Les modes cognitifs actuels du langage s’intéressent en priorité au traitement des informations dans des réseaux distribués avec certaines zones corticales se comportant comme des nœuds pour gérer ...des fonctions spécifiques, reliés via des structures sous-corticales.
Étudier la connectivité de la région temporale basale en fonction de ses articulations avec le langage.
Des stimulations électriques corticales (SEC) à haute fréquence, ayant l’avantage de produire des perturbations transitoires des fonctions cognitives, ont été utilisées chez des patients présentant une épilepsie pharmaco-résistante et explorés par des électrodes profondes dans le cadre du bilan préopératoire afin de délimiter la zone temporale basale du langage (BTLA). Toujours par des SEC mais à basse fréquence, des mesures de connectivité fonctionnelle et effective ont été réalisées par la technique des potentiels évoqués cortico-corticaux.
Les zones apparentant à la BTLA présentent statistiquement plus des connections notamment avec les structures temporales internes. Le gyrus fusiforme est, en parallèle, fortement impliqué dans des réseaux n’appartenant pas à la BTLA.
La mesure de la connectivité effective couplée à la stimulation corticale fonctionnelle dans l’étude du langage et notamment de la BTLA amène une nouvelle dimension dans la compréhension des articulations entre une zone fonctionnelle et ses connections au-delà de sa localisation anatomique. En plus, des patterns de connectivité spécifiques nous permettent d’avancer des hypothèses concernant les relations du langage avec d’autres processus cognitifs, notamment la mémoire.
Objectif
: Évaluer la satisfaction des usagers du Samu 47 à la suite de leur appel dans le cadre de l’aide médicale urgente.
Matériel et méthodes
: Étude de cohorte observationnelle, monocentrique, ...prospective, menée entre le 25 août et le 13 septembre 2020 par un même opérateur, à partir d’un questionnaire téléphonique. Le critère de jugement principal était le niveau de satisfaction générale des requérants après leur appel. Les critères secondaires étaient le ressenti du délai de décroché ARM (assistant de régulation médicale) et médical, le niveau de satisfaction de la prise en charge ARM et médicale, le ressenti du délai d’arrivée des effecteurs envoyés, le niveau de satisfaction de leur prise en charge, la mise en application du conseil formulé par le médecin régulateur et la suggestion d’axes d’amélioration.
Résultats
: Parmi les 596 requérants inclus, 419 (70 %) étaient « très satisfaits », 132 (22 %) « assez satisfaits », 25 (4 %) « peu satisfaits » et 20 (3 %) « pas du tout satisfaits ». Les analyses univariées montraient que cela était lié à l’entretien avec l’ARM et le médecin régulateur, à la prise en charge des secours, à certaines attentes, à la suite donnée à l’appel, au délai ressenti d’arrivée des secours et au délai ressenti de décroché. L’analyse multivariée révélait que les éléments liés à l’insatisfaction étaient l’attente « faire venir les secours » (RC : 5,71, IC 95 % : 2,56–13,01), le fait de recevoir un conseil à l’issue de l’appel (RC : 9,28, IC 95 % : 4,15–21,96) et le délai ressenti de décroché de l’appel jugé « long » (RC : 3,64, IC 95 % : 1,06–10,75).
Conclusion
: Les usagers sont globalement satisfaits à la suite de leur appel au Samu 47.
Aim
: The aim of this study is to evaluate the satisfaction among the users of the emergency medical helpline of the Samu (urgent medical aid service) 47.
Procedure
: We conducted an observational, monocentric, prospective, cohort study, on the basis of a phone feedback form conducted from August 25 to September 13, 2020, by a same operator. The primary endpoint was global satisfaction after the call. The secondary endpoints were the telephone pick-up time, the rescue time, the satisfaction after the interviews with the emergency care dispatcher and the triage doctor, the satisfaction of the care provided by first responders, the application of the medical advice given, and the definition of areas for improvement.
Results
: Among the 596 users included, 419 (70%) were “very satisfied”, 132 (22%) “somewhat satisfied”, 25 (4%) “somewhat dissatisfied”, and 20 (3%) “very dissatisfied”. Univariate analysis showed that it was related to the interviews with the emergency care dispatcher and the triage doctor, the care provided by the first responders, certain expectations, the result of the call, the perception of telephone pick-up, and rescue time. Multivariate analysis revealed that the items which were related to dissatisfaction were the expectation of getting the emergency services (odds ratio OR = 5.71, 95% confidence interval CI = 2.56, 13.01), the call resulting in an advice (OR = 9.28, 95% CI = 4.15, 21.96), and the perception of telephone pick-up time as “long” (OR = 3.64, 95% CI = 1.06, 10.75).
Conclusion
: The users of the emergency medical helpline of the Samu 47 were globally satisfied after their call.
Several studies have described cranio–cerebral correlations in accordance with the 10–20 electrode placement system. These studies have made a significant contribution to human brain imaging ...techniques, such as near-infrared spectroscopy and trans-magnetic stimulation. With the recent development of high resolution EEG, an extension of the 10–20 system has been proposed. This new configuration, namely the 10–10 system, allows the placement of a high number (64–256) of EEG electrodes. Here, we describe the cranio–cerebral correlations with the 10–10 system. Thanks to the development of a new EEG-MRI sensor and an automated algorithm which enables the projection of electrode positions onto the cortical surface, we studied the cortical projections in 16 healthy subjects using the Talairach stereotactic system and estimated the variability of cortical projections in a statistical way. We found that the cortical projections of the 10–10 system could be estimated with
a grand standard deviation of 4.6 mm in
x, 7.1 mm in
y and 7.8 mm in
z. We demonstrated that the variability of projections is greatest in the central region and parietal lobe and least in the frontal and temporal lobes. Knowledge of cranio–cerebral correlations with the 10–10 system should enable to increase the precision of surface brain imaging and should help electrophysiological analyses, such as localization of superficial focal cortical generators.
