Philadelphia chromosome-like (Ph-like) ALL is a recent subtype of acute lymphoblastic leukemia. Although it does not express the BCR-ABL fusion gene, it has a behavior like true BCR/ABL1–positive ...cases. This subtype harbors different molecular alterations most commonly CRLF2 rearrangements. Most cases of Ph-like ALL are associated with high white blood cell count, high minimal residual disease level after induction therapy, and high relapse rate. Efforts should be encouraged for early recognition of Ph-like ALL to enhance therapeutic strategies. Recently, many trials are investigating the possibility of adding the tyrosine kinase inhibitor (TKI) to chemotherapy to improve clinical outcomes. The role and best timing of allogeneic bone marrow transplant in those cases are still unclear. Precision medicine should be implemented in the treatment of such cases. Abstract Acute leukemia is a hematological malignancy affecting different organ systems including the eye and orbit through direct infiltration of tissues or secondary to hematological abnormalities. ...Ophthalmological manifestations in acute leukemia are variable ranging from asymptomatic presentation to serious manifestations that can alter the disease course and treatment. The purpose of this study is to detect the incidence of different ophthalmological manifestations in newly diagnosed acute leukemia patients and to assess the relationship between ocular findings and hematological characteristics and the sequel of these neoplasms. A cross-sectional study with analytical components was conducted on 222 newly diagnosed acute myeloid and acute lymphoblastic leukemia patients who presented at Oncology Center Mansoura University (OCMU) between January 2022 and February 2023. All patients underwent a complete ophthalmic evaluation at Mansoura Ophthalmology Center (MOC). The mean age was 43.45 ± 17.35 years (range, 17–85), and M/F was 137 (61.7%)/85 (38.3%). One-hundred and forty-four (64.9%) had acute myeloid leukemia (AML), and 78 (35.1%) had acute lymphoblastic leukemia (ALL). Ophthalmic manifestations were detected in 96 patients (43.2%). Among them, 4 (1.8%) had poor visual acuity. Retinal hemorrhage (19.8%) and Roth spots (17.1%) were the most common ocular manifestations. Other ophthalmological manifestations observed were orbital involvement (3.2%), ocular motility issues (1.4%), subconjunctival hemorrhage (5.9%), conjunctival chemosis (0.9%),lid swelling (4.1%), lid ecchymosis (3.2%), lagophthalmos (0.5%), lid ptosis (1.8%), retinal venous congestion & tortuosity (4.1%), preretinal hemorrhage (3.2%), vitreous hemorrhage (3.2%), macular affection (2.3%), retinal infiltration (1.8%), exudative retinal detachment (ERD) (1.8%), cotton-wool spots (0.9%), retinal vein occlusion (0.5%), papilledema (2.8%), optic disc infiltration (1.8%), disc pallor (1.8%).AML patients were significantly associated with a higher frequency of ocular affection, retinal hemorrhages, and Roth spots ( P 0.028, 0.003, and 0.046, respectively) compared to ALL patients. Retinal hemorrhage was statistically significantly associated with anemia ( P 0.021). Ophthalmological manifestations of acute leukemia are heterogeneous; they can be detected at initial presentations or relapse. Some manifestations are asymptomatic, others can affect visual acuity or even alter the disease course. Cooperation between ophthalmologists and haemato-oncologists is crucial for recognizing ocular involvement and disease management.
Background
To estimate the diagnostic utility of chest CT qualitative assessment and chest CT total severity score (TSS) to predict mortality in oncology patients with COVID-19 infection.
Methods
...This retrospective study included 151 oncology patients with COVID-19 infection. 67, 84 were male and female, respectively. Their mean age (years) ± SD was 49.7 ± 14.9. Two radiologists individually reviewed the chest CT and scored the pulmonary abnormalities using TSS. Inter-observer agreement was determined using the Bland–Altman plot. Correlation between TSS and COVID-19 severity, complication, mortality, cancer status and effect in anticancer therapy plan was done.
