Forest edges influence more than half of the world's forests and contribute to worldwide declines in biodiversity and ecosystem functions. However, predicting these declines is challenging in ...heterogeneous fragmented landscapes. Here we assembled a global dataset on species responses to fragmentation and developed a statistical approach for quantifying edge impacts in heterogeneous landscapes to quantify edge-determined changes in abundance of 1,673 vertebrate species. We show that the abundances of 85% of species are affected, either positively or negatively, by forest edges. Species that live in the centre of the forest (forest core), that were more likely to be listed as threatened by the International Union for Conservation of Nature (IUCN), reached peak abundances only at sites farther than 200-400 m from sharp high-contrast forest edges. Smaller-bodied amphibians, larger reptiles and medium-sized non-volant mammals experienced a larger reduction in suitable habitat than other forest-core species. Our results highlight the pervasive ability of forest edges to restructure ecological communities on a global scale.
The atomic nucleus and its electrons are often thought of as independent systems that are held together in the atom by their mutual attraction. Their interaction, however, leads to other important ...effects, such as providing an additional decay mode for excited nuclear states, whereby the nucleus releases energy by ejecting an atomic electron instead of by emitting a γ-ray. This 'internal conversion' has been known for about a hundred years and can be used to study nuclei and their interaction with their electrons. In the inverse process-nuclear excitation by electron capture (NEEC)-a free electron is captured into an atomic vacancy and can excite the nucleus to a higher-energy state, provided that the kinetic energy of the free electron plus the magnitude of its binding energy once captured matches the nuclear energy difference between the two states. NEEC was predicted in 1976 and has not hitherto been observed. Here we report evidence of NEEC in molybdenum-93 and determine the probability and cross-section for the process in a beam-based experimental scenario. Our results provide a standard for the assessment of theoretical models relevant to NEEC, which predict cross-sections that span many orders of magnitude. The greatest practical effect of the NEEC process may be on the survival of nuclei in stellar environments, in which it could excite isomers (that is, long-lived nuclear states) to shorter-lived states. Such excitations may reduce the abundance of the isotope after its production. This is an example of 'isomer depletion', which has been investigated previously through other reactions, but is used here to obtain evidence for NEEC.
A single-group, phase 1-2 study indicated that eltrombopag improved the efficacy of standard immunosuppressive therapy that entailed horse antithymocyte globulin (ATG) plus cyclosporine in patients ...with severe aplastic anemia.
In this prospective, investigator-led, open-label, multicenter, randomized, phase 3 trial, we compared the efficacy and safety of horse ATG plus cyclosporine with or without eltrombopag as front-line therapy in previously untreated patients with severe aplastic anemia. The primary end point was a hematologic complete response at 3 months.
Patients were assigned to receive immunosuppressive therapy (Group A, 101 patients) or immunosuppressive therapy plus eltrombopag (Group B, 96 patients). The percentage of patients who had a complete response at 3 months was 10% in Group A and 22% in Group B (odds ratio, 3.2; 95% confidence interval CI, 1.3 to 7.8; P = 0.01). At 6 months, the overall response rate (the percentage of patients who had a complete or partial response) was 41% in Group A and 68% in Group B. The median times to the first response were 8.8 months (Group A) and 3.0 months (Group B). The incidence of severe adverse events was similar in the two groups. With a median follow-up of 24 months, a karyotypic abnormality that was classified as myelodysplastic syndrome developed in 1 patient (Group A) and 2 patients (Group B); event-free survival was 34% and 46%, respectively. Somatic mutations were detected in 29% (Group A) and 31% (Group Β) of the patients at baseline; these percentages increased to 66% and 55%, respectively, at 6 months, without affecting the hematologic response and 2-year outcome.
The addition of eltrombopag to standard immunosuppressive therapy improved the rate, rapidity, and strength of hematologic response among previously untreated patients with severe aplastic anemia, without additional toxic effects. ULTRACAM is a portable, high-speed imaging photometer designed to study faint astronomical objects at high temporal resolutions. ULTRACAM employs two dichroic beamsplitters and three frame-transfer ...CCD cameras to provide three-colour optical imaging at frame rates of up to 500 Hz. The instrument has been mounted on both the 4.2-m William Herschel Telescope on La Palma and the 8.2-m Very Large Telescope in Chile, and has been used to study white dwarfs, brown dwarfs, pulsars, black hole/neutron star X-ray binaries, gamma-ray bursts, cataclysmic variables, eclipsing binary stars, extrasolar planets, flare stars, ultracompact binaries, active galactic nuclei, asteroseismology and occultations by Solar System objects (Titan, Pluto and Kuiper Belt objects). In this paper we describe the scientific motivation behind ULTRACAM, present an outline of its design and report on its measured performance.
Bone marrow has been shown to be superior to peripheral blood, as a stem cell source, in young patients (<20 years of age) with acquired aplastic anemia undergoing a matched sibling transplant. The ...aim of this study was to test whether this currently also holds true for older patients with acquired aplastic anemia.
