In this report we show that a vacuum is a nonlinear optical medium and discuss what the optical phenomena are that should exist in the framework of the standard model of particle physics. We pay ...special attention to the low energy limit. The predicted effects for photons of energy smaller than the electron rest mass are of such a level that none have yet been observed experimentally. Progress in field sources and related techniques seem to indicate that in a few years vacuum nonlinear optics will be accessible to human investigation.
Il existe de nombreuses techniques de réparation des fibres profondes du TFCC qui ont toutes pour point commun de nécessiter un abord du versant ulnaire du poignet. Nous rapportons ici les résultats ...d’une technique « full arthroscopic knotless » utilisée depuis début 2014.
Entre janvier 2014 et août 2017, 91 patients ont été opérés par 4 voies d’abord arthroscopique. Aucun abord par arthrotomie jamais n’a été nécessaire. Il s’agissait de 68 hommes pour 23 femmes. L’âge moyen était de 32,4 ans, la main dominante était atteinte 65 fois, 42 était des travailleurs manuels. Le mécanisme était une chute dans 64 cas, une supination forcée du poignet dans 19 cas. Le délai moyen entre accident et intervention chirurgicale était de 6,2 mois. Dans 53 cas, il s’agissait d’une lésion isolée des fibres profondes (Atzei 3) et dans 38 cas d’une lésion associée des fibres profondes et superficielles (Atzei 2).
L’intervention utilise 4 voies d’abord (34R, 6R, 6U et RUD). L’originalité de la technique est que la voie d’abord 6U est utilisée pour un abord habituel de l’articulation radio-carpienne mais également pour un abord RUD en passant sous le TFCC.
Quatre-vingt-onze patients ont été revus à un délai moyen de 28,6 mois. Les mobilités étaient comprises entre 97,5 et 98,8 % par rapport au côté controlatéral. La force de serrage est passée de 74,8 à 95,9 % par rapport au côté sain. La douleur a été réduite de 77 % en aigu et de 82 % en chronique. Le qDASH est passé de 37,3 à 5,7. Il n’y a eu pour seule complication que 2 neurapraxie transitoires et résolutives de la branche sensitive du nerf ulnaire (BSNU) dans les 8 premiers cas, aucun dans les 83 suivants. Tous les patients sont retournés à leur emploi au même poste.
Nous comparons nos résultats à la littérature. S’ils sont au minimum identiques sur la plupart des critères évalués, nous ne retrouvons que 2,2 % de neurapraxie de la BSNU, contre 10,4 % pour la série d’Atzei, que 5,4 % d’instabilité SOE 1 de la tête ulnaire contre 8,3 à 58 % dans la littérature.
Cette technique « full arthroscopique knotless » apparaît donc fiable en termes de résultats pour des taux de complications nerveuses et d’instabilité résiduelle inférieurs à ceux observés dans la littérature.
Depuis mars 2008, le remplacement du trapèze dans la rhizarthrose par un implant Pi2 est devenu notre intervention de référence. Nous rapportons les résultats d’une série de 208 implants posés en ...10,5 ans.
Entre mars 2014 et septembre 2018, 208 implants ont été posés chez 187 patients. Au total, 78 patients ont été revus (84 implants) à un recul minimum de 5 ans (5,1–10,2). Dix ont été perdus de vue. Il s’agissait de 72 femmes pour 6 hommes. L’âge moyen était de 66,3 ans, la main dominante était atteinte 48 fois, 58 étaient retraités. La rhizarthrose était classée Dell 0 2 fois, Dell 1 14 fois, Dell 2 31 fois, Dell 3 26 fois et Dell 4 1A fois.
Par une voie d’abord antéroexterne de 3cm, une trapézectomie « monobloc » est effectuée avec un respect particulier la capsule dorsale, élément clé de la stabilisation de l’implant. Une trapezoïdectomie partielle pour médialiser l’implant est réalisée. Une fois positionné, une capsuloplastie antérieure est confectionnée à l’aide d’une bandelette de l’APL puis la capsule refermée. Une attelle est mise en place pour 4 semaines avec exercices d’auto-rééducation à partir du quinzième jour.
Les mobilités de la MP et de l’IP se sont améliorées de 9 à 15°. La force de serrage a progressé de 19,8 à 26,2kg, celle de la pince pouce-index de 3,4 à 5,4kg La douleur aiguë est passée de 7,4 à 1,9, la douleur chronique de 3,2 à 0,4 et le qDASH de 63,2 à 16,9. Quatre-vingt-quatorze pour cent des patients se disaient satisfaits ou très satisfaits. Pour ce qui est des complications, 2 patients (sur 208) ont été réopérés pour persistance des douleurs, et nous avons déplorés 5 luxations sur 208 cas (2,5 %), dont seulement 2 ont été réopérées. Il n’y a eu aucune infection ni algodystrophie.
Nous comparons nos résultats à ceux de la littérature, non seulement pour ce qui est des résultats cliniques mais également radiologiques, et plus particulièrement en termes de complications (luxations : de 4,6 à 33,0 % - réinterventions : de 0,0 à 33,0 %).
Si la stabilisation de l’implant reste la difficulté principale, le faible taux de reprise et la qualité des résultats paraissent des éléments intéressants justifiant le choix de cet implant.

In this paper we set a framework in which experiments whose goal is to test QED predictions can be used in a more general way to test nonlinear electrodynamics (NLED) which contains low-energy QED as ...a special case. We review some of these experiments and we establish limits on the different free parameters by generalizing QED predictions in the framework of NLED. We finally discuss the implications of these limits on bound systems and isolated charged particles for which QED has been widely and successfully tested.
