Systematic literature review and narrative synthesis.
A systematic review of published literature using MEDLINE, Embase and PsycINFO without geographical or temporal limits was conducted. We included studies that explicitly linked high ambient temperatures and/or heat waves to mental health outcomes. Evidence was graded using a National Institute of Health framework.
A total of 35 studies were included in the review, among which 34 were observational studies and one was a case–control study. Six broad mental health outcome categories were identified: suicide and heat; bipolar disorder, mania and depression and heat; schizophrenia and heat; organic mental health outcomes and heat including dementia; alcohol and substance misuse and heat; and multiple mental health outcomes/mental health service usage and heat. The studies included were heterogeneous in terms of study design, population, setting, exposure measures, outcomes and location making meta-analysis unfeasible. Fifteen studies showed an increased suicide risk with heat (relative risk RR 1.014–1.37 per 1 °C, P < 0.05; r = 0.10–0.64, P < 0.05). Increased risks of mental health-related admissions and emergency department visits at higher temperatures were also found. No evidence of change in community care use was found. Evidence for the impact of heat on other mental health outcomes was mixed.
High ambient temperatures have a range of mental health effects. The strongest evidence was found for increased suicide risk. Limited evidence was found for an increase in heat-related morbidity and mortality among people with known mental health problems. Knowledge gaps exist on the impact of high temperatures on many common mental health disorders. Mental health impacts should be incorporated into plans for the public health response to high temperatures, and as evidence evolves, psychological morbidity and mortality temperature thresholds should be incorporated into hot weather–warning systems.

For women with ductal carcinoma in situ (DCIS) of the breast, the risk of developing an ipsilateral breast event (IBE; defined as local recurrence of DCIS or invasive carcinoma) after surgical ...excision without radiation is not well defined by clinical and pathologic characteristics.
The Oncotype DX breast cancer assay was performed for patients with DCIS treated with surgical excision without radiation in the Eastern Cooperative Oncology Group (ECOG) E5194 study. The association of the prospectively defined DCIS Score (calculated from seven cancer-related genes and five reference genes) with the risk of developing an IBE was analyzed using Cox regression. All statistical tests were two-sided.
There were 327 patients with adequate tissue for analysis. The continuous DCIS Score was statistically significantly associated with the risk of developing an IBE (hazard ratio HR = 2.31, 95% confidence interval CI = 1.15 to 4.49; P = .02) when adjusted for tamoxifen use (prespecified primary analysis) and with invasive IBE (unadjusted HR = 3.68, 95% CI = 1.34 to 9.62; P = .01). For the prespecified DCIS risk groups of low, intermediate, and high, the 10-year risks of developing an IBE were 10.6%, 26.7%, and 25.9%, respectively, and for an invasive IBE, 3.7%, 12.3%, and 19.2%, respectively (both log rank P ≤ .006). In multivariable analyses, factors associated with IBE risk were DCIS Score, tumor size, and menopausal status (all P ≤ .02).
The DCIS Score quantifies IBE risk and invasive IBE risk, complements traditional clinical and pathologic factors, and provides a new clinical tool to improve selecting individualized treatment for women with DCIS who meet the ECOG E5194 criteria.
Summary Background A serogroup A meningococcal polysaccharide–tetanus toxoid conjugate vaccine (PsA–TT, MenAfriVac) was licensed in India in 2009, and pre-qualified by WHO in 2010, on the basis of ...its safety and immunogenicity. This vaccine is now being deployed across the African meningitis belt. We studied the effect of PsA–TT on meningococcal meningitis and carriage in Chad during a serogroup A meningococcal meningitis epidemic. Methods We obtained data for the incidence of meningitis before and after vaccination from national records between January, 2009, and June, 2012. In 2012, surveillance was enhanced in regions where vaccination with PsA–TT had been undertaken in 2011, and in one district where a reactive vaccination campaign in response to an outbreak of meningitis was undertaken. Meningococcal carriage was studied in an age-stratified sample of residents aged 1–29 years of a rural area roughly 13–15 and 2–4 months before and 4–6 months after vaccination. Meningococci obtained from cerebrospinal fluid or oropharyngeal swabs were characterised by conventional microbiological and molecular methods. Findings Roughly 1·8 million individuals aged 1–29 years received one dose of PsA–TT during a vaccination campaign in three regions of Chad in and around the capital N'Djamena during 10 days in December, 2011. The incidence of meningitis during the 2012 meningitis season in these three regions was 2·48 per 100 000 (57 cases in the 2·3 million population), whereas in regions without mass vaccination, incidence was 43·8 per 100 000 (3809 cases per 8·7 million population), a 94% difference in crude incidence (p<0·0001), and an incidence rate ratio of 0·096 (95% CI 0·046–0·198). Despite enhanced surveillance, no case of serogroup A meningococcal meningitis was reported in the three vaccinated regions. 32 serogroup A carriers were identified in 4278 age-stratified individuals (0·75%) living in a rural area near the capital 2–4 months before vaccination, whereas only one serogroup A meningococcus was isolated in 5001 people living in the same community 4–6 months after vaccination (adjusted odds ratio 0·019, 95% CI 0·002–0·138; p<0·0001). Interpretation PSA–TT was highly effective at prevention of serogroup A invasive meningococcal disease and carriage in Chad. How long this protection will persist needs to be established. The dot-probe task is a widely used experimental paradigm that evaluates attention biases within anxiety disorders. Considerable research has focused on improving the reliability of dot-probe scores ...because the task's original attention bias index has shown very low test-retest reliability. The current study serves as a replication and extension of Price et al. (2015), who systematically examined the effects of methodological choices on reliability of dot-probe task results. Fifty-six adults diagnosed with social anxiety disorder were asked to complete a facial dot-probe task twice, approximately 1 week apart. Test-retest reliability and internal consistency were examined for 10 dot-probe attention bias indices across 55 sets of outlier cutoffs. Both Pearson's r and intraclass correlation coefficients were used. Trial-level bias score indices of mean bias toward threat and attention bias variability, which measure attention bias dynamically using individual pairs of trials, demonstrated the highest reliability and were less sensitive to changes in outlier cutoffs as compared with the dot probe's classic attention bias index and others. Results demonstrate the potential for post hoc outlier cutoff selection to artificially inflate reliability, particularly for unreliable indices. A priori cutoff selection is recommended for future research. Intraclass correlation coefficients are also recommended for assessment of reliability because Pearson's r does not account for poor agreement between scores.

