W
-boson mass measurements performed by the ATLAS, LHCb, CDF, and D0 experiments is studied using a coherent framework with theory uncertainty correlations. The measurements are ...combined using a number of recent sets of parton distribution functions (PDF), and are further combined with the average value of measurements from the Large Electron–Positron collider. The considered PDF sets generally have a low compatibility with a suite of global rapidity-sensitive Drell–Yan measurements. The most compatible set is CT18 due to its larger uncertainties. ABSTRACT
PKS 1830 -211 is a γ-ray emitting, high-redshift (z =2.507 ± 0.002), lensed flat-spectrum radio quasar. During the period 2019 mid-February to mid-April, this source underwent a series of ...strong γ-ray flares that were detected by both AGILE-GRID (Gamma-Ray Imaging Detector) and Fermi Large Area Telescope (Fermi-LAT), reaching a maximum γ-ray flux of $F_{\rm E\gt 100\, MeV}\approx 2.3\times 10^{-5}$ photons cm−2 s−1. Here, we report on a coordinated campaign from both on-ground Medicina, Owens Valley Radio Observatory (OVRO), Rapid Eye Mount (REM), and Sardinia Radio Telescope (SRT) and orbiting facilities (AGILE, Fermi, INTEGRAL, NuSTAR, Swift, and Chandra), with the aim of investigating the multiwavelength properties of PKS 1830-211 through nearly simultaneous observations presented here for the first time. We find a possible break in the radio spectra in different epochs above 15 GHz, and a clear maximum of the 15 GHz data approximately 110 d after the γ-ray main activity periods. The spectral energy distribution shows a very pronounced Compton dominance (> 200) which challenges the canonical one-component emission model. Therefore, we propose that the cooled electrons of the first component are re-accelerated to a second component by, for example, kink or tearing instability during the γ-ray flaring periods. We also note that PKS 1830-211 could be a promising candidate for future observations with both Compton satellites e.g. enhanced ASTROGAM (e-ASTROGAM) and Cherenkov arrays Cherenkov Telescope Array Observatory (CTAO) which will help, thanks to their improved sensitivity, in extending the data availability in energy bands currently uncovered.
Men with erectile dysfunction (ED) frequently have a disproportionate burden of comorbid vascular disorders including atherosclerotic disease. We investigated whether scheduled tadalafil is better ...than on-demand (OD) in improving endothelium-dependent vasodilatation of cavernous arteries in men with ED and whether this effect is also exerted on markers of endothelial function. We did an open-label, randomized, crossover study including 20 male outclinic patients aged 18 years or older (mean age 54 years) who had at least a 3-month history of ED of any severity or etiology. Tadalafil (20 mg) on alternate days (ADs) or OD was administered for 4 weeks. Primary end points were variations of basal inflow (peak systolic velocity (PSV)) and flow-mediated dilatation (FMD) of cavernous arteries compared with baseline at penile Duplex ultrasound. Secondary end points were variations of Q13-SIEDY scores regarding morning erections and of markers of endothelial function, that is, vascular cell adhesion molecule (VCAM), intercellular cell adhesion molecule, endothelin-1 (ET-1), insulin and C-reactive protein (CRP). PSVs and FMD were higher after AD treatment when compared with OD and baseline, respectively (P=0.0001), and improvements were maintained from 2 weeks after discontinuation (P<0.005). Patients receiving tadalafil AD experienced a significant improvement of morning erections as compared to AD treatment (P<0.0001); ET1, VCAM and CRP showed a robust decrease after chronic vs OD regimes (P<0.05), with concomitant increase in insulin levels (P<0.05), without any variation in blood pressure and other laboratory parameters. Chronic but not OD tadalafil improves endothelial function with sustained effects from its discontinuation. Chronic treatment also produces a dramatic increase in morning erections, which determines better oxygenation to the penis, thus providing a rationale for vascular rehabilitation.
