Computational methods have made substantial progress in improving the accuracy and throughput of pathology workflows for diagnostic, prognostic, and genomic prediction. Still, lack of ...interpretability remains a significant barrier to clinical integration. We present an approach for predicting clinically-relevant molecular phenotypes from whole-slide histopathology images using human-interpretable image features (HIFs). Our method leverages >1.6 million annotations from board-certified pathologists across >5700 samples to train deep learning models for cell and tissue classification that can exhaustively map whole-slide images at two and four micron-resolution. Cell- and tissue-type model outputs are combined into 607 HIFs that quantify specific and biologically-relevant characteristics across five cancer types. We demonstrate that these HIFs correlate with well-known markers of the tumor microenvironment and can predict diverse molecular signatures (AUROC 0.601-0.864), including expression of four immune checkpoint proteins and homologous recombination deficiency, with performance comparable to 'black-box' methods. Our HIF-based approach provides a comprehensive, quantitative, and interpretable window into the composition and spatial architecture of the tumor microenvironment.
Stellar-activity features such as spots can complicate the determination of planetary parameters through spectroscopic and photometric observations. The overlap of a transiting planet and a stellar ...spot, for instance, can produce anomalies in the transit light-curves that may lead to an inaccurate estimation of the transit duration, depth, and timing. These inaccuracies can for instance affect the precise derivation of the planet radius. We present the results of a quantitative study on the effects of stellar spots on high-precision transit light-curves. We show that spot anomalies can lead to an estimate of a planet radius that is 4% smaller than the real value. Likewise, the transit duration may be estimated about 4%, longer or shorter. Depending on the size and distribution of spots, anomalies can also produce transit-timing variations (TTVs) with significant amplitudes. For instance, TTVs with signal amplitudes of 200 s can be produced when the spot is completely dark and has the size of the largest Sun spot. Our study also indicates that the smallest size of a stellar spot that still has detectable effects on a high-precision transit light-curve is around 0.03 time the stellar radius for typical Kepler Telescope precision. We also show that the strategy of including more free parameters (such as transit depth and duration) in the fitting procedure to measure the transit time of each individual transit will not produce accurate results for active stars.

: The liver is morphologically and functionally modulated by sex hormones. Long‐term use of oral contraceptives (OCs) and anabolic androgenic steroids (AASs) can induce both benign (hemangioma, ...adenoma, and focal nodular hyperplasia FNH) and malignant (hepatocellular carcinoma HCC) hepatocellular tumors. Hepatic adenomas (HAs) are rare, benign neoplasms usually occurring in young women, the development and the complications of which have been related to the strength of OCs and the duration of their use. HA incidence has fallen since the introduction of pills containing smaller amounts of estrogens. FNH is a benign lesion, most commonly seen in young women, which is thought to represent a local hyperplastic response of hepatocytes to a vascular abnormality. Because of the female predominance and the young age at onset, a role of female hormones has been suggested. Furthermore, a large proportion of women with FNH (50–75%) are OC users. Liver hemangiomas (LHs) are the most common benign liver tumors and are seen more commonly in young adult females. The female predilection and clinical observations of LH growth under conditions of estrogenic exposure suggest a possible role for estrogen in the pathogenesis of LHs. HCC has become one of the most widespread tumors in the world in recent years, representing the sixth leading cancer and the third most common cause of death from cancer. Apart from liver cirrhosis, numerous other factors responsible for its onset have been proposed: hepatitis infections from virus B (HBV) and C (HCV), alcohol, smoking, and aflatoxin. However, regardless of etiology, chronic liver diseases progress at unequal rates in the two sexes, with the major sequelae, such as cirrhosis and HCC, being more frequent in men than in women. These epidemiological data have prompted researchers to investigate the relationship between sex hormones and liver tumors. The human liver expresses estrogen and androgen receptors and experimentally both androgens and estrogens have been implicated in stimulating hepatocyte proliferation and may act as liver tumor inducers or promoters.
The Rossiter-McLaughlin (hereafter RM) effect is a key tool for measuring the projected spin-orbit angle between stellar spin axes and orbits of transiting planets. However, the measured radial ...velocity (RV) anomalies produced by this effect are not intrinsic and depend on both instrumental resolution and data reduction routines. Using inappropriate formulas to model the RM effect introduces biases, at least in the projected velocity Vsini⋆ compared to the spectroscopic value. Currently, only the iodine cell technique has been modeled, which corresponds to observations done by, e.g., the HIRES spectrograph of the Keck telescope. In this paper, we provide a simple expression of the RM effect specially designed to model observations done by the Gaussian fit of a cross-correlation function (CCF) as in the routines performed by the HARPS team. We derived a new analytical formulation of the RV anomaly associated to the iodine cell technique. For both formulas, we modeled the subplanet mean velocity vp and dispersion βp accurately taking the rotational broadening on the subplanet profile into account. We compare our formulas adapted to the CCF technique with simulated data generated with the numerical software SOAP-T and find good agreement up to Vsini⋆ ≲ 20 km s-1. In contrast, the analytical models simulating the two different observation techniques can disagree by about 10σ in Vsini⋆ for large spin-orbit misalignments. It is thus important to apply the adapted model when fitting data.

Emphasis on adolescent HIV has increased worldwide as antiretroviral treatment has greatly extended life expectancies of HIV-positive children. Few evidence-based guidelines exist on the optimal time ...to disclose to an adolescent living with HIV (ALHIV); little is known about the medical effects of disclosure. This study looked to determine whether disclosure is associated with improved medical outcomes in ALHIV. Prior work has tended to be qualitative, cross-sectional, and with an emphasis on psychosocial outcomes. This paper addresses the adolescent cohort retrospectively (longitudinally), building upon what is already known about disclosure.
Retrospective, longitudinal clinical record reviews of ALHIV seen at Kericho District Hospital between April 2004 and November 2012 were performed. Patient demographics and clinical outcomes were systematically extracted. The student's t-test was used to calculate changes in mean CD4 count, antiretroviral therapy (ART), and cotrimoxazole adherence pre- vs. post-disclosure. Linear regression modelling assessed for trends in those clinical outcomes associated with age of disclosure.
Ninety-six ALHIV (54 female, 42 male) were included; most (73%) entered care through the outpatient department. Nearly half were cared for by parents, and 20% experienced a change in their primary caregiver. The mean time in the study was 2.47 years; mean number of visits 10.97 per patient over the mean time in the study. Mean disclosure age was 12.34 years. An increase in mean ART adherence percentage was found with disclosure (0.802 vs. 0.917; p = 0.0015). Younger disclosure age was associated with significantly higher mean CD4 counts over the course of the study (p = 0.001), and a nonsignificant trend toward a higher mean ART adherence percentage (p = 0.055).
ART adherence and improved immunologic status are both associated with disclosure of HIV infection to adolescent patients. Disclosure of an HIV diagnosis to an adolescent is an important means to improve HIV care.
