In this study, we analyzed 340 whole genomes of SARS-CoV-2, which were sampled between April and November 2020 in 33 cities of Rio Grande do Sul, South Brazil. We demonstrated the circulation of two ...novel emergent lineages, VUI-NP13L and VUI-NP13L-like, and five major lineages that had already been assigned (B.1.1.33, B.1.1.28, P.2, B.1.91, B.1.195). P.2 and VUI-NP13L demonstrated a massive spread in October 2020. Constant and consistent genomic surveillance is crucial to identify newly emerging SARS-CoV-2 lineages in Brazil and to guide decision making in the Brazilian Public Healthcare System.
ABSTRACT Introduction: Knowledge about risky sexual behavior among young people has been widely acknowledged as a key tool to controlling the spread of HIV. This article aimed at presenting the risk ...behavior of Brazilian Army conscripts toward HIV infection according to the country’s geographic regions. . Method: We collected data from 37,282 conscripts, aged 17 to 22, during enlistment in the Brazilian Army in 2016. The prevalence of HIV infection, both self-reported and measured through laboratory results, and risk behavior factors were estimated by region. Results: 75% of the sample of conscripts reported to have already started sexual activity, and the average age of their sexual initiation was 15. Condom use varied according to the type of sexual relationship, being lower among steady partners and greater among less stable relationships. HIV prevalence assessed by laboratory tests was 0.12% across the country and the highest prevalence was observed in the North region (0.24%). Alcohol and illicit drug usage was higher in the South region. Discussion: The study allowed the observation of risk behavior monitoring for HIV infection among young Brazilians. Lower condom usage among steady partners may be contributing to an increase in the number of HIV-infected individuals. Conclusion: Results suggest the need to intensify prevention campaigns to disseminate safe sex practices among young people, in addition to the expansion of testing offer to this population.
RESUMO Introdução: O conhecimento do comportamento sexual de risco entre jovens tem sido amplamente reconhecido como ferramenta-chave para controlar a propagação do HIV. Este artigo tem o objetivo de retratar o comportamento de risco dos conscritos do Exército brasileiro à infecção pelo HIV segundo as macrorregiões brasileiras. Métodos: Foram utilizados dados de 37.282 conscritos, entre 17 e 22 anos, durante apresentação ao Exército em 2016. Estimaram-se as prevalências de HIV autorreferida e medida por exame laboratorial, além de indicadores de comportamento de risco, por macrorregiões geográficas. Resultados: Dos conscritos que compuseram a amostra, 75% relatou já ter iniciado atividade sexual, e a média de idade de início foi de aproximadamente 15 anos. O uso do preservativo variou de acordo com o tipo de parceria sexual, sendo menor na relação com parceiros(as) fixos(as) e maior nas parcerias menos estáveis. A prevalência da infecção pelo HIV medida através do diagnóstico laboratorial foi de 0,12% no Brasil, sendo mais prevalente na região Norte (0,24%). O consumo de álcool e drogas ilícitas foi maior na região Sul. Discussão: O estudo permitiu observar o monitoramento do comportamento de risco à infecção pelo HIV entre os jovens brasileiros. O menor uso de preservativo quando a parceria sexual é considerada estável pode estar contribuindo para aumentar o número de indivíduos infectados pelo HIV. Conclusão: Os resultados sugerem a necessidade de campanhas de divulgação para os jovens sobre práticas de sexo seguro, além da ampliação da oferta de testagem nessa população.

Behavioral surveillance surveys have been carried among military conscripts, in Brazil, since 1996. This paper presents the results of the 2007 survey and compares actual findings with those obtained ...in previous studies carried out in the period 1999-2002. The conscripts were selected with a two stage sampling stratified by geographical region. The study included a self-reported questionnaire and blood collection for HIV and syphilis testing. Data from 35,432 conscripts aged 17-20 years old were analyzed. The findings show a reduction in regular condom use, with fixed and casual partners, mainly among those with poor educational level. The proportion of conscripts that have sex with other men was maintained (3.2%), but the index of risky sexual behavioral showed worsening results in this group. Also, the HIV prevalence rate increased from 9 to 11.3 per 10,000 in 2007, though the increase was not statistically significant. In conclusion, the evidences found in this paper indicate the need of reformulate policy among young adults.

