Seizures immediately preceding the occurrence of an ischemic or hemorrhagic stroke are a rare but well-documented phenomenon, for which the term "heraldic seizure" has been proposed. Cerebrovascular ...disease is the most common cause of epileptic seizures in elderly patients; thus, screening and management of vascular risk factors should be performed systematically in cases of late-onset epilepsy. MRI may help to distinguish heraldic seizure from stroke-elicited seizure by showing abnormalities confined to the cortex that spare vascular territories, increased magnetic resonance angiography flow in the ipsilateral cerebral arteries, and enhancement of the leptomeninges on postcontrast MRI. We present a case report that illustrates the difficulty of making the diagnosis of a heraldic seizure at onset.

Stroke is the most common cause of disability. Brain repair mechanisms are often insufficient to allow a full recovery. Stroke damage involve all brain cell type and extracellular matrix which represent the crucial “glio-neurovascular niche” useful for brain plasticity. Regenerative medicine including cell therapies hold great promise to decrease post-stroke disability of many patients, by promoting both neuroprotection and neural repair through direct effects on brain lesion and/or systemic effects such as immunomodulation. Mechanisms of action vary according to each grafted cell type: “peripheral” stem cells, such as mesenchymal stem cells (MSC), can provide paracrine trophic support, and neural stem/progenitor cells (NSC) or neurons can act as direct cells’ replacements. Optimal time window, route, and doses are still debated, and may depend on the chosen medicinal product and its expected mechanism such as neuroprotection, delayed brain repair, systemic effects, or graft survival and integration in host network. MSC, mononuclear cells (MNC), umbilical cord stem cells and NSC are the most investigated. Innovative approaches are implemented concerning combinatorial approaches with growth factors and biomaterials such as injectable hydrogels which could protect a cell graft and/or deliver drugs into the post-stroke cavity at chronic stages. Through main publications of the last two decades, we provide in this review concepts and suggestions to improve future translational researches and larger clinical trials of cell therapy in stroke.
Whether endovascular therapy (EVT) added on best medical management (BMM), as compared to BMM alone, is beneficial in acute ischemic stroke with isolated posterior cerebral artery occlusion is ...unknown.
We conducted a multicenter international observational study of consecutive stroke patients admitted within 6 hours from symptoms onset in 26 stroke centers with isolated occlusion of the first (P1) or second (P2) segment of the posterior cerebral artery and treated either with BMM+EVT or BMM alone. Propensity score with inverse probability of treatment weighting was used to account for baseline between-groups differences. The primary outcome was 3-month good functional outcome (modified Rankin Scale mRS score 0-2 or return to baseline modified Rankin Scale). Secondary outcomes were 3-month excellent recovery (modified Rankin Scale score 0-1), symptomatic intracranial hemorrhage, and early neurological deterioration.
Overall, 752 patients were included (167 and 585 patients in the BMM+EVT and BMM alone groups, respectively). Median age was 74 (interquartile range, 63-82) years, 329 (44%) patients were female, median National Institutes of Health Stroke Scale was 6 (interquartile range 4-10), and occlusion site was P1 in 188 (25%) and P2 in 564 (75%) patients. Baseline clinical and radiological data were similar between the 2 groups following propensity score weighting. EVT was associated with a trend towards lower odds of good functional outcome (odds ratio, 0.81 95% CI, 0.66-1.01;
=0.06) and was not associated with excellent functional outcome (odds ratio, 1.17 95% CI, 0.95-1.43;
=0.15). EVT was associated with a higher risk of symptomatic intracranial hemorrhage (odds ratio, 2.51 95% CI, 1.35-4.67;
=0.004) and early neurological deterioration (odds ratio, 2.51 95% CI, 1.64-3.84;
<0.0001).
In this observational study of patients with proximal posterior cerebral artery occlusion, EVT was not associated with good or excellent functional outcome as compared to BMM alone. However, EVT was associated with higher rates of symptomatic intracranial hemorrhage and early neurological deterioration. EVT should not be routinely recommended in this population, but randomization into a clinical trial is highly warranted.

