Introducción: La nutrición oral del prematuro no es posible hasta la adquisición del reflejo de la succión-deglución-respiración. Su retraso prolonga la estancia hospitalaria y aumenta la incidencia ...de trastornos oro-motores en los primeros años de la infancia. Objetivos: Analizar la transición de la nutrición enteral a la oral, la comorbilidad asociada con su retraso y el impacto de una estimulación precoz de la succión en una cohorte de prematuros. Pacientes y métodos: Revisión retrospectiva de 95 prematuros menores de 32 semanas de gestación (SG) ingresados en una UCI neonatal en los últimos 4 años. Se revisó la edad gestacional, somatometría al nacimiento y al alta, comorbilidad asociada, duración de la ventilación mecánica, necesidades de oxígeno, momento de inicio/fin de nutrición enteral/oral, inicio del método canguro y estimulo de la succión y ganancia ponderal media diaria. Resultados: La estimulación de la succión se inició entre la semana 29 y 40 SG (media y mediana 32 SG). La nutrición oral se inició entre la 31-40 SG (media y mediana 33SG) y completó entre la 33-44 SG (media y mediana 35 SG). La nutrición oral se retrasó en los pacientes que necesitaron más tiempo de ventilación mecánica y oxigenoterapia. Hubo una correlación positiva entre el inicio de la estimulación de la succión y el momento de adquisición de una nutrición oral completa (84% test de correlación de Spearman) y el tiempo de estancia hospitalaria (80% test de correlación de Spearman). Conclusiones: El estímulo precoz de la succión en el paciente pretérmino parece facilitar la nutrición oral completa de manera precoz, asociándose a una disminución en la estancia hospitalaria y a una mejora en la ganancia ponderal media diaria.Introduction: Premature baby's oral feeding is not possible until the reflex of sucking-swallowing-breathing adquisition. Its delay extends hospital stay and increases the incidence of oral motor disorders in early childhood. Aims: To analyze the transition from enteral to oral nutrition, the comorbidity associated with its delay and the impact of an early suction stimulation in a cohort of premature babies. Patients and methods: Retrospective checking of 95 infants less than 32 gestation weeks (GW) admitted to a neonatal ICU in the last 4 years. It was revised the gestational age, anthropometric at birth and discharge, comorbidity, duration of mechanical ventilation, oxygen requirements, time of beginning and end of enteral/oral nutrition, beginning of Kangaroo method and the suction stimulation and the daily weight gain average. Results: Suction stimulation began between weeks 29 and 40 GW (average and median 32 GW). Oral nutrition was initiated between 31-40 GW (average and median 33 GW) and completed between 33-44 GW (average and median 35 GW). Oral nutrition was delayed in patients who required longer mechanical ventilation and oxygen therapy. There was a positive correlation between the beginning of suction stimulation and the time of acquisition of a complete oral nutrition (84% Spearman correlation test) and length of hospital stay (80% Spearman correlation test). Conclussions: Early suction stimulation in a preterm patient seems to facilitate full oral nutrition at an early stage and it is associated with a hospital stay decrease and the improvement in the daily weight gain average.
Objectives
The rate of eosinophilic esophagitis (EoE) diagnosis is increasing. This study aims to determine the incidence of EoE in the pediatric population residing in the southwestern Madrid and to ...analyze whether absolute monthly pollen counts, modified or not by the principal atmospheric pollutants, are associated with it.
Methods
A cross‐sectional study on prospectively recruited patients was designed to calculate the incidence of EoE in children aged under 15 years who were diagnosed between September 2014 and August 2016 in twelve hospitals.
We collected demographic and symptoms data, date of onset of symptoms, date of medical consultation, and date of endoscopic diagnosis of each included patient. Relative risk estimation was performed to assess the association between the incidence of diagnosis and monthly pollen counts and levels of atmospheric pollutants. All these models were adjusted for the number of total patients that underwent endoscopy at first time.
Results
One hundred forty‐eight patients were included. The most frequent symptoms were abdominal pain 42.57%, dysphagia 42.57%, and impaction 39%–86%. The median overall monthly incidence was 1.13 interquartile rank: 0.97–1.43 cases/100,000 children, and the annual mean was 15.2. The overall analysis of the relationship between incidence and absolute monthly counts, corrected for the number of first‐time endoscopies performed, revealed no statistically significant association with pollen and air pollutants. There was a higher frequency of diagnosis during the pollination period of Cupressaceae relative risk 1.647; 95% CI (1.192–2.276) p < .002 and during February and November (relative risk 1.67; p < .01).
