The assessment of small airways obstruction (SAO) using spirometry is practiced in population-based studies. However, it is not clear what are the most used parameters and cut-offs to define abnormal ...results.
We searched three databases (Medline, Web of Science, Google Scholar) for population-based studies, published by 1 May 2021, that used spirometry parameters to identify SAO and/or provided criteria for defining SAO. We systematically reviewed these studies and summarised evidence to determine the most widely used spirometry parameter and criteria for defining SAO. In addition, we extracted prevalence estimates and identified associated risk factors. To estimate a pooled prevalence of SAO, we conducted a meta-analysis and explored heterogeneity across studies using meta regression.
Twenty-five studies used spirometry to identify SAO. The most widely utilised parameter (15 studies) was FEF
, either alone or in combination with other measurements. Ten studies provided criteria for the definition of SAO, of which percent predicted cut-offs were the most common (5 studies). However, there was no agreement on which cut-off value to use. Prevalence of SAO ranged from 7.5% to 45.9%. As a result of high heterogeneity across studies (I
= 99.3%), explained by choice of spirometry parameter and WHO region, we do not present a pooled prevalence estimate.
There is a lack of consensus regarding the best spirometry parameter or defining criteria for identification of SAO. The value of continuing to measure SAO using spirometry is unclear without further research using large longitudinal data. PROSPERO registration number CRD42021250206.
A top-down nanofabrication approach is used to develop silicon nanowires from silicon-on-insulator (SOI) wafers and involves direct-write electron beam lithography (EBL), inductively coupled ...plasma-reactive ion etching (ICP-RIE) and a size reduction process. To achieve nanometer scale size, the crucial factors contributing to the EBL and size reduction processes are highlighted. The resulting silicon nanowires, which are 20 nm in width and 30 nm in height (with a triangular shape) and have a straight structure over the length of 400 μm, are fabricated precisely at the designed location on the device. The device is applied in biomolecule detection based on the changes in drain current (Ids), electrical resistance and conductance of the silicon nanowires upon hybridization to complementary target deoxyribonucleic acid (DNA). In this context, the scaled-down device exhibited superior performances in terms of good specificity and high sensitivity, with a limit of detection (LOD) of 10 fM, enables for efficient label-free, direct and higher-accuracy DNA molecules detection. Thus, this silicon nanowire can be used as an improved transducer and serves as novel biosensor for future biomedical diagnostic applications.
The most effective surgical approach for total hip arthroplasty (THA) remains controversial. The direct anterior approach may be associated with a reduced risk of dislocation, faster recovery, ...reduced pain and fewer surgical complications. This systematic review aims to evaluate the current evidence for the use of this approach in THA.
Following the Cochrane collaboration, an extensive literature search of PubMed, Medline, Embase and OvidSP was conducted. Randomised controlled trials, comparative studies, and cohort studies were included. Outcomes included the length of the incision, blood loss, operating time, length of stay, complications, and gait analysis.
A total of 42 studies met the inclusion criteria. Most were of medium to low quality. There was no difference between the direct anterior, anterolateral or posterior approaches with regards to length of stay and gait analysis. Papers comparing the length of the incision found similar lengths compared with the lateral approach, and conflicting results when comparing the direct anterior and posterior approaches. Most studies found the mean operating time to be significantly longer when the direct anterior approach was used, with a steep learning curve reported by many. Many authors used validated scores including the Harris hip score, and the Western Ontario and McMaster Universities Arthritis Index. These mean scores were better following the use of the direct anterior approach for the first six weeks post-operatively. Subsequently there was no difference between these scores and those for the posterior approach.
