Background: Adenovirus-mediated overexpression of the apoptosis-inducing protein Bax can induce apoptosis in prostate cancer cell lines. Constitutive overexpression of Bax could result in unwanted ...apoptosis in every site of accidental Bax accumulation in vivo. Therefore, we developed an adenoviral construct (Av-ARR2PB-Bax) in which the probasin promoter, modified to contain two androgen response elements, drives Bax expression. This promoter would be expected to limit expression of Bax to cells expressing the androgen receptor. Methods: A variety of androgen receptor (AR)-positive and -negative cell lines of prostatic or nonprostatic origin were infected with Av-ARR2PB-Bax or a control virus, Av-ARR2PB-CAT, in which the same promoter drives expression of the chloramphenicol acetyl transferase-reporter gene. Bax expression and apoptosis in vitro were assessed by western blot analysis. Tumor size and apoptosis in vivo were assessed after four weekly injections of Av-ARR2PB-Bax or Av-ARR2PB-CAT into subcutaneous LNCaP xenografts growing in uncastrated male mice. All statistical tests were two-sided. Results: Bax was overexpressed in an androgen-dependent way in AR-positive cell lines of prostatic origin but not in AR-positive cells of nonprostatic origin or in AR-negative cell lines of either prostatic or nonprostatic origin. The androgen dihydrotestosterone activated apoptosis in LNCaP cells infected with Av-ARR2PB-Bax but not in those infected with Av-ARR2PB-CAT. Av-ARR2PB-Bax-injected LNCaP xenograft tumors decreased in tumor size from 34.1 mm3 (95% confidence interval CI = 25.1 mm3 to 43.1 mm3) to 24.6 mm3 (95% CI = –2.5 mm3 to 51.7 mm3), but the difference was not statistically significant (P = .5). Tumors injected with Av-ARR2PB-CAT increased in size, from 28.9 mm3 (95% CI = 12.7 mm3 to 45.1 mm3) to 206 mm3 (95% CI = 122 mm3 to 290 mm3) (P = .002) and contained statistically significant more apoptotic cells (23.3% 95% CI = 21.1% to 25.6% versus 9.5% 95% CI = 8.0% to 11.1) (P<.001). Conclusions: Av-ARR2PB-Bax induces androgen-dependent therapeutic apoptosis in vitro and in vivo by activating apoptosis in AR-positive cells derived specifically from prostatic epithelium and does not affect nonprostatic cells.
Prospective clinical study to evaluate patients suffering from solid tumor using a totally implanted venous access device (TIVAD) to determine: (1) if there is a relationship between cutaneous ...contamination at port insertion site and catheter-related bloodstream infection (CRBI); (2) development modalities of CRBI; (3) if there is a relationship between chemotherapy administration modalities by push/ bolus versus continuous infusion and CRBI.
We studied 41 consecutive patients who needed a TIVAD positioned for chemotherapy administration by bolus/ push or continuous infusion. In every patient, we performed blood cultures from blood samples from port catheters and cutaneous cultures from cutaneous tampons of the skin surrounding the implant area on the first (T0) and eight day (T1) postoperatively, after 1 month (T2), and after 3 months (T3) from insertion.
The study was completed on 40 patients; in one case, the port was removed at T2 for septic complications. We obtained four positive blood cultures (two, 5%), two in the same patient, all caused by staphylococcus. Positive cutaneous tampons were 21 (13%) in 11 patients (27%); the four CRBI occurred in this group of patients with none in the remaining 30 patients (73%) for a total number of 120 tampons (p<0.01). In two cases, the same germ was isolated from both the skin and blood. None of the patients presented a local infection of the subcutaneous pocket. Positive cutaneous cultures decrease over time: T0-T2; 24-5%; T1-T3, 20-5% (p<0.04). There were no differences in CRBI incidence and positive cutaneous tampons between the two chemotherapy administration modalities.
Cutaneous microbial flora has a primary role in CRBI development within TIVADs; there is a relationship between cutaneous colonization and CRBI; colonization reaches its maximum during the first days after catheterization in which the use of the system is at high risk; colonization occurs both via extraluminal and endoluminal routes; there is no difference in CRBI incidence between bolus and continuous infusion administration.
