The subjects discussed in the article are the curious use of the term creativity by neoliberalism, how this has had an effect on educational policies in recent years, and how this effect can be ...counteracted by suggesting an option in a critical educational model. We characterize the concept of creativity applying a multidisciplinary approach with an analytical foundation that diverges with the perceptions coined by the business speech in the latest decades. Because of this disparity, we question the manner in which pedagogy ought to use creativity for the best development of students’ critical capacity, which allows them to criticize the strategies and the objectives of their learning process, as well as willingly animate their subjectivity and foment their critical thought. In relation to the methodology, for all the phases of this documentary research project, we have worked with a bibliographical review of sources which, due to their concrete theme and academic impact, are more relevant for the idea and purposes.
Los temas que aborda el presente artículo son el uso interesado del término creatividad por parte del neoliberalismo, cómo éste ha influido en las políticas educativas durante los últimos años, si es ...posible contrarrestar esta influencia y, en tal caso, de qué modo puede plantearse una alternativa en forma de modelo pedagógico crítico. A tal efecto se intenta definir preliminarmente un concepto de creatividad a través de un enfoque multidisciplinar (psicológico, sociológico, antropológico, psicológico…) con una base crítica que se contrapone a las ideas que en las últimas décadas ha acuñado el discurso neoliberal. A la luz de esta contraposición se indaga en el modo en que la pedagogía debe implementar la creatividad para el desarrollo óptimo de la capacidad crítica de los estudiantes, para así plantear un modelo educativo que les permita cuestionarse las estrategias y la finalidad de su propio proceso de aprendizaje, así como formar libremente su propia subjetividad y potenciar su pensamiento crítico. Este recorrido argumental recurre a una metodología basada en la revisión bibliográfica –investigación documental– de las fuentes apropiadas a los conceptos tratados directamente y de modo contextual. Finalmente, se presentan una serie de conclusiones que demuestran la necesidad de plantear un modelo educativo alternativo, resultado de un sistema pedagógico afín, que fomente la creatividad de tal modo que favorezca la generación de procesos de aprendizaje y enseñanza críticos.

The University Hospital of Salamanca, in Spain, had its first COVID-19 case on March 1st and as of May 11th, we had 1,100 positive cases. Based on the vulnerability of patients with blood cancers, on ...March 9th, the Hematology Department developed a protocol, amended as the new information was available, to maintain the Hematology Unit as a “free COVID-19 island.” The protocol included symptom-based surveys and screening tests to patients, caregivers, and healthcare personnel to identify early potential positive cases and prevent its spread. Between March 9 and April 28, 32 asymptomatic patients and caregivers were tested and 68 rT-PCR diagnostic assays have been performed with two positive results. A 106 healthcare workers have been tested (208 rT-PCR) and seven of them were positive. In summary, the implementation of preemptive measures after the first case appeared allowed us to be able to provide treatment to our patients.
This randomized clinical trial investigated the effectiveness of surgery compared with physical therapy consisting of manual therapies including desensitization maneuvers in carpal tunnel syndrome ...(CTS). The setting was a public hospital and 2 physical therapy practices in Madrid, Spain. One hundred twenty women with CTS were enrolled between February 2013 and January 2014, with 1-year follow-up completed in January 2015. Interventions consisted of 3 sessions of manual therapies including desensitization maneuvers of the central nervous system (physical therapy group, n = 60) or decompression/release of the carpal tunnel (surgical group, n = 60). The primary outcome was pain intensity (mean pain and the worst pain), and secondary outcomes included functional status and symptoms severity subscales of the Boston Carpal Tunnel Questionnaire and the self-perceived improvement. They were assessed at baseline and 1, 3, 6, and 12 months by a blinded assessor. Analysis was by intention to treat. At 12 months, 111 (92%) women completed the follow-up (55/60 physical therapy, 56/60 surgery). Adjusted analyses showed an advantage (all, P < .01) for physical therapy at 1 and 3 months in mean pain (Δ -2.0 95% confidence interval (CI) -2.8 to -1.2/-1.3 95% CI -2.1 to -.6), the worst pain (Δ -2.9 -4.0 to -2.0/-2.0 -3.0 to -.9), and function (Δ -.8 -1.0 to -.6/-.3 -.5 to -.1), respectively. Changes in pain and function were similar between the groups at 6 and 12 months. The 2 groups had similar improvements in the symptoms severity subscale of the Boston Carpal Tunnel Questionnaire at all follow-ups. In women with CTS, physical therapy may result in similar outcomes on pain and function to surgery.

