Schnitzler's syndrome is characterized by recurrent urticarial rash and monoclonal gammopathy, associated with clinical and biological signs of inflammation and a long‐term risk of AA amyloidosis and ...overt lymphoproliferation. An extensive literature review was performed, and the following questions were addressed during an expert meeting: In whom should Schnitzler's syndrome be suspected? How should the diagnosis of Schnitzler's syndrome be established? How should a patient with Schnitzler's syndrome be treated? How should a patient with Schnitzler's syndrome be followed up?. A diagnosis of Schnitzler's syndrome is considered definite in any patient with two obligate criteria: a recurrent urticarial rash and a monoclonal IgM gammopathy, and two of the following minor criteria: recurrent fever, objective signs of abnormal bone remodeling, elevated CRP level or leukocytosis, and a neutrophilic infiltrate on skin biopsy. It is considered probable, if only 1 minor criterion is present. In patients with monoclonal IgG gammopathies, diagnosis is definite if three minor criteria are present and possible if two are present. First‐line treatment in patients with significant alteration of quality of life or persistent elevation of markers of inflammation should be anakinra. Follow‐up should include clinical evaluation, CBC and CRP every 3 months and MGUS as usually recommended.

Intra-articular osteoid osteoma (OO) is a rare and difficult diagnosis. We report the case of an 11-year-old boy who presented with inflammatory monoarticular arthritis in the left elbow. This monoarthritis was resistant to all types of treatment. He had an OO, diagnosed late because the first symptoms developed subsequent to the ablation of a wart in the same elbow and were suggestive of arthritis.
Le tocilizumab (TCZ) est un anticorps anti-récepteur de l’IL-6 humanisé, indiqué dans le traitement de la polyarthrite rhumatoïde (PR) active, modérée à sévère, la maladie de Still de l’adulte, la ...maladie de Castleman et, de façon plus récente, l’arthrite juvénile idiopathique dans sa forme systémique. De même que les anti-TNFα, le rituximab et plus rarement l’abatacept, le TCZ est capable d’induire des réactions cutanées paradoxales de type psoriasique essentiellement à type de pustulose palmo-plantaire.
Nous rapportons l’observation d’une patiente suivie pour un rhumatisme psoriasique qui développait sous anti-TNFα puis sous tocilizumab une éruption psoriasiforme palmo-plantaire.
Le mécanisme de ces réactions reste imparfaitement compris, mais des réactions croisées avec d’autres biothérapies sont observées.
Tocilizumab (TCZ) is a humanized antihuman interleukin (IL)-6 receptor antibody recommended for the treatment of moderate to severe active rheumatoid arthritis, adult-onset Still disease, Castleman disease and more recently, systemic juvenile idiopathic arthritis. Like anti-TNFα, rituximab and less frequently abatacept, TCZ can induce paradoxical cutaneous eruption like psoriasis with predominantly palmoplantar presentation.
We report a 47-year-old woman with psoriastic arthritis who developed under anti-TNFα therapy and later under tocilizumab a paradoxical palmoplantar eruption.
The specific underlying mechanisms of this side effect are unclear but relapse of these lesions seems to be observed with certain biological agents.
