Notions of identity engendered from the 20th century Latin American & Caribbean Feminist Encuentros are studied to illustrate how such identities are amalgamations of multiple national & ...transnational feminist identities & discursive practices. The principal areas of debate within the first seven Encuentros are identified: (1) the reconsideration of autonomy; (2) the relationship between various feminisms & other movements & institutions; (3) the mediation of tensions involving competing understandings of inclusion & exclusion within the context of the feminist movement; & (4) the negotiation of difference, inequality, & power relations among different groups of women. Attention is then dedicated to analyzing the various topics addressed at the eighth Encuentro in Juan Dolio, Dominican Republic, including the formation of a young feminist generation that has problematized previous generations' debates about diversity & pluralism & the increased realization of globalization's & neoliberalism's affects upon the global women's movement. The significance of the Encuentros for comprehending sundry national & international feminist movements is also considered. 82 References. J. W. Parker
The COVID-19 pandemic significates an enormous number of patients with pneumonia that get complicated with severe acute respiratory distress syndrome (ARDS), some of them with refractory hypercapnia ...and hypoxemia that need mechanical ventilation (MV). Those patients who are not candidate to extracorporeal membrane oxygenation (ECMO), the extracorporeal removal of CO
(ECCO
R) can allow ultra protective MV to limit the transpulmonary pressures and avoid ventilatory induced lung injury (VILI). We report a first case of prolonged ECCO
R support in 38 year male with severe COVID-19 pneumonia refractory to conventional support. He was admitted tachypneic and oxygen saturation 71% without supplementary oxygen. The patient's clinical condition worsens with severe respiratory failure, increasing the oxygen requirement and initiating MV in the prone position. After 21 days of protective MV, PaCO
rise to 96.8 mmHg, making it necessary to connect to an ECCO
R system coupled continuous veno-venous hemodialysis (CVVHD). However, due to the lack of availability of equipment in the context of the pandemic, a pediatric gas exchange membrane adapted to CVVHD allowed to maintain the removal of CO
until completing 27 days, being finally disconnected from the system without complications and with a satisfactory evolution.

A pesar de ser una referencia internacional en donación y trasplante, España precisa mejorar los procesos de donación en niños, en particular la donación tras la determinación de la muerte por ...criterios circulatorios (donación en asistolia). El presente artículo, resumen del documento de consenso elaborado por la Organización Nacional de Trasplantes y la Asociación Española de Pediatría, pretende facilitar los procesos de donación en niños y neonatos y analizar los conflictos éticos que plantea. Se comentan los fundamentos éticos de la donación pediátrica, los principios de la evaluación clínica de los posibles donantes, los criterios diagnósticos de muerte encefálica en niños, los cuidados intensivos para el mantenimiento de los donantes, los conceptos básicos de la donación en asistolia y los procesos de donación en neonatos con malformaciones muy graves del sistema nervioso incompatibles con la vida y en niños en cuidados paliativos. Considerar sistemáticamente la donación de órganos y tejidos cuando un niño fallece en condiciones de ser donante es un imperativo ético y ha de constituir un estándar profesional, tanto por la necesidad de órganos para trasplante, como por asegurar un cuidado integral centrado en la familia.
Despite being an international reference in donation and transplantation, Spain needs to improve pediatric donation, including donation after the circulatory determination of death. The present article, a summary of the consensus report prepared by the Organización Nacional de Trasplantes and the Spanish Pediatrics Association, intends the facilitation of donation procedures in newborns and children and the analysis of associated ethical dilemma. The ethical basis for donation in children, the principles of clinical assessment of possible donors, the criteria for the determination of death in children, intensive care management of donors, basic concepts of donation after the circulatory determination of death and the procedures for donation in newborns with severe nervous system's malformation incompatible with life, as well as in children receiving palliative care are commented. Systematically considering the donation of organs and tissues when a child dies in conditions consistent with donation is an ethical imperative and must become an ethical standard, not only because of the need of organs for transplantation, but also to ensure family centered care.
The electronics of the CMS Drift Tube (DT) chambers will need to be replaced in order to cope with the increased occupancy and rates at the high luminosity phase in the HL-LHC (High-Luminosity Large ...Hadron Collider). A new electronic system is being designed, that will allow forwarding signals from all DT chambers at the maximum resolution to the backend system, where improved trigger primitives generation will take place. The on-detector boards, which will be attached to the DT chambers inside the CMS wheels are called OBDT (On Board electronics for Drift Tubes) and are built around a Microsemi Polarfire FPGA that performs the time digitization of the chamber signals at ~1 ns resolution, and forwards them to a high-speed link, optimizing bandwidth and latency. Thirteen prototypes of this board have been installed in parallel to the present system in the CMS detector, fully instrumenting one out of sixty sectors, and are taking data routinely integrated in the CMS DAQ chain. The firmware implementation of this FPGA performs the time digitization of up to 240 input channels, the high-speed link implementation and several slow control functionalities, including a very thorough clock and reset signals distribution that allows ensuring that the time measurements are stable across power cycles and reconfigurations of all the CMS clock chain. The description and implementation details, together with the results of the performance of this firmware in the data taking campaigns at CMS are presented in this contribution.