Objective
To assess the in vitro efficacy on antioxidant potential, protection against global oxidative stress, and effect on collagen neosynthesis of minimalist formula (Peptide‐C ampoules product) ...containing 10% natural vitamin C, rice and lupin bio‐peptides, hyaluronic acid, and Vichy volcanic mineralizing water (active mix).
Methods
In‐tube quantitative tests (“in‐tube screening”) assessed global antioxidant properties, anti‐lipid peroxidation, anti‐protein glycosylation, and metalloproteinase inhibition (anti‐collagenase, anti‐elastase, and anti‐hyaluronidase activity) properties of the formula. Protection against oxidative stress was evaluated on human keratinocyte monolayer cultures, and collagen neosynthesis was quantified on fibroblast monolayer cultures treated with supernatants from product‐treated reconstructed human epidermis.
Results
Product (5% concentration) showed high antioxidant ability (blocking 99.0% oxidation), protection against oxidative stress damage (51.8% lipid peroxidation and 37.8% protein glycosylation decreases), and inhibition of hyaluronidase (21.9%), elastase (47.1%), and collagenase (61.8%). The protective effect was validated on human keratinocyte monolayer cultures in the presence of active mix (0.025%). Oxidative stress (ROS) was reduced by 99.0%, while global oxidative stress (RMS) induced by pollution, UVA radiation, and a combination of both factors was reduced by 48.94%, 8.7%, and 96.28%, respectively. The product increased collagen neosynthesis (11.21%) by cellular dialogue in fibroblasts incubated with product/mix‐treated‐RHE supernatants.
Conclusion
The combination of ingredients in the product showed high global antioxidant capacity, as well as a protective effect against oxidative stress induced by UVA, pollution, or both combined factors and an ability to stimulate collagen neosynthesis in in vitro studies, which support the clinical efficacy of this product.
Neanderthals have been claimed to have had a selective adaptation to rugged, wooded landscapes that would have partially compensate their high basal metabolic rate and locomotor energetic costs ...through reducing search time and increasing diet breadth. The archaeological site of the Navalmaíllo rockshelter (Pinilla del Valle, Madrid), located in a mountain environment in central Iberian Peninsula, has been interpreted as a repeated short-term occupation hunting camp by Neanderthal groups, mainly focused on primary access to large bovids and cervids. Navalmaíllo rockshelter has also furnished fish, toads, frogs, tortoises, lizards and snakes remains. This association of amphibians and reptiles suggests a much warmer climate (+2.8 °C) for layer F than at present, with similar temperatures during the summer but higher temperatures throughout the remaining months. Rainfall was slightly more abundant (+180.6 mm) than today, with a more contrasted regime during the year, with rainier winters and drier summers. A two-month period of aridity is observed during summer, representing a similar duration to present-day climatic conditions. Reconstructed landscapes are mainly constituted by open areas with dry grassland and rocky or stony areas, evolving laterally to humid meadows, probably close to the aquatic and peri-aquatic areas, such as rivers or ponds with riverside vegetation. Woodland environments are also well represented around the site, with medium scrublands to forest formations. Fish complete this reconstruction by documenting the presence of a pre-mountain well-developed river system characterized by relatively cold, permanent, oxygen-rich, and running waters. Such reconstruction is in disagreement with previous pollen interpretation for Layer F that suggested a very open and cold environment. Our new interpretation suggests that the Neanderthal occupation of the Navalmaíllo rockshelter occurred during a somewhat temperate and humid period, probably within the later part of the Marine Isotope Stage 5, effectively favouring the presence of a high biodiversity around the site.

Chronic kidney disease (CKD) patients are at high-risk for severe Covid-19. The multicentric, observational and prospective SENCOVAC study aims to describe the humoral response and safety of ...SARS-CoV-2 vaccines in CKD patients. Safety and immediate humoral response results are reported here.
Four cohorts of patients were included: kidney transplant (KT) recipients, haemodialysis (HD), peritoneal dialysis (PD) and non-dialysis CKD patients from 50 Spanish centres. Adverse events after vaccine doses were recorded. At baseline and on day 28 after the last vaccine dose, anti-Spike antibodies were measured and compared between cohorts. Factors associated with development of anti-Spike antibodies were analyzed.