Post-treatment platelet reactivity (PR) is associated with ischemic and bleeding events in patients receiving P2Y12 receptor antagonists.
We aimed to study the relationship between post-treatment PR ...after a 60-mg loading dose (LD) of prasugrel and 1-year thrombotic and bleeding events.
Patients were prospectively included in this multicenter study if they had a successful percutaneous coronary intervention (PCI) for acute coronary syndrome (ACS) and received prasugrel. The platelet reactivity index (PRI) was measured using the Vasodilator-Stimulated Phosphoprotein index (VASP) after a prasugrel LD. Endpoints included the rate of thrombotic events and bleeding events at 1 year.
Among the 301 patients enrolled, 9 (3%) were lost to follow-up at 1 year. The rates of thrombotic and bleeding events at 1 year were of 7.5% and 6.8%, respectively. Receiver-operating curve (ROC) analysis demonstrated an optimal cut-off value of 53.5% of PRI to predict thrombotic events at 1 year. Using this cut-off value we observed that patients exhibiting high on-treatment platelet reactivity (HTPR) had a higher rate of thrombotic events (22.4% vs. 2.9%; P < 0.001). In parallel the optimal cut-off value of PRI to predict bleeding was 16%. Patients with a PRI ≤ 16% had a higher rate of bleeding events compared with those with a PRI > 16% (15.6% vs. 3.3%; P < 0.001). In multivariate analysis, the PRI predicted both thrombotic and bleeding events (OR: 1.44, 95% confidence interval CI: 1.2-1.72; P < 0.001 and OR: 0.75, 95% CI: 0.59-0.96; P = 0.024 respectively, per 10% increase).
Platelet reactivity measurement after a prasugrel LD predicts both ischemic and bleeding events at 1 year follow-up for ACS patients undergoing PCI.
Feedbacks from tropical instability waves (TIWs) on the seasonal cycle of the eastern Pacific Ocean are studied using two eddy‐rich ocean simulations, with and without TIWs. By warming the equatorial ...waters by up to 0.4°C through nonlinear advection in boreal summer and fall, TIWs reduce the amplitude of the seasonal cycle in upper ocean temperatures. In addition, TIWs stabilize the upper part of the Equatorial Undercurrent (EUC) through enhanced barotropic energy conversion, leading to a year‐round weakening by −0.15 m s−1 and preventing an unrealistic re‐intensification in boreal fall usually found in non‐eddy resolving models. A coarser simulation at 1‐degree horizontal resolution fails to reproduce the TIW‐induced nonlinear warming of equatorial waters, but succeeds in inhibiting the EUC re‐intensification. This suggests a threshold effect in TIW strength, associated with the model's ability to simulate eddies, which may be responsible for long‐standing biases displayed by global climate models in this region.

Tropical instability waves (TIWs) are meandering ocean waves that originate off the coast of Ecuador between June and December and propagate in the eastern equatorial Pacific Ocean. By mixing cold equatorial with warm off‐equatorial waters, TIWs influence the climate of this key region for the global carbon cycle. In this study, we use a regional ocean model to quantify the impact of these waves on the upper‐ocean seasonal cycle. We find that TIWs do not interact in the same way with ocean surface temperatures and currents. TIWs warm surface equatorial waters during the boreal summer and fall, reducing the amplitude of the seasonal temperature cycle. In contrast, TIWs reduce the strength of one of the strongest Pacific equatorial currents—the Equatorial Undercurrent—all year long. Moreover, TIWs prevent an unrealistic re‐intensification of this current in boreal fall often found in models unable to simulate these waves accurately. In particular, we provide evidence that the horizontal resolution of most climate models is not sufficient to simulate accurately TIW impacts on the Pacific Ocean, particularly its temperatures. These findings highlight the necessity to incorporate TIW effects in the future generation of global climate models, via enhanced parameterization or increased horizontal resolution.

Résumé L’électroencéphalogramme (EEG) permet une analyse fonctionnelle du signal électrique cérébral et constitue l’examen de référence pour analyser les processus électrophysiologiques sous-tendant ...l’épilepsie mais également de nombreux autres dysfonctionnements du système nerveux central. Les examens d’imagerie morphologique apportent des renseignements complémentaires de l’EEG mais ne peuvent se substituer à cet outil d’analyse fonctionnelle. L’EEG a de plus l’avantage d’être non invasif, facile à réaliser et à contrôler quand un suivi est nécessaire, y compris au lit du patient. Face à l’évolution des connaissances, des techniques et des indications, une actualisation des recommandations sur l’EEG a été jugée nécessaire par la Société de Neurophysiologie Clinique de Langue Française (SNCLF) et la Ligue Française contre l’Épilepsie (LFCE). Cet article aborde la méthodologie adoptée pour ce travail de recommandations, précise les différentes thématiques détaillées dans les chapitres suivants et reprend les fiches synthétiques de recommandation de chacun de ces chapitres ainsi que des propositions de rédaction du compte-rendu d’EEG. La revue de la littérature n’a pas permis d’apporter une réponse à certaines questions pour lesquelles un avis d’experts a cependant été exprimé par le groupe de travail et de lecture en complément des recommandations.
