Natriuretic peptides are promising markers in diagnosing acute and chronic heart failure and assessing prognosis in these patients. Increasing routine use to unselected patients is challenged by ...false-positive results. The aims of this study were to assess (a) the distributions of N-terminal B-type natriuretic peptide (NT-proBNP) values in various diagnostic groups, (b) factors that influence NT-proBNP, and (c) the value of NT-proBNP in risk stratification in unselected emergency room (ER) patients.
NT-proBNP was measured in 876 unselected consecutive patients mean (SD) age, 58 (18) years; 53% male attending the ERs of 2 university hospitals and 1 community hospital. Diagnoses, age, sex, hemoglobin, creatinine (CREA), C-reactive protein (CRP), troponin T, and intensity of care were documented. In a subset consisting of all 417 patients at 1 center, in-hospital follow-up was completed with respect to a complicated clinical course, including intensive care treatment and death.
NT-proBNP was significantly increased in patients with cardiac diagnoses or histories compared with patients with only pulmonary or other diagnoses. In patients with other diagnoses, NT-proBNP values increased significantly with the number of atherosclerotic risk factors (P=0.044). Age, renal function, CRP, and to a much lesser extent, hemoglobin significantly influenced NT-proBNP values. The amount of care was positively correlated with NT-proBNP (P<0.001). Classification and regression tree analysis showed a superior impact of NT-proBNP for identification of high-risk patients.
NT-proBNP is a promising marker for identification of patients with structural heart disease in the ER and a suitable tool for risk stratification. Its use in the ER should be limited to clearly clinically defined patient groups at present to avoid a potential excess of additional diagnostic procedures in positive but asymptomatic patients.

Our objective was to characterize monoclonal antiphospholipid antibodies (APL) and identify disease-associated antigens in patients with the antiphospholipid syndrome (APS). We used the monoclonal ...antibody HL-5B, derived from a patient with APS suffering from multiple ischemic events, to screen a 12-mer peptide phage display library (New England Biolabs, London, England). The identified phage clones were sequenced and the derived consensus peptide was synthesized. The peptide was used to perform competitive inhibition experiments for their ability to inhibit the binding of the monoclonal antibody and of serum antibodies to cardiolipin and phosphatidylserine. Additionally patients and control sera were screened for their binding reactivities to this peptide. Using this 12-mer phage display library the peptide APHKHKASLSIY as consensus peptide for the monoclonal antiphospholipid antibody HL-5B could be identified. In competitive inhibition studies we showed that this peptide is able to inhibit the binding of HL-5B to cardiolipin and phosphatidylserine and furthermore another antiphospholipid antibody used as control was also inhibited in its binding to phospholipids. Using 21 sera from APS patients 67% showed a binding to the peptide in a specific ELISA above the cutoff level, generated with sera from 20 healthy controls. Out of the reactive patients’ sera we used two exemplarily to perform inhibition studies. Both sera could be inhibited more than 40% in their binding to cardiolipin in a commercially available antiphospholipid antibody assay (Aescu.diagnostics, Wendelsheim, Germany). The identified peptide APHKHKASLSIY simulates the antigenic structure recognized from a subpopulation of serum antiphospholipid antibodies. This might indicate that the diversity of the antiphospholipid antibodies is limited and only few epitopes or few common structures are responsible for the development of those antibodies. Tests using these epitopes will strongly improve laboratory diagnosis of the APS.

Das Zentrum für ...Psychotherapie und Psychosomatik des Otto-Wagner-Spitals in Wien bietet innerhalb der Versorgungspsychiatrie ein elektives stationäres und tagesklinisches Behandlungsangebot mit dem Schwerpunkt Psychotherapie an.
Methodik
Im Zeitraum von Ende 2011 bis Beginn 2013 wurden 120 PatientInnen als Stichprobe eingeschlossen, von denen 88 (PatientInnengruppe
n
= 60; Kontrollgruppe
n
= 28) an der Zweittestung teilnahmen. Die Kontrollgruppe bestand aus einer Wartegruppe mit treatment as usual.
Ergebnisse
Es konnten signifikante Verbesserungen bezüglich der erhobenen Beeinträchtigungen im Prä-/Postvergleich für die PatientInnengruppe in allen eingesetzten Erhebungsinstrumenten erzielt werden, während sich für die Kontrollgruppe das ursprüngliche Niveau (mit Ausnahme der depressiven Symptomatik) unverändert zeigte: Für die PatientInnengruppe zeigte sich eine Abnahme der generellen psychischen Beeinträchtigung im Global Assessment of Functioning (GAF), ein Anstieg der physischen und psychischen gesundheitsbezogenen Lebensqualität im Short-Form Health Survey (SF-36), eine Reduktion von Depression im Beck-Depressions-Inventar II (BDI-II), eine Abnahme der allgemeinen psychischen Belastung im Brief Symptom Inventory (BSI-53) und eine deutliche Reduktion der Borderline-Symptomatik in der Kurzversion der Borderline Symptom Liste (BSL-23). Darüber hinaus zeigten die PatientInnen eine hohe Zufriedenheit mit der Behandlung (ZUF-8).
Schlussfolgerungen
Durch den Vergleich mit Ergebnissen anderer ähnlicher Untersuchungen kann postuliert werden, dass stationäre beziehungsweise tagesklinische Psychotherapie, wie sie am Zentrum für Therapie und Psychosomatik durchgeführt wird, eine hochwirksame Behandlung für PatientInnen mit schweren und komplexen psychosomatischen und psychiatrischen Erkrankungen ist.
