Introduction-Objectif: L'émergence de la tuberculose multirésistante (MDR-TB) est une
menace pour la santé publique mondiale. Le but de notre étude était de déterminer
les facteurs de risque d'échec ...du traitement de la TB-MR.Méthodes: Étude
rétrospective menée entre janvier 2000 et mars 2019 incluant des patients atteints
de TB-MR. Les caractéristiques des patients en échec thérapeutique ont été comparées
à celles des patients guéris. Une analyse de régression logistique a été utilisée
pour identifier les facteurs de risque d'échec du traitement.Résultats: Notre étude
a inclus 140 patients âgés de 42±13 ans (18-80). Cinquante-sept pour cent des
patients ont eu un succès thérapeutique et 12 % ont eu un échec thérapeutique. En
analyse de régression logistique multivariée, l'échec du traitement était associé à
l'âge supérieur à 45 ans (OR=1,05 ;IC95%1,024-7,736;p=0,014), au niveau
d'éducation primaire et à l'analphabétisme (OR=5,022 ;IC95%1,316-19,161;p=0,018),
antécédents d'incarcération (OR=3,291 ; IC95%1,291-21,083;p=0,016), dénutrition
(OR=4,544 ;IC95%2,304-54,231;p=0,027), TB étendue (OR=6,406 ; IC95%1,761-23,922;
p=0,038) , frottis initiaux positifs de haut grade (OR=1,210 ;IC95%1,187-32,657;
p=0,045), culture de frottis positive à 90 jours de traitement (OR=6,871 ;
IC95%3,824-23,541; p=0,003), mauvaise observance (OR=6,110 ; IC95%2,740-12,450;
p=0,021) et la survenue d'événements indésirables psychiatriques (OR=3,644 ;
IC95%2,560-27,268; p=0,041).Conclusion: Une éducation thérapeutique, un soutien
nutritionnel et psychologique et un suivi rapproché sont fortement recommandés pour
optimiser le pronostic de la TB-MR.

The main objective of this paper is to propose a new approach of European multilateral assistance to Central and Eastern European Countries (CEECs) that joined the European Union (EU) based on the ...prism of International political economy (IPE). The first part defines the notion of Aid, explains why IPE is pertinent and shows how it could be adapted here. The second part then focuses on the European multilateral assistance to the CEECs showing that it forms a series of institutional arrangements to properties comparable to those of a regime, concluded by actors with differentiated power, knowledge and interest and which effects are very different from those predicted.

A new approach via the international political economy is proposed for interpreting European multilateral assistance to the central and eastern European countries that joined the European Union. After defining aid, an argument is presented for applying the concept of an international political economy, even though methodological caveats must be formulated when using it to analyze intra-European aid. This European multilateral assistance stems from a series of institutional arrangements resembling a system. These arrangements between players with different powers, knowledge and interests have had effects quite different from what was predicted.
The wind energy industry continuously demands significant improvements in wind turbine maintenance strategies through the use of condition monitoring systems (CMS). Regardless of their design, wind ...turbines have rotating parts that need to be monitored during operation in order to avoid
unpredicted failures. In this paper, an experiment conducted at the University of Newcastle is described and initial data analysis is presented. The experiment was performed in a gear test-rig of the Design Unit department, where gearbox contact fatigue tests are frequently performed. The
testing procedure involves running of the gears to destruction; starting with 'fresh' gears and operating for a period of time until they fail. In this experiment, the gear being monitored is periodically checked to give an indication of the tooth's cross-section loss. A combination of vibration
analysis and acoustic emission (AE) analysis is utilised in this experiment as it is believed that, for reliable diagnosis of rotating machinery, multi-sensing technology should be used. Accelerometers and acoustic emission sensors were deployed throughout the tests, which were conducted in
five stages over seven weeks. For the vibration data processing and calculation of the data, the root mean square (RMS) and crest factor values are calculated and frequency spectrum analysis is performed. For the acoustic emission data processing, RMS and energy parameters are calculated.
These parameters are shown giving information about the potential gear deterioration over time. Finally, the results of the experiment are presented and show minimal changes in the vibration and acoustic emission parameters in the first two stages of the tests, indicating a reliable baseline
reading. However, in the later stages of the tests, data showed a very clear indication of possible gear deterioration, with an increase in the values of RMS for both acoustic emission and vibration data and a modal shift in the vibration spectrum. Visual inspection performed afterwards confirmed
the onset of severe macro-pitting failures in the gears.
The optimal transmit power of a sensor node while satisfying different rate constraints is derived. First, an optimization problem with an instantaneous transmission rate constraint is addressed. ...Next, the optimal power is analyzed, but now with an average transmission rate constraint. The optimal solution for a class of fading channels, in terms of system parameters, is presented and a suboptimal solution is also proposed for an easier, yet efficient, implementation. Insightful asymptotical analysis for both schemes, considering a Rayleigh fading channel, are shown. Finally, the optimal power allocation for a sensor node in a cognitive radio environment is analyzed where an optimum solution for a class of fading channels is again derived. In all cases, numerical results are provided for either Rayleigh or Nakagami-m fading channels.
Introduction. The purpose of this study is to investigate chest pain evaluations after initial coronary computed tomography angiography (CCTA) based upon coronary artery disease (CAD) burden. ...Methods. CCTA results of 1,518 patients were grouped based on the CCTA results into no CAD, nonobstructive CAD (<50% maximal diameter stenosis), or obstructive CAD (≥50% stenosis). Chest pain evaluation after initial CCTA and rates of major adverse cardiovascular events (MACE) defined as the incidence of all-cause mortality, nonfatal MI, ischemic stroke, and late revascularization (>90 days following CCTA) were evaluated. Results. MACE rates were higher with obstructive CAD compared to nonobstructive CAD and no CAD (8.9% versus 0.7%, P<0.001; 8.9 versus 1.6%, P<0.001). One hundred seventy-four patients (11.5%) underwent evaluation for chest pain after index CCTA with rates significantly higher with obstructive CAD compared to both nonobstructive CAD and no CAD (7.5% versus 13.9% versus 17.8%, P<0.001). The incidence of repeat testing was more frequent in patients with obstructive CAD (no CAD 36.5% versus nonobstructive CAD 54.9% versus obstructive CAD 67.7%, P=0.015). Conclusion. Absence of obstructive disease on CCTA is associated with lower rates of subsequent evaluations for chest pain and repeat testing with low MACE event rates over a 22-month followup.
