Abstract
Aims
Cardiac amyloidosis is common in elderly patients with aortic stenosis (AS) referred for transcatheter aortic valve implantation (TAVI). We hypothesized that patients with dual aortic ...stenosis and cardiac amyloid pathology (AS-amyloid) would have different baseline characteristics, periprocedural and mortality outcomes.
Methods and results
Patients aged ≥75 with severe AS referred for TAVI at two sites underwent blinded bone scintigraphy prior to intervention (Perugini Grade 0 negative, 1–3 increasingly positive). Baseline assessment included echocardiography, electrocardiogram (ECG), blood tests, 6-min walk test, and health questionnaire, with periprocedural complications and mortality follow-up. Two hundred patients were recruited (aged 85 ± 5 years, 50% male). AS-amyloid was found in 26 (13%): 8 Grade 1, 18 Grade 2. AS-amyloid patients were older (88 ± 5 vs. 85 ± 5 years, P = 0.001), with reduced quality of life (EQ-5D-5L 50 vs. 65, P = 0.04). Left ventricular wall thickness was higher (14 mm vs. 13 mm, P = 0.02), ECG voltages lower (Sokolow–Lyon 1.9 ± 0.7 vs. 2.5 ± 0.9 mV, P = 0.03) with lower voltage/mass ratio (0.017 vs. 0.025 mV/g/m2, P = 0.03). High-sensitivity troponin T and N-terminal pro-brain natriuretic peptide were higher (41 vs. 21 ng/L, P < 0.001; 3702 vs. 1254 ng/L, P = 0.001). Gender, comorbidities, 6-min walk distance, AS severity, prevalence of disproportionate hypertrophy, and post-TAVI complication rates (38% vs. 35%, P = 0.82) were the same. At a median follow-up of 19 (10–27) months, there was no mortality difference (P = 0.71). Transcatheter aortic valve implantation significantly improved outcome in the overall population (P < 0.001) and in those with AS-amyloid (P = 0.03).
Conclusions
AS-amyloid is common and differs from lone AS. Transcatheter aortic valve implantation significantly improved outcome in AS-amyloid, while periprocedural complications and mortality were similar to lone AS, suggesting that TAVI should not be denied to patients with AS-amyloid.
The chemical composition of exhaled breath was examined for volatile organic compound (VOC) indicators of sexual arousal in human beings. Participants (12-male, 12-female) were shown a randomized ...series of three emotion-inducing 10-min film clips interspersed with 3-min neutral film clips. The films caused different arousals: sports film (positive-nonsexual); horror film (negative-nonsexual); and erotic (sexual) that were monitored with physiological measurements including genital response and temperature. Simultaneously the breath was monitored for VOC and CO
. While some breath compounds (methanol and acetone) changed uniformly irrespective of the film order, several compounds did show significant arousal associated changes. For both genders CO
and isoprene decreased in the sex clip. Some male individuals showed particularly strong increases of indole, phenol and cresol coincident with sexual arousal that decreased rapidly afterwards. These VOCs are degradation products of tyrosine and tryptophan, precursors for dopamine, noradrenalin, and serotonin, and therefore represent potential breath markers of sexual arousal.
