Primary cutaneous lymphomas (PCL) are uncommon. Observations based on the first year of data from the Spanish Registry of Primary Cutaneous Lymphomas (RELCP, in its Spanish abbreviation) of the ...Spanish Academy of Dermatology and Venereology (AEDV) were published in February 2018. This report covers RELCP data for the first 5 years.
RELCP data were collected prospectively and included diagnosis, treatments, tests, and the current status of patients. We compiled descriptive statistics of the data registered during the first 5 years.
Information on 2020 patients treated at 33 Spanish hospitals had been included in the RELCP by December 2021. Fifty-nine percent of the patients were men; the mean age was 62.2 years. The lymphomas were grouped into 4 large diagnostic categories: mycosis fungoides/Sézary syndrome, 1112 patients (55%); primary B-cell cutaneous lymphoma, 547 patients (27.1%); primary CD30+lymphoproliferative disorders, 222 patients (11%), and other T-cell lymphomas, 116 patients (5.8%). Nearly 75% of the tumors were registered in stage I. After treatment, 43.5% achieved complete remission and 27% were stable at the time of writing. Treatments prescribed were topical corticosteroids (1369 67.8%), phototherapy (890 patients 44.1%), surgery (412 patients 20.4%), and radiotherapy (384 patients 19%).
The characteristics of cutaneous lymphomas in Spain are similar to those reported for other series. The large size of the RELCP registry at 5 years has allowed us to give more precise descriptive statistics than in the first year. This registry facilitates the clinical research of the AEDV's lymphoma interest group, which has already published articles based on the RELCP data.

The aims of this study were to describe the demographic and clinical characteristics of patients with scabies seen by dermatologists in Spain, to identify the possible emergence of atypical forms of scabies, and to explore the frequency of treatment failures and associated risk factors.
We conducted an observational, cross-sectional, multicenter study of data collected prospectively in April and May 2023 using the CLINI-AEDVp platform created by the Spanish Academy of Dermatology and Venereology (AEDV).
Participating dermatologists from 31 hospitals in 15 of Spain's autonomous communities recorded 186 cases of active scabies (51% in women) during the study period. A diagnostic certainty level of A, B or C as per the International Alliance for the Control of Scabies Consensus Criteria was required for diagnosis. Overall, 92% of patients had typical scabies and 66% had already been treated with a scabicide for the current episode. Of the treated patients, only 36% had received and completed adequate treatment (including the simultaneous treatment of all household members) and 50% had not received clear written recommendations.
In a high proportion of the cases of scabies studied, the patient had already received treatment. In those cases, we observed several remediable shortcomings that could explain why some of these treatments had failed. Remedying these deficiencies should lead to better control of scabies and an improved assessment of the actual effectiveness of currently available scabicides.

We estimated the cost of treating CTCL over a period of 1year from the perspective of the Spanish National Health System. Twenty-three dermatologists and hematologists from 15 public hospitals analyzed data for adult patients with mycosis fungoides (MF) or Sézary syndrome (SS).
A total of 141 patients (57.4% male) with a mean age of 63.6 years (95%CI: 61.4-65.7 years) were included. The mean direct annual cost of treating CTCL was €34,214 per patient. The corresponding costs by stage were €11,952.47 for stageI disease, €23,506.21 for stageII disease, €38,771.81 for stageIII disease, and €72,748.84 for stageIV disease. The total direct annual cost of treating MF/SS in public hospitals in Spain was estimated at €78,301,171; stageI disease accounted for 81% of all costs, stageII for 7%, and stagesIII andIV for 6% each.
The MICADOS study offers an accurate picture of the direct cost of treating CTCL in patients with MF/SS in Spain and shows that costs vary significantly according to disease stage. Patient-borne and indirect costs should be analyzed in future studies.

No se dispone de datos españoles sobre el coste asociado al linfoma cutáneo de célulasT (LCCT). Además, la incorporación de nuevos tratamientos hace necesario analizar el coste real de la enfermedad. El estudio MICADOS analizó dos objetivos principales: Por un lado, evaluó el impacto en la calidad de vida en los pacientes con LCCT, y por otro lado, estudió los costes de la enfermedad. En esta publicación se recoge el segundo de los objetivos del estudio.

El coste anual directo total estimado de todos los pacientes en España con MF/SS resultó en 78.301.171€, donde el 81% de los costes fueron atribuibles a pacientes en estadioI, el 7% al estadioII, el 6% al estadioIII y el 6% al estadioIV.
Este estudio ofrece una evaluación precisa del coste directo del LCCT en pacientes con MF/SS en España, mostrando costes que varían sustancialmente en función del estadio. Los costes soportados por el paciente y los costes indirectos deberán considerarse en futuras investigaciones.

