Background: Multinational time‐trend analyses of atopic disease have shown that the East–West gradients in prevalence are shrinking. We set out to clarify whether the disparities in the occurrence of atopy and atopic diseases in Finnish and Russian Karelia during the past 10 years have diminished and how the prevalence of atopy has evolved with successive years of birth.
Methods: Two surveys with identical methodology were performed in 1997/1998 and 2007. The study population comprised randomly selected adults, aged 25–54 years, from Finnish and Russian Karelia. Serum samples were collected for total and specific IgE measurements. Clinical data were obtained by questionnaires.
Results: Sensitization rates to birch pollen increased from 7.8% to 14.8% (P < 0.001) and to cat from 6.1% to 10.8% (P < 0.001) in Finland. In Russia, no significant increase was found. Contrary to this, total IgE remained stable in Finland but decreased significantly (P < 0.001) in Russia. Analyses based on years of birth revealed that the prevalence of sensitization to allergens increased with successive birth years in Finland, but remained stable in Russia. Over the 10 years, self‐reported physician‐diagnosed asthma increased from 5.5% to 8.1% (P = 0.05) and hay fever from 8.1% to 13.2% (P < 0.001) in Finland.
Conclusions: Disparities in the prevalence of atopy and atopic disease between Finnish and Russian Karelia have further grown. The ‘allergy epidemic’ continues in Finland and is mainly attributable to the years of birth effect shown in atopy prevalence. In Russia, no signs of the epidemic are discernible, although the decrease in total IgE may indicate a change in environmental exposure.
► We analyze stellar occultations by Titan observed by the Cassini/UVIS instrument. ► We derive density and temperature profiles for the upper atmosphere of Titan. ► We derive extinction profiles for ...Titan aerosols near their production region. ► We find evidence for the presence of low frequency gravity waves in the atmosphere. ► The results are compared with previous Voyager and Cassini measurements.
Stellar occultations observed by the Cassini/UVIS instrument provide unique data that probe the mesosphere and lower thermosphere of Titan at altitudes between 400 and 1400km. This region is a site of complex photochemistry that forms hydrocarbon and nitrile species, and plays a crucial role in the formation of the organic hazes observed in the stratosphere, but has yet to be adequately characterized. We analyzed publicly available data obtained between flybys Tb in December 2004 and T58 in July 2009, with an emphasis on two stable occultations obtained during flybys T41 and T53. We derived detailed density profiles for CH4, C2H2, C2H4, C4H2, HCN, HC3N and C6H6 between ∼400 and 1200km and extinction coefficients for aerosols between 400 and 900km. Our analysis reveals the presence of extinction layers in the occultation data that are associated with large perturbations in the density profiles of the gaseous species and extinction profiles of the aerosols. These relatively stable features vary in appearance with location and change slowly over time. In particular, we identify a sharp extinction layer between 450 and 550km that coincides with the detached haze layer. In line with recent images obtained by Cassini/ISS, the altitude of this layer changes rapidly around the equinox in 2009. Our results point to unexpected complexity that may have significant consequences for the dynamics and physical processes taking place in the upper atmosphere of Titan.
Intramammary infection (IMI), also known as mastitis, is the most frequently occurring and economically the most important infectious disease in dairy cattle. This study provides a validation of the ...analytical specificity and sensitivity of a real-time PCR-based assay that identifies 11 major pathogen species or species groups responsible for IMI, and a gene coding for staphylococcal β-lactamase production (penicillin resistance). Altogether, 643 culture isolates originating from clinical bovine mastitis, human, and companion animal samples were analyzed using the assay. The isolates represented 83 different species, groups, or families, and originated from 6 countries in Europe and North America. The analytical specificity and sensitivity of the assay was 100% in bacterial and β-lactamase identification across all isolates originating from bovine mastitis (n=454). When considering the entire culture collection (including also the isolates originating from human and companion animal samples), 4 Streptococcus pyogenes, 1 Streptococcus salivarius, and 1 Streptococcus sanguis strain of human origin were identified as Streptococcus uberis, and 3 Shigella spp. strains were identified as Escherichia coli, decreasing specificity to 99% in Strep. uberis and to 99.5% in E. coli. These false-positive results were confirmed by sequencing of the 16S rRNA gene. Specificity and sensitivity remained at 100% for all other bacterial targets across the entire culture collection. In conclusion, the real-time PCR assay shows excellent analytical accuracy and holds much promise for use in routine bovine IMI testing programs. This study provides the basis for evaluating the assay's diagnostic performance against the conventional bacterial culture method in clinical field trials using mastitis milk samples.