Le processus styloïde est un os implanté sur la face externe de l’os temporal ayant plusieurs rapports anatomiques (ex : artère carotide interne). Le syndrome d’Eagle désigne les symptômes en lien ...avec les contraintes anatomiques de ce processus quand il est allongé.
Nous rapportons le cas d’un patient ayant présenté un infarctus cérébral en lien avec une sténose de l’artère carotide interne par un processus styloïde allongé. Un homme de 52 ans est admis aux urgences pour une hémiparésie droite et une aphasie. L’examen neurologique réalisé initialement retrouvait une plégie du bras droit, une parésie de la jambe droite, une paralysie faciale centrale droite, une aphasie fluente. Le score NIHSS était côté à 11. L’IRM cérébrale montre un hypersignal en séquence de diffusion et T2 FLAIR dans les territoires de l’artère sylvienne gauche et jonctionnel postérieur gauche (Fig. 1). L’angioscanner des troncs supra-aortiques a révélé une sténose de l’artère carotide interne gauche estimée à 70 % en lien avec un processus styloïde mesurant 5,7cm. Quatre mois avant cet épisode, un angioscanner des troncs supra-aortiques avait été réalisé dans un contexte de traumatisme cervical, les processus styloïdes droits et gauches étaient déjà allongés, mais sans compression des artères carotides internes (Fig. 2–4).
Ce cas est intéressant, car dans la littérature plusieurs accidents vasculaires cérébraux, en lien avec un processus styloïde allongé, sont décrits, mais le plus souvent en relation avec une dissection artérielle provoquée. Il n’y a que peu de sténoses carotidiennes qui ont été décrites. Chez notre patient, le reste du bilan étiologique s’est avéré normal. La topographie jonctionnelle de l’infarctus oriente vers un mécanisme hémodynamique. Un écho-doppler dynamique a été réalisé, mais n’a pas retrouvé de perturbations de flux position dépendante.
Ce cas met donc en lumière une présentation rare du syndrome d’Eagle. La prise en charge n’est pas clairement définie. Après discussions avec le patient et les chirurgiens, une styloïdectomie gauche par voie cervicale a été réalisée.
Les syndromes neurologiques paranéoplasiques affectant le système nerveux périphérique peuvent rarement être des atteintes motrices pures et peuvent mimer des pathologies plus fréquentes, comme la ...SLA.
Nous présentons le cas d’un patient de 63 ans qui a développé une tétraparésie d’installation rapide sur 24h, initialement attribuée à une myélopathie cervicarthrosique. Il présente un déficit moteur pur des quatre membres prédominant aux membres inférieurs, des fasciculations des cuisses, une camptocormie et une dysarthrie, avec des réflexes conservés, sans syndrome pyramidal. L’électroneuromyogramme est évocateur d’une atteinte du deuxième motoneurone avec des fasciculations des membres et du tronc et une diminution du potentiel global d’action musculaire aux membres inférieurs. L’analyse du liquide cérébrospinal (LCS) retrouve une pléïcoytose, une hyperprotéinorachie et des bandes oligoclonales spécifiques au LCS. Les anticorps anti-Hu sont positifs à taux élevés dans le sérum et le LCS. Le PET-scan montre une hyperfixation prostatique et ganglionnaire (médiastinale, sus-claviculaire droite et inguinale bilatérale). La biopsie ganglionnaire retrouve une métastase d’un adénocarcinome peu différencié (étude immunohistochimique négative). Des biopsies prostatiques retrouvent un adénocarcinome prostatique, traité par chimiothérapie et chirurgie. Des cures d’immunoglobulines IV ont permis une stabilité clinique avec un recul de 4 mois.
Les neuronopathies motrices pures secondaires à un syndrome paranéoplasique sont exceptionnellement décrites et peuvent être prises pour une sclérose latérale amyotrophique. Les syndromes paranéoplasiques neurologiques liés aux cancers prostatiques sont rares. La présence de deux cancers concomitants ne peut être exclue chez ce patient ; le cancer primitif de la lésion ganglionnaire ne peut être déterminé avec certitude.
Une neuronopathie motrice pure, aiguë ou subaiguë, doit faire rechercher un syndrome neurologique paranéoplasique.