ABSTRACT Objective: To analyze the distribution of health care services for viral hepatitis and reported cases of viral hepatitis according to the health regions of Northern Brazil. Method: It is an ...evaluative, descriptive and quantitative research considering viral hepatitis care services and reported cases in the Northern region of Brazil, using data collected from the National Registry of Health Establishments and the Notifiable Diseases Information System. Descriptive statistics and georeferencing, through software, were used to demonstrate the spatial distribution of services and reported cases. Results: Viral hepatitis health services are distributed in a differentiated way; rapid tests are capillaries in the states; confirmatory tests and treatment are performed in some health regions, with a greater grouping of services in the capitals and their surroundings. Cases were reported across all regions, with areas of higher concentration near services. Conclusion: The availability of services can favor access to prevention, diagnosis and monitoring of cases. However, organizational peculiarities of the health system and services highlight fragilities that have repercussions on the access and entirety of viral hepatitis care.
RESUMO Objetivo: Analisar a distribuição dos serviços de saúde de atenção às hepatites virais e os casos notificados de hepatites virais segundo as regiões de saúde dos estados do Norte do Brasil. Método: Trata-se de pesquisa avaliativa, descritiva e quantitativa considerando os serviços de atenção e casos notificados de hepatites virais na região Norte do Brasil. Foram coletados dados do Cadastro Nacional de Estabelecimentos de Saúde e do Sistema de Informação de Agravos e Notificação. Utilizou-se estatística descritiva e georreferenciamento por meio de software para visualizar a distribuição espacial dos serviços e os casos notificados. Resultados: Os serviços são distribuídos de maneira diferenciada; testes rápidos apresentam-se capilarizados nos estados; demais exames para confirmar o diagnóstico e o tratamento são realizados em algumas regiões de saúde, com maior agrupamento de serviços nas capitais e suas cercanias. Verificam-se casos notificados de maneira pulverizada nas regiões, com áreas de maior concentração próximas aos serviços. Conclusão: A disponibilidade de serviços pode favorecer o acesso e a adoção de medidas de prevenção, diagnóstico e monitoramento de casos. Entretanto, peculiaridades organizacionais do sistema e serviços de saúde evidenciam fragilidades que repercutem no acesso e na integralidade da atenção às hepatites virais.
The Zika virus outbreak has triggered a set of local and global actions for a rapid, effective, and timely public health response. A World Health Organization (WHO) initiative, supported by the ...Department of Chronic Condition Diseases and Sexually Transmitted Infections (DCCI) of the Health Surveillance Secretariat (SVS), Brazil Ministry of Health (MoH) and other public health funders, resulted in the start of the "Study on the persistence of Zika virus in body fluids of patients with ZIKV infection in Brazil - ZIKABRA study". The ZIKABRA study was designed to increase understanding of how long ZIKV persists in bodily fluids and informing best measures to prevent its transmission. Data collection began in July 2017 and the last follow up visit occurred in 06/26/2020.
A framework for the ZIKABRA Cooperation initiative is provided through a description and analysis of the mechanisms, strategies and the ethos that have guided the models of international governance and technical cooperation in health for scientific exchange in the context of a public health emergency. Among the methodological strategies, we included a review of the legal documents that supported the ZIKABRA Cooperation; weekly documents produced in the meetings and working sessions; technical reports; memorandum of understanding and the research protocol.
We highlight the importance of working in cooperation between different institutional actors to achieve more significant results than that obtained by each group working in isolation. In addition, we point out the advantages of training activities, ongoing supervision, the construction of local installed research capacity, training academic and non-academic human resources, improvement of laboratory equipment, knowledge transfer and the availability of the ZIKABRA study protocol for development of similar studies, favoring the collective construction of knowledge to provide public health emergency responses. Strategy harmonization; human resources and health services; timing and recruiting particularities and processing institutional clearance in the different sites can be mentioned as challenges in this type of initiative.
IntroductionAcute viral hepatitis is a disease of great clinical importance. This study proposes actions to better characterise cases of acute hepatitis in Brazil and to provide relevant information ...to institutionalised health policies within the Unified Health System. Available data on acute hepatitis in Brazil need to be re-evaluated regarding the different hepatotropic agent (hepatitis A to E virus) frequencies, as well as other agents that can cause similar clinical conditions, such as Herpes Simplex Virus 1 and 2(HSV1, HSV2), Varicella Zoster Virus (VZV), Cytomegalovirus (CMV), Epstein Barr Virus (EBV), Human Herpes Virus 6 and 7 (HHV6, HHV7), arbovirus (yellow fever, dengue, chikungunya, Zika), parvovirus B19, adenovirus, parechovirus, enterovirus, HIV, leptospirosis, toxoplasmosis and syphilis, in addition to autoimmune hepatitis. In this context, the primary aim of this study is the clinical-epidemiological and molecular characterisation of acute viral hepatitis in Brazilian health services from all geographical regions of the country. The present article describes the study protocol.Methods and analysisThis study will evaluate 2280 patients with symptoms and/or signs suggestive of acute liver disease in Brazilian health institutions in all five geographic Brazilian regions. Demographic, epidemiological and clinical data will be collected, as well as blood samples to be analysed at Hospital Israelita Albert Einstein Clinical Laboratory.Ethics and disseminationEthics approval was obtained at the national research ethics committee (Conselho Nacional de Ética em Pesquisa— CONEP–CAAE 00952818.4.1001.0071) and at all participating sites. Results will be published in journals and presented at scientific meetings.