Objectives This study sought to determine the diagnostic accuracy of 64-slice computed tomographic coronary angiography (CTCA) to detect or rule out significant coronary artery disease (CAD). ...Background CTCA is emerging as a noninvasive technique to detect coronary atherosclerosis. Methods We conducted a prospective, multicenter, multivendor study involving 360 symptomatic patients with acute and stable anginal syndromes who were between 50 and 70 years of age and were referred for diagnostic conventional coronary angiography (CCA) from September 2004 through June 2006. All patients underwent a nonenhanced calcium scan and a CTCA, which was compared with CCA. No patients or segments were excluded because of impaired image quality attributable to either coronary motion or calcifications. Patient-, vessel-, and segment-based sensitivities and specificities were calculated to detect or rule out significant CAD, defined as ≥50% lumen diameter reduction. Results The prevalence among patients of having at least 1 significant stenosis was 68%. In a patient-based analysis, the sensitivity for detecting patients with significant CAD was 99% (95% confidence interval CI: 98% to 100%), specificity was 64% (95% CI: 55% to 73%), positive predictive value was 86% (95% CI: 82% to 90%), and negative predictive value was 97% (95% CI: 94% to 100%). In a segment-based analysis, the sensitivity was 88% (95% CI: 85% to 91%), specificity was 90% (95% CI: 89% to 92%), positive predictive value was 47% (95% CI: 44% to 51%), and negative predictive value was 99% (95% CI: 98% to 99%). Conclusions Among patients in whom a decision had already been made to obtain CCA, 64-slice CTCA was reliable for ruling out significant CAD in patients with stable and unstable anginal syndromes. A positive 64-slice CTCA scan often overestimates the severity of atherosclerotic obstructions and requires further testing to guide patient management.
We report our identification of the optical afterglow and host galaxy of the short-duration gamma-ray burst sGRB 160821B. The spectroscopic redshift of the host is z = 0.162, making it one of the ...lowest redshift short-duration gamma-ray bursts (sGRBs) identified by Swift. Our intensive follow-up campaign using a range of ground-based facilities as well as Hubble Space Telescope, XMM-Newton, and Swift, shows evidence for a late-time excess of optical and near-infrared emission in addition to a complex afterglow. The afterglow light curve at X-ray frequencies reveals a narrow jet, deg, that is refreshed at >1 day post-burst by a slower outflow with significantly more energy than the initial outflow that produced the main GRB. Observations of the 5 GHz radio afterglow shows a reverse shock into a mildly magnetized shell. The optical and near-infrared excess is fainter than AT2017gfo associated with GW170817, and is well explained by a kilonova with dynamic ejecta mass Mdyn = (1.0 0.6) × 10−3 M and a secular (post-merger) ejecta mass with Mpm = (1.0 0.6) × 10−2 M , consistent with a binary neutron star merger resulting in a short-lived massive neutron star. This optical and near-infrared data set provides the best-sampled kilonova light curve without a gravitational wave trigger to date.
Health Canada (HC) has, since 2013, issued safety alerts restricting the use of codeine-containing drugs among breastfeeding women and children/adolescents under 18 years of age. These products are ...linked to breathing problems among ultra-rapid CYP2D6 metabolizers and early use of opioid can lead to future opioid misuse. Using a multi-province population-based cohort study, we estimate the impact of federal safety alerts on annual rates of codeine use in the Canadian pediatric population. We analyzed data from 8,156,948 children/adolescents in five Canadian provinces between 1996 and 2021, using a common protocol. Children/adolescents were categorized as: ≤ 12 years (children) or > 12 years (adolescents). We defined codeine exposure by ≥ 1 prescription filled for codeine alone or combined with other medications. For both age categories, we obtained province-specific codeine prescription filling rates per calendar year by dividing the number of children/adolescents with ≥ 1 codeine prescription filled by the number of person-time. Annual rates of codeine use per 1000 persons vary by province from 3.0 (Quebec) to 10.1 (Manitoba) in children, and from 5.5 to 51.3 in adolescents. After the 2013 HC advisory, exposure decreased in all provinces (adjusted level change from - 0.6 to - 18.4%) in children and from - 2.1 to - 17.9% in adolescents after the 2016 advisory. Annual rates declined over time in all provinces, following HC safety alerts specific to each of the two age categories.