We analyzed 1886 patients with acquired aplastic anemia who received a first transplant from a human leukocyte antigen identical sibling between 1999 and 2009, with either bone marrow (n=1163) or peripheral blood (n=723) as the source of stem cells.
In multivariate Cox analysis negative predictors for survival were: patient's age over 20 years (RR 2.0, P<0.0001), an interval between diagnosis and transplantation of more than 114 days (RR 1.3, P=0.006), no anti-thymocyte globulin in the conditioning (RR 1.6, P=0.0001), a conditioning regimen other than cyclophosphamide (RR=1.3, P=0.008) and the use of peripheral blood as the source of stem cells (RR 1.6, P<0.00001). The survival advantage for recipients of bone marrow rather than peripheral blood was statistically significant in patients aged 1-19 years (90% versus 76% P<0.00001) as well as in patients aged over 20 years (74% versus 64%, P=0.001). The advantage for recipients of bone marrow over peripheral blood was maintained above the age of 50 years (69% versus 39%, P=0.01). Acute and chronic graft-versus-host disease were more frequent in peripheral blood transplants. Major causes of death were graft-versus-host disease (2% versus 6% in bone marrow and peripheral blood recipients, respectively), infections (6% versus 13%), and graft rejection (1.5% versus 2.5%).
This study shows that bone marrow should be the preferred stem cell source for matched sibling transplants in acquired aplastic anemia, in patients of all age groups.
This is the sixth special report that the European Society for Blood and Marrow Transplantation regularly publishes on the current practice and indications for haematopoietic SCT for haematological ...diseases, solid tumours and immune disorders in Europe. Major changes have occurred in the field of haematopoietic SCT over the last years. Cord blood units as well as haploidentical donors have been increasingly used as stem cell sources for allo-SCT, thus, augmenting the possibility of finding a suitable donor for a patient. Continuous refinement of conditioning strategies has also expanded not only the number of potential indications but also has permitted consideration of older patients or those with co-morbidity for a transplant. There is accumulating evidence of the role of haematopoietic SCT in non-haematological disorders such as autoimmune diseases. On the other hand, the advent of new drugs and very effective targeted therapy has challenged the role of SCT in some instances or at least, modified its position in the treatment armamentarium of a given patient. An updated report with revised tables and operating definitions is presented.
We have analyzed 1448 patients with acquired aplastic anemia grafted between 2005 and 2009, and compared outcome of identical sibling (n=940) versus unrelated donor (n=508) transplants. When compared ...to the latter, sibling transplants were less likely to be performed beyond 180 days from diagnosis (39% vs. 85%), to have a cytomegalovirus negative donor/recipient status (15% vs. 23%), to receive antithymocyte globulin in the conditioning (52% vs. 61%), and more frequently received marrow as a stem cell source (60% vs. 52%). Unrelated donor grafts had significantly more acute grade II-IV (25% vs. 13%) and significantly more chronic graft-versus-host disease (26% vs. 14%). In multivariate analysis, the risk of death of unrelated donor grafts was higher, but not significantly higher, compared to a sibling donor (P=0.16). The strongest negative predictor of survival was the use of peripheral blood as a stem cell source (P<0.00001), followed by an interval of diagnosis to transplant of 180 days or more (P=0.0005), patient age 20 years or over (P=0.0005), no antithymocyte globulin in the conditioning (P=0.003), and donor/recipient cytomegalovirus sero-status, other than negative/negative (P=0.04). In conclusion, in multivariate analysis, the outcome of unrelated donor transplants for acquired aplastic anemia, is currently not statistically inferior when compared to sibling transplants, although patients are at greater risk of acute and chronic graft-versus-host disease. The use of peripheral blood grafts remains the strongest negative predictor of survival.
Background
Increased uptake of robotic surgery has led to interest in learning curves for robot‐assisted procedures. Learning curves, however, are often poorly defined. This systematic review was ...conducted to identify the available evidence investigating surgeon learning curves in robot‐assisted surgery.
Methods
MEDLINE, Embase and the Cochrane Library were searched in February 2018, in accordance with PRISMA guidelines, alongside hand searches of key congresses and existing reviews. Eligible articles were those assessing learning curves associated with robot‐assisted surgery in patients.
Results
Searches identified 2316 records, of which 68 met the eligibility criteria, reporting on 68 unique studies. Of these, 49 assessed learning curves based on patient data across ten surgical specialties. All 49 were observational, largely single‐arm (35 of 49, 71 per cent) and included few surgeons. Learning curves exhibited substantial heterogeneity, varying between procedures, studies and metrics. Standards of reporting were generally poor, with only 17 of 49 (35 per cent) quantifying previous experience. Methods used to assess the learning curve were heterogeneous, often lacking statistical validation and using ambiguous terminology.
Conclusion
Learning curve estimates were subject to considerable uncertainty. Robust evidence was lacking, owing to limitations in study design, frequent reporting gaps and substantial heterogeneity in the methods used to assess learning curves. The opportunity remains for the establishment of optimal quantitative methods for the assessment of learning curves, to inform surgical training programmes and improve patient outcomes.
