We report the case of a 54 years old patient monitored for a monoclonal IgG kappa light chain refractory relapsed multiple myeloma and receiving daratumumab immunotherapy. Daratumumab (DARA), a ...monoclonal anti-CD38 antibody, belongs to the new generation of immunotherapy in refractory relapse multiple myeloma which the medical pathologist should be aware of to avoid misinterpretation of biological tests performed for patients followed for this disease. By its IgG1K humanized monoclonal antibody backbone, DARA interferes in both serum protein electrophoresis and immunofixation by the presence of an alternate IgGK monoclonal peak, leading to a possible difficulty to assess treatment's response in monoclonal IgG kappa light chain myeloma. By its intrinsic anti-CD38 activity DARA also interferes in the screening and identification of red blood cells alloantibodies, due to stabilized red cells reagent expressing weakly the CD38 molecule. We manage to overcome this last interference by using dithiotreitol.
Évaluer un kit de détection et quantification de l’hémorragie fœto-maternelle par cytométrie en flux utilisant un double marquage antihémoglobine fœtale (anti-HbF) et anti-anhydrase carbonique ...(anti-AC).
Vingt-neuf prélèvements de femmes enceintes, dont 22 présentent un résultat ininterprétable et 7 un résultat positif avec le réactif de routine anti-HbF (FMH QuikQuant, IQ Products) ont été testés avec un réactif avec double marquage anti-HbF/anti-AC (Fetal Cell Count Kit, IQ Products), sur cytomètre en flux (Navios, Beckman-Coulter). Deux niveaux de contrôles internes de qualité (FetalTrol, IQ Products) ont été utilisés pour valider chaque série. En parallèle : réalisation d’une électrophorèse capillaire de l’hémoglobine en cas de résultat ininterprétable avec le réactif FMH QuikQuant.
Les 22 échantillons ininterprétables avec le réactif anti-HbF seul ont tous été trouvés négatifs en double marquage anti-HbF/anti-AC. Parmi ces 22 patientes, 5 présentent une hémoglobinopathie (dont une persistance héréditaire probable de l’HbF, avec 29 % d’HbF), et 3 une augmentation modérée et isolée de l’HbF probablement acquise et liée à la grossesse. Les 7 échantillons positifs ont été également trouvés positifs avec le réactif anti-HbF/anti-AC avec une corrélation satisfaisante des résultats entre les 2 techniques.
Ce kit est intéressant en 2e intention chez les patientes présentant un premier résultat ininterprétable avec un réactif anti-HbF seul, notamment en cas d’augmentation importante de l’HbF. Chez ces patientes, il apparaît souhaitable de compléter le bilan par une électrophorèse de l’hémoglobine en l’absence d’antériorité.
Summary
Acquired factor V inhibitor (AFVI) is an extremely rare disorder that may cause severe bleeding. To identify factors associated with bleeding risk in AFVI patients, a national, multicentre, ...retrospective study was made including all AFVI patients followed in 21 centres in France between 1988 and 2015. All patients had an isolated factor V (FV) deficiency <50% associated with inhibitor activity. Patients with constitutional FV deficiency and other causes of acquired coagulation FV deficiencies were excluded. The primary outcome was incident bleeding and factors associated with the primary outcome were identified. Thirty‐eight (74 36–100 years, 42·1% females) patients with AFVI were analysed. Bleeding was reported in 18 (47·4%) patients at diagnosis and in three (7·9%) during follow‐up (7 0·2–48.7 months). At diagnosis, FV was <10% in 31 (81·6%) patients. Bleeding at diagnosis was associated with a prolonged prothrombin time that strongly correlated with the AFVI level measured in plasma {r = 0·63, 95% confidence interval (CI) 0·36–0·80, P < 0·05}. Bleeding onset during follow‐up was associated with a slow AFVI clearance (P < 0·001). The corresponding receiver operating characteristics curve showed that AFVI clearance was predictive of bleeding onset with an AFVI clearance of seven months with a sensitivity of 100% (95% CI: 29–100) and a specificity of 86% (95% CI: 57–98, P = 0·02). Kaplan–Meier analysis showed that AFVI clearance >7 months increased the risk of bleeding by 8 (95% CI: 0·67–97, P = 0·075). Prothrombin time at diagnosis and time for clearance of FV inhibitor during follow‐up are both associated with bleeding in patients with AFVI.

