process‐based model that simulated substrate‐limited respiration and CH4 production using multiple carbon pools. Models were parameterized by using a model‐data fusion approach with multiple years of eddy covariance data collected in a recently restored wetland and a mature restored wetland. A third recently restored wetland site was used for model validation. During model validation, the process‐based model explained 70% of the variance in net ecosystem exchange of CO2 (NEE) and 50% of the variance in CH4 exchange. Not accounting for high respiration following restoration led to empirical models overestimating annual NEE by 33–51%. By employing a model‐data fusion approach we provide rigorous estimates of uncertainty in model predictions, accounting for uncertainty in data, model parameters, and model structure. The PEPRMT model is a valuable tool for understanding carbon cycling in restored wetlands and for application in carbon market‐funded wetland restoration, thereby advancing opportunity to counteract the vast degradation of wetlands and flooded peatlands.

In Korea, patients with chronic hepatitis C virus (HCV) infection are typically treated with pegylated interferon‐alpha plus ribavirin, but interferons are contraindicated in many patients ...and are often poorly tolerated, particularly by the elderly and those with advanced liver disease. No interferon‐free treatment regimens are approved in Korea. Sofosbuvir is an oral nucleotide analog inhibitor of the HCV nonstructural 5B RNA polymerase. It is approved in the USA, European Union and Japan for treating a number of HCV genotypes, including genotype 2. Genotype 2 has a seroprevalence of 38–46% in Korea. This single‐arm, phase 3b study (NCT02021643) examined the efficacy and safety of sofosbuvir plus ribavirin (12‐week duration) in chronic genotype 2 HCV‐infected treatment‐naive and treatment‐experienced Korean patients with and without cirrhosis. The proportion of patients with sustained virologic response 12 weeks after treatment discontinuation (SVR12) was 97% (125/129), with 96% (101/105) of treatment‐naive and 100% (24/24) of treatment‐experienced patients achieving SVR12. Two patients experienced virologic failure (n = 1, on‐treatment failure; n = 1, relapse). No patient discontinued study treatment due to an adverse event (AE). The most common treatment‐emergent AEs were headache (18%, 23/129) and pruritus (15%, 19/129). Few patients had grade 3 AEs (5%, 6/129) or grade 3 laboratory abnormalities (12%, 15/129). No grade 4 AE was reported. These data suggest that 12 weeks of treatment with the all‐oral, interferon‐free regimen of sofosbuvir plus ribavirin is effective and well tolerated in Korean patients with chronic genotype 2 HCV infection.
We mapped tidal wetland gross primary production (GPP) with unprecedented detail for multiple wetland types across the continental United States (CONUS) at 16‐day intervals for the years 2000–2019. ...To accomplish this task, we developed the spatially explicit Blue Carbon (BC) model, which combined tidal wetland cover and field‐based eddy covariance tower data into a single Bayesian framework, and used a super computer network and remote sensing imagery (Moderate Resolution Imaging Spectroradiometer Enhanced Vegetation Index). We found a strong fit between the BC model and eddy covariance data from 10 different towers (r2 = 0.83, p < 0.001, root‐mean‐square error = 1.22 g C/m2/day, average error was 7% with a mean bias of nearly zero). When compared with NASA's MOD17 GPP product, which uses a generalized terrestrial algorithm, the BC model reduced error by approximately half (MOD17 had r2 = 0.45, p < 0.001, root‐mean‐square error of 3.38 g C/m2/day, average error of 15%). The BC model also included mixed pixels in areas not covered by MOD17, which comprised approximately 16.8% of CONUS tidal wetland GPP. Results showed that across CONUS between 2000 and 2019, the average daily GPP per m2 was 4.32 ± 2.45 g C/m2/day. The total annual GPP for the CONUS was 39.65 ± 0.89 Tg C/year. GPP for the Gulf Coast was nearly double that of the Atlantic and Pacific Coasts combined. Louisiana alone accounted for 15.78 ± 0.75 Tg C/year, with its Atchafalaya/Vermillion Bay basin at 4.72 ± 0.14 Tg C/year. The BC model provides a robust platform for integrating data from disparate sources and exploring regional trends in GPP across tidal wetlands.

Measurements of hyperspectral canopy reflectance provide a detailed snapshot of information regarding canopy biochemistry, structure and physiology. In this study, we collected 5 years of repeated ...canopy hyperspectral reflectance measurements for a total of over 100 site visits within the flux footprints of two eddy covariance towers at a pasture and rice paddy in northern California. The vegetation at both sites exhibited dynamic phenology, with significant interannual variability in the timing of seasonal patterns that propagated into interannual variability in measured hyperspectral reflectance. We used partial least-squares regression (PLSR) modeling to leverage the information contained within the entire canopy reflectance spectra (400-900 nm) in order to investigate questions regarding the connection between measured hyperspectral reflectance and landscape-scale fluxes of net ecosystem exchange (NEE) and gross primary productivity (GPP) across multiple timescales, from instantaneous flux to monthly integrated flux. With the PLSR models developed from this large data set we achieved a high level of predictability for both NEE and GPP flux in these two ecosystems, where the R2 of prediction with an independent validation data set ranged from 0.24 to 0.69. The PLSR models achieved the highest skill at predicting the integrated GPP flux for the week prior to the hyperspectral canopy reflectance collection, whereas the NEE flux often achieved the same high predictive power at daily to monthly integrated flux timescales. The high level of predictability achieved by PLSR in this study demonstrated the potential for using repeated hyperspectral canopy reflectance measurements to help partition NEE into its component fluxes, GPP and ecosystem respiration, and for using quasi-continuous hyperspectral reflectance measurements to model regional carbon flux in future analyses.