Conclusions
This study confirms the high incidence of eosinophilic esophagitis and also suggests a period of higher incidence of diagnosis in the months of February and November as well as in the period of high pollination of Cupressaceae.
Purpose To examine the intrasession and intersession reliability and the absolute reliability of three functional dynamic tests—forward-lunge, step-up-over and sit-to-stand tests—using computerized ...dynamic posturography. Subjects and Methods An intra-test and test-retest, repeated measure study was designed. Forty-five healthy subjects twice carried out the forward-lunge test, step-up-over test, and sit-to-stand test on two days, one week apart. The intrasession and intersession reliabilities as judged by the intraclass correlation coefficient (ICC) and the minimal detectable change of the three functional tests were calculated. Results Excellent to very good intrasession reliability of the forward-lunge test (ICC range of 0.9–0.8) was found. Very good to good intrasession reliability of the step-up-over test (ICC range of 0.9–0.5) was found and very good intrasession reliability of the sit-to-stand test (ICC range of 0.8–0.7) was found. The minimal detectable change at the 95% confidence level of most of the measures was lower than 30%. Conclusion The forward-lunge, step-up-over and sit-to-stand tests are reliable measurement tools.
La infección por Helicobacter pylori (H. pylori) afecta a más del 50% de la población mundial. El aumento en las resistencias antibióticas es la principal causa del fracaso del tratamiento. El ...objetivo principal fue analizar el éxito erradicador tras la aplicación de las nuevas recomendaciones de tratamiento ESPGHAN e introducción de la PCR como técnica de diagnóstico directo, describir la evolución del patrón local de resistencias antibióticas y valorar el coste-efectividad de la aplicación de la PCR aislada o en conjunto con el cultivo como estrategia diagnóstica.

Se incluyó a 192 pacientes, 98 fueron detectados por cultivo (2013-2016) y 94 por cultivo o PCR (2017-2019). Se instauró tratamiento antibiótico en 153 pacientes, 90 en el primer periodo (pautas ESPGHAN 2011: porcentaje erradicación 62,2%), 63 en el segundo (pautas ESPGHAN 2017: porcentaje erradicación: 73%). Se observó un aumento en las resistencias a claritromicina, pasando de un 16,3% (n=16) en el primer periodo a un 53,2% (n=48) entre 2017-2019 (98% detectadas por PCR, 60% por cultivo). No hubo diferencias en el resto de resistencias antibióticas. La solicitud aislada de la PCR presentó una ratio de análisis de coste-efectividad (CEAR) de 71,91, en comparación con un 92,16 del cultivo y un 96,35 del cultivo y la PCR de forma conjunta.
La aplicación de las pautas ESPGHAN 2017 consiguió un mayor éxito de erradicación, aunque menor que lo observado en publicaciones previas, sin llegar al objetivo marcado de al menos un 90%. Se observó un incremento en las resistencias a macrólidos, sin poder discriminar si se trata de un aumento real o de una mayor sensibilidad diagnóstica de las técnicas moleculares, siendo la solicitud aislada de la PCR la estrategia más coste efectiva.

La esofagitis eosinofílica es una enfermedad emergente, crónica, mediada por el sistema inmune y caracterizada por síntomas de disfunción esofágica e inflamación con infiltración eosinofílica aislada ...en el esófago. Es más frecuente en varones y en sujetos atópicos y los síntomas varían con la edad: en niños pequeños se manifiesta con vómitos, dolor abdominal y problemas con la alimentación y en niños mayores y adolescentes con disfagia e impactación alimentaria. El diagnóstico se basa en la presencia de síntomas e inflamación esofágica con ≥ 15 eosinófilos/campo de gran aumento, tras descartar otras causas de eosinofilia esofágica. Sin tratamiento, la enfermedad suele persistir y puede evolucionar a formas fibroestenóticas más frecuentes en el adulto. Las opciones terapéuticas incluyen inhibidores de la bomba de protones, dieta de eliminación empírica y corticoides deglutidos. Tras el tratamiento de inducción es aconsejable la terapia de mantenimiento. La dieta es el único tratamiento que se dirige a la causa de la enfermedad, al identificar los alimentos desencadenantes. La respuesta a los tratamientos requiere la evaluación histológica, por la escasa concordancia entre los síntomas y la inflamación esofágica.