Lattice vibrations have been investigated in TiB2, ZrB2 and HfB2 by temperature‐dependent extended X‐ray absorption fine structure (EXAFS) experiments. Data clearly show that the EXAFS oscillations ...are characterized by an anomalous behavior of the Debye–Waller factor of the transition‐metal–boron pair, which is suggested to be associated with a superposition of an optical mode corresponding to phonon vibrations induced by the B sublattice and an acoustic mode corresponding to the transition‐metal (TM) sublattice. Data can be interpreted as a decoupling of the metal and boron vibrations observed in these transition‐metal diborides (TMB2), a mechanism that may be responsible for the significant reduction of the superconducting transition temperature observed in these systems with respect to the parent MgB2 compound. The vibrational behavior of TM–TM bonds has also been investigated to study the occurrence of anisotropy and anomalies in the lattice vibrational behavior of TM–TM bonds.
Les patients atteints du syndrome de Sjögren primaire (SSp) ont un risque accru de lymphome non hodgkinien (LNH). Il n’y a pas de consensus sur la prise en charge thérapeutique des LNH de bas grade : ...surveillance active, traitement local ou immuno-chimiothérapie. Les objectifs de notre étude étaient de décrire les caractéristiques du LNH chez des patients pSS, les stratégies thérapeutiques et l’impact de ces stratégies sur le pronostic du lymphome et du pSS.
Cette étude rétrospective multicentrique a inclus tous les patients atteints de lymphome de la cohorte ASSESS, enrichie de patients recrutés dans des services de médecine interne et de Rhumatologie. Nous avons recueilli les manifestations biologiques et cliniques du pSS, les caractéristiques du lymphome et la stratégie de traitement. La survie sans progression (PFS) lymphome et pSS et la survie globale (OS) ont été analysées.
Au total, 106 patients pSS ayant présenté un lymphome B entre 1985 et 2019 ont été inclus. Parmi eux, 14 (13 %) avaient un lymphome diffus à grandes cellules B et 82 présentaient un LNH B de la zone marginale (LZM), les lymphomes du tissu lymphoïde associé aux muqueuses (MALT) étant le sous-type histologique le plus fréquent survenant chez 68/82 (83 %) des malades. La localisation la plus fréquente était l’atteinte des glandes salivaires (47/82, 57 %). Il existait une atteinte diffuse (≥2 localisations) chez 37/82 (45 %) patients. L’activité du SSp au diagnostic était élevée avec un ESSDAI médian à 19.
Parmi ces 82 patients, une stratégie de surveillance a été choisie chez 19 (23 %) d’entre eux ; 63 patients ont reçu un traitement spécifique du lymphome au diagnostic, comprenant un traitement systémique (chimiothérapie et/ou immunothérapie) chez 49 (60 %) patients et un traitement local (chirurgie ou radiothérapie) chez 14 (17 %) patients ; 10 patients ont en outre reçu un traitement d’entretien par rituximab (RTX).
Comparativement aux patients traités, les patients non traités étaient plus âgés (moyenne 64 vs 56, p=0,041) avaient moins souvent une localisation pulmonaire (0 % vs 23 %, p=0,032). Il n’y avait aucune différence dans les caractéristiques cliniques et biologiques du SSp.
Nous avons ensuite analysé le pronostic après un recul moyen de 6,5 ans. Neuf patients (11 %) sont décédés au cours du suivi. En analyse multivariée, l’âge (HR=1,16 1,06–1,27, p=0,001) et la localisation pulmonaire (HR=8,15 1,57–42,3, p=0,013) étaient associés au décès.
Enfin, nous avons comparé l’OS, la PFS -lymphome et la PFS - SSp chez les patients traités par rapport aux patients non traités après pondération par un score de propension. Nous avons observé que le démarrage d’un traitement actif du LNH au moment du diagnostic du lymphome n’avait pas d’impact sur l’OS et la PFS du lymphome (HR=4,81 0,48–47,9, p=0,2 et HR=1,12 0,50–2,48, p=0,8, respectivement). À l’inverse, le traitement du lymphome avait un effet protecteur sur le risque de rechute du SSp (HR=0,4 0,17–0,95, p=0,038). Enfin, parmi les patients traités pour un lymphome, nous avons observé qu’aucune rechute de lymphome n’est survenue chez les patients ayant reçu un traitement d’entretien par RTX (0/10 événements vs 18/53, p=0,04).