Extremely irradiated, close-in planets to early-type stars might be prone to strong atmospheric escape. We review the literature showing that X-ray-to-optical measurements indicate that for ...intermediate-mass stars (IMSs) cooler than 8250 K, the X-ray and extreme-ultraviolet (XUV) fluxes are on average significantly higher than those of solar-like stars, while for hotter IMSs, because of the lack of surface convection, it is the opposite. We construct spectral energy distributions for prototypical IMSs, comparing them to solar. The XUV fluxes relevant for upper-planet atmospheric heating are highest for the cooler IMSs and lowest for the hotter IMSs, while the ultraviolet (UV) fluxes increase with increasing stellar temperature. We quantify the influence of this characteristic of the stellar fluxes on the mass loss of close-in planets by simulating the atmospheres of planets orbiting EUV-bright (WASP-33) and EUV-faint (KELT-9) A-type stars. For KELT-9b, we find that atmospheric expansion caused by heating due to absorption of the stellar UV and optical light drives mass-loss rates of 1011 g s−1, while heating caused by absorption of the stellar XUV radiation leads to mass-loss rates of 1010 g s−1, thus underestimating mass loss. For WASP-33b, the high XUV stellar fluxes lead to mass-loss rates of 1011 g s−1. Even higher mass-loss rates are possible for less massive planets orbiting EUV-bright IMSs. We argue that it is the weak XUV stellar emission, combined with a relatively high planetary mass, which limit planetary mass-loss rates, to allow the prolonged existence of KELT-9-like systems.
Context.
Several observational and theoretical results indicate that the atmospheric temperature of the ultra-hot Jupiter KELT-9b in the main line formation region is a few thousand degrees higher ...than predicted by self-consistent models.
Aims.
Our aim was to test whether non-local thermodynamic equilibrium (NLTE) effects are responsible for the presumably higher temperature.
Methods.
We employed the Cloudy NLTE radiative transfer code to self-consistently compute the upper atmospheric temperature-pressure (TP) profile of KELT-9b, assuming solar metallicity and accounting for Roche potential. In the lower atmosphere, we used an updated version of the HELIOS radiative-convective equilibrium code to constrain the Cloudy model.
Results.
The Cloudy NLTE TP profile is ≈2000 K hotter than that obtained with previous models assuming LTE. In particular, in the 1–10
−7
bar range the temperature increases from ≈4000 to ≈8500 K, remaining roughly constant at lower pressures. We find that the high temperature in the upper atmosphere of KELT-9b is driven principally by NLTE effects modifying the Fe and Mg level populations, which strongly influence the atmospheric thermal balance. We employed Cloudy to compute LTE and NLTE synthetic transmission spectra on the basis of the TP profiles computed in LTE and NLTE, respectively, finding that the NLTE model generally produces stronger absorption lines, particularly in the ultraviolet, than the LTE model (up to 30%). We compared the NLTE synthetic transmission spectrum with the observed H
α
and H
β
line profiles obtaining an excellent match, thus supporting our results.
Conclusions.
The NLTE synthetic transmission spectrum can be used to guide future observations aiming at detecting features in the KELT-9b transmission spectrum. Metals, such as Mg and Fe, and NLTE effects shape the upper atmospheric temperature structure of KELT-9b, and thus affect the mass-loss rates derived from it. Finally, our results call for checking whether this is the case also for cooler planets.
Abstract
Joint effusion due to elbow fractures are common among adults and children. Radiography is the most commonly used imaging procedure to diagnose elbow injuries. The purpose of the study was ...to investigate the diagnostic accuracy of deep convolutional neural network algorithms in joint effusion classification in pediatric and adult elbow radiographs. This retrospective study consisted of a total of 4423 radiographs in a 3-year period from 2017 to 2020. Data was randomly separated into training (n = 2672), validation (n = 892) and test set (n = 859). Two models using VGG16 as the base architecture were trained with either only lateral projection or with four projections (AP, LAT and Obliques). Three radiologists evaluated joint effusion separately on the test set. Accuracy, precision, recall, specificity, F1 measure, Cohen’s kappa, and two-sided 95% confidence intervals were calculated. Mean patient age was 34.4 years (1–98) and 47% were male patients. Trained deep learning framework showed an AUC of 0.951 (95% CI 0.946–0.955) and 0.906 (95% CI 0.89–0.91) for the lateral and four projection elbow joint images in the test set, respectively. Adult and pediatric patient groups separately showed an AUC of 0.966 and 0.924, respectively. Radiologists showed an average accuracy, sensitivity, specificity, precision, F1 score, and AUC of 92.8%, 91.7%, 93.6%, 91.07%, 91.4%, and 92.6%. There were no statistically significant differences between AUC's of the deep learning model and the radiologists (
p
value > 0.05). The model on the lateral dataset resulted in higher AUC compared to the model with four projection datasets. Using deep learning it is possible to achieve expert level diagnostic accuracy in elbow joint effusion classification in pediatric and adult radiographs. Deep learning used in this study can classify joint effusion in radiographs and can be used in image interpretation as an aid for radiologists.