Los linfomas cutáneos primarios (LCP) son un conjunto de entidades poco frecuentes. En febrero del 2018 se describieron los resultados del primer año de funcionamiento del Registro de linfomas cutáneos primarios de la AEDV. En el presente trabajo actualizamos los resultados tras 5 años de funcionamiento.
Registro de enfermedad de pacientes con LCP. Se recogieron datos prospectivamente de los pacientes, incluyendo diagnóstico, tratamientos, pruebas realizadas y estado actual del paciente. Se realizó un análisis descriptivo.
En diciembre del 2021 se había incluido a un total de 2.020 pacientes en el Registro, pertenecientes a 33 hospitales españoles. El 59% fueron hombres, y la edad media fue de 62,2 años. Se agruparon en 4 grandes grupos diagnósticos: micosis fungoide/síndrome de Sézary (1.112 55%), LCP de células B (547 27,1%), trastornos linfoproliferativos de células T CD30+ (222 11%) y otros linfomas T (116 5,8%). La mayoría presentó estadio T1, encontrándose actualmente casi el 75% en remisión completa (43,5%) o enfermedad estable (EE: 27%). Los tratamientos más usados fueron corticoides tópicos (1.369 67,8%), fototerapia (890 44,1%), cirugía (412 20,4%) y radioterapia (384 19%).
Las características del paciente con LCP en España no difieren de otras series. El mayor tamaño del registro permite precisar mejor los datos con respecto a los resultados del primer año. Este registro facilita al grupo de linfomas de la AEDV realizar investigación clínica, surgiendo ya trabajos publicados de dicho registro.
Background: Buprenorphine is a low‐molecular‐weight, lipophilic, opioid analgesic. The transdermal delivery system (TDS) containing it has skin irritation potential, but at least two cases of ...contact allergy to the active principal have been described previously.
Objective: To confirm allergic contact dermatitis from transdermal buprenorphine (TDB) in five older patients suffering from chronic pain and who developed persistent, pruritic erythematous plaques at the contact sites, with two of them also presenting with a generalized skin eruption.
Methods: Besides the baseline patch test series, all five patients were tested with the TDB, four of whom were also tested with the placebo transdermal delivery system as provided by the manufacturer; one patient was also tested with other preparations containing buprenorphine.
Results: All reacted to the TDB containing the active principal, the placebo being negative in the four patients tested. The patient tested with the other buprenorphine preparations did react positively to them as well. Tests with TDB in 28 healthy controls were negative.
Conclusion: We report five cases of delayed hypersensitivity reactions to a TDS containing buprenorphine. Such adverse reactions might be under‐reported. A fentanyl‐containing TDS is a good alternative in these cases.

Diversos trabajos apoyan la hipótesis de que en España se está produciendo un aumento de incidencia de la escabiosis, y existen dudas sobre el posible desarrollo de resistencias y el ...incremento de formas clínicas atípicas. Los objetivos de este estudio fueron caracterizar el perfil demográfico y clínico de los pacientes de escabiosis atendidos por dermatólogos en España, identificar la posible aparición de escabiosis atípicas, así como describir la frecuencia y los posibles factores de riesgo de los fracasos terapéuticos previos.
Realizamos un estudio observacional, transversal, multicéntrico con recogida de datos prospectiva, en abril y mayo de 2023 dentro de la plataforma CLINI-AEDVp de la Academia Española de Dermatología y Venereología.
Se reclutaron 186 casos de escabiosis activa (51% mujeres) en 31 centros participantes de 15 comunidades autónomas. Se requirió un nivel A, B o C de los criterios de consenso de la International Alliance for the Control of Scabies (IACS) para el diagnóstico. El 92% de los pacientes presentaron formas clínicas típicas de escabiosis y un 66% había recibido tratamiento escabicida previo para el episodio en curso. De los pacientes previamente tratados, solo un 36% había recibido y cumplimentado una pauta terapéutica adecuada que incluyera el tratamiento simultáneo de convivientes, y un 50% careció de un documento escrito y claro con las recomendaciones.
Una elevada proporción de los casos de escabiosis atendidos actualmente ha recibido tratamiento previo. En estos se observan defectos corregibles que pueden justificar parte de los fracasos terapéuticos. Trabajar en la mejora de las deficiencias encontradas ayudará a un mejor control de la enfermedad y a evaluar la efectividad actual de los escabicidas disponibles.
