From the perspective of discourse analysis, we understand genres as discursive constructions that can take varied forms and convey in different modalities, with conventions established and shared by ...the community. Rather than a textual category, a genre is a kind of socio-communicative event that is generated in a specific community, with certain communicative purposes. Healthcare professionals have a wide range of written and oral genres, each of which is characterized by some purposes, as well as a series of formal, linguistic and content conventions. Based on our work with clinical case reports in Catalan, English and Spanish, in this article we try to characterize the genre, as well as to review critically the bibliography from the fields of discourse analysis and the theory of genres, and also the prescriptive texts generated within the discursive community itself, about how clinical cases are or should be.
Clinical case reports are a genre developed within the communities of healthcare professionals and based on the clinical experience of one or a few patients. Unlike other authors, we consider that ...they have a mainly narrative nature and, therefore, can be characterized by a structure following the five canonical sections of the narrative: initial situation, node, (re) action or evaluation, denouement and final situation. We verify the presence of these components in a prototypical clinical case report from the field of neurology, comparing them with analogous samples of another genre that is widely recognised as narrative: clinical tales.
Els casos clínics (CC) constitueixen un gènere propi dels professionals sanitaris, caracteritzat per l'ús d'una terminologia i d'unes abreviacions específiques que encaixen amb els propòsits ...comunicatius del gènere i de la comunitat de pràctica. Juntament amb aquests recursos, hi podem trobar determinades construccions que no necessàriament contenen mots especialitzats, però que també caracteritzen aquest gènere. Es tracta d'estructures que se solen repetir amb unes formes idèntiques o molt similars en nombrosos CC i amb un mateix objectiu. Identifiquem aquestes construccions convencionalitzades aplicant un programari de gestió de corpus i d'anàlisi de textos a 115 casos clínics en català, castellà i anglès sobre afeccions mentals, provinents dels àmbits de la neurologia, la psiquiatria i la psicologia. En presentem les més destacades i les relacionem amb quatre de les mogudes retòriques analitzades per Helán: la justificació del cas; la presentació del cas; la presentació del pacient quant a la raó de la consulta o l'ingrés; la investigació del problema.
This article deals with the norm and the standard of the Catalan language in the context of the recent reform of its grammar. We formulate our linguistic analyses and interpretations in relation to ...historical and geographical backgrounds, paying particular attention to the main varieties of Catalan. In the successive development stages of the linguistic norm, we show how codifiers have faced the challenging task of taking sufficient account of specific dialectal peculiarities in a language that is not backed by a nation-state and is spoken in different countries and regions.

The article focuses on the language situation in Valencia, an important autonomous community of Spain. There are two official languages: Castilian as a language of the whole of Spain (i.e. Spanish) ...and Valencian as the Valencian people’s language proper. The paper describes the relations between Spanish and Valencian both from a linguistic and a socio-political point of view and offers a statistical summary of the use of both languages in the territory of the Valencian Community. Two tendencies important for the future of Valencia are discussed, a tendency to use “countrywide” Spanish and a tendency to preserve Valencian (the latter tendency is frequently based on respect for tradition as well as on personal decisions). In addition, the article considers relations between Valencian and Catalan.
Resumen: Los casos clínicos son un género propio de las comunidades de profesionales de la salud que se basa en la experiencia clínica de uno o unos pocos pacientes. A diferencia de otros autores, ...consideramos que tienen una naturaleza principalmente narrativa y que, por lo tanto, se pueden caracterizar por una estructuración en los cinco apartados canónicos de la narración: situación inicial, nudo, (re)acción o evaluación, desenlace y situación final. Verificamos la presencia de estos componentes en un caso clínico prototípico del campo de la neurología, comparándolos con sus análogos en textos provenientes del género reconocidamente narrativo de los cuentos clínicos.Abstract: Clinical case reports are a genre developed within the communities of healthcare professionals and based on the clinical experience of one or a few patients. Des de la perspectiva de l'anàlisi del discurs, entenem els gèneres com a construccions discursives que poden adoptar formes variades i vehicular-se en diverses modalitats, amb convencions ...establertes i compartides per la comunitat. Més que una categoria textual, un gènere és un tipus d'esdeveniment sociocomunicatiu que es genera en una comunitat concreta, amb uns propòsits comunicatius determinats. Els professionals sanitaris disposen d'un conjunt de gèneres orals i escrits, cadascun dels quals es caracteritza per unes finalitats, com també per unes convencions formals, lingüístiques i de continguts. A partir del nostre treball amb els casos clínics en català, anglès i castellà, en aquest article mirem de caracteritzar-ne el gènere, tot fent una revisió crítica de la bibliografia provinent de l'anàlisi del discurs i la teoria dels gèneres, com també dels textos prescriptius, generats per la comunitat discursiva mateixa, sobre com són o han de ser els casos clínics.
Assagem una aproximació a les característiques formals del gènere dels casos clínics: l’extensió, l’estructuració, l’autoria, els títols i els trets dels diversos apartats que convencionalment els ...componen. Partim d’un corpus en català, castellà i anglès de casos clínics sobre salut mental, provinents sobretot dels camps de la neurologia, la psiquiatria i la psicologia. En presentem dades numèriques i observacions qualitatives susceptibles de revelar alguns dels trets i de les tendències d’aquest gènere, en relació amb els propòsits comunicatius i amb les peculiaritats de les comunitats de pràctica en les quals s’inscriuen. Contrastem les dades i les observacions recollides al corpus amb les obres prescriptives sobre el gènere, provinents de les comunitats discursives mateixes, com també amb la bibliografia pertinent produïda sobretot en l’àmbit de l’anàlisi del discurs.
En aquest article revisem les potencialitats de les eines d’intel·ligència artificial (IA) per a millorar la comunicació metge-pacient, especialment en la reinterpretació dels informes mèdics. ...Explorem l’ús de bots de conversa, com ChatGPT, per a adaptar i simplificar textos mèdics complexos per als pacients. Destaquem els beneficis de la IA en la millora de la llegibilitat dels informes, amb un estudi pilot que demostra una millora del 19-31 % en la seva comprensió. Subratllem també la necessitat de mantenir la supervisió humana, especialment per a garantir la correcció i la confidencialitat de la informació clínica. Apuntem futures aplicacions, inclosa l’adaptació a diversos idiomes i les eines d’assistència durant la conversa metge-pacient.
In this article, we explore the potential of artificial intelligence (AI) tools to improve doctor-patient communication, particularly in the reinterpretation of medical reports. We explore the use of chatbots, such as ChatGPT, to adapt and simplify complex medical texts for patients. We highlight the benefits of AI in improving the readability of reports, with a pilot study demonstrating a 19–31% improvement in patient understanding. We also emphasise the need for human oversight, particularly to ensure the accuracy and confidentiality of clinical information. We mention future applications, including multilingual adaptation and tools to support the doctor-patient conversation.
En este artículo revisamos las potencialidades de las herramientas de inteligencia artificial (IA) para mejorar la comunicación médico-paciente, especialmente en la reinterpretación de los informes médicos. Exploramos el uso de bots conversacionales, como ChatGPT, para adaptar y simplificar textos médicos complejos para los pacientes. Destacamos los beneficios de la IA en la mejora de la legibilidad de los informes, con un estudio piloto que demuestra una mejora del 19-31 % en su comprensión. Subrayamos también la necesidad de mantener la supervisión humana, especialmente para garantizar la corrección y la confidencialidad de la información clínica. Señalamos futuras aplicaciones, incluyendo la adaptación a varios idiomas y las herramientas de asistencia durante la conversación médico-paciente.
