Introduction: Accidents involving spiders bites usually cause mild medical reactions that lead to local symptoms and, less commonly, systemic effects. The most medically significant spiders belong to ...the genera Latrodectus and Loxosceles. This paper presents a posible case of steatodism in a young woman and her pet.Case description: 26-year-old female patient, who reports a clinical history characterized by paresthesia, malaise, fever, diarrea and a painful papule in the left cheek after being bitten by a spider. Immediately after being bit, the patient hit the spider with the back of her hand and it fell to the ground, where her dog swallowed it. The dog presented with vomiting and general discomfort after ingestion. Symptomatic therapy was given for comfort, and neither the patient nor the dog required antivenin therapy. Both evolved favorably.Discussion: The relevance of this case is the involvement of two mammals (a human and her dog) due to the accidental contact with a spider, possibly of the genus Latrodectus or Steatoda. Conclusion: Two possible cases of steatodism are described. Since spider bites are a relatively frequent reason for medical consultation in Colombia, it is important to diagnose and manage them properly.
Introduction: Delusional infestation is a rare psychiatric disorder defined as a condition in which the patient has the unshakable belief and perception of being infested with parasites. Its ...treatment is difficult, and frequently includes antipsychotic medications (such as olanzapine or aripiprazole). Non-pharmacological treatment, particularly psychotherapy, can be used for less severe cases. Dermatologists and psychiatrists must take a multi-disciplinary approach (preferably in a psycodermatology dedicated clinic) since this type of patients sometimes refuse treatment.Case description: A 53-year-old female businesswoman describes a clinical history of five years of visual hallucinations, depressive symptoms, and generalized pruritus, along with the use of toxic substances to “clean” her skin and cloths. She reports similar symptoms in some relatives but they were not evaluated. Blood tests and analyses of the “specimen” brought by the patient were performed, yielding negative results. The patient had never been assessed by any specialist, and showed disoriented during the consultation. Follow-up was not possible due to the reluctance of the patient to follow the indications and seek psychiatric treatment. Moreover, the patient did not respond to further communication attempts.Discussion: Delusional infestation is an uncommon disease that endangers the patients and the people around them. Its treatment is difficult and long, and not conducting proper follow-up is a great risk. Its prevalence and incidence is variable and generally unknown. It can affect the patient, their next of kin, pets or the environment, and the “pathogen” can be a living organism or an inanimate object. Conclusion: This case is important as it shows the hardships of treatment, adequate follow-up and care, as well as the need to improve how these patients are approached. Additionally, both classical and uncommon signs and symptoms could be observed as the patient stated that her relatives were affected (possible delusional infestation by proxy).
BackgroundObstetric morbidity (OM) is higher in SLE women than in healthy ones. Few data on SLE pregnancy outcomes in Latin America (LA) have been reported.ObjectivesTo study SLE pregnancy outcomes ...in LA.MethodsGLADEL 2.0 is an observational prevalent/incident cohort started in 2019. To date, 43 centers from 10 LA countries have enrolled 1030 SLE patients (1982/1997 ACR or SLICC criteria). Women with ≥1 pregnancy were included. Past and ongoing (6, 12, 24 months follow-up) OM (miscarriages, fetal deaths, preeclampsia, prematurity, neonatal lupus) were evaluated.ResultsAt inclusion, 329 women have had at least one pregnancy median (IQR): 2 (1–3): table 1. Of them, 293 (89.1%) had ≥1 live birth and 183 (55.6%) developed OM. Preeclampsia occurred in 49 (14.9%). Among 71 (21.6%) women with anti-SS-A(Ro)/SS-B(La) antibodies, 3 (4.2%) developed