Cette étude montre que l’âge et la localisation pulmonaire sont indépendamment associés au risque de décès au cours des LZM compliquant un SSp. Le choix de traiter ou non le lymphome au moment de son diagnostic n’affecte pas l’OS et la PFS pour le lymphome. Cependant, le traitement du lymphome semble réduire le risque de rechute du SSp. Ceci devrait être pris en compte lors du choix de la stratégie thérapeutique chez les patients SSp qui développent un lymphome.
The occurrence of thyroid abnormalities and the appearance of organ- and non-organ-specific autoantibodies during long-term recombinant interferon alpha-2a (IFN-alpha) therapy were studied in 86 and ...51 consecutive outpatients with hepatitis C and B virus-related chronic active hepatitis (CAH-HCV and CAH-HBV), respectively. Most patients had longstanding community-acquired hepatitis. At baseline, 9.3% of CAH-HCV and 3.9% of CAH-HBV patients showed clinical and/or biochemical signs of thyroid dysfunction. The remaining patients were euthyroid, although anti-thyroid autoantibodies were found in 33/78 (42.3%) of CAH-HCV and in 5/49 (10.2%) of CAH-HBV patients. During IFN-alpha treatment, increased anti-thyroid autoantibody levels were seen in 40% of CAH-HCV initially negative patients, while they became detectable in no more than 10% of CAH-HBV patients. Interferon-alpha-induced hypo- or hyperthyroidism was recorded in 12 of 35 CAH-HCV patients treated for 12 months (34.3%). Only one CAH-HBV patient developed hyperthyroidism. High titers of anti-nuclear autoantibodies (ANA) were recorded at enrollment in 5/36 (13.8%) of CAH-HCV and in 3/16 (18.7%) of CAH-HBV patients. Only one CAH-HCV patient displayed anti-parietal cell antibodies (PCA). After IFN-alpha treatment, ANA were found in 10/28 (35.7%) and PCA in 2/28 (7.1%) of CAH-HCV patients, while an additional CAH-HBV patient developed PCA, but not ANA. However, no signs of systemic autoimmune disease were recorded.
Introduction La mesure de l’HbA1c est insuffisamment répétée dans l’année chez les patients diabétiques : 3 dosages chez seulement 44 % en France (ENTRED 2007) et chez 51 % dans le Nord (données ARS ...2010). L’étude observationnelle DIANORD a pour objectif d’évaluer, dans un échantillon de patients, l’impact d’une campagne de sensibilisation à une fréquence suffisante de ce dosage. Nous rapportons l’analyse rétrospective de la situation avant lancement de la campagne. Patients et méthodes La base de données annuelle d’un groupement de laboratoires du Nord (BIOLILLE) a été interrogée. 22 047 prélèvements ont été analysés ; ils concernent 13 076 patients (48,4 % de femmes 51,6 % d’hommes) âgés en moyenne de 60,4 ans. Résultats La fréquence de dosage de l’HbA1c dans cet échantillon est en moyenne 2,04/an. A été exclu de cette analyse globale par prudence le sous groupe < 6 % + 1 seul dosage (5 093 patients), considérant que le dosage de l’HbA1c est souvent utilisé par le généraliste comme « test de dépistage ». Le déséquilibre du diabète n’induit pas une fréquence accrue de dosages : 2,05 sous groupe > 7,5 % (4 371) vs 1,98 sous groupe < 7 % (7 569). La répartition dans cet échantillon selon la fréquence est la suivante : 36,5 % 1 dosage (2 914 patients), 32,3 % 2 dosages (2 579), 31,2 % > = 3 dosages (2 490). Conclusion Des efforts sont nécessaires pour améliorer la fréquence du dosage de l’HbA1c. La campagne DIANORD a pour esprit de s’appuyer sur le tissu professionnel local : médecins généralistes-pharmaciens-diabétologues libéraux et d’informer le patient diabétique certes sur la signification de ce dosage mais aussi sur la pertinence de sa mesure plus régulière. Une analyse identique et comparative est prévue à l’issu de cette campagne.