Results
There was a statistically significant excellent agreement between the independent observers in quantitative pulmonary assessment using TSS with interclass correlation (ICC) > 0.9 (

< 0.001). ROC curve analysis revealed that TSS was statistically significantly higher in non-survivors using an optimum cut-off value of 5 to predict in-hospital mortality. Univariate analysis showed that age, pulmonary predominant pattern, pleural effusion, tree-in-bud, ECOG PS, tumour stage 4 and post-COVID cancer status were a statistically significant predictor of mortality. Multivariate analysis reported that consolidation versus ground-glass opacity (GGO), crazy paving pattern versus GGO and progressive versus remittent cancer diseases were statistically significant independent predictors of mortality among those patients.
Conclusions
TSS demonstrated excellent inter-observer agreement to assess COVID-19 in oncology patients with low cut-off value to predict in-hospital mortality, thus raising the attention to rapid proper care in this setting. There was a statistically significant positive correlation between TSS and delayed chemotherapeutic schedule.
Purpose
Assessment of individual VTE risk in cancer patients prior to chemotherapy is critical for determining necessity of interventions. Risk assessment models (RAM) are available but have not been ...validated for haematological malignancy. We aimed to assess the validity of the Vienna Cancer and Thrombosis Study (V-CATS) score in prediction of VTE in a variety of haematological malignancies.
Methods
This is a prospective cohort study conducted on 81 newly diagnosed cancer patients undergoing chemotherapy. Demographic, clinical and cancer related data were collected, patients were followed up for 6 months, and VTE events were recorded. Khorana score (KS) was calculated. Plasma D-dimer and sP-selectin were measured, and then, V-CATS score was calculated. Receiver operator curve (ROC) was used to assess the sensitivity and specificity of RAMs. A modified V-CATS was generated and subsequently assessed by using new cut-off levels of d-dimer and sP-selectin based on ROC curve of the patients’ results and compared the probability of VTE occurrence using all three RAMs.
Results
Among the 81 patients included in this study, a total of 2.7% were diagnosed with advanced metastatic cancer. The most frequent cancer was non-Hodgkin lymphoma (39.5%), and 8 patients (9.8%) developed VTE events. The calculated probability of VTE occurrence using KS, V-CATS and modified V-CATS scores at cut-off levels ≥ 3 was 87.5%, 87.5% and 100%, respectively. The AUC in ROC curve of modified Vienna CATS score showed significant difference when compared to that of V-CATS and KS (
P
= 0.047 and 0.029, respectively).
Conclusion
The findings of our study highlight the value of three VTE risk assessment models in haematological malignancies. The modified V-CATS score demonstrated higher specificity compared to both V-CATS and KS, while all three scores exhibited similar sensitivity. We encourage the implementation of RAMs in haematological cancers for an appropriate use of thromboprophylaxis.
Podoplanin (D2-40) is a lymphatic endothelial marker that is considered as a specific marker for lymphatic endothelial cells and lymphangiogenesis in salivary gland carcinomas (SGCs). Aim: ...the present study aimed to investigate the immunohistochemical expression of podoplanin in SGCs and to correlate its expression with the clinicopathological parameters and patients' survival. Forty-nine SGC cases were electronically selected. Demographic, clinical, laboratory, and survival data were reviewed and tabulated. Immunohistochemistry was performed using antipodoplanin. Cases were divided into low and high expression based on a scoring system. A score of 0 and 1 was considered low expression, while > 1 was considered high expression. Podoplanin high expression was seen in 46.9% of cases, and 53.1% of cases showed low expression. Significant statistical associations were seen between podoplanin expression and tumour grade ( p ≤ 0.001), tumour-nodal- metastasis (TNM) stage (p ≤ 0.001), tumour size (p ≤ 0.001), nodal metastasis (p ≤ 0.001), tumour type (p = 0.03), prognosis (p ≤ 0.001), and mortality (p ≤ 0.001). The overall survival and progression-free survival differed significantly in cases with high and low expression (p ≤ 0.001). Podoplanin overexpression might be a significant prognostic indicator for patients with SGCs, implicating that it is a potential therapeutic target to improve survival in these cancer patients.
