Les progrès en oncologie ont permis une amélioration considérable de l’espérance de vie des patients y compris au stade métastatique soulevant la question de la qualité de vie des patients ...stabilisés. Pour les métastases osseuses, le point crucial est l’évaluation du risque fracturaire de l’os atteint afin de guider au mieux la reprise d’activité physique. La simple lecture de la radiographie ou du scanner est insuffisante. Dans le cadre de l’étude clinique multicentrique MEKANOS, notre laboratoire développe un processus de simulation numérique de la résistance mécanique d’un os tumoral à partir des images DICOM de scanner. Cependant, si les propriétés mécaniques de l’os à utiliser dans la simulation sont bien connues, celles à utiliser pour la tumeur ne le sont pas. Notre objectif est donc de déterminer ces propriétés.
Pour déterminer les propriétés visco-élastiques de la tumeur, nous avons collecté prospectivement des fragments humains frais de tumeurs fréquemment responsables de dissémination osseuse : 6 cancers du sein, 5 cancers du rein et 4 cancers de la thyroïde. Chaque fragment comportait des parties saines et des parties tumorales. Chaque échantillon a été testé mécaniquement en plusieurs points (3×3×25 points/échantillon) en indentation par AFM (Atomic Force Microscopy, Nanowizard3 AFM-JPK Instruments AG – équipé d’un MLCT, Bruker).
Les échantillons de cancer du sein, composé de tissu fibreux dense, présentaient une augmentation du module élastique par rapport au tissu normal, indiquant une augmentation de la rigidité. Pour le cancer du rein, les propriétés dépendaient plutôt de l’histologie. Les carcinomes à cellules claires, avec leur forte densité cellulaire, présentaient une diminution du module élastique. À l’inverse le carcinome papillaire, beaucoup plus fibreux, avait une augmentation du module élastique. Les résultats pour les échantillons de thyroïde étaient concordants avec le sein et le rein. Pour la thyroïde, il a également été observé une diminution de la fraction élastique, indiquant une augmentation de la viscosité.
En conclusion, nous avons observé que les propriétés mécaniques tissulaires tumorales étaient hautement dépendantes de la structure tissulaire et de l’histologie. Ces différences seront importantes à intégrer dans le modèle de simulation numérique pour obtenir une prédiction fiable et personnalisée de la résistance mécanique d’une métastase osseuse. À terme, ce travail contribuera à mieux guider la reprise d’activité physique des patients métastatiques osseux stabilisés.
Summary Objective Home polysomnography is being increasingly developed for sleep studies, with various grades of quality. This study aimed to determine the feasibility of affordable, high quality ...home polysomnographic recordings prescribed for suspected sleep-related neurological disorders. Patients and methods We prospectively screened all patients referred to the specialist sleep disorders clinic in Nancy University Hospital between May 2011 and August 2011. Patients were eligible for inclusion if they required polysomnography for the diagnosis of a sleep-related neurological disorder. One-night, polysomnography was performed in each patient's home by a trained sleep technician. Financial cost was determined prior to inclusion. A recording was considered as satisfactory if all the following criteria were present: at least, one EEG channel with continuous signal allowing determination of sleep stages and wake during more than 66% of sleep time; at least, one usable respiratory channel (airflow or either band) during more than 66% of sleep time; and usable oximetry during more than 66% of sleep time. Results Forty-eight of the 139 screened patients were included. Among the 48 home polysomnography recordings, 35 (72.9%) were satisfactory. Thirteen (27.1%) tracings displayed an unsatisfactory loss of EEG data, including seven (14.6%) tracings with an unsatisfactory loss of respiratory data. Conclusion Home polysomnography prescribed for suspected sleep-related neurological disorders is feasible, with affordable costs, whilst maintaining high quality recording. Further studies are needed to measure the real medico-economic impact of promoting outpatient domiciliary explorations for sleep-related neurological disorders.

Les auteurs montrent que l’éducation thérapeutique du malade présentant des crises épileptiques et de son entourage est nécessaire, mais non encore organisée en particulier en cas d’épilepsie ...pharmaco-résistante. Les objectifs de cette éducation thérapeutique sont détaillés à partir des constats existants et des besoins. Les auteurs précisent l’exigence d’un abord global subjectif autant qu’organique intégrant des dimensions psychosociales et le vécu du patient. La nécessité d’une formation spécifique des médecins et autres professionnels de santé est soulignée dans l’attente d’un statut de l’éducation thérapeutique. Les auteurs proposent un projet d’éducation thérapeutique d’un malade atteint d’une épilepsie pharmaco-résistante.