Antecedentes
La aceptación creciente de la cirugía robótica ha generado interés en las curvas de aprendizaje para los procedimientos asistidos por robot. Sin embargo, las curvas de aprendizaje a menudo están mal definidas. Esta revisión sistemática se realizó para identificar la evidencia disponible en relación a las curvas de aprendizaje del cirujano en la cirugía asistida por robot.
Métodos
En Febrero de 2018, se realizaron búsquedas en MEDLINE, Embase y Cochrane Library, de acuerdo con las recomendaciones PRISMA, junto con búsquedas manuales de congresos clave y de revisiones ya existentes. Los artículos elegibles fueron aquellos que evaluaron las curvas de aprendizaje asociadas con la cirugía asistida por robot efectuada en pacientes.
Resultados
Las búsquedas bibliográficas identificaron 2.316 registros de los cuales 68 cumplían los criterios de elegibilidad y correspondían a 68 estudios primarios. De estos 68 estudios, 49 evaluaron las curvas de aprendizaje basadas en datos de pacientes de 10 especialidades quirúrgicas. Los 49 estudios eran todos estudios observacionales, en su mayoría de un solo brazo (35/49 (71%)) e incluían pocos cirujanos. Las curvas de aprendizaje mostraban una notable heterogeneidad, variando entre procedimientos, estudios y parámetros analizados. Los estándares de presentación de informes fueron generalmente deficientes, con solo 17/49 (35%) cuantificando la experiencia previa. Los métodos utilizados para evaluar la curva de aprendizaje fueron heterogéneos, a menudo carecían de validación estadística y usaban terminología ambigua.
Conclusión
Las estimaciones de la curva de aprendizaje estaban sujetas a una considerable incertidumbre, careciendo de evidencia robusta por las limitaciones en el diseño del estudio, lagunas de información en los artículos y heterogeneidad sustancial en los métodos utilizados para evaluar las curvas de aprendizaje. Queda pendiente establecer métodos cuantitativos óptimos para evaluar las curvas de aprendizaje, informar de los programas de formación quirúrgica y mejorar los resultados del paciente.

Little consistency between studies
We present photometry and spectroscopy for 27 pulsating hydrogen-atmosphere white dwarfs (DAVs; a.k.a. ZZ Ceti stars) observed by the Kepler space telescope up to K2 Campaign 8, an extensive ...compilation of observations with unprecedented duration (>75 days) and duty cycle (>90%). The space-based photometry reveals pulsation properties previously inaccessible to ground-based observations. We observe a sharp dichotomy in oscillation mode line widths at roughly 800 s, such that white dwarf pulsations with periods exceeding 800 s have substantially broader mode line widths, more reminiscent of a damped harmonic oscillator than a heat-driven pulsator. Extended Kepler coverage also permits extensive mode identification: we identify the spherical degree of 87 out of 201 unique radial orders, providing direct constraints of the rotation period for 20 of these 27 DAVs, more than doubling the number of white dwarfs with rotation periods determined via asteroseismology. We also obtain spectroscopy from 4 m-class telescopes for all DAVs with Kepler photometry. Using these homogeneously analyzed spectra, we estimate the overall mass of all 27 DAVs, which allows us to measure white dwarf rotation as a function of mass, constraining the endpoints of angular momentum in low- and intermediate-mass stars. We find that 0.51-0.73 M white dwarfs, which evolved from 1.7-3.0 M ZAMS progenitors, have a mean rotation period of 35 hr with a standard deviation of 28 hr, with notable exceptions for higher-mass white dwarfs. Finally, we announce an online repository for our Kepler data and follow-up spectroscopy, which we collect at http://k2wd.org.
The Low-Frequency Array radio telescope discovered the 707 Hz binary millisecond pulsar (MSP) J0952−0607 in a targeted radio pulsation search of an unidentified Fermi gamma-ray source. This source ...shows a weak energy flux of Fγ = 2.6 × 10−12 erg cm−2 s−1 in the energy range between 100 MeV and 100 GeV. Here we report the detection of pulsed gamma-ray emission from PSR J0952−0607 in a very sensitive gamma-ray pulsation search. The pulsar's rotational, binary, and astrometric properties are measured over 7 years of Fermi-Large Area Telescope data. For this we take into account the uncertainty on the shape of the gamma-ray pulse profile. We present an updated radio-timing solution now spanning more than 2 years and show results from optical modeling of the black-widow-type companion based on new multiband photometric data taken with HiPERCAM on the Gran Telescopio Canarias on La Palma and ULTRACAM on the New Technology Telescope at ESO La Silla (based on observations collected at the European Southern Observatory, Chile; programme 0101.D-0925, PI: Clark, C. J.). PSR J0952−0607 is now the fastest-spinning pulsar for which the intrinsic spin-down rate has been reliably constrained ( ). The inferred surface magnetic field strength of is among the 10 lowest of all known pulsars. This discovery is another example of an extremely fast spinning black-widow pulsar hiding within an unidentified Fermi gamma-ray source. In the future such systems might help to pin down the maximum spin frequency and the minimum surface magnetic field strength of MSPs.
