Numerous studies have demonstrated that the levels of cyclooxygenase (COX), the enzyme that catalyzes the conversion of arachidonic acid to prostaglandin H
2, and of prostaglandins are higher in ...various tumors and cells during inflammation than in normal tissues. The aim of the present study was to analyze whether COX-2 isoform expression was noticeably higher in fine-needle aspirates (FNA) from breast carcinoma than in FNA from fibroadenoma and fibrocystic breast tissue. COX-2 expression was detected by immunocytochemical (IC) staining and was analyzed by microscopic scoring and computer gray-scale analysis. Evaluation of COX-2 IC positivity in FNA from three groups of patients (nine with breast carcinoma, nine with fibroadenoma, eight with fibrocystic breasts) revealed high COX-2 IC positivity in the majority of patients with breast carcinoma and low or absent COX-2 IC positivity in patients with fibrocystic breast changes. In addition, low or medium COX-2 IC positivity was found in the majority of patients with fibroadenoma, only three of these patients having high COX-2 IC positivity.
Extracorporeal shock wave lithotripsy (ESWL) treatment has been used at Department of Urology, University Hospital "Osijek", Croatia, since July 1988. Until December 2001 seven thousand and eight ...hundred patients underwent ESWL for urinary stones, 68 of them were children (0.87%). Sixty-eight children aged 4 to 15 years (average 10.14 years) underwent ESWL. They were treated for the total of 91 stones: 35 (38.46%) caliceal, 23 (25.27%) in pyelon, 7 (7.69%) in pyeloureteric segment and 14 (15.38%) ureteral. Staghorn calculi were found in 6 (6.59%) patients and multiple stones (four or more stones in the same kidney) in 6 (6.59%). There was total of 95 ESWL sessions performed in 68 patients (1.39 session per patient). Fifty-six patients (82.35%) without residual stones found at the control plain film and sonography of urinary tract were considered "stone free". Addition of 5 patients with clinically insignificant residual fragments (less than 4 mm) increases overall success rate to 89.70%. ESWL is a simple, safe and effective procedure in the management of urolithiasis in childhood. Clinical experience of our institution confirms ESWL as the first line treatment for kidney stones in the pediatric age patients.
Ewing Sarcoma is the second most common bone tumor in children and young adults. Primary vertebral Ewing sarcoma (ES)/primary neuroectodermal tumor (PNET) is very rare, especially with intraspinal ...infiltration. Most cases of ES contain somatic translocation t(11;22) (q24;q12). We present a case of a boy who was diagnosed with Non Hodgkin lymphoma (B immunophenotype) of the stomach at the age of 12. He was treated according to BFM 90 protocol. Complete remission (CR) was achieved. At the age of 17 he was admitted to our department because of the anemia, thrombocytopenia, fatigue and hypertrophy of the gums. The diagnosis of acute myeloid leukemia (AML-M4) was established cytomorfologically while immunophenotypic analysis suggested pro T acute lymphoblastic leukemia (proT-ALL) with co-expression of myeloid markers (due to presence CD2+,CD3-,citCD3+,CD7+,TdT+) with 60% of CD 117 marker. Cytogenetic analysis revealed complex chromosome abnormalities (46XY,t(q26;q27),del(7q22),t(8;19)(q23;p12),der(18q). Polymerase chain reaction (PCR) testing for the most common mutations was negative. He received induction therapy with Ara-C and idarubicine. CR was achieved and consolidation therapy (NOVIA) followed. Allogenic bone marrow transplantation (allo-BMT) from the HLA identical family donor was performed after standard conditioning regimen (Busulfan iv and Endoxan) in February 2007 with three lineage engraftment and 100% chimera on the lymphocytes and granulocytes of the peripheral blood. Eight months after allo-BMT he presented with paraparesis of the lower extremities. Magnetic resonance imaging showed massive intraspinal infiltration with tumorous tissue from L4 to S1. An operation proceeded and histologically a small-blue-cell tumor (CD99 positive) was observed, indicating ES/PNET. Immunophenotypic analysis of bone marrow aspirate revealed CD45-CD90+CD117+ immunophenotype typical for tumors of neuroectodermal origin. No leukemia or lymphoma cells were found. Cytogenetic analysis showed complex pseudodiploid clone with numeric and structural abnormalities (46-47,XY+del(1p22),+2del(6q),t(11;22)(q24;q13 .3),-16cp10/46XY) of which translocation t(11;22) is characteristic for ES, although PCR analysis for (EWS/FLI1 type 1 and 2) was negative. Conclusion: We presented a case of 17-year -old patient with three malignant diseases in a five-year period, which is to our knowledge not described in the literature.
Cytomorphologic and cytochemical bone marrow analysis is essential in the diagnosis of acute leukemia. Immunophenotyping and conventional cytogenetics, just as fluorescent in situ hybridization ...(FISH) are other diagnostic procedures, as well as genome analysis by PCR (polymerase chain reaction). PCR is inevitable in searching for minimal residual disease, because it may detect very small amount of malignant hematopoietic cells even when a patient is in complete remission (less than 5% malignant cells in bone marrow and disappearance from peripheral blood) which helps better monitoring of patients. By in situ hybridization (ISH) it is possible to associate specific cell type with genome alteration, but the method is not sensitive enough. By combining ISH and PCR a novel technique with increased sensitivity was developed, PCR in situ, which enables nucleic acid amplification in an intact cell. In this case report we present two patients whose bone marrow aspirates were analyzed also by PCR in situ.
During 18 months of the 1991/92 war against Croatia, 4,545 injured were treated at the Department of Surgery of Osijek Clinical Hospital. Some, 2,544 (55.9%) sustained shell fragment injuries and 807 ...(17.8%) bullet injuries. Shell fragments lead to a higher number of polytraumatized patients than did bullets (6.8% versus 5.3%). The postoperative complication rate for fragment injury (5.8%) was significantly higher than that for bullet injury (4.1%). Mortality rate did not differ significantly (3.9% versus 4.5% respectively).

During the war in Croatia, the surgical department of Osijek Hospital was the place of primary and definitive surgical management of the wounded from the city of Osijek and the entire northeastern ...region of Croatia. Our surgical experience with the management of war injuries of major extremity vessels, collected from May 2, 1991, through April 12, 1992, is reviewed. During that time, 60 wounded were admitted to the hospital with injuries of major blood vessels of the extremities, all of them directly from the battlefield or from the streets of Osijek. The method of surgical treatment depended on the type of vascular injury and associated injuries. Our methods were arterial injury reconstruction and monitoring of the early results when autogenous vein grafts or synthetic prostheses were used. The results indicate that one should not resist using synthetics for reconstruction of arterial injuries in a highly contaminated area.