Inflammatory bowel disease (IBD) with a subsequent diagnosis of non-Hodgkin lymphoma has been reported. Gastrointestinal tract involvement in mantle cell lymphoma (MCL) is rare and many case reports ...of MCL simulating IBD have been reported. Lymphomas have been reported in associations with chronic ulcerative colitis (UC). The prognosis of lymphoma in patients with IBD is not well known.
We report a case of MCL occurring in a patient with UC.
This is a case report of a patient presented at Oncology Center Mansoura University.
A 56-year-old male was referred for evaluation of thrombocytopenia and microcytic hypochromic anemia with a history of prior bleeding per rectum. Clinical examination revealed ecchymosis, mild splenomegaly, and small right submandibular lymph node. Colonoscopy showed diffuse mucosal erythema and edema of the transverse, descending, sigmoid colon, and rectum; ulcers showed loss of the vascular pattern. Extensive ulcerative colitis had a Mayo score 1.
Multiple biopsies were taken and microscopic examination revealed mild focal active colitis with multiple hyperplastic lymphoid follicles and no evidence of cellular atypia in the examined material. Gastroenterologist started the patient on corticosteroids. Virology screening, ANA, anti-ds-DNA, and Helicobacter pylori stool antigen tests were negative. Thrombocytopenia was refractory to corticosteroids, and the patient suffered from recurrent episodes of melena; the patient was shifted to eltrombopag. For the UC, mesalazine 500mg bid and proton pump inhibitors were prescribed. On further evaluation bone marrow examination revealed infiltration with small B-cell lymphomatous deposits (49%) positive for CD20, CD5 and Cyclin D1: MCL. Lymph node biopsy was also consistent with MCL. A suspicion that IBD could be an extranodal lymphomatous infiltration was raised; revision of the GIT pathology was consistent with UC and no malignancy was detected.
Our patient received R-CHOP/DHAP protocol and achieved complete remission.
The patient remains in complete remission on rituximab maintenance. The patient is ongoing follow up with gastroenterology for management of abdominal pain and gastrointestinal bleeding.
We must pay special attention for lymphomas occurring in IBD; exclusion of extranodal infiltration is mandatory for patient management.
Papillary breast lesions and neoplasms (PBLs/Ns) are diagnostically challenging lesions in both core needle biopsy (CNB) and radiology.
To determine the accuracy and upgrade rate of CNB and BI-RADS ...diagnosis of PBLs/Ns compared to final excision diagnosis and the factors linked to upgrade.
The favored CNB diagnosis and BI-RADS category for 82 PBLs/Ns were assessed based on histopathology, myoepithelial marker immunohistochemistry, mammographic/ultrasonographic findings. The radiological findings were compared to the pathological diagnoses. The accuracies of CNB and BI-RADS were compared to the excision diagnosis of the corresponding PBLs/Ns. The upgrade rates to malignancy were evaluated for both CNB and BI-RADS.
The presence of solid, irregular masses in breasts with composition A/B with calcification in radiology was significantly associated with the diagnosis of suspicious/malignant CNB, and malignant excision specimens (p<0.05). CNB was more accurate (90%), sensitive and specific with high positive and negative predictive values than BI-RADS. Combined CNB/BI-RADS accuracy was 90.2%. Overall upgrade rate came up to 9.8%. Upgrade rates to carcinoma were 7.3% for CNB and 8.5% for BI-RADS. Factors linked to upgrade were the age, lesion-size, BI-RADS category 4A and C, and histopathological/radiological discordance. All the upgraded PBLs/Ns were diagnosed as benign lesions in CNB with present/focally present myoepithelial diagnosis reflecting a sampling error.
Up to 9.8% of PBLs/Ns diagnosed on CNB and BI-RADS undergo upgrading upon final excision, despite the high diagnostic accuracy. These evidences should be considered for final decision on whether to excise the lesion or not.
