Te. The limiting factor in large-scale bolometric searches for this rare process is the background induced by surface radioactive contamination, as shown by the results of the CUORE experiment. The basic concept of CROSS consists of rejecting this challenging background component by pulse-shape discrimination, assisted by a proper coating of the faces of the crystal containing the isotope of interest and serving as energy absorber of the bolometric detector. In this paper, we demonstrate that ultra-pure superconductive Al films deposited on the crystal surfaces act successfully as pulse-shape modifiers, both with fast and slow phonon sensors. Rejection factors higher than 99.9% of
α
surface radioactivity have been demonstrated in a series of prototypes based on crystals of Li
2
MoO
4
and TeO
2
. We have also shown that point-like energy depositions can be identified up to a distance of

A stress reaction involving increased cortisol release, which has not been documented thus far, might affect the assessment of selectivity of catheterization during adrenal venous sampling (AVS).
To ...investigate whether an ACTH-driven cortisol release occurs during AVS and whether it influences the assessment of selectivity by the step-up of cortisol (plasma cortisol concentrations, PCC) between the adrenal vein blood (PCC(SIDE)) and the inferior vena cava (PCC(IVC)), e.g. the selectivity index (SI).
We determined the SI in samples obtained simultaneously at starting AVS (t-15) and again after 15 min (t0) in 34 consecutive patients with proven aldosterone-producing adenoma. We then calculated the SI with PCC(SIDE) obtained at t-15 and at t0, and the PCC(IVC) values obtained at the different time point, thus simulating sequential AVS.
The PCC(SIDE) and the SI fell significantly from t-15 to t0 on both the sides. When PCC(SIDE) obtained at t-15 was combined with PCC(IVC) at t0, the SI values were higher than those obtained with simultaneously drawn samples. This led to label as selective more AVS studies than with bilaterally simultaneous data, especially when using higher cutoffs for the SI.
A transient increase in cortisol release from both adrenal glands occurs in the majority of the patients who undergo AVS. This stress reaction can influence the assessment of both the selectivity of the catheterization during the sequential AVS technique and the lateralization of aldosterone excess.

This paper aims at presenting the application for lake water quality monitoring which has been developed in the framework of SIMILE (Informative System for the Integrated Monitoring of Insubric Lakes ...and their Ecosystems) Interreg Italy-Switzerland project. The objective of SIMILE project is to facilitate the monitoring of the Maggiore, Como, and Lugano lakes through the integration of different techniques: in situ monitoring with buoys, remote sensing and citizen science. A mobile application has been designed in agreement with the project partners, who are also actors working for lake quality monitoring, such as CNR (Italian National Research Council) and ARPA (Agency for prevention and environmental protection). The developed application allows to collect data over the area of interest, such as pictures and parameters which can be acquired by visual inspection as well as with appropriate tools, depending on the user typology. The application has then been implemented with open source software to foster its use also for other projects with similar goals. In the paper, the design choices, the architecture and the implementation details are described.
Abstract Background and aims The independent role of serum uric acid (SUA) as a marker of cardio-renal risk is debated. The aim of this study was to assess the relationship between SUA, metabolic ...syndrome (MS), and other cardiovascular (CV) risk factors in an Italian population of hypertensive patients with a high prevalence of diabetes. Methods and results A total of 2429 patients (mean age 62 ± 11 years) among those enrolled in the I-DEMAND study were stratified on the basis of SUA gender specific quartiles. MS was defined according to the NCEP-ATP III criteria, chronic kidney disease (CKD) as an estimated GFR (CKD-Epi) <60 ml/min/1.73 m2 or as the presence of microalbuminuria (albumin-to-creatinine ratio ≥2.5 mg/mmol in men and ≥3.5 mg/mmol in women). The prevalence of MS, CKD, and positive history for CV events was 72%, 43%, and 20%, respectively. SUA levels correlated with the presence of MS, its components, signs of renal damage and worse CV risk profile. Multivariate logistic regression analysis revealed that SUA was associated with a positive history of CV events and high Framingham risk score even after adjusting for MS and its components (OR 1.10, 95% CI 1.03–1.18; P = 0.0060; OR 1.28, 95% CI 1.15–1.42; P < 0.0001). These associations were stronger in patients without diabetes and with normal renal function. Conclusions Mild hyperuricemia is a strong, independent marker of MS and high cardio-renal risk profile in hypertensive patients under specialist care. Intervention trials are needed to investigate whether the reduction of SUA levels favorably impacts outcome in patients at high CV risk.
Factors related to the development of microalbuminuria in hypertension are not well known. We did a prospective study to investigate whether glomerular hyperfiltration precedes the development of ...microalbuminuria in hypertension. We assessed 502 never-treated subjects screened for stage 1 hypertension without microalbuminuria at baseline and followed up for 7.8 years. Creatinine clearance was measured at entry. Urinary albumin and ambulatory blood pressure were measured at entry and during the follow-up until subjects developed sustained hypertension needing antihypertensive treatment. Subjects with hyperfiltration (creatinine clearance >150 ml/min/1.73 m2, top quintile of the distribution) were younger and heavier than the rest of the group and had a greater follow-up increase in urinary albumin than subjects with normal filtration (P<0.001). In multivariable linear regression, creatinine clearance adjusted for confounders was a strong independent predictor of final urinary albumin (P<0.001). In multivariable Cox regression, patients with hyperfiltration had an adjusted hazard ratio for the development of microalbuminuria based on at least one positive measurement of 4.0 (95% confidence interval (CI), 2.1–7.4, P<0.001) and an adjusted hazard ratio for the development of microalbuminuria based on two consecutive positive measurements of 4.4 (95% CI, 2.1–9.2, P<0.001), as compared with patients with normal filtration. Age, female gender, and 24 h systolic blood pressure were other significant predictors of microalbuminuria. In conclusion, stage 1 hypertensive subjects with glomerular hyperfiltration are at increased risk of developing microalbuminuria. Early intervention with medical therapy may be beneficial in these subjects even if their blood pressure falls below normal limits during follow-up.
This study focuses on the role of VGI visualization in encouraging participation to environmental Citizen Science, taking as a case study the mobile application developed within SIMILE (Informative ...System for the Integrated Monitoring of Insubric Lakes and their Ecosystems) project activities. The main project goal is the protection of water quality for Lugano, Maggiore and Como lakes through a geoinformatics coordination of existing monitoring systems with new data collection methods (satellite images, high frequency sensors and citizen science). This tool enables the producers to perform a community-based monitoring side by side with local authorities and research bodies improving the efficiency of the current protocols.Referring to ontologies of geospatial data, the VGI samples are analysed under different aspects as their quality, quantity, variety, granularity, spatial and temporal distribution, highlighting their temporary, moving and changing nature. This analysis shows the presence of potential contradictions existing between the application use and the perceived lake water quality, to be mitigated mainly working on the user interface of the map view. So, a study of existing conventions, especially in terms of colours, is compared with the results coming from the application of consolidated survey methodologies to this specific context. Reflections on markers, symbols and attributes of features and maps are presented introducing geovisual analytical solutions for dynamic phenomena. The design solutions adopted are finally presented and validated with a combination of different usability evaluation methods like heuristics and field tests.