Abstract Objective To report the efficacy and safety of anti-TNF agents in patients with severe and/or refractory manifestations of Behçet's disease (BD). Methods We performed a multicenter study of ...main characteristics and outcomes of anti-TNF alpha treatments mainly infliximab (62%), and adalimumab (30%) in 124 BD patients 48% of men; median age of 33.5 (28–40) years. Results Overall response (i.e. complete and partial) rate was 90.4%. Clinical responses were observed in 96.3%, 88%, 70%, 77.8%, 92.3% and 66.7% of patients with severe and/or refractory ocular, mucocutaneous, joint, gastro-intestinal manifestations, central nervous system manifestations and cardiovascular manifestations, respectively. No significant difference was found with respect to the efficacy of anti-TNF used as monotherapy or in association with an immunosuppressive agent. The incidence of BD flares/patient/year was significantly lower during anti-TNF treatment (0.2 ± 0.5 vs 1.7 ± 2.4 before the use of anti-TNF, p < 0.0001). The prednisone dose was significantly reduced at 6 and 12 months (p < 0.0001). In multivariate analysis, retinal vasculitis was negatively associated with complete response to anti-TNF (OR = 0.33 0.12–0.89; p = 0.03). The efficacy and relapse free survival were similar regardless of the type of anti-TNF agent used. After a median follow-up of 21 7–36 months, side effects were reported in 28% of patients, including infections (16.3%) and hypersensitivity reactions (4.1%). Serious adverse events were reported in 13% of cases. Conclusion Anti-TNF alpha therapy is efficient in all severe and refractory BD manifestations. Efficacy appears to be similar regardless of the anti-TNF agent used (infliximab or adalimumab).
Les auto-anticorps antiphospholipides sont des anticorps hétérogènes qui reconnaissent le plus souvent des complexes composés de protéines et de phospholipides anioniques. La nature de ces complexes ...est aujourd’hui mieux connue, de même que la structure des anticorps antiphospholipides grâce à l’analyse d’anticorps monoclonaux permettant de définir leurs spécificités précises et leurs réactivités croisées. En revanche, l’origine de ces auto-anticorps reste mal comprise, de même que le lien éventuel entre les antiphospholipides observés chez le sujet normal et ceux retrouvés en pathologie auto-immune. Seuls certains antiphospholipides sont pathogènes et directement responsables des manifestations cliniques du syndrome des antiphospholipides, mais il n’existe pas aujourd’hui de test permettant de rechercher précisément et uniquement les antiphospholipides pathogènes. Les mécanismes qui lient ces auto-anticorps à l’apparition des symptômes, en particulier obstétricaux, sont mieux compris dans leur diversité et suggèrent de nouvelles voies thérapeutiques.
Antiphospholipid antibodies constitute a group of heterogeneous antibodies, which mainly recognize complexes made of proteins and anionic phospholipids. The nature of these complexes is currently better defined, as well as known, the structure of the antiphospholipid antibodies owing to the analysis of the monoclonal forms of these antibodies which were also studied both in terms of their precise specificities and cross-reactivity. However, the origin of these autoantibodies is not clearly understood, as well as the possible link between antiphospholipid antibodies present in healthy individuals, and those observed in autoimmune diseases. Only a fraction of antiphospholipid antibodies are pathogenic and directly responsible for the clinical manifestations of the antiphospholipid syndrome, but there is, to date, no biological test able to accurately detect pathogenic antiphospholipid antibodies. The diverse mechanisms which link these autoantibodies to the occurrence of symptoms, mainly during obstetrical complications, are better understood, and suggest new therapeutic avenues.
Le Lupus érythémateux systémique (LES) est une maladie auto-immune rare et chronique. L’activité de la maladie est marquée par des phases de rémissions et de rechutes spontanées ou induites par des ...modifications thérapeutiques. Le LES expose à des complications graves nécessitant un suivi médical rapproché. Cependant, les poussées de la maladie sont imprévisibles et ne coïncident pas toujours avec la fréquence programmée du suivi médical. L’activité du LES est mesurée à l’aide du questionnaire « Systemic Lupus Érythematosus Disease Activity Index » (SLEDAI-2K), mais ce score médical ne peut pas être complété par les patients. L’enjeu est donc de développer des outils permettant aux patients de mesurer l’activité de leur maladie, de détecter les poussées et, par la suite, d’adapter leur suivi médical. En utilisant le même cadre qu’une précédente étude nationale liée à l’impact du COVID-19 (enquête EPICURE), nous visons à développer et valider, en collaboration avec les Centres de référence et de compétence du lupus et la Société Hometrix Health, un outil adapté au patient nommé « SLEDAI-P/LUPIN ». Afin de valider le SLEDAI-P/LUPIN, nous souhaitons recruter 500 patients répondants aux critères de classification ACR/EULAR 2019 du LES. Tous les patients participant à l’étude rempliront l’autoquestionnaire et auront une consultation de suivi avec leur spécialiste qui remplira le SLEDAI-2K clinique dans un délai maximal de 7jours. La validité de l’autoquestionnaire patient sera évaluée en calculant la corrélation entre le SLEDAI-P/LUPIN et le SLEDAI-2K complété par le clinicien (en aveugle des résultats du SLEDAI-P/LUPIN). Nous collecterons également des informations sur la qualité de vie via le questionnaire SF-36 pour évaluer si le SLEDAI-P/LUPIN reflète les données rapportées par les patients. Les résultats de cette enquête permettront de valider le SLEDAI-P/LUPIN en tant qu’autoquestionnaire porté par le patient. Le développement de questionnaires d’activité de la maladie auto-administrés tels que le SLEDAI-P/LUPIN peut permettre d’optimiser le suivi des patients atteints de LES, et d’/de autonomiser/responsabiliser les patients atteints de LES pour le contrôle et la gestion de leur maladie au quotidien.