Resumo: Este estudo teve por objetivo avaliar a cascata de cuidado da redução da transmissão vertical do HIV nos estados do Amazonas, Ceará, Espírito Santo, Rio de Janeiro, Rio Grande do Sul e no ...Distrito Federal, usando dados do Sistema de Informação de Agravos de Notificação (SINAN). Entre os anos de 2007 e 2012, cresceu a taxa de detecção de HIV na gestação em 5 estados, variando de 7,3% no Distrito Federal a 46,1% no Amazonas, com redução de 18,6% no Rio de Janeiro. Menos de 90% das mulheres usaram antirretroviral durante o pré-natal, incluídas as que já se sabiam portadoras do HIV. A realização de cesárea eletiva foi baixa. A taxa de detecção de AIDS em crianças menores de 5 anos como proxy da transmissão vertical do HIV apresentou uma redução de 6,3% entre 2007 e 2012, sendo a maior no Rio Grande do Sul (50%), que apresentou as maiores taxas do período, enquanto no Espírito Santo ocorreu o maior aumento (50%). A avaliação da cascata do cuidado do HIV na gestante apontou falhas em todos os pontos. É necessária uma conexão entre a atenção básica e os centros de referência para HIV/AIDS, ordenando o cuidado da família e o melhor desfecho para a criança.
Abstract: This study aimed to assess the cascade of care in the reduction of mother-to-child HIV transmission in the states of Amazonas, Ceará, Espírito Santo, Rio de Janeiro, and Rio Grande do Sul and the Distrito Federal, Brazil, using data from the Brazilian Information System on Diseases of Notification (SINAN). From 2007 to 2012, there was an increase (from 7.3% in Distrito Federal to 46.1% in Amazonas) in intra-gestational detection of HIV in 5 states, with a 18.6% reduction in Rio de Janeiro. Fewer than 90% of the women received antiretroviral therapy during their prenatal care, including those that already knew they were HIV-positive. The elective cesarean rate was low. The AIDS detection rate in children under 5 years as a proxy for mother-to-child HIV transmission showed a reduction of 6.3% from 2007 to 2012, and was highest in Rio Grande do Sul (50%), the state with the highest rates in the period, while Espírito Santo showed the highest increase (50%). Evaluation of the cascade of HIV care in pregnant women identified flaws in all the points. A link is needed between primary care and referral centers for HIV/AIDS, organizing care for the family and better outcomes for the children.
Resumen: Este estudio tuvo por objetivo evaluar la cascada de cuidado de la reducción de la transmisión vertical del VIH en los Estados del Amazonas, Ceará, Espírito Santo, Río de Janeiro, Río Grande do Sul y en el Distrito Federal, usando datos del Sistema de Información de Enfermedades de Notificación Obligatoria (SINAN por sus siglas en portugués). Entre los años de 2007 y 2012 creció la tasa de detección de VIH durante la gestación en 5 estados, variando de 7,3% en el Distrito Federal a 46,1% en Amazonas, con una reducción de 18,6% en Río de Janeiro. Menos de un 90% de las mujeres usaron antirretrovirales durante el período prenatal, incluidas quienes ya se sabían portadoras del VIH. La realización de cesárea electiva fue baja. La tasa de detección de SIDA en niños menores de 5 años como proxy de la transmisión vertical del VIH presentó una reducción de 6,3% entre 2007 y 2012, siendo la mayor en Río Grande do Sul (50%), la cual presentó las mayores tasas del período, mientras que en Espírito Santo se produjo el mayor aumento (50%). La evaluación de la cascada del cuidado del VIH en la gestante apuntó fallos en todos los puntos. Es necesaria una conexión entre la atención básica y los centros de referencia para VIH/SIDA, buscando el cuidado de la familia y el mejor desenlace para el niños.
