Accuracy of preoperative imaging methods for the diagnosis of intraductal papillary mucinous tumor of the pancreas (IPMT) is not well known.
To compare diagnostic accuracy of various preoperative ...imaging methods with pathology data following surgical resection.
Fourteen consecutive patients underwent pancreatic surgical resection for IPMT between January 1988 and May 2002. Imaging methods included endoscopic retrograde cholangiopancreatography and/or magnetic resonance cholangiopancreatography and/or endoscopic ultrasonography. Results of preoperative morphological examinations were compared with histopathological findings from surgical specimens.
IPMT was located to the pancreatic head or body in 72% of patients and involved the main pancreatic duct (MPD) in 79% of cases Carcinoma was diagnosed in 35% of the cases. Sensitivity of imaging methods varied from 64 to 80% to accurately locate the tumor and from 73 to 80% in distinguishing between types involving the MPD or the accessory ducts. Planned surgical resection based on preoperative imagery corrrelated with final surgery in 57% of the patients. Histological study of whole pancreas specimens revealed lesions in undilated ducts, and also dilated ducts without histological lesions.
The sensitivity of preoperative imaging methods is moderate in defining intraductal extension of IMPT. Duct dilatation is not predictive of histological involvement by tumors and ducts may be pathological without dilatation.
La place et les performances des examens morphologiques dans le diagnostic des tumeurs intracanalaires papillaires et mucineuses du pancréas (TIPMP) sont encore mal définies.
Comparer les performances diagnostiques des examens d’imagerie pré-opératoire entre eux sur la base de données anatomo-pathologiques de pièces opératoires.
14 malades consécutifs opérés pour TIPMP de janvier 1988 à mai 2002 ont été retenus. Les examens morphologiques réalisés étaient une cholangio-pancréatographie endoscopique et/ou par résonance magnétique et/ou une écho-endoscopie bilio-pancréatique. Une relecture standardisée de chacun de ces examens a été faite ainsi que de toutes les données histologiques.
La TIPMP avait une localisation céphalo-corporéale dans 72 % des cas et la maladie atteignait le canal principal dans 79 % des cas. Une dégénérescence était présente dans 35 % des cas. La sensibilité des 3 examens morphologiques variait de 64 à 80 % pour localiser la tumeur et de 73 à 80 % pour distinguer les formes du canal principal ou des canaux secondaires. Le type de résection proposé à l’issue du bilan morphologique correspondait à celui effectué dans 57 % des cas. L’analyse des pièces de pancréatectomie totale a permis de révéler des lésions histologiques sur des canaux non dilatés et inversement des canaux dilatés sans lésion histologique.
La sensibilité des examens morphologiques actuels est médiocre pour définir en pré-opératoire l’extension intracanalaire des tumeurs papillaires et mucineuses du pancréas. Cette étude confirme que la dilatation canalaire n’est pas prédictive d’une atteinte histologique et inversement qu’en l’absence de dilatation canalaire, il peut exister une atteinte histologique.
