Achilles tendinitis is a common sport-induced injury. The clinical findings of several cases of recurrent Achilles tendinitis show thickened tendon, painful with movement and with palpation, and with ...the existence of nodules in the distal end of the tendon. Musculoskeletal ultrasound shows volume increase, hypoechoic and hyperechoic images in the tendon's structure and intratendinous calcifications. When the athlete refers repetitive pain at the Achilles level, degeneration and intratendinous rupture must be considered.
This study evaluated the scale-up of a remote monitoring (RM) service, capturing monthly Rheumatoid Arthritis Impact of Disease scores and patient-generated text messages, for patients with ...rheumatoid arthritis (RA; in remission or with low disease activity) attending routine outpatient clinics across six hospitals. We explored patients and staff experiences and implementation outcomes.
A pragmatic, mixed methods approach was used, with active patient involvement throughout. We undertook a rapid review, analysed service-level data, and conducted a patient survey and patient and staff interviews, informed by the Capability, Opportunity, Motivation, Behaviour (COM-B) and Exploration, Preparation, Implementation, Sustainment (EPIS) theoretical frameworks.
The review included 37 articles, covering themes of patient and clinician acceptability, engagement, feasibility and clinical impact. Service-level data (n = 202) showed high levels of patient engagement with the service. The patient survey (n = 155) showed patients felt the service was easy to use, had confidence in it and felt it improved access to care. Patient interview (n = 22) findings mirrored those of the survey. Motivating factors included increased responsiveness and ease of contact with clinical teams. Views from staff interviews (n = 16) were more mixed. Some implementation barriers were specific to roll-out sites. Prioritisation of staff needs was emphasised.
Patients were positive about the service and engagement was high. Staff views and engagement were more mixed. Results suggest that equal levels of patient and staff engagement are required for sustainability. These findings further our understanding of the implementation challenges to scaling RM interventions for patients with RA in routine care settings.
A 71-year-old white female experienced a nondisplaced right central acetabular fracture in a motor vehicle accident. Despite anteroposterior (AP) roentgenograms of the pelvis, Judet views, and plain ...tomograms, no fracture was detected. These negative findings did not entirely explain the continued presence of pain. The patient was treated with physical therapy and encouraged to bear weight on the leg as tolerated. The fracture then progressed to a central fracture dislocation readily apparent on AP views of the pelvis. This case illustrates the need for further diagnostic studies (bone scanning, computed tomography, and magnetic resonance imaging) when, despite negative objective findings, a high degree of suspicion persists.
On May 25, 2005, Ireland lodged the first example of a so-called "partial submission" to the Commission on the Limits of the Continental Shelf in respect of an allegedly "undisputed" area of seabed ...beyond 200 nm limits sandwiched between disputed and extensive seabed claims to the north (Hatton-Rockall Area) by Denmark/Faroes and Iceland, and an as yet undelimited seabed area to the south (the Celtic Shelf) where the United Kingdom, France, and Spain may also have seabed claims. This article considers the problems that may arise even from this supposedly uncontroversial type of submission in the light of the diplomatic reaction by both Denmark/Faroes and Iceland in August 2005.
Commercial food packaging, in both bulk and unit sizes, can be found in archaeological assemblages. Newspaper advertisements from the ca. 1740 to 1820 period provide information on the shape, size, ...and contents of wood, metal, glass, ceramic, and fiber containers. The advertisements also show that elements of consumerism, in the form of brand names, distinctive packaging, unit packaging, international markets, fixed prices, ready-made items, and targeted markets, were practiced in the food industry by the early years of the 19th century.
Although malaria was officially declared eradicated from Europe in 1975, its former vectors, mainly members of the Anopheles maculipennis (Meigen) complex, are still distributed throughout the ...continent. The present situation of Anophelism without malaria indicates that current socio-economic and environmental conditions maintain the basic case reproduction number, Ro, below 1. Recently, it has been speculated that predicted climate changes may increase anopheline abundance and biting rates (as well as reduce the Plasmodium parasite extrinsic incubation period), allowing the reemergence of malaria transmission in Europe. As a preliminary step toward predicting future scenarios, we have constructed models to test whether the current distribution of the five former European malaria vectors An. atroparvus (Van Thiel), An. labranchiae (Falleroni), An. messeae (Swellengrebel & De Buck), An. sacharovi (Favr) and An. superpictus (Grassi) can be explained by environmental parameters, including climate. Multivariate logistic regression models using climate surfaces derived from interpolation of meteorological station data (resolution 0.5 × 0.5°) and remotely sensed land cover (resolution 1 × 1 km) were fitted to 1,833 reported observations of the presence and absence of each species across Europe. These relatively crude statistical models predicted presence and absence with a sensitivity of 74–85.7% and specificity of 73.4–98.1% (with climate a significantly better predictor than land cover type). A geographically independent validation of the models gave a sensitivity of 72.9–88.5% and a specificity of 72.7–99.6%. This allowed us to generate risk maps for each species across Europe. Assuming that high risk equates with the potential for high abundance, these models should permit the development of risk maps for European mosquitoes under future climate scenarios. These techniques would be equally useful for estimating the risk of reemergence in other nonendemic areas such as the United States and Australia, as well as changes to risk within endemic areas.