El manejo práctico de la esofagitis eosinofílica presenta desafíos debido, entre otras causas, a la falta de disponibilidad actual de fármacos específicos y a su abordaje con tratamientos dietéticos, en ocasiones, complejos. El presente documento, elaborado por el Grupo de Trabajo de Trastornos Gastrointestinales Eosinofílicos de la Sociedad Española de Gastroenterología, Hepatología y Nutrición Pediátricas, tiene como objetivo facilitar el abordaje diagnóstico y terapéutico de la esofagitis eosinofílica pediátrica, con base en las recientes guías de consenso basadas en la evidencia.
Eosinophilic oesophagitis is an emerging and chronic disorder mediated by the immune system, and is characterised by symptoms of oesophageal dysfunction and inflammation with isolated eosinophil infiltration in the oesophagus. It is more common in males and in atopic subjects, and the symptoms vary with age. In younger children, there is vomiting, abdominal pain and dietary problems, with dysphagia and food impaction in older children and adolescents. The diagnosis is based on the presence of symptoms and oesophageal inflammation with ≥ 15 eosinophils / high power field, and after ruling out other causes of oesophageal eosinophilia. Without treatment, the disease usually persists and can progress to fibrostenotic forms more common in adults. The treatment options included proton pump inhibitors, empirical elimination diets, and swallowed topical corticosteroids. Maintenance therapy is advisable after the induction treatment. Diet is the only treatment that is directed at the cause of the disease, on identifying the triggering food or foods. The response to the treatments requires a histological assessment due to the poor agreement between the symptoms and the oesophageal inflammation.
The practical management of Eosinophilic oesophagitis presents with challenges, due to, among other causes, the current lack of availability of specific drugs, and to its approach with, occasionally complex, diet treatments. The present document, prepared by the Working Group on Eosinophilic Gastrointestinal Disorders of the Spanish Society of Paediatric Gastroenterology, Hepatology and Nutrition, has as its objective to help in the diagnostic and therapeutic approach to paediatric eosinophilic oesophagitis, based on the recent evidence-based consensus guidelines.

Las recomendaciones de introducción de la alimentación complementaria han variado paralelamente al incremento de la incidencia de enfermedad celíaca, alergias e intolerancias alimentarias y de ...obesidad infantojuvenil, base de muchas enfermedades crónicas del adulto. Existen controversias sobre cuándo es el momento idóneo de inicio, qué alimento es el más adecuado y cuál es la forma de presentar esos alimentos al lactante para mejorar el comportamiento alimentario. No existen guías específicas para lactantes con alto riesgo alérgico o prematuros. El baby-led weaning se ha propuesto como una manera relajada para incorporar al lactante al patrón familiar de alimentación. La evidencia actual apoya que la respuesta a todas estas preguntas se encuentra en el grado de maduración y necesidades específicas de cada lactante; sin embargo, la controversia está servida.

We use, olive cake and tomato peel (55% in level incorporation) added to Iberian pigs compound feed, during 28 days -plus 8 days in adaptation- in growing phase (from 72.6 kg to 104.4 kg of live ...weight). At the final experimental periods we obtain average daily gain of 646 and 664 g/day and 6.1 kg and 5.9 kg of average daily feed intake to olive cake and tomato peel, respectively. The ingestion of olive cake increased the content oleic fatty acid (3.2 porcentuals points) and decreased saturated fatty acid and polyunsaturated fatty acid in adipose subcutaneous tissue. By contrast, tomato peel increased the linoleic fatty acid (1.9 porcentuals points) and decreased the monounsatured fatty acid in this tissue. It can be concluded that due to high feed conversion ratio (9.4 and 8.9 to olive cake and tomato peel, respectively), the use of these subproducts would be conditioned to the cheapening of the diet depending on the cost of products. Also, if the aim is achieve quality standards in terms of fatty acid composition (high level to oleic acid in adipose tissue), the olive cake in contrast to the tomato peel, it seems more appropriate for inclusion in the diet of Iberian pigs during the pre-fattening phase.