neonatal lupus (no cardiac involvement). Antiphospholipid syndrome (APS) was associated with higher risk of OM (52.2% vs 10.0%; p< 0.001). Of the 755 pregnancies reported, 551 (73.0%) resulted in live births, of which 79 (14.3%) were premature. The remaining pregnancies ended in 178 (23.6%) miscarriages and 41 (5.4%) fetal deaths. During 2-follow-up years (figure 1), 24 singles pregnancies occurred. All were under antimalarials; 16 (66.7%) resulted in live births, 4 (25.0%) premature; 12 (50.0%) developed OM. There were seven (29.2%) miscarriages and one fetal loss (4.2%) related to severe preeclampsia. One cholestasis gravidarum (4.2%) lead to prematurity. There were no new cases of neonatal lupus.ConclusionIn GLADEL 2.0 cohort, around half of the studied women presented OM being frequently related to APS. Miscarriages, prematurity, preeclampsia and fetal deaths were the most common pregnancy complications. The incidence of neonatal lupus was lower than previously reported.1 ReferenceCimaz R, et al. Incidence and spectrum of neonatal lupus erythematosus: a prospective study of infants born to mothers with anti-Ro autoantibodies. J Pediatr 2003;142:678–83.Lay summaryPregnancy in patients with lupus carries increased risk to both the mother and baby. Black and Hispanic patients are the most seriously affected, but pregnancy in lupus is not well understood in Latin American patients. Results from a multiethnic, multinational Latin American cohort of patients with lupus showed that miscarriages, prematurity, preeclampsia, and fetal deaths were the most common pregnancy complications. The results of this study highlight the great unmet need in improved care of patients with lupus during pregnancy to protect the health of the mother and baby.Abstract 609 Table 1Characteristics of SLE women with ≥1 pregnancy at cohort inclusion related to OM1 Variables OM1 p value2 Variables OM1 p value2 Epidemiological/Comorbidities No (n=146) Yes (n=183) SLE background No (n=146) Yes (n=183) Age (years)3 41(34–47) 39(31.5–50) 0.542 Disease duration (months)3 76(28–53) 100(36.5–62.5) 0.136 Education (years)3 12(10.2–15) 12(10–15) 0.664 Antiphospholipid syndrome 3/30 (10%) 24/46 (52.2%) 0.001 Ethnicity 0.299 Laboratory Afro-Latin American 14/146 (9.6%) 8/183 (4.4%) Anti-dsDNA antibodies 107/134 (79.9%) 139/172 (80.8%) 0.885 White 30/146 (20.5%) 42/183 (23.0%) Anti-Ro antibodies 51/109 (46.8%) 57/135 (42.2%) 0.518 Amerindian 3/146 (2.1%) 4/183 (2.2%) Anti-La antibodies 20/107 (18.7%) 18/132 (13.6%) 0.293 Mestizo 99/146 (67.8%) 129/183 (70.5%) C3 and/or C4, low 117/141 (83.0%) 147/174 (84.5%) 0.760 Socioeconomic level 0.184 Lupus anticoagulant 17/92 (18.5%) 34/121 (28.1%) 0.109 High 29/143 (20.3%) 41/181 (22.7%) aCL4 IgG 21/101 (20.8%) 34/129 (26.4%) 0.353 Medium 42/143 (29.4%) 67/181 (37.0%) aCL4 IgM 15/101 (14.9%) 33/130 (25.4%) 0.071 Low 72/143 (50.3%) 73/181 (40.3%) Anti-ß2GP15 IgG 11/79 (13.9%) 15/95 (15.8%) 0.832 Medical coverage 0.260 Anti-ß2GP15 IgM 10/79 (12.7%) 15/95 (15.8%) 0.666 Complete/partial 94/141 (66.7%) 133/182 (73.1%) Treatment No Coverage 47/141 (33.3%) 49/182 (26.9%) Corticosteroids 143/146 (97.9%) 176/181 (97.2%) 0.736 Hypertension 47/83 (56.6%) 73/110 (66.4%) 0.180 Antimalarials 141/146 (96.6%) 177/181 (97.8%) 0.520 Diabetes mellitus 9/83 (10.8%) 6/110 (5.5%) 0.184 Immunosuppressors 118/146 (80.8%) 152/179 (84.9%) 0.373 Dyslipidemia 23/78 (29.5%) 31/109 (28.4%) 0.872 Aspirin 30/42 (71.4%) 53/65 (81.5%) 0.243 Smoking 22/50 (44.0%) 34/58 (58.6%) 0.176 Anticoagulation 18/42 (42.9%) 33/66 (50.0%) 0.554 1Obstetric morbidity; 2 statistically significance: p< 0.05; 3 median (interquartile range); 4 anti-cardiolipin antibodies; 5 beta-2 glycoprotein I antibodies.Abstract 609 Figure 1SLE pregnancy outcome 2-year follow-upDisclosure of interestRosa Maria Serrano Morales: None declared, Romina Nieto: None declared, Rosana Quintana: None declared, Paula Alba: None declared, Sabrina POrta: None declared, Lucia Hernández: None declared, Guillermo Berbotto: None declared, Verónica Inés Bellomio: None declared, Nílzio da Silva: None declared, Odirlei Monticielo: None declared, Fernando Cavalcanti: None