Every workday we wrestle with cumbersome and unintuitive technologies. Our response is usually "That's just the way it is." Even technology designers and workplace managers believe that certain ...technological changes are inevitable and that they will bring specific, unavoidable organizational changes. In this book, Paul Leonardi offers a new conceptual framework for understanding why technologies and organizations change as they do and why people think those changes had to occur as they did. He argues that technologies and the organizations in which they are developed and used are not separate entities; rather, they are made up of the same building blocks: social agency and material agency. Over time, social agency and material agency become imbricated--gradually interlocked--in ways that produce some changes we call "technological" and others we call "organizational." Drawing on a detailed field study of engineers at a U.S. auto company, Leonardi shows that as the engineers developed and used a a new computer-based simulation technology for automotive design, they chose to change how their work was organized, which then brought new changes to the technology.Each imbrication of the social and the material obscured the actors' previous choices, making the resulting technological and organizational structures appear as if they were inevitable. Leonardi suggests that treating organizing as a process of sociomaterial imbrication allows us to recognize and act on the flexibility of information technologies and to create more effective work organizations.
Objectives
To address the relationship between years lived with a disability (YLDs), prevalence and cost of neurological diseases, and to test whether there is a European North–South gradient for ...national health expenditure, disability, costs and prevalence of neurological diseases.
Materials and methods
Information on costs, prevalence and YLDs referred to 2010 were taken from the Study on the Cost of Disorders of the Brain and from the Global Burden of Disease study; data on health expenditure were taken from OECD reports. Selected conditions were as follows: brain tumours, stroke, dementia, Parkinson's disease, epilepsy, multiple sclerosis, migraine and tension‐type headache; selected countries were from North (Denmark, Finland, Norway, Sweden) and South (Greece, Italy, Portugal, Spain) Europe. The association between the variables for each condition was tested using Spearman's correlation; Wilcoxon signed ranks test was used to test North–South Europe differences.
Results
Correlations were largely non‐significant (except for stroke). YLDs and cost were generally lower in South‐European countries, and prevalence was lower in North‐European countries, but no significant differences were found.
Conclusions
Health expenditure, YLDs, costs and prevalence of neurological conditions were generally not correlated across the eight countries. A clear North–South gradient was found for health expenditures, and partially for YLDs, costs and diseases' prevalence. We hypothesized that this is a consequence of the expansion of morbidity of neurological conditions connected to ageing, that health and welfare systems of selected countries were not prepared to face.
Abstract
The use of instrumental tools for improving both the diagnostic accuracy and the prognostic soundness in patients with disorders of consciousness (DOC) plays an important role. However, the ...most recent international guidelines on DOC published by the American and the European Academies of Neurology and by the UK Royal College of Physicians contain heterogeneous recommendations on the implementation of these techniques in the clinical routine for both diagnosis and prognosis. With the present work, starting from the comparison of the DOC guidelines’ recommendations, we look for possible explanations behind such discrepancies considering the adopted methodologies and the reference health systems that could have affected the guidelines’ perspectives. We made a provocative argument about the need to find the most appropriate common methodology to retrieve and grade the evidence, increase the meta-analytic studies, and reduce the health policies that influence on the guidelines development that, in turn, should inform the health policies with the strongest scientific evidence.
Flow disruption with the WEB-DL device has been used safely for the treatment of wide-neck bifurcation aneurysms, but the stability of aneurysm occlusion after this treatment is unknown. This ...retrospective multicenter European study analyzed short- and midterm data in patients treated with WEB-DL.
Twelve European neurointerventional centers participated in the study. Clinical data and pre- and postoperative short- and midterm images were collected. An experienced interventional neuroradiologist independently analyzed the images. Aneurysm occlusion was classified into 4 grades: complete occlusion, opacification of the proximal recess of the device, neck remnant, and aneurysm remnant.
Forty-five patients (34 women and 11 men) 35-74 years of age (mean, 56.3 ± 9.6 years) with 45 aneurysms treated with the WEB device were included. Aneurysm locations were the middle cerebral artery in 26 patients, the posterior circulation in 13 patients, the anterior communicating artery in 5 patients, and the internal carotid artery terminus in 1 patient. Forty-two aneurysms were unruptured. Good clinical outcome (mRS < 2) was observed in 93.3% of patients at the last follow-up. Adequate occlusion (complete occlusion, opacification of the proximal recess, or neck remnant) was observed in 30/37 patients (81.1%) in short-term follow-up (median, 6 months) and in 26/29 patients (89.7%) in midterm follow-up (median, 13 months). Worsening of the aneurysm occlusion was observed in 2/28 patients (7.1%) at midterm follow-up.
The results suggest that the WEB endovascular treatment of wide-neck bifurcation aneurysms offers stable occlusion in a class of aneurysms that are historically unstable. Additionally, our data show that opacification of the WEB recess can be delineated from true neck or aneurysm remnants.
The goal of this study is to augment explanations of how newly implemented technologies enable network change within organizations with an understanding of when such change is likely to happen. ...Drawing on the emerging literature on technology affordances, the paper suggests that informal network change within interdependent organizational groups is unlikely to occur until users converge on a shared appropriation of the new technology's features such that the affordances the technology enables are jointly realized. In making the argument for the importance of shared affordances, this paper suggests that group-level network change has its most profound implications at the organization level when individuals use the same subset of a new information technology's features. To explore this tentative theory, we turn to a comparative, multimethod, longitudinal study of computer-based simulation technology use in automotive engineering. The findings of this explanatory case study show that engineers used the new technology for more than three months, during which time neither group experienced changes to their advice networks. Initially, divergent uses of the technology's features by engineers in both groups precluded them from being able to coordinate their work in ways that allowed them to structure their advice networks differently. Eventually, engineers in only one of the two groups converged on the use of a common set of the technology's features to enact a shared affordance. This convergence was necessary to turn the technology into a resource that could collectively afford group members the ability to compare their simulation outputs with one another and, in so doing, alter the content and structure of the group's advice network. The implications of these findings for the literatures on technology feature use, affordances, social networks, and post-adoption behaviors in organizations are discussed.
This paper compares two alternative theoretical foundations upon which the study of sociomateriality can be built: agential realism and critical realism. It begins by providing a brief overview of ...the sociomaterial perspective on organizational practices and considers why this perspective holds great appeal at this point in time. I then engage with Mutch's (2013) critique of the agential realist foundation upon which most current discussions of sociomateriality are constructed to highlight what practical problems are generated when authors attempt to map agential realism's philosophical discussion onto empirical phenomena. Next, I attempt to make explicit what Mutch leaves implicit in his paper: how building studies of sociomateriality on the theoretical foundation offered by critical realism can, potentially, overcome some of the practical problems created by a footing on agential realism. Finally, I push Mutch's arguments one step further to compare what practical consequences arise when researchers attempt to construct studies of sociomateriality on either of these two theoretical foundations. I suggest that there are important implications for what one can study, how one can study it, and how scholars can contribute to theory on technology and organizing based on the theoretical foundation they choose to build upon.