There is little evidence for improved kinematics or better long-term outcomes following the use of the direct anterior approach for THA. There is a steep learning curve with similar rates of complications, length of stay and outcomes. Well-designed, multi-centre, prospective randomised controlled trials are required to provide evidence as to whether the direct anterior approach is better than the lateral or posterior approaches when undertaking THA. Fracture mechanics characterization tests for adhesive joints are analyzed and reviewed in order to understand their advantages and disadvantages. Data reduction techniques for analytical methods are ...summarized to understand the improvements implemented in each test. Numerical approaches are also used complementing tests information. Both linear and non-linear methods to obtain the fracture energy release rate are presented. Pure mode I and mode II tests are described. Simple mixed-mode tests, varying only the specimen geometry, with limited mode-mixity are also presented. Performing a wider mode-mixity range requires sophisticated apparatus that are studied in detail. There is no general agreement about the test suitability for mixed-mode fracture assessment of adhesive joints. A universal test that can easily be performed and give accurate results is essential to optimize the expensive testing at the design stage.
On-chip nonlinear optics is a thriving research field, which creates transformative opportunities for manipulating classical or quantum signals in small-footprint integrated devices. Since the length ...scales are short, nonlinear interactions need to be enhanced by exploiting materials with large nonlinearity in combination with high-Q resonators or slow-light structures. This, however, often results in simultaneous enhancement of competing nonlinear processes, which limit the efficiency and can cause signal distortion. Here, we exploit the frequency dependence of the optical density-of-states near the edge of a photonic bandgap to selectively enhance or inhibit nonlinear interactions on a chip. We demonstrate this concept for one of the strongest nonlinear effects, stimulated Brillouin scattering using a narrow-band one-dimensional photonic bandgap structure: a Bragg grating. The stimulated Brillouin scattering enhancement enables the generation of a 15-line Brillouin frequency comb. In the inhibition case, we achieve stimulated Brillouin scattering free operation at a power level twice the threshold.
Recent observations of NGC 1068 and other AGN support the idea of a geometrically and optically thick dust torus surrounding the central supermassive black hole and accretion disk of AGN. In ...type 2 AGN, the torus is seen roughly edge-on, leading to obscuration of the central radiation source and a silicate absorption feature near $10~{\rm\mu m}$. While most of the current torus models distribute the dust smoothly, there is growing evidence that the dust must be arranged in clouds. We describe a new method for modeling near- and mid-infrared emission of 3-dimensional clumpy tori using Monte Carlo simulations. We calculate the radiation fields of individual clouds at various distances from the AGN and distribute these clouds within the torus region. The properties of the individual clouds and their distribution within the torus are determined from a theoretical approach of self-gravitating clouds close to the shear limit in a gravitational potential. We demonstrate that clumpiness in AGN tori can overcome the problem of over-pronounced silicate features. Finally, we present model calculations for the prototypical Seyfert 2 galaxy NGC 1068 and compare them to recent high-resolution measurements. Our model is able to reproduce both the SED and the interferometric observations of NGC 1068 in the near- and mid-infrared.
In the present work we analyzed seven globular clusters (GCs) selected from their location in the Galactic bulge and with metallicity values in the range −1.30 Fe/H −0.50. The aim of this work is ...first to derive cluster ages assuming single stellar populations and second to identify the stars from first (1G) and second generations (2G) from the main sequence, subgiant, and red giant branches, and to derive their age differences. Based on a combination of UV and optical filters used in this project, we apply the Gaussian mixture models to distinguish the multiple stellar populations. Applying statistical isochrone fitting, we derive self-consistent ages, distances, metallicities, and reddening values for the sample clusters. An average age of 12.3 0.4 Gyr was obtained both using DSED and BaSTI (accounting atomic diffusion effects) isochrones, without a clear distinction between the moderately metal-poor and the more metal-rich bulge clusters, except for NGC 6717 and the inner halo NGC 6362 with ∼13.5 Gyr. We derived a weighted mean age difference between the multiple populations hosted by each GC of 41 170 Myr adopting canonical He abundances; whereas for higher He in 2G stars, this difference reduces to 17 170 Myr, but with individual uncertainties of 500 Myr.