The ability to see behind flames is a key challenge for the industrial field and particularly for the safety field. Development of new technologies to detect live people through smoke and flames in ...fire scenes is an extremely desirable goal since it can save human lives. The latest technologies, including equipment adopted by fire departments, use infrared bolometers for infrared digital cameras that allow users to see through smoke. However, such detectors are blinded by flame-emitted radiation. Here we show a completely different approach that makes use of lensless digital holography technology in the infrared range for successful imaging through smoke and flames. Notably, we demonstrate that digital holography with a cw laser allows the recording of dynamic human-size targets. In this work, easy detection of live, moving people is achieved through both smoke and flames, thus demonstrating the capability of digital holography at 10.6 μm.
Context.
GRB 211106A and GRB 211227A are two recent gamma-ray bursts (GRBs) whose initial X-ray position enabled us to possibly associate them with bright, low-redshift galaxies (
z
< 0.7). The ...prompt emission properties suggest that GRB 211106A is a genuine short-duration GRB and GRB 211227A is a short GRB with extended emission. Therefore, they are likely to be produced by a compact binary merger. However, a classification based solely on the prompt emission properties can be misleading.
Aims.
The possibility of having two short GRBs occurring in the local Universe makes them ideal targets for the search of associated kilonova (KN) emission and for detailed studies of the host galaxy properties.
Methods.
We carried out deep optical and near-infrared (NIR) follow-up with the ESO-VLT FORS2, HAWK-I, and MUSE instruments for GRB 211106A and with ESO-VLT FORS2 and X-shooter for GRB 211227A, starting from hours after the X-ray afterglow discovery up to days later. We performed photometric analysis to look for afterglow and KN emissions associated with the bursts, together with imaging and spectroscopic observations of the host galaxy candidates. We compared the results obtained from the optical/NIR observations with the available
Swift
X-Ray Telescope (XRT) and others high-energy data of both events.
Results.
For both GRBs we placed deep limits to the optical/NIR afterglow and KN emission. We identified their associated host galaxies, GRB 211106A at a photometric redshift
z
= 0.64, GRB 211227A at a spectroscopic
z
= 0.228. From MUSE and X-shooter spectra we derived the host galaxy properties, which turned out to be consistent with short GRBs typical hosts. We also compared the properties of GRB 211106A and GRB 211227A with those of the short GRBs belonging to the S-BAT4 sample, here extended up to December 2021, in order to further investigate the nature of these two bursts.
Conclusions.
Our study of the prompt and afterglow phase of the two GRBs, together with the analysis of their associated host galaxies, allows us to confirm the classification of GRB 211106A as a short GRB, and GRB 211227A as a short GRB with extended emission. The absence of an optical/NIR counterpart down to deep magnitude limits is likely due to high local extinction for GRB 211106A and a peculiarly faint kilonova for GRB 211227A.
ABSTRACT
Short gamma-ray bursts (SGRBs) are produced by the coalescence of compact binary systems which are remnants of massive stars. GRB 160410A is classified as a short-duration GRB with extended ...emission and is currently the farthest SGRB with a redshift determined from an afterglow spectrum and also one of the brightest SGRBs to date. The fast reaction to the Neil Gehrels Swift Observatory alert allowed us to obtain a spectrum of the afterglow using the X-shooter spectrograph at the Very Large Telescope (VLT). The spectrum shows several absorption features at a redshift of z = 1.7177, in addition, we detect two intervening systems at z = 1.581 and z = 1.444. The spectrum shows Ly α in absorption with a column density of log (N(H i)/cm2) = 21.2 ± 0.2 which, together with Fe ii, C ii, Si ii, Al ii, and O i, allow us to perform the first study of chemical abundances in a SGRB host galaxy. We determine a metallicity of X/H = −2.3 ± 0.2 for Fe ii and −2.5 ± 0.2 for Si ii and no dust depletion. We also find no evidence for extinction in the afterglow spectral energy distribution modelling. The environment has a low degree of ionization and the C iv and Si iv lines are completely absent. We do not detect an underlying host galaxy down to deep limits. Additionally, we compare GRB 160410A to GRB 201221D, another high-z short GRB that shows absorption lines at z = 1.045 and an underlying massive host galaxy.