This study examined the reliability of different data analytic strategies for the dot-probe task, a computer task designed to measure attention bias toward threat within anxiety disorders. It found the highest reliability for methods that measure temporal fluctuations in attention bias and found that methods of handling outlier values can substantially affect estimates of reliability.
Complex associations exist among socioeconomic status (SES) in early life, beliefs about oral health care (held by individuals and their parents), and oral health–related behaviors. The pathways to ...poor adult oral health are difficult to model and describe, especially due to a lack of longitudinal data. The study aim was to explore possible pathways of oral health from birth to adulthood (age 38 y). We hypothesized that higher socioeconomic position in childhood would predict favorable oral health beliefs in adolescence and early adulthood, which in turn would predict favorable self-care and dental attendance behaviors; those would lead to lower dental caries experience and better self-reported oral health by age 38 y. A generalized structural equation modeling approach was used to investigate the relationship among oral health–related beliefs, behaviors in early adulthood, and dental health outcomes and quality of life in adulthood (age, 38 y), based on longitudinal data from a population-based birth cohort. The current investigation utilized prospectively collected data on early (up to 15 y) and adult (26 and 32 y) SES, oral health–related beliefs (15, 26, and 32 y), self-care behaviors (15, 28, and 32 y), oral health outcomes (e.g., number of carious and missing tooth surfaces), and oral health–related quality of life (38 y). Early SES and parental oral health–related beliefs were associated with the study members’ oral health–related beliefs, which in turn predicted toothbrushing and dental service use. Toothbrushing and dental service use were associated with the number of untreated carious and missing tooth surfaces in adulthood. The number of untreated carious and missing tooth surfaces were associated with oral health–related quality of life. Oral health toward the end of the fourth decade of life is associated with intergenerational factors and various aspects of people’s beliefs, SES, dental attendance, and self-care operating since the childhood years.
We present the 2SXPS (Swift-XRT Point Source) catalog, containing 206,335 point sources detected by the Swift X-ray Telescope (XRT) in the 0.3-10 keV energy range. This catalog represents a ...significant improvement over 1SXPS, with double the sky coverage (now 3790 deg2), and several significant developments in source detection and classification. In particular, we present for the first time techniques to model the effect of stray light-significantly reducing the number of spurious sources detected. These techniques will be very important for future, large effective area X-ray missions, such as the forthcoming Athena X-ray observatory. We also present a new model of the XRT point-spread function and a method for correctly localizing and characterizing piled-up sources. We provide light curves-in four energy bands, two hardness ratios, and two binning timescales-for every source, and from these deduce that over 80,000 of the sources in 2SXPS are variable in at least one band or hardness ratio. ABSTRACT
The tick microbiota may influence the colonization of Ixodes scapularis by Borrelia burgdorferi, the Lyme disease bacterium. Using conserved and pathogen-specific primers we performed a ...cross-kingdom analysis of bacterial, fungal, protistan and archaeal communities of I. scapularis nymphs (N = 105) collected from southern Vermont, USA. The bacterial community was dominated by a Rickettsia and several environmental taxa commonly reported in I. scapularis, as well as the human pathogens B. burgdorferi and Anaplasma phagocytophilum, agent of human granulocytic anaplasmosis. With the fungal primer set we detected primarily plant- and litter-associated taxa and >18% of sequences were Malassezia, a fungal genus associated with mammalian skin. Two 18S rRNA gene primer sets, intended to target protistan communities, returned mostly Ixodes DNA as well as the wildlife pathogen Babesia odocoilei (7% of samples), a Gregarines species (14%) and a Spirurida nematode (18%). Data from pathogen-specific and conserved primers were consistent in terms of prevalence and identification. We measured B. burgdorferi presence/absence and load and found that bacterial beta diversity varied based on B. burgdorferi presence/absence. Load was weakly associated with bacterial community composition. We identified taxa associated with B. burgdorferi infection that should be evaluated for their role in vector colonization by pathogens.
This study describes the microbial communities of I. scapularis nymphs, in relation to B. burgdorferi presence/absence and identifies specific taxa associated with infection that should be further explored for their role in vector colonization by human pathogens.