declared, Francinne Machado Ribeiro: None declared, Eduardo Borba: None declared, Eloisa Bonfa: None declared, loreto massardo: None declared, Gustavo Aroca Martínez: None declared, Andrés Cadena Bonfanti: None declared, GERARDO QUINTANA LOPEZ: None declared, Mario Javier MORENO ALVAREZ: None declared, Jorge Antonio Esquivel Valerio: None declared, Isabel Acosta-Colman: None declared, Astrid Paats: None declared, CLAUDIA MORA: None declared, Marina Scolnik: None declared, Diana Fernández Ávila: None declared, Carmen Funes Soaje: None declared, Veronica Saurit: None declared, Mercedes García: None declared, Eduardo Kerzberg: None declared, Graciela Gomez: None declared, Cecilia Pisoni: None declared, Edgard Reis Neto: None declared, Iris Guerra Herrera: None declared, Oscar Neira: None declared, Carlos Cañas: None declared, Miguel A Saavedra: None declared, Margarita Portela: None declared, Hilda Fragoso loyo: None declared, Luis Humberto Silveira Torre: None declared, Ignacio Garcia-De La Torre: None declared, Manuel F. Immobilized enzymes offer different benefits such as the feasibility to be reused for reproducible bioprocesses. The challenge is to establish the appropriate storage conditions that allow the ...maintenance of their properties for long periods. In this study, we immobilized a recombinant leucine aminopeptidase (I-rLAP) on a siliceous support synthesized from tetraethyl orthosilicate (TEOS) activated with glutaraldehyde to evaluate its residual activity after storage in 20% v/v ethanol and sodium azide solutions at 4 and 25 °C. The characterization of the support by X-ray diffraction (XRD), diffuse reflectance infrared Fourier transform (DRIFT) and field emission probe microanalyzer (EPMA) was consistent with previous characterization reports of silica gel matrices. Particle size ≤420 μm exhibited a suitable performance that avoided high backpressure into the columns and increased the amount of immobilized enzyme. I-rLAP recovered up to 90% of the applied activity after 64 days of storage at 4 and 25 °C in 20% v/v ethanol. Conversely, no effect was observed when the insoluble enzyme was stored in sodium azide. Activity recovery of I-rLAP after storage in ethanol solution could be related to the formation of disulfide bonds as suggested by free thiol analyses. Reverse phase-ultra performance liquid chromatography (RP-UPLC) and Mass Spectrometry confirmed that the immobilized enzyme maintained its specificity to remove N-terminal methionine from a recombinant hormone. The obtained results indicate that this methodology constitutes an alternative for bioprocesses involving long-term storage of immobilized enzymes.

Introduction: The study of the nervous system can be challenging for students in the basic cycle of medical sciences. This study is specifically focused on the teaching-learning process of the ...cranial nerves.
Objective: The aim of this study is to describe the visibility and impact of scientific production related to the teaching-learning process of the cranial nerves in Scopus.
Method: A bibliometric analysis of articles on the teaching-learning process of the cranial nerves published between 2017 and 2021 in the Scopus database was carried out. Production, visibility, and impact variables were analyzed, including the number of documents, number of citations, and weighted citation impact.
Results: A total of 64 articles were published, with the highest production in 2019 (25%; n = 16). The articles received a total of 282 citations, with 30.6% of them corresponding to 2017. The highest weighted citation impact value was reported in 2017 (1.3). Collaborative international articles accounted for 13.3%, while single authorship represented 7.2%. National collaboration works had a higher average citation per publication (4.9; n = 163) and a better weighted citation impact index (1.14).
Conclusions: The study found a low scientific production on the teaching-learning of the cranial nerves, with a decreasing trend and a predominance of multiple authorship, mainly national and institutional. The impact index of publications was found to be low based on their citations.