QED vacuum can be polarized and magnetized by an external electromagnetic field, therefore acting as a birefringent medium. This effect has not yet been measured. In this paper, after having recalled ...the main facts concerning vacuum magnetic birefringence polarimetry detection method and the related noise sources, we detail our Monte Carlo simulation of a pulsed magnetic field data run. Our Monte Carlo results are optimized to match BMV experiment 2014 data. We show that our Monte Carlo approach can reproduce experimental results giving an important insight into the systematic effects limiting experiment sensitivity.

Pharmacophore modeling incorporates geometric and chemical features of known inhibitors and/or targeted binding sites to rationally identify and design new drug leads. In this study, we have encoded ...a three-dimensional pharmacophore matching similarity (FMS) scoring function into the structure-based design program DOCK. Validation and characterization of the method are presented through pose reproduction, crossdocking, and enrichment studies. When used alone, FMS scoring dramatically improves pose reproduction success to 93.5% (∼20% increase) and reduces sampling failures to 3.7% (∼6% drop) compared to the standard energy score (SGE) across 1043 protein–ligand complexes. The combined FMS+SGE function further improves success to 98.3%. Crossdocking experiments using FMS and FMS+SGE scoring, for six diverse protein families, similarly showed improvements in success, provided proper pharmacophore references are employed. For enrichment, incorporating pharmacophores during sampling and scoring, in most cases, also yield improved outcomes when docking and rank-ordering libraries of known actives and decoys to 15 systems. Retrospective analyses of virtual screenings to three clinical drug targets (EGFR, IGF-1R, and HIVgp41) using X-ray structures of known inhibitors as pharmacophore references are also reported, including a customized FMS scoring protocol to bias on selected regions in the reference. Overall, the results and fundamental insights gained from this study should benefit the docking community in general, particularly researchers using the new FMS method to guide computational drug discovery with DOCK.
The endocannabinoid anandamide (AEA) is an antinociceptive lipid that is inactivated through cellular uptake and subsequent catabolism by fatty acid amide hydrolase (FAAH). Fatty acid binding ...proteins (FABPs) are intracellular carriers that deliver AEA and related N-acylethanolamines (NAEs) to FAAH for hydrolysis. The mammalian brain expresses three FABP subtypes: FABP3, FABP5, and FABP7. Recent work from our group has revealed that pharmacological inhibition of FABPs reduces inflammatory pain in mice. The goal of the current work was to explore the effects of FABP inhibition upon nociception in diverse models of pain. We developed inhibitors with differential affinities for FABPs to elucidate the subtype(s) that contributes to the antinociceptive effects of FABP inhibitors. Inhibition of FABPs reduced nociception associated with inflammatory, visceral, and neuropathic pain. The antinociceptive effects of FABP inhibitors mirrored their affinities for FABP5, while binding to FABP3 and FABP7 was not a predictor of in vivo efficacy. The antinociceptive effects of FABP inhibitors were mediated by cannabinoid receptor 1 (CB1) and peroxisome proliferator-activated receptor alpha (PPARα) and FABP inhibition elevated brain levels of AEA, providing the first direct evidence that FABPs regulate brain endocannabinoid tone. These results highlight FABPs as novel targets for the development of analgesic and anti-inflammatory therapeutics.
We report unrestrained, all-atom molecular dynamics simulations of HIV-1 protease that sample large conformational changes of the active site flaps. In particular, the unliganded protease undergoes ...multiple conversions between the "closed" and "semiopen" forms observed in crystal structures of inhibitor-bound and unliganded protease, respectively, including reversal of flap "handedness." Simulations in the presence of a cyclic urea inhibitor yield stable closed flaps. Furthermore, we observe several events in which the flaps of the unliganded protease open to a much greater degree than observed in crystal structures and subsequently return to the semiopen state. Our data strongly support the hypothesis that the unliganded protease predominantly populates the semiopen conformation, with closed and fully open structures being a minor component of the overall ensemble. The results also provide a model for the flap opening and closing that is considered to be essential to enzyme function.
Prostate cancer is a leading cause of cancer-related deaths in men in the United States. Although treatable when detected early, prostate cancer commonly transitions to an aggressive ...castration-resistant metastatic state. While taxane chemotherapeutics such as docetaxel are mainstay treatment options for prostate cancer, taxane resistance often develops. Fatty acid binding protein 5 (FABP5) is an intracellular lipid chaperone that is upregulated in advanced prostate cancer and is implicated as a key driver of its progression. The recent demonstration that FABP5 inhibitors produce synergistic inhibition of tumor growth when combined with taxane chemotherapeutics highlights the possibility that FABP5 may regulate other features of taxane function, including resistance. Employing taxane-resistant DU145-TXR cells and a combination of cytotoxicity, apoptosis, and cell cycle assays, our findings demonstrate that FABP5 knockdown sensitizes the cells to docetaxel. In contrast, docetaxel potency was unaffected by FABP5 knockdown in taxane-sensitive DU145 cells. Taxane-resistance in DU145-TXR cells stems from upregulation of the P-glycoprotein ATP binding cassette subfamily B member 1 (ABCB1). Expression analyses and functional assays confirmed that FABP5 knockdown in DU145-TXR cells markedly reduced ABCB1 expression and activity, respectively. Our study demonstrates a potential new function for FABP5 in regulating taxane sensitivity and the expression of a major P-glycoprotein efflux pump in prostate cancer cells.
