Bone is more difficult to break than to split. Although this is well known, and many studies exist on the behaviour of long cracks in bone, there is a need for data on the orientation-dependent ...crack-growth resistance behaviour of human cortical bone that accurately assesses its toughness at appropriate size scales. Here, we use in situ mechanical testing to examine how physiologically pertinent short (<600 microm) cracks propagate in both the transverse and longitudinal orientations in cortical bone, using both crack-deflection/twist mechanics and nonlinear-elastic fracture mechanics to determine crack-resistance curves. We find that after only 500 microm of cracking, the driving force for crack propagation was more than five times higher in the transverse (breaking) direction than in the longitudinal (splitting) direction owing to major crack deflections/twists, principally at cement sheaths. Indeed, our results show that the true transverse toughness of cortical bone is far higher than previously reported. However, the toughness in the longitudinal orientation, where cracks tend to follow the cement lines, is quite low at these small crack sizes; it is only when cracks become several millimetres in length that bridging mechanisms can fully develop leading to the (larger-crack) toughnesses generally quoted for bone.
Abstract Quantitative assessment of the strength and toughness of bone has become an integral part of many biological and bioengineering studies on the structural properties of bone and their ...degradation due to aging, disease and therapeutic treatment. Whereas the biomechanical techniques for characterizing bone strength are well documented, few studies have focused on the theory, methodology, and various experimental procedures for evaluating the fracture toughness of bone, i.e., its resistance to fracture, with particular reference to whole bone testing in small animal studies. In this tutorial, we consider the many techniques for evaluating toughness and assess their specific relevance and application to the mechanical testing of small animal bones. Parallel experimental studies on wild-type rat and mouse femurs are used to evaluate the utility of these techniques and specifically to determine the coefficient of variation of the measured toughness values.
Severe Acute Respiratory Syndrome coronavirus-2 (SARS-CoV-2) has challenged public health agencies globally. In order to effectively target government responses, it is critical to identify the ...individuals most at risk of coronavirus disease-19 (COVID-19), developing severe clinical signs, and mortality. We undertook a systematic review of the literature to present the current status of scientific knowledge in these areas and describe the need for unified global approaches, moving forwards, as well as lessons learnt for future pandemics.
Medline, Embase and Global Health were searched to the end of April 2020, as well as the Web of Science. Search terms were specific to the SARS-CoV-2 virus and COVID-19. Comparative studies of risk factors from any setting, population group and in any language were included. Titles, abstracts and full texts were screened by two reviewers and extracted in duplicate into a standardised form. Data were extracted on risk factors for COVID-19 disease, severe disease, or death and were narratively and descriptively synthesised.
One thousand two hundred and thirty-eight papers were identified post-deduplication. Thirty-three met our inclusion criteria, of which 26 were from China. Six assessed the risk of contracting the disease, 20 the risk of having severe disease and ten the risk of dying. Age, gender and co-morbidities were commonly assessed as risk factors. The weight of evidence showed increasing age to be associated with severe disease and mortality, and general comorbidities with mortality. Only seven studies presented multivariable analyses and power was generally limited. A wide range of definitions were used for disease severity.
The volume of literature generated in the short time since the appearance of SARS-CoV-2 has been considerable. Many studies have sought to document the risk factors for COVID-19 disease, disease severity and mortality; age was the only risk factor based on robust studies and with a consistent body of evidence. Mechanistic studies are required to understand why age is such an important risk factor. At the start of pandemics, large, standardised, studies that use multivariable analyses are urgently needed so that the populations most at risk can be rapidly protected.

The decision support system for agrotechnology transfer (DSSAT) has been in use for the last 15 years by researchers worldwide. This package incorporates models of 16 different crops with software ...that facilitates the evaluation and application of the crop models for different purposes. Over the last few years, it has become increasingly difficult to maintain the DSSAT crop models, partly due to fact that there were different sets of computer code for different crops with little attention to software design at the level of crop models themselves. Thus, the DSSAT crop models have been re-designed and programmed to facilitate more efficient incorporation of new scientific advances, applications, documentation and maintenance. The basis for the new DSSAT cropping system model (CSM) design is a modular structure in which components separate along scientific discipline lines and are structured to allow easy replacement or addition of modules. It has one Soil module, a Crop Template module which can simulate different crops by defining species input files, an interface to add individual crop models if they have the same design and interface, a Weather module, and a module for dealing with competition for light and water among the soil, plants, and atmosphere. It is also designed for incorporation into various application packages, ranging from those that help researchers adapt and test the CSM to those that operate the DSSAT–CSM to simulate production over time and space for different purposes. In this paper, we describe this new DSSAT–CSM design as well as approaches used to model the primary scientific components (soil, crop, weather, and management). In addition, the paper describes data requirements and methods used for model evaluation. We provide an overview of the hundreds of published studies in which the DSSAT crop models have been used for various applications. The benefits of the new, re-designed DSSAT–CSM will provide considerable opportunities to its developers and others in the scientific community for greater cooperation in interdisciplinary research and in the application of knowledge to solve problems at field, farm, and higher levels.