Earlier research is lacking on the prevalence and nature of objective working hour characteristics in the retail sector. We developed a codification in the retail sector and investigated differences ...in objective working hour characteristics by part-time work, sex and age. The payroll-based registry data of objective working hours consisted >12,000 employees of the retail sector in Finland for 2018–2020. Descriptive statistics for means, standard deviations (SD) and range of annual working hour characteristics were calculated, the differences in means were tested, partially based on the protocol established for health care sector. The final sample had 60–63% part-time employees and 23% men. Morning shifts were more frequent (48–51%) among full-time employees compared to 27–30% of the part-time employees. Evening shifts, 43–46%, were frequent among part-time employees vs. 26% in full-time. No sex differences were detected, and age group differences only among part-time employees. To conclude, the codification for registry-based working hour data enables us to identify individual differences in working hour characteristics. The working hour characteristics differed between part-time and full-time employees, not between sexes, whereas age differences were minor and among part-time employees. The codification could be used in studies of the retail sector in association with health and wellbeing.
We present an empirical model of H2 densities and temperatures above the homopause for the Saturn thermosphere, based on stellar occultations observed by the Cassini/UVIS instrument. These ...observations span the whole Cassini mission and probe a wide range of latitudes, longitudes and local times. Although the model is based on Cassini stellar occultations only, the results agree well with Voyager occultations and reasonably well with other measurements of temperature and density in the thermosphere. The principal meridional trend that the model captures is an increase in temperature with latitude away from the equator, associated with significantly elevated density level altitudes at latitudes higher than about 40° in both hemispheres. These results confirm the importance of auroral heating in Saturn’s thermosphere and constrain models of global circulation. We estimate the uncertainty in the model based on the standard deviation of the data points around the mean state. We do not attempt to distinguish retrieval errors from spatial and temporal trends. We find that the most important source of uncertainty is an apparent semi-annual variation in H2 density and temperature, associated with expansion and contraction of the pressure levels in the thermosphere. Similar variation occurs in the Earth’s thermosphere where it is likely driven by seasonal changes to global circulation. The model presented here is a useful tool that constrains physical models, helps to interpret existing observations, and supports the planning of future observations and missions.

There is little information about the associations of intakes of cholesterol and eggs, a major source of dietary cholesterol, with the risk of cognitive decline in general populations or in carriers ...of apolipoprotein E ɛ4 (APO-E4), a major risk factor for dementia.
We investigated the associations of cholesterol and egg intakes with incident dementia, Alzheimer disease (AD), and cognitive performance in middle-aged and older men from Eastern Finland.
A total of 2497 dementia-free men, aged 42-60 y in 1984-1989 at the baseline examinations of the prospective, population-based Kuopio Ischaemic Heart Disease Risk Factor Study, were included in the study. Information on the apolipoprotein E (Apo-E) phenotype was available for 1259 men. Data on cognitive performance tests at the 4-y re-examinations were available for 480 men. Dietary intakes were assessed with the use of 4-d food records at baseline. Dementia and AD diagnoses were based on Finnish health registers. Cox regression and ANCOVA were used for the analyses.
During the 21.9-y follow-up, 337 men were diagnosed with dementia, and 266 men were diagnosed with AD. Neither cholesterol nor egg intake was associated with a higher risk of incident dementia or AD. For example, when evaluated continuously, each intake of 100 mg cholesterol/d was associated with a multivariable-adjusted HR of 0.90 (95% CI: 0.79, 1.02) for incident dementia, and each additional 0.5 egg (27 g)/d was associated with an HR of 0.89 (95% CI: 0.78, 1.01). However, egg intake was associated with better performance on neuropsychological tests of the frontal lobe and executive functioning, the Trail Making Test, and the Verbal Fluency Test. The Apo-E4 phenotype did not modify the associations of cholesterol or egg intake (P-interactions > 0.11).
Neither cholesterol nor egg intake is associated with an increased risk of incident dementia or AD in Eastern Finnish men. Instead, moderate egg intake may have a beneficial association with certain areas of cognitive performance.
Purpose
To assess the healing of costal cartilage fractures (CCFX) in patients with blunt polytrauma with follow-up imaging and clinical examination. Effect on physical performance and quality of ...life (QoL) was also evaluated.
Methods
The study group comprised twenty-one patients with diagnosed CCFX in trauma CT. All the patients underwent MRI, ultrasound, ultra-low-dose CT examinations, and clinical status control. The patients completed QoL questionnaires. Two radiologists evaluated the images regarding fracture union, dislocation, calcifications, and persistent edema at fracture site. An attending trauma surgeon clinically examined the patients, with emphasis on focal tenderness and ribcage mobility. Trauma registry data were accessed to evaluate injury severity and outcome.
Results
The patients were imaged at an average of 34.1 months (median 36, range 15.8–57.7) after the initial trauma. In 15 patients (71.4%), CCFX were considered stable on imaging. Cartilage calcifications were seen on healed fracture sites in all the patients. The fracture dislocation had increased in 5 patients (23.8%), and 1 patient (4.8%) showed signs of a non-stable union. Four patients (19.0%) reported persistent symptoms from CCFX.
Conclusion
Non-union in CCFX is uncommon but may lead to decreased stability and discomfort. Both clinical and radiological examinations play an important part in the post-traumatic evaluation of CCFX. CT and MRI visualize the healing process, while dynamic ultrasound may reveal instability. No significant difference in QoL was detected between patients with radiologically healed and non-healed CCFX. Post-traumatic disability was mostly due to other non-thoracic injuries.