Premature ejaculation (PE) is thought to be the most common male sexual dysfunction; however, the prevalence of lifelong (LL)-PE is relatively low. The aim of this study was to investigate the ...effects of on-demand vardenafil (10 mg) to modify the intravaginal ejaculatory latency time (IELT) in men with LL-PE without erectile dysfunction. Forty-two men (18-35 years) were enrolled in a 16-week, double-blind, placebo-controlled, cross-over study. Primary end point was the modification from baseline of IELT assessed by stopwatch technique; secondary end points were post-ejaculatory refractory time (PERT) and variations of scores at the Index of Premature Ejaculation questionnaire. The changes in geometric mean IELT were superior after taking vardenafil (0.6+/-0.3 vs 4.5+/-1.1 min, P<0.01), compared with placebo (0.7+/-0.3 vs 0.9+/-1.0 min, ns). PERT dropped significantly after vardenafil (16.7+/-2.0 vs 4.3+/-0.9 min, P<0.001), compared with placebo (15.3+/-2.2 vs 15.8+/-2.3 min). Patients who took vardenafil (vs placebo) reported significantly (P<0.01) increased ejaculatory control (6+/-2 vs 16+/-2), improved overall sexual satisfaction (7+/-2 vs 15+/-1) and distress (4+/-1 vs 8+/-1) scores, respectively. Multiple regression analysis (r(2)=0.86) for IELT by the number of attempts at sexual intercourse showed significant differences between the slopes of lines for placebo and vardenafil (P<0.0001). The most common adverse events for vardenafil (vs placebo) were headache (10 vs 3%), flushing (12 vs 0%) and dyspepsia (10 vs 0%), which tended to disappear over the time. In conclusion, in our study, vardenafil increased IELT and reduced PERT in men with LL-PE. Besides, improvements in confidence, perception of ejaculatory control and overall sexual satisfaction were reported.
Lumbar canal stenosis (LCS) is a condition that can potentially cause disability. It often occurs in aging populations. The aim of this study was to analyze the correlation between the clinical ...outcomes of postoperative patients and classifications that were based on MRI assessments. This prospective cohort study was carried out at Cipto Mangunkusumo General Hospital from January to July 2016 using consecutive sampling. Thirty-eight patient samples were obtained, all of whom were managed with the same surgical technique of decompression and posterior stabilization. The patients were categorized in four types based on MRI examination using the Schizas classification. Pre- and post-treatment (three months and six months) assessments of the patients were conducted according to Visual Analogue Scale (VAS), the Oswestry Disability Index (ODI), the Japanese Orthopedic Association Score (JOA), and the Roland-Morris Disability Questionnaire (RMDQ). The statistical analysis was performed using the statistical program for social science (SPSS) v.19. The average age of the patients in this sample was 58.92 years (range 50-70 years). There were 16 males and 22 females. Most patients were classified as type C (21 subjects) based on MRI examination. The improvement in the clinical scores of male subjects was better than in the female subjects. Significant differences were found in the six-month postoperative VAS (p = 0.003) and three-month postoperative JOA scores (p = 0.029). The results at follow-up showed that the VAS, ODI, JOA and RMDQ scores were improved. There were no statistical differences between the MRI-based classification and the clinical outcomes at preoperative, three and six months postoperative according to VAS (p = 0.451, p = 0.738, p = 0.448), ODI (p = 0.143, p = 0.929, p = 0.796), JOA (p = 0.157, p = 0.876, p = 0.961), and RMDQ (p = 0.065, p = 0.057, p = 0.094). There was clinical improvement after decompression and posterior stabilization in lumbar canal stenosis, which was manifested at three and six month post-operation follow-up in the VAS ODI, JOA and RMDQ scores. There was no association between the degree of LCS and VAS, ODI, JOA and RMDQ scores.
