Introduction Certaines formes graves d’orbitopathie basedowienne peuvent nécessiter une chirurgie urgente. Nous décrivons ici l’observation d’une patiente pour laquelle une exceptionnelle ...décompression orbitaire par voie postérieure s’est avérée nécessaire. Observation Mme M., 51 ans, présente une maladie de Basedow depuis 2013 (T4L = 60 pmol/L, TRAK = 10,2 UI/L) avec orbitopathie sévère : œdème des paupières, exophtalmie de stade III. Nous retrouvons dans ses antécédents un cancer du sein droit et une artériopathie oblitérante des membres inférieurs. Elle poursuit cependant un tabagisme toujours actif et important : un paquet/jour depuis l’âge de 12 ans. Un traitement par thiamazole est proposé, mais devant d’importantes fluctuations de la TSH, la persistance de TRAK élevés (> 40 UI/L) et l’absence de contrôle de la maladie, une thyroïdectomie totale est proposée en mai 2015, encadrée par une corticothérapie par voie intraveineuse. Il est cependant observé une aggravation de l’orbitopathie avec acuité visuelle à 5/10e à droite, diplopie, et à l’IRM orbitaire une hypertrophie des muscles extraoculaires plus évidents sur les muscles médiaux, avec comblement des apex orbitaux et atteinte des deux nerfs optiques. Une corticothérapie IV (méthylprednisolone 1 g/24 heures sur 3 jours) est inefficace. L’atteinte étant trop sévère pour une utilisation de rituximab, il est décidé la réalisation d’une décompression du nerf optique droit par voie postérieure, d’abord nasal endoscopique en milieu neurochirurgical, permettant la récupération d’une acuité visuelle à 8/10e. Discussion La décompression neurochirurgicale d’un nerf optique par voie postérieure en urgence dans une maladie de Basedow est d’indication exceptionnelle. Le suivi doit être spécialisé et l’arrêt du tabac, impératif.
Introduction La neuropathie optique est une complication redoutable de l’orbitopathie Basedowienne avec une incidence estimée entre 3 à 5 %. La décompression chirurgicale demeure une solution ...efficace en cas de menace visuelle. Les solutions chirurgicales classiques par voies maxillo-faciales et ORL connaissent des limitations particulièrement en termes de décompression du canal optique proprement dit. La voie endoscopique endonasale permet une décompression intégrant le canal optique et la paroi médiale de l’orbite. Matériels et méthodes Les patients porteurs d’une neuropathie optique réfractaire dysthyroïdienne réfractaire à la corticothérapie ont été sélectionnés sur des critères neuro-ophtalmologiques lors d’une consultation multidisciplinaire dédiée. Tous les patients ont été explorés en pré-opératoire et post-opératoire immédiats puis à 3 mois par un bilan neuro-ophtalmologique (champs visuel, acuité visuelle, fond d’œil, OCT, exophtalmométrie, bilan oculomoteur, vision des couleurs et score d’activité), une imagerie orbitaire ainsi qu’une évaluation endocrinienne. Résultats Onze patients présentant une neuropathie optique dysthyroïdienne réfractaire ont été sélectionnés sur 12 mois. Seize canaux optiques ont été décomprimés. La morbidité et la mortalité opératoires sont nulles. Dix patients (90,9 %) ont été améliorés dès l’évaluation post-opératoire immédiate. Aucune dégradation post-opératoire ne fut notée. Discussion La décompression endoscopique endonasale du canal optique et de l’apex orbitaire constitue une solution simple et efficace dans la gestion de la neuropathie optique dysthyroïdienne réfractaire à la corticothérapie. Une étude prospective en cours comprenant plus de patients sur une période de suivi plus longue devrait permettre de confirmer ces données et d’affiner le processus de sélection des patients éligibles.