Objectif Valider une stabilité pré-analytique >48h pour rendre possible la réalisation de cet examen sur des prélèvements non décantés ayant un délai d’acheminement long.
(1) Étude de la stabilité pré-analytique sur tubes EDTA conservés à température réfrigérée lorsque la centrifugation est réalisée à J5 du prélèvement, pour 21 échantillons de patientes dont le fœtus a préalablement été déterminé de génotype RHD positif. (2) Étude de la stabilité sur tubes Cell-Free DNA (Streck) centrifugés à J10 et J14 : comparaison aux Résultats obtenus sur EDTA pour 31 patientes.
(1) 48 % de génotypages indéterminés pour les prélèvements sur tubes EDTA centrifugés à J5 du prélèvement, mais aucun faux négatif. (2) Concordance satisfaisante entre tubes EDTA dans les conditions standards et tubes Cell-Free DNA : 77,4 % de concordance à J10 et 83,3 % à J14. Une seule discordance : résultat positif sur EDTA trouvé négatif à J10 sur Cell-Free DNA, à relativiser car tout premier résultat négatif fait l’objet d’un contrôle sur un second prélèvement.
Validation de la stabilité pré-analytique sur tubes Cell-Free DNA centrifugés jusqu’à 14jours après le prélèvement, permettant d’éviter une décantation manuelle pour les échantillons au délai d’acheminement long. L’étude de la stabilité de l’ADN fœtal libre sur tubes EDTA centrifugés à J5 mérite d’être poursuivie.
The quantitation of human hepatitis B virus (HBV) in the serum of infected patients is recommended to characterize the course of chronic HBV infection. The better sensitivity and broader dynamic range of HBV DNA quantitation using the Abbott RealTime PCR assay was confirmed by the study of 362 serum samples from 311 patients. In addition, data analysis revealed the concordance of HBV DNA quantitations between the two assays
. When this evaluation was assessed as a function of HBV genotype, there was discordance for HBV genotype C samples. Thus, we performed an in-house PCR to confirm the discrepancy observed regarding the HBV genotypes. The in-house PCR results agreed better with the Abbott RealTime PCR method when compared with the standard hybridization assay.
In conclusion, the wide dynamic range of HBV DNA quantitation achieved with the Abbott RealTime PCR assay makes it appropriate for the clinical monitoring of HBV infected patients. However, a change of HBV DNA quantitation method could influence results on the follow-up of HBV genotype C infected patients.
Dry eye symptoms are among the leading complaints in ophthalmology. Dry eye disease (DED) is associated with significant pain affecting quality of life. Cellular and molecular mechanisms underlying ...ocular pain associated with DED are not fully understood. In this study, we investigated the ocular surface of patients with DED using in vivo confocal microscopy (IVCM) to quantify corneal nerve density and its relation with corneal inflammation. Gene expression of the proinflammatory markers HLA-DR, IL-6, CXCL12, and CCL2 and the receptors CXCR4 and CCR2, as well as PENK (enkephalin precursor), was therefore quantified in conjunctival impression cytology specimens. Thirty-two patients with DED and 15 age-matched controls were included. Subbasal nerve density was significantly lower in DED patients compared to controls. IVCM analysis revealed that DED patients had a significantly higher corneal dendritic cell density compared to controls. Conjunctival impression cytology analysis revealed that HLA-DR, IL-6, CXCR4, and CCL2/CCR2 mRNA levels were significantly increased in DED patients compared to controls, whereas PENK mRNA levels were significantly decreased. Similar results were obtained in vitro on immortalized human conjunctiva-derived epithelial cells challenged with osmotic stress that mimics the DED condition. These results demonstrate that proinflammatory molecules and endogenous enkephalin have opposite gene regulation during DED.