The application of digital pulse amplitude by fingertip peripheral arterial tonometry (PAT) device in patients with erectile dysfunction (ED) has never been performed. We investigated the diagnostic ...value of reactive hyperaemia (RH) and augmentation index (AI) as evaluated using PAT in men with ED of any origin. A total of 40 patients underwent diagnostic investigation for ED, including dynamic penile duplex ultrasound (PDU) and PAT device. Moreover, 30 patients without ED served as controls. According to PDU cutoff at 35 cm/sec, patients were divided into vascular (n = 30) and nonvascular (n = 10) ED aetiology. Moreover, controls with (n = 10) or without (n = 20) vascular risk factors (VRFs) were studied in a separate analysis. Average RH‐PAT was not different in men with or without ED (P = 0.56) independently of VRFs. The AI was higher in men with ED compared with the controls (P < 0.0001) as well as when controlled for the presence or absence of VRFs (P < 0.0001). An inverse relationship between AI and PSV was also found (r2 = −0.72, P < 0.0001). In conclusion, an increased AI but not an impaired RH‐PAT is present in men with vascular ED independently of VRFs and may represent an early detection of vascular impairment that may precede endothelial dysfunction in populations at low risk for developing vascular ED.
Celiac disease (CD) diagnosis can be extremely challenging in the case of atypical patterns. In this context, oral signs seem to play a decisive role in arousing suspicion of these forms of the ...disease. At the same time, the different expressions of the HLA-DQB1*02 allele apparently seem to facilitate the interpretation of signs and highlighted symptoms. The aim of this work was to verify whether it is possible to identify a correlation between the development of oral signs and different DQ2 haplotypes in celiac pediatric patients. 44 celiac patients with a medium age of 9.9 were studied. Oral examinations were performed in order to identify recurrent aphthous stomatitis (RAS) and dental enamel defects (DED). The diagnosis of DED resulted as being related to allele expression (P value = 0.042) while it was impossible to find a similar correlation with RAS. When both oral signs were considered, there was an increase in correlation with HLA-DQB1*02 expression (P value = 0.018). The obtained results identified both the fundamental role that dentists can play in early diagnosis of CD, as well as the possible role of HLA haplotype analysis in arousing suspicion of atypical forms of the disease.

45 (17 men, 28 women) morbid-obese patients (age: 19-49 y, BMI>40 kg/m2). All patients were selected on the basis of medical history, physical and biochemical evaluation and of psychiatric tests, which were performed on all individuals admitted to our Day Hospital to verify the safety of surgical intervention.
Body weight, body composition (by dual X-ray absorptiometry, DXA), blood pressure, lipid profile, fibrinogen and glucose metabolism were monitored at baseline and 1, 3, 6, 9, 12, 24 and 36 months after surgery.
A significant and persistent weight loss was present in all patients at the end of the 3 y follow-up period (P<0.001), with a progressive reduction of total and trunk fat mass as evaluated by means of DXA. Additionally, a parallel significant reduction in systolic (P<0.001) and diastolic (P<0.001) blood pressure was observed. Total and LDL cholesterol were significantly reduced (P<0.001), while HDL showed no modifications; triglycerides declined progressively during the 3 y follow-up (P<0.001). Fibrinogen decreased from 364.5+/-82.4 to 266.4+/-45.7 mg/dl at the end of the period (P<0.001). Fasting glucose levels and glucose levels 120 min after an oral glucose tolerance test were reduced from 95.1+/-20.3 to 78.6+/-9.1 mg/dl (P<0.001) and from 116.9+/-34.7 to 77.6+/-15.5 mg/dl (P<0.001), respectively, at baseline and at the end of the study. Moreover, fasting insulin decreased from 30.0+/-20.4 to 8.6+/-2.9 microUI/ml (P<0.001) after 3 y, while insulin levels after (120 min) oral glucose load decreased from 105.5+/-61.5 to 12.0+/-6.0 microUI/ml (P<0.001).
Our results show that biliary-intestinal bypass may represent a valid and alternative therapeutic approach in patients with morbid obesity since it induces a significant and stable reduction of body weight and obesity-related risk factors for CVD.