La surrénalectomie pour adénome de Conn (AC) guérit 35 % des patients. L’ARS est validé aux États-Unis comme un score prédictif de guérison. L’objectif est de valider ce score en France. Cette étude ...rétrospective a colligé les données des patients opérés pour AC dans 4 CHU de 2002 à 2014. Le score ARS est noté sur 5 points (femme = 1, IMC ≤ 25 = 1, durée d’HTA < 6 ans = 1, nombre d’antiHTA < 2 = 2). La guérison est définie par une TA < 140/90 mmHg au moins 6 mois après la chirurgie sans traitement (ttt). Cent cinquante patients ont été opérés pour AC, 98 % par cœlioscopie, 2 conversions. Une laparotomie d’emblée. Les données concernant le score ARS et le suivi à 6 mois ont été établies pour 120 patients. L’âge moyen était de 51,7 ± 10,7 ans. La durée moyenne de l’HTA avant chirurgie était de 6,7 ± 6,6 ans avec en moyenne 2,4 ± 1,2 antiHTA ; 38,1 % ont normalisé leur TA sans ttt, 12,9 % n’avaient pas normalisé leur TA, 49 % ont normalisé leur TA mais requièrent un ttt antiHTA. La morbidité était de 5 %, la mortalité nulle. Les femmes étaient guéries dans 53,8 % des cas contre 29,6 % des hommes ( p = 0,004). Les patients non guéris de leur HTA avaient en moyenne 1,6 antiHTA 6 mois après la chirurgie versus 2,8 antiHTA avant la chirurgie. La guérison de l’HTA pour les patients avec un ARS de 0–1 est de 18,8 %, 29,8 % pour un ARS de 2–3, 73,2 % pour un ARS de 4–5 ( p = 0,0001). Le score ARS est prédictif de guérison de l’HTA en France.
Toxic serum cefepime trough concentrations are not well defined in the current literature. We aimed to define a more precise plasma trough concentration threshold for this antibiotic's neurological ...toxicity and to identify individuals at risk for developing neurotoxic side effects.
Retrospective study including all individuals who underwent cefepime therapeutic drug monitoring (TDM) between 2013 and 2017. Individuals with cefepime concentrations other than trough were excluded. The primary outcome was to assess the incidence of neurotoxicity and its relationship with cefepime plasma trough concentrations. Secondary outcomes were the relationship of renal function, cefepime daily dose, age, and cerebral and general co-morbidities with the occurrence of neurotoxicity. We also compared the mortality rate during hospitalization in individuals with and without neurotoxicity, and the possible impact of neuroprotective co-medications on outcomes.
Cefepime concentrations were determined in 584 individuals. Among 319 individuals with available trough concentrations included, the overall incidence of neurotoxicity was 23.2% (74 of 319 individuals). Higher cefepime plasma trough concentrations were significantly associated with risk of neurotoxicity (no neurotoxicity 6.3 mg/L (interquartile range (IQR) 4.1–8.6) versus with neurotoxicity 21.6 mg/L (IQR 17.0–28.6), p <0.001). Individuals with presumed cefepime neurotoxicity had a significantly lower renal function (estimated glomerular filtration rate 82.0 mL/min/1.73 m2 (IQR 45.0–105.0) versus 35.0 mL/min/1.73 m2 (IQR 23.3–53.3, p <0.001), and significantly higher in-hospital mortality (19 (7.8%) versus 26 (35.1%) individuals, p <0.001). No neurotoxic side effects were seen below a trough concentration of 7.7 mg/L. Levels ≥38.1 mg/L always led to neurological side effects.
In individuals with risk factors for cefepime neurotoxicity, such as renal insufficiency, TDM should be systematically performed, aiming at trough concentrations <7.5 mg/L.
Although whole‐exome sequencing (WES) is the gold standard for the diagnosis of neurodevelopmental disorders (NDDs), it remains expensive for some genetic centers. Commercialized panels comprising ...all OMIM‐referenced genes called “medical exome” (ME) constitute an alternative strategy to WES, but its efficiency is poorly known. In this study, we report the experience of 2 clinical genetic centers using ME for diagnosis of NDDs. We recruited 216 consecutive index patients with NDDs in 2 French genetic centers, corresponded to the daily practice of the units and included non‐syndromic intellectual disability (NSID, n = 33), syndromic ID (NSID = 122), pediatric neurodegenerative disorders (n = 7) and autism spectrum disorder (ASD, n = 54). We sequenced samples from probands and their parents (when available) with the Illumina TruSight One sequencing kit. We found pathogenic or likely pathogenic variants in 56 index patients, for a global diagnostic yield of 25.9%. The diagnosis yield was higher in patients with ID as the main diagnosis (32%) than in patients with ASD (3.7%). Our results suggest that the use of ME is a valuable strategy for patients with ID when WES cannot be used as a routine diagnosis tool.

When and how adaptive immunity emerged is one of the fundamental questions in immunology. Accumulated evidence suggests that the key components of adaptive immunity, rearranging receptor genes and ...the MHC, are unique to jawed vertebrates. Recent studies in protochordates, in particular, the draft genome sequence of the ascidian Ciona intestinalis, are providing important clues for understanding the origin of antigen receptors and the MHC. We discuss a group of newly identified protochordate genes along with some cold-blooded vertebrate genes, the ancestors of which might have provided key elements of antigen receptors. The organization of the proto-MHCs in protochordates provides convincing evidence that the MHC regions of jawed vertebrates emerged as a result of two rounds of chromosomal duplication.