Unemployment of the unqualified young workers in Canada has always been a preoccupation and is doomed to increase with the mondialisation of the economy. The first source of non qualification is to ...dropout of school. In this paper, we consider the effect of working while in school on the probability of dropping out at the secondary level. However, contrary to previous papers we have realised on this subject, we consider here the relation decision to dropout and working while in school in a more complex setting. In this paper, we entertain the possibility of two models to explain the decisions of the individuals in our sample of Statistics Canada's School Leavers Survey. In a first model, school is the0501n preoccupation, while in the second model experience of work while in school is the leading decision. Not knowing a priori, which model a specific individual belongs to, we use a sophisticated econometric model to compute the probability that an individual with given characterictics is part of the first or the second model. The research identifies several determinants of dropping out and different policies which can be put in action by the government in order to reduce dropout. Le chômage chez les jeunes non qualifiés au Canada est particulièrement élevé et la mondialisation actuelle de l'économie ne fait qu'amplifier le problème. La première source de non-qualification des jeunes travailleurs est l'abandon des études secondaires. Dans cet article, nous prenons en compte l'effet du travail pendant les études sur la probabilité de quitter l'école au secondaire. Contrairement aux autres articles que nous avons réalisés sur le sujet, nous considérons ici la relation décision de quitter l'école et travail pendant les études d'une manière plus complexe à l'aide de l'enquête auprès des sortants de 1991 de Statistique Canada. En effet, nous introduisons une réflexion sur le processus décisionnel qui conduit les étudiants à quitter l'école à l'aide de deux modèles. Ne sachant pas a priori dans quel modèle un individu spécifique se trouvera, nous utilisons un modèle économétrique afin de déterminer la probabilité qu'un individu se retrouve dans le premier ou le second modèle. Les recherches identifient plusieurs déterminants du décrochage scolaire et différentes politiques qui pourraient être mise en oeuvre par le gouvernement afin de réduire le décrochage scolaire.

D'autres individus, quant à eux, arrêtent complètement de travailler afin de parfaire leur éducation, pour ensuite retourner sur le marché du travail. Le concept de transition entre l'école et le travail devrait donc être employé afin de reconnaître les nombreux mouvements qui existent entre le monde de l'éducation et celui du marché du travail ainsi qu'à l'intérieur de ces deux mondes. Cette étude s'attarde particulièrement aux déterminants du retour aux études. Pour cela, nous avons utilisé l'enquête auprès des sortants de 1991 de Statistique Canada combinée à l'enquête du suivi auprès des sortants de 1995 de Statistique Canada. La modélisation du processus décisionnel des individus face à l'abandon et au raccrochage scolaire est un exercice important suggérant l'influence des caractéristiques personnelles, socioéconomiques ou institutionnelles, sur des décisions primordiales pour l'avenir du capital humain au Canada.

Pigs are confronted with changes in farming practices that may affect performance and animal well-being. The sanitary conditions of the farm can have an impact on the ability of pigs to adapt to ...these changes. This study aimed to analyse how weaned pigs respond to common farming practices of changes in diet and housing in terms of performance, health and behaviour, and how these responses are affected by the sanitary housing conditions, qualified here as good or poor. At weaning at 4 weeks of age, 20 piglets were assigned to 10 blocks of two littermates and each pig within a litter was randomly assigned to one of two sanitary conditions. Pigs were housed individually and received a starter diet. A diet change occurred on day 12 post weaning (starter to weaner diets) and pigs were transferred to the grower unit on day 33 post weaning and continued to receive the weaner diet. From 43 days post weaning, pigs were offered a grower diet and were vaccinated against swine influenza on day 47 and 61 post weaning. On the basis of this design, three post-weaning phases were identified: phase I from day 1 to 11 (post weaning), phase II from day 12 to 32 (after the diet change) and phase III from day 33 to 42 (after the housing change). Individual BW was measured every 3 days, and feed refusals and faecal scores were recorded on a daily basis. Behavioural observations were performed during 28 days by using the instantaneous scan sampling method. Individual blood samples were collected at the end of each phase to analyse the plasma concentration of haptoglobin and on day 68 post weaning to analyse the anti-influenza immunoglobulins G (IgG). Poor sanitary conditions resulted in a decrease in daily gain, feed intake and gain to feed ratio of, respectively, 11%, 5% and 7% (P,0.05). Pigs in poor sanitary conditions had higher faecal scores (P,0.05), tended to have higher plasma haptoglobin concentration in phase II (P50.06) and had a higher anti-influenza IgG titre (P50.11). The diet change affected performance and behavioural responses of pigs in poor but not in good sanitary conditions. Housing change resulted in a 30% decrease in growth and an increase in behaviour oriented towards exploration and excitement. The results of this study show an effect of sanitary conditions on the responses of pigs to a diet change, hereas those to a housing change were little affected by the sanitary conditions.