crystals and employed in the CROSS and CUPID experiments can eventually exploit the coincident fast signal detected in a light detector to reduce this background. However, the scintillation provides a modest signal-to-noise ratio, making difficult a pile-up pulse-shape recognition and rejection at timescales shorter than a few ms. Neganov–Trofimov–Luke assisted light detectors (NTL-LDs) offer the possibility to effectively increase the signal-to-noise ratio, preserving a fast time-response, and enhance the capability of pile-up rejection via pulse shape analysis. It is unclear whether revascularization of renal artery stenosis (RAS) by means of percutaneous renal angioplasty and stenting (PTRAS) is advantageous over optimal medical therapy. Hence, we designed ...a randomized clinical trial based on an optimized patient selection strategy and hard experimental endpoints. Primary objective of this study is to determine whether PTRAS is superior or equivalent to optimal medical treatment for preserving glomerular filtration rate (GFR) in the ischemic kidney as assessed by 99mTcDTPA sequential renal scintiscan. Secondary objectives of this study are to establish whether the two treatments are equivalent in lowering blood pressure, preserving overall renal function and regressing target organ damage, preventing cardiovascular events and improving quality of life. The study is designed as a prospective multicentre randomized, un-blinded two-arm study. Eligible patients will have clinical and angio-CT evidence of RAS. Inclusion criteria is RAS affecting the main renal artery or its major branches either >70% or, if <70, with post-stenotic dilatation. Renal function will be assessed with 99mTc-DTPA renal scintigraphy. Patients will be randomized to either arms considering both resistance index value in the ischemic kidney and the presence of unilateral/bilateral stenosis. Primary experimental endpoint will be the GFR of the ischemic kidney, assessed as quantitative variable by 99TcDTPA, and the loss of ischemic kidney defined as a categorical variable.
Since hyperaldosteronism has been experimentally related to myocardial interstitial fibrosis, we investigated the effects of hypertension and excess aldosterone due to aldosterone-producing adenomas ...(APAs) on the heart.
In 52 hypertensive individuals, we performed Doppler echocardiography for estimation of left ventricular (LV) wall thickness and dimensions, transmitral LV filling flow velocity indexes, and 24-hour ambulatory blood pressure monitoring. Consecutive patients with APAs (n = 26) and essential hypertension (EH, n = 26) were individually matched for age, sex, race, body mass index, casual blood pressure, and known duration of hypertension. The matched groups were similar for demography, casual and 24-hour blood pressure values and variability, and duration of hypertension but differed for serum potassium, plasma renin activity, and aldosterone levels (all P < .001). A thicker interventricular septum (P = .015) and posterior wall (P = .009) and a higher LV mass index (118 +/- 5 versus 100 +/- 4 g/m2, P = .009) were observed in APA compared with EH patients. Both septum and posterior wall thicknesses had a significant direct relationship with age, plasma aldosterone, and mean blood pressure. The integral of the early diastolic filling wave (Ei) (P = .011) and the ratio Ei/Ai (A wave integral) (P = .038) were lower and the atrial contribution to LV filling was higher (52 +/- 2% versus 46 +/- 2%, P = .038) in APA than in EH patients. The ratio Ei/Ai was significantly (P = .008) inversely related only to age and plasma aldosterone.
In APA patients, the excess aldosterone is associated with both increased LV wall thickness and mass and decreased early diastolic LV filling indexes compared with demographically similar EH with superimposable blood pressure values, profile, and variability.