Self-assembled phospholipid bilayer Nanodiscs have become an important and versatile tool among model membrane systems to functionally reconstitute membrane proteins. Nanodiscs consist of lipid ...domains encased within an engineered derivative of apolipoprotein A-1 scaffold proteins, which can be tailored to yield homogeneous preparations of disks with different diameters, and with epitope tags for exploitation in various purification strategies. A critical aspect of the self-assembly of target membranes into Nanodiscs lies in the optimization of the lipid:protein ratio. Here we describe strategies for performing this optimization and provide examples for reconstituting bacteriorhodopsin as a trimer, rhodopsin, and functionally active P-glycoprotein. Together, these demonstrate the versatility of Nanodisc technology for preparing monodisperse samples of membrane proteins of wide-ranging structure.
Indentation techniques for assessing fracture toughness are attractive due to the simplicity and expediency of experiments, and because they potentially allow the characterization of both local and ...bulk fracture properties. Unfortunately, rarely have such techniques been proven to give accurate fracture toughness values. This is a concern, as such techniques are seeing increasing usage in the study of biomaterials and biological hard tissues. Four available indentation techniques are considered in the present article: the Vickers indentation fracture (VIF) test, the cube corner indentation fracture (CCIF) test, the Vickers crack opening displacement (VCOD) test and the interface indentation fracture (IIF) test. Each technique is discussed in terms of its suitability for assessing the absolute and relative toughness of materials or material interfaces based on the published literature on the topic. In general, the VIF and CCIF techniques are found to be poor for quantitatively evaluating toughness of any brittle material, and the large errors involved (∼±50%) make their applicability as comparative techniques limited. Indeed, indentation toughness values must differ by at least by a factor of three to conclude a significant difference in actual toughness. Additionally, new experimental results are presented on using the CCIF test to evaluate the fracture resistance of human cortical bone. Those new results indicate that inducing cracking is difficult, and that the cracks that do form are embedded in the plastic zone of the indent, invalidating the use of linear elastic fracture mechanics based techniques for evaluating the toughness associated with those cracks. The VCOD test appears to be a good quantitative method for some glasses, but initial results suggest there may be problems associated with applying this technique to other brittle materials. Finally, the IIF technique should only be considered a comparative or semi-quantitative technique for comparing material interfaces and/or the neighboring materials.
Summary
High‐dose chemotherapy (HDC) and autologous stem cell transplantation (ASCT) are used as consolidation in first remission (CR1) in some centres for untreated, transformed indolent B‐cell ...lymphoma (Tr‐iNHL) but the evidence base is weak. A total of 319 patients with untreated Tr‐iNHL meeting prespecified transplant eligibility criteria age <75, LVEF ≥45%, no severe lung disease, CR by positron emission tomography or computed tomography ≥3 months after at least standard cyclophosphamide, doxorubicin, vincristine and prednisolone with rituximab (R‐CHOP) intensity front‐line chemotherapy were retrospectively identified. Non‐diffuse large B‐cell lymphoma transformations were excluded. About 283 (89%) patients had follicular lymphoma, 30 (9%) marginal‐zone lymphoma and six (2%) other subtypes. Forty‐nine patients underwent HDC/ASCT in CR1, and a 1:2 propensity‐score‐matched cohort of 98 patients based on age, stage and high‐grade B‐cell lymphoma with MYC, BCL2 and/or BCL6 rearrangements (HGBL‐DH) was generated. After a median follow‐up of 3·7 (range 0·1–18·3) years, ASCT was associated with significantly superior progression‐free survival hazard ratio (HR) 0·51, 0·27–0·98; P = 0·043 with a trend towards inferior overall survival (OS; HR 2·36;0·87–6·42; P = 0·1) due to more deaths from progressive disease (8% vs. 4%). Forty (41%) patients experienced relapse in the non‐ASCT cohort — 15 underwent HDC/ASCT with seven (47%) ongoing complete remission (CR); 10 chimeric antigen receptor‐modified T‐cell (CAR‐T) therapy with 6 (60%) ongoing CR; 3 allogeneic SCT with 2 (67%) ongoing CR. Although ASCT in CR1 improves initial duration of disease control in untreated Tr‐iNHL, the impact on OS is less clear with effective salvage therapies in this era of CAR‐T.