The authors show that therapeutic education of the epileptic patient and his proxies is necessary but not yet organized for drug-resistant epilepsy; the objectives of this education are detailed after looking at the current status and the needs. The authors insist on the necessity of a global consideration of the patient: subjective and organic as well, taking into account the psychological and social dimensions and the experiences of the patient. The need of a specific formation for the physician and other Health workers is emphasized, awaiting a status for therapeutic education. The authors propose a project of therapeutic education of the patient suffering from drug-resistant epilepsy.
Abstract Objectives Type 1 schizencephaly (SZ) is a cerebral malformation characterised by a cleft lined and surrounded by a polymicrogyric cortex, extending from the pial region to the ...peri-ventricular heterotopia. Our purpose was to combine and compare dipole source imaging technique and Stereo-EEG (SEEG) technique in determining the irritative and epileptogenic zones in a case of type 1 schizencephaly. Methods High-resolution (64-channel) video-EEG with electrical source imaging and SEEG recordings were performed during a pre-surgical evaluation for medically intractable epilepsy. Results Anatomo-electro-clinical correlations based on SEEG and source localisation identified two irritative and epileptogenic zones partially overlapping the polymicrogyric cortex surrounding the SZ: an anterior medio-lateral network primarily involving dysplasic limbic structures and a lateral network involving the anterior and middle part of the cleft and polymicrogyric cortex. The most posterior part (at the temporo-parieto-occipital junction) displayed a normal background activity. Conclusions Both epileptogenic and electrophysiologically normal cortices coexisted within the same widespread malformation: only the anterior part belonged to the anterior medio-lateral epileptogenic network defined by the SEEG. Significance In cases of widespread cortical malformation such as SZ, source localization techniques can help to define the irritative zone and relevant targets for SEEG.
Electroencephalography allows the functional analysis of electrical brain cortical activity and is the gold standard for analyzing electrophysiological processes involved in epilepsy but also in ...several other dysfunctions of the central nervous system. Morphological imaging yields complementary data, yet it cannot replace the essential functional analysis tool that is EEG. Furthermore, EEG has the great advantage of being non-invasive, easy to perform and allows control tests when follow-up is necessary, even at the patient's bedside. Faced with the advances in knowledge, techniques and indications, the Société de Neurophysiologie Clinique de Langue Française (SNCLF) and the Ligue Française Contre l'Épilepsie (LFCE) found it necessary to provide an update on EEG recommendations. This article will review the methodology applied to this work, refine the various topics detailed in the following chapters. It will go over the summary of recommendations for each of these chapters and underline proposals for writing an EEG report. Some questions could not be answered by the review of the literature; in those cases, an expert advice was given by the working and reading groups in addition to the guidelines.
La paralysie vélopalatine unilatérale de l’enfant est une situation rare qui pose principalement le problème de son diagnostic étiologique et donc de l’extension du bilan pour y parvenir.
Nous ...rapportons les cas des deux enfants âgés respectivement de 10 et 11 ans, jusque-là en bonne santé, présentant une paralysie vélopalatine transitoire unilatérale, associée dans un cas à une paralysie de l’hémipharynx homolatéral.
La confrontation des données de ces deux observations à celles de la littérature suggère le diagnostic d’une neuropathie périphérique idiopathique.
Compte tenu de la rareté de ce tableau clinique, il paraît nécessaire de colliger d’autres cas similaires afin d’affirmer le caractère bénin de l’affection, et d’éviter la multiplication d’examens complémentaires invasifs.
Unilateral spinal and accessory palsy is uncommon in children. The main problem is the extensive exploration undertaken to achieve etiological diagnosis.
We report two cases of transitory, unilateral, spinal and accessory palsy, associated in one case with homolateral hemi-pharyngeal palsy, that occurred in two previously healthy children.
Few reports have been published in the literature to date. From those two cases and data of the literature, we suggest that the clinical presentation results from idiopathic peripheral nerve palsy.
Further case reports would be useful to confirm the benign nature of this syndrome so that complementary investigations may be limited in future cases with a similar presentation.