1746 participants were recruited: 1116 HD, 171 PD, 176 non-dialysis CKD patients and 283 KT recipients. Most patients (98%) received mRNA vaccines. At least one vaccine reaction developed after the first dose in 763 (53.5%) and after the second dose in 741 (54.5%) of patients. Anti-Spike antibodies were measured in the first 301 patients. At 28 days, 95% of patients had developed antibodies: 79% of KT, 98% of HD, 99% of PD and 100% of non-dialysis CKD patients (p<0.001). In a multivariate adjusted analysis, absence of an antibody response was independently associated to KT (OR 20.56, p = 0.001) and to BNT162b2 vaccine (OR 6.03, p = 0.023).
The rate of anti-Spike antibody development after vaccination in KT patients was low but in other CKD patients it approached 100%; suggesting that KT patients require persistent isolation measures and booster doses of a Covid-19 vaccine. Potential differences between Covid-19 vaccines should be explored in prospective controlled studies.
Background: A multidisciplinary approach and Doppler ultrasound (DU) assessment for the creation and maintenance of arteriovenous fistulas (AVF) for haemodialysis can improve prevalence and patency. ...The aim of this study was to analyze the impact of a new multidisciplinary vascular access (VA) clinic with routine DU. Material and methods: We analyzed the VA clinic results from 2014 and 2015, before and after the implementation of a multidisciplinary team protocol (vascular surgeon/nephrologist) with routine DU in preoperative mapping and prevalent AVF. Results: We analyzed 345 and 364 patients from 2014 and 2015 respectively. The number of surgical interventions was similar in both periods (p = .289), with a trend toward an increase in preventive surgical repair of AVF in 2015 (17 vs. 29, p = .098). 155 vs. 169 new AVF were performed in 2014 and 2015, with a significantly lower primary failure rate in 2015 (26.4 vs. 15.3%, p = .015), and a non-significant increase in radiocephalic AVF, 25.8 vs. 33.2% (n = 40 vs. 56), p = .159. The concordance between the indication at the clinic and the surgery performed also increased (81.3 vs. 93.5%, p = .001). Throughout 2015 fewer complementary imaging test were requested from the clinic (78 vs. 35, p < .001), with a corresponding reduction in costs (€ 87,716 vs. € 59,445). Conclusions: Multidisciplinary approach with routine DU can improve VA results, with a decrease in primary failure rate, higher likelihood of radiocephalic AVF, better management of dis-functioning AVF and lower radiological test costs. Resumen: Antecedentes y objetivo: El abordaje multidisciplinar y el uso de ecografía doppler (ED) en la creación y vigilancia del acceso vascular (AV) puede mejorar la prevalencia y permeabilidad de las fístulas arteriovenosas (FAV) para hemodiálisis. El objetivo de este estudio es analizar el impacto de una nueva consulta multidisciplinar (CMD) de AV con ED de rutina. Material y métodos: Evaluamos los resultados de la consulta de AV en 2014 (pre-CMD) y 2015 (CMD), antes y después de la implantación de un equipo multidisciplinar (cirujano vascular/nefrólogo) con ED de rutina en mapeo prequirúrgico y FAV prevalente. Resultados: Se evaluaron 345 pacientes en 2014 (pre-CMD) y 364 pacientes en 2015 (CMD). En ambos periodos se realizó un número similar de cirugías, 172 vs. 198, p = 0,289, con tendencia a aumentar las cirugías preventivas de reparación de FAV en el periodo CMD, 17 vs. 29 (p = 0,098). En FAV de nueva creación (155 vs. 169), disminuyó la tasa de fallo primario en el periodo CMD, 26,4 vs. 15,3%, p = 0,015 y aumentó de forma no significativa la realización de FAV radiocefálicas distales, 25,8 vs. 33,2% (n = 40 vs. 56), p = 0,159. También aumentó la concordancia entre la indicación quirúrgica en la consulta y la cirugía realizada (81,3 vs. 93,5%, p = 0,001). En el periodo CMD se solicitaron menos exploraciones radiológicas desde la consulta, 78 vs. 35 (p < 0,001), con una reducción del gasto sanitario (81.716€ vs. 59.445€). Conclusiones: El manejo multidisciplinar y la utilización del ED de rutina permiten mejorar los resultados de AV, con disminución de la tasa de fallo primario de FAV, más opciones de FAV distal nativa, mejor manejo de la FAV prevalente disfuncionante y menor coste en exploraciones radiológicas.