Aim
To compare the relative efficacy of thiopurines and anti‐TNF therapy in patients at high risk of ...disease recurrence.
Methods
As part of a larger study comparing post‐operative management strategies, patients at high risk of recurrence (smoker, perforating disease, ≥2nd operation) were treated after resection of all macroscopic disease with 3 months metronidazole together with either azathioprine 2 mg/kg/day or mercaptopurine 1.5 mg/kg/day. Thiopurine‐intolerant patients received adalimumab induction then 40 mg fortnightly. Patients underwent colonoscopy at 6 months with endoscopic recurrence assessed blind to treatment.
Results
A total of 101 patients 50% male; median (IQR) age 36 (25–46) years were included. There were no differences in disease history between thiopurine‐ and adalimumab‐treated patients. Fifteen patients withdrew prior to 6 months, five due to symptom recurrence (of whom four were colonoscoped). Endoscopic recurrence (Rutgeerts score i2–i4) occurred in 33 of 73 (45%) thiopurine vs. 6 of 28 (21%) adalimumab‐treated patients intention‐to‐treat (ITT); P = 0.028 or 24 of 62 (39%) vs. 3 of 24 (13%) respectively per‐protocol analysis (PPA); P = 0.020. Complete mucosal endoscopic normality (Rutgeerts i0) occurred in 17/73 (23%) vs. 15/28 (54%) (ITT; P = 0.003) and in 27% vs. 63% (PPA; P = 0.002). The most advanced disease (Rutgeerts i3 and i4) occurred in 8% vs. 4% (thiopurine vs. adalimumab).
Conclusions
In Crohn's disease patients at high risk of post‐operative recurrence adalimumab is superior to thiopurines in preventing early disease recurrence.
The British Society of Paediatric and Adolescent Gynaecology (BritSPAG) was created in 2000 with specific aims to include raising the profile of paediatric and adolescent gynaecology (PAG) within the ...United Kingdom (UK). The Society has since developed a set of clinical standards for all acute hospitals providing gynaecological services to enable successful provision of paediatric and adolescent gynaecology care.
To determine the depth of knowledge that obstetric and gynaecology trainees have with regards to the PAG services provided at their Hospital, reflecting how widely PAG services have had an impact on trainees.
The national survey was distributed to all deaneries in the UK for circulation to all their trainees via e mail during Nov 2017-March 2018.
28% of the trainees said there was a PAG clinic at their hospital, 46.9% did not have a clinic and 24.7% were unsure. 41.6% of the respondents were aware of BritSPAG, however only 10.4% were aware of the BritSPAG clinical standards for service planning with regards to PAG clinics. Nearly half were aware of the PAG specialist centre for their region but only 6.5% were aware of the BritSPAG UK map of services. A large majority (93.24%) didn’t believe that trainees in O&G received adequate exposure to PAG in their training.
This study represents the largest and first national survey to seek obstetric and gynaecology trainees’ thoughts on the provision of PAG training in the UK today. Given that only 28% of trainees answering said that they were aware of a PAG clinic at their hospital, this indicates not only that many hospitals did not have a dedicated PAG clinic but more worryingly five of the trainees were not aware of the existence of a confirmed PAG clinic at their hospital, and therefore are potentially losing out on training opportunities. Disappointingly the results of our survey reveal that trainees in Obstetrics and Gynaecology still have very little experience or exposure to PAG during their training despite there being opportunities to do so.
We present data from an induced gallium arsenide (GaAs) quantum wire that exhibits an additional conductance plateau at 0.5(2e²/h), where e is the charge of an electron and h is Planck's constant, in ...zero magnetic field. The plateau was most pronounced when the potential landscape was tuned to be symmetric by using low-temperature scanning-probe techniques. Source-drain energy spectroscopy and temperature response support the hypothesis that the origin of the plateau is the spontaneous spin-polarization of the transport electrons: a ferromagnetic phase. Such devices may have applications in the field of spintronics to either generate or detect a spin-polarized current without the complications associated with external magnetic fields or magnetic materials.