The importance of the gut-brain axis in maintaining homeostasis has long been appreciated. However, the past 15 yr have seen the emergence of the microbiota (the trillions of microorganisms within ...and on our bodies) as one of the key regulators of gut-brain function and has led to the appreciation of the importance of a distinct microbiota-gut-brain axis. This axis is gaining ever more traction in fields investigating the biological and physiological basis of psychiatric, neurodevelopmental, age-related, and neurodegenerative disorders. The microbiota and the brain communicate with each other via various routes including the immune system, tryptophan metabolism, the vagus nerve and the enteric nervous system, involving microbial metabolites such as short-chain fatty acids, branched chain amino acids, and peptidoglycans. Many factors can influence microbiota composition in early life, including infection, mode of birth delivery, use of antibiotic medications, the nature of nutritional provision, environmental stressors, and host genetics. At the other extreme of life, microbial diversity diminishes with aging. Stress, in particular, can significantly impact the microbiota-gut-brain axis at all stages of life. Much recent work has implicated the gut microbiota in many conditions including autism, anxiety, obesity, schizophrenia, Parkinson's disease, and Alzheimer's disease. Animal models have been paramount in linking the regulation of fundamental neural processes, such as neurogenesis and myelination, to microbiome activation of microglia. Moreover, translational human studies are ongoing and will greatly enhance the field. Future studies will focus on understanding the mechanisms underlying the microbiota-gut-brain axis and attempt to elucidate microbial-based intervention and therapeutic strategies for neuropsychiatric disorders.
Retrospective studies suggest that subjects with chronic hepatitis C and advanced fibrosis who achieve a sustained virological response (SVR) have a lower risk of hepatic decompensation and ...hepatocellular carcinoma (HCC). In this prospective analysis, we compared the rate of death from any cause or liver transplantation, and of liver‐related morbidity and mortality, after antiviral therapy among patients who achieved SVR, virologic nonresponders (NR), and those with initial viral clearance but subsequent breakthrough or relapse (BT/R) in the HALT‐C (Hepatitis C Antiviral Long‐Term Treatment Against Cirrhosis) Trial. Laboratory and/or clinical outcome data were available for 140 of the 180 patients who achieved SVR. Patients with nonresponse (NR; n = 309) or who experienced breakthrough or relapse (BT/R; n = 77) were evaluated every 3 months for 3.5 years and then every 6 months thereafter. Outcomes included death, liver‐related death, liver transplantation, decompensated liver disease, and HCC. Median follow‐up for the SVR, BT/R, and NR groups of patients was 86, 85, and 79 months, respectively. At 7.5 years, the adjusted cumulative rate of death/liver transplantation and of liver‐related morbidity/mortality in the SVR group (2.2% and 2.7%, respectively) was significantly lower than that of the NR group (21.3% and 27.2%, P < 0.001 for both) but not the BT/R group (4.4% and 8.7%). The adjusted hazard ratio (HR) for time to death/liver transplantation (HR = 0.17, 95% confidence interval CI = 0.06‐0.46) or development of liver‐related morbidity/mortality (HR = 0.15, 95% CI = 0.06‐0.38) or HCC (HR = 0.19, 95% CI = 0.04‐0.80) was significant for SVR compared to NR. Laboratory tests related to liver disease severity improved following SVR. Conclusion: Patients with advanced chronic hepatitis C who achieved SVR had a marked reduction in death/liver transplantation, and in liver‐related morbidity/mortality, although they remain at risk for HCC. In this paper we investigate a particular ghost-free bimetric theory that exhibits the partially massless (PM) symmetry at quadratic order. At this order the global SO(1,4) symmetry of the theory is ...enhanced to SO(1,5). We show that this global symmetry becomes inconsistent at cubic order, in agreement with a previous calculation. Furthermore, we find that the PM symmetry of this theory cannot be extended beyond cubic order in the PM field. More importantly, it is shown that the PM symmetry cannot be extended to quartic order in any theory with one massless and one massive spin-2 fields.
