Abstract
Introduction
The role of elastin in tympanic retractions and chronic otitis media is not well established. Williams Syndrome (WS), a pathology related to a mutation in the elastin gene, ...could generate tympanic retractions.
Objective
To compare the prevalence of tympanic retractions among patients with WS and controls.
Methods
WS patients (
n
= 43 ears) and controls (
n
= 130 ears) were evaluated by digital otoscopic examination and the degree of tympanic membrane retraction was classified by 2 blinded experienced otolaryngologists.
Results
The agreement rate between the evaluators was 71.1% for pars tensa and 65% for pars flaccida retraction (
p
< 0.001). The pars tensa and pars flaccida retractions are present in patients with WS after an adjusted residue of respectively - 2.8 and - 2.6 (
p
= 0.011 and
p
= 0.022) compared with controls.
Conclusions
Tympanic membrane retractions are not more common in the WS group when compared with controls.
Fundamentação: A fibrose cística (FC) é uma das doenças genéticas letais mais comuns. Nos últimos anos, a sobrevida dos pacientes portadores dessa doença tem aumentado em decorrência principalmente ...do uso de antibióticos profiláticos e terapêuticos. Estes apresentam potencial ototóxico, sendo responsáveis por perdas auditivas sensorioneurais (SSN). Além disso, os pacientes com FC apresentam rinossinusopatias, sintomáticas e assintomáticas, que também necessitam de acompanhamento adequado. Objetivos: Definir a prevalência de alterações auditivas sensorioneurais (SSN) e rinossinusopatias nos pacientes com FC do Hospital de Clínicas de Porto Alegre (HCPA); descrever os possíveis fatores relacionados com tais incidências; e, a partir dessas informações, projetar o funcionamento do ambulatório de otorrinolaringologia (ORL) e FC do HCPA. Material e Métodos: Estudo retrospectivo, em que foram revisados os prontuários de pacientes com FC do Ambulatório de Pneumologia Infantil e de Adultos do HCPA. Foi criado um protocolo a ser preenchido em que se buscava a descrição do perfil da amostra em relação à idade do diagnóstico da doença, idade atual, tipos de antibióticos já utilizados, média do número de tratamentos, percentual que possui avaliação auditiva com resultados, e, por último, investigação radiológica e resultados. Resultados: Foram revisados 107 prontuários de pacientes com FC. A idade média da amostra foi de 7,87 anos e 1,33 anos a idade média de diagnóstico de FC. Os exames audiométricos foram realizados por 39,3% (42) das crianças, encontrando-se 28,56% (12) casos de SSN de graus variados. Um percentual mais elevado apresentava ausência de reflexo estapédico (36,3%). Somente 24,3% (26) realizaram RX dos seios da face nos quais 96.9% estavam alterados. Um total de 667 tratamentos com aminoglicosídeos, sendo 49.5% com tobramicina, 43.4% com amicacina e 7% com gentamicina. Conclusão: Diante desses resultados mostra-se necessário um ambulatório específico de otorrinolaringologia e de FC para acompanhamento desses pacientes. O Ambulatório terá como objetivo auxiliar na prevenção e tratamento das perdas auditivas decorrentes dos tratamentos com aminoglicosídeos, assim como tratar, quando necessário, das vias aéreas superiores em relação à obstrução e inflamação associadas a presença de pólipos rinossinusais. Este ambulatório já está em funcionamento.
To evaluate clinical and evolutive characteristics of patients admitted in an intensive care unit after cardiopulmonary resuscitation, identifying prognostic survival factors.
A retrospective study ...of 136 patients admitted between 1995 and 1999 to an intensive care unit, evaluating clinical conditions, mechanisms and causes of cardiopulmonary arrest, and their relation to hospital mortality.
A 76% mortality rate independent of age and sex was observed. Asystole was the most frequent mechanism of death, and seen in isolation pulmonary arrest was the least frequent. Cardiac failure, need for mechanical ventilation, cirrhosis and previous stroke were clinically significant (p < 0.01) death factors.
Prognostic factors supplement the doctor's decision as to whether or not a patient will benefit from cardiopulmonary resuscitation.

Besides the well-known prevalence of chronic ...rhinosinusitis, neurosensory hearing loss (NSHL) due to the large use of ototoxic antibiotics may be another problem that should receive special attention in otolaryngology (ENT).
Define retrospectively the prevalence of NSHL among patients with CF from the Hospital de Clínicas de Porto Alegre (HCPA) and based on this data discuss the need for a specific out patient ENT-CF clinic.
Files from CF HCPA patients were reviewed for age at diagnosis, the current age, types of antibiotics used, number of treatments, percentage with audiologic evaluation and its respective results.
One hundred and seven files were reviewed. The mean age at diagnosis was 1.33 years old (SD 1.97, range 0-12). The mean age of the sample was 7.87 (SD 4.49, range 1-22). Audiologic evaluation was performed in 39.3% (42) of the patients. 28.56% (12) had some degree of NSHL, while 36.3% did not present acoustic reflex. From 667 treatments with antibiotics, 49.5% were with tobramycin, 43.4% with amycacin and 7% with gentamicin.

Objetivo: Comparar os resultados encontrados no exame otoscópico em pacientes com síndrome de Williams e participantes sem a síndrome, a fim de estabelecer correlação entre anormalidades na elastina ...e retrações da membrana timpânica. Desenho do estudo: Estudo de prevalência, controlado. Local: Serviço de Otorrinolaringologia do Hospital de Clínicas de Porto Alegre Pacientes: Pacientes com diagnóstico definitivo de Síndrome de Williams e controles. Avaliação: Avaliação otoscópica digital de pacientes com Síndrome de Williams e controles, e classificação do grau da retração da membrana timpânica por dois avaliadores independentes. Principal medida de desfecho: Concordância entre avaliadores, prevalência de retrações em ambos os grupos. Resultados: A concordância entre os avaliadores foi de 71,1% para a retração da pars tensa e 65% para a retração da pars flaccida (p<0,001). As retrações da pars tensa e da pars flaccida apresentaram um resíduo ajustado de -2,8 (p=0.011) e -2,6 (p=0,022), respectivamente, em paciente portadores da síndrome de Williams, em comparação com os pacientes sem a síndrome. Conclusão: Neste estudo as retrações da membrana timpânica não foram significativamente prevalentes em pacientes com alterações na elastina.