Several studies have suggested that obesity could have a negative effect on response to anti-tumor necrosis factor α (anti-TNFα) in rheumatoid arthritis (RA). Little is known about the impact of body ...mass index (BMI) on other biologic agents. We aimed to evaluate the effect of BMI on response to tocilizumab (TCZ) in RA. RA patients treated with TCZ were included in this multicenter retrospective study. BMI was calculated at the initiation of treatment. After 6 months of treatment, change from baseline in DAS28, pain on a visual analog scale, erythrocyte sedimentation rate and C-reactive protein level, and tender and swollen joints were analyzed. The primary endpoint was decrease in DAS28 ≥ 1.2. Secondary outcomes were good response and remission by EULAR criteria. At baseline, among 115 RA patients included, the median (interquartile range) BMI was 25.4 (22.0–28.8) kg/m

. The number of patients with normal weight, overweight, and obesity was 53 (46 %), 37 (32 %), and 25 (22 %), respectively. Baseline characteristics did not differ between the three subgroups of BMI. Background: B cell activation may result in an increased secretion of immunoglobulin free light chains (FLCs) in autoimmune diseases. Objective: To analyse serum FLC levels in patients with ...rheumatoid arthritis and in those with primary Sjögren’s syndrome (pSS). Patients and methods: Blood samples were collected from 80 healthy blood donors, 50 patients with rheumatoid arthritis and 139 patients with pSS. Serum FLC level was measured using a new quantitative immunoassay. Results: Mean (standard error (SE)) serum κ and λ FLC levels were significantly higher in patients with rheumatoid arthritis and in those with pSS than in controls (κ : 18.9 (1.1) and 16.3 (1.4) v 10.5 (0.4) mg/l, p<0.001 and p = 0.001, respectively; λ: 16.7 (1.2) and 19.3 (1.5) v 11.6 (0.6) mg/l, p<0.001 for both). 18 (36%) patients with rheumatoid arthritis and 31 (22.3%) patients with pSS had abnormal serum FLC levels (increased κ or λ levels and abnormal ratio of κ:λ). Serum κ and λ levels were correlated with other B cell activation markers in both diseases. FLC levels increased with disease activity, because, unlike total gammaglobulin and immunoglobulin G levels, they were significantly correlated with Disease Activity Score 28 in patients with rheumatoid arthritis (p = 0.004 for κ, p = 0.05 for λ) and with extraglandular involvement in pSS (p = 0.01 for κ, p = 0.04 for λ). Conclusion: FLC levels are increased and correlate with disease activity in patients with rheumatoid arthritis and in those with pSS, two diseases in which increased risk of lymphoma could result from persistent B cell activation and disease activity. Further studies are required to determine whether FLC assessment could represent a relevant biomarker for response to treatment (especially B cell depletion) and for the risk of lymphoma in autoimmune diseases.