During the High Luminosity LHC, the Drift Tube chambers installed in the CMS detector need to operate with an integrated dose ten times higher than expected at the LHC due to the increase in ...integrated luminosity from 300 fb-1 to 3000 fb-1. Irradiations have been performed to assess the performance of the detector under such conditions and to characterize the radiation aging of the detector. The presented analysis focuses on the behaviour of the high voltage currents and the dose measurements needed to extrapolate the results to High Luminosity conditions, using data from the photon irradiation campaign at GIF++ in 2016 as well as the efficiency analysis from the irradiation campaign started in 2017. Although the single-wire loss of high voltage gain observed of 70% is very high, the muon reconstruction efficiency is expected to decrease less than 20% during the full duration of High Luminosity LHC in the areas under highest irradiation.

To evaluate the efficacy and safety of percutaneous tracheostomy by means of single-step dilation with fiber optic bronchoscopy assistance in critical care patients under mechanical ventilation.
...Between the years 2004 and 2014, 512 patients with indication of tracheostomy according to clinical criteria, were prospectively and consecutively included in our study. One-third of them were high-risk patients. Demographic variables, APACHE II score, and days on mechanical ventilation prior to percutaneous tracheostomy were recorded. The efficacy of the procedure was evaluated according to an execution success rate and based on the necessity of switching to an open surgical technique. Safety was evaluated according to post-operative and operative complication rates.
The mean age of the group was 64 ± 18 years (203 women and 309 males). The mean APACHE II score was 21 ± 3. Patients remained an average of 11 ± 3 days on mechanical ventilation before percutaneous tracheostomy was performed. All procedures were successfully completed without the need to switch to an open surgical technique. Eighteen patients (3.5%) presented procedure complications. Five patients experienced transient desaturation, 4 presented low blood pressure related to sedation, and 9 presented minor bleeding, but none required a transfusion. No serious complications or deaths associated with the procedure were recorded. Eleven patients (2.1%) presented post-operative complications. Seven presented minor and transitory bleeding of the percutaneous tracheostomy stoma, 2 suffered displacement of the tracheostomy cannula, and 2 developed a superficial infection of the stoma.
Percutaneous tracheostomy using the single-step dilation technique with fiber optic bronchoscopy assistance seems to be effective and safe in critically ill patients under mechanical ventilation when performed by experienced intensive care specialists using a standardized procedure.

Basta con un vistazo a algunos de nuestros más difundidos manuales de Historia Moderna de Europa para percibir que hasta que no incorporemos el mundo colonial ibérico seguiremos ofreciendo un relato ...deficiente. ¿Acaso se puede entender el apogeo comercial de Holanda en el siglo xvii obviando los hallazgos en materia náutica realizados por los exploradores españoles y portugueses años antes? Los artículos de este dossier pretenden responder a este interrogante, y lo hacen desde tres niveles de aproximación: el que proporciona el estudio de casos poco o nada conocidos; el que analiza modelos de colonización preconcebidos con un claro fin de legitimación de otras estructuras políticas; y el que establece una comparación con experiencias semejantes a las realizadas en otros territorios. Il suffit de regarder certains de nos manuels les plus répandus d’histoire moderne de l’Europe pour se rendre compte que tant que nous n’y incorporerons pas le monde colonial ibérique, nous continuerons à offrir un récit déficient. Ainsi, pouvons-nous comprendre l’apogée commerciale de la Hollande au xviie siècle, sans prendre en compte les découvertes nautiques faites par les explorateurs espagnols et portugais les années précédentes ? Les articles de ce dossier tentent de répondre à cette question, et ce à partir de trois niveaux d’approche : celui fourni par l’étude de cas peu ou pas connus ; celui qui analyse des modèles préconçus de colonisation dans le but clair de légitimer d’autres structures politiques ; et celui qui établit une comparaison avec des expériences similaires à celles menées dans d’autres territoires. If we look at some of our most widespread textbooks on modern European history, we will realize that as long as we do not incorporate the Iberian colonial world into them, we will continue to offer a deficient narrative. Thus, can we understand the commercial apogee of Holland in the 17th century, without taking into account the nautical discoveries made by Spanish and Portuguese explorers in previous years? The articles of this dossier attempt to answer this question, based on three levels of approach: the one provided by case studies that are little or unknown; the one that analyses preconceived models of colonization with the clear aim of legitimizing other political structures; and the one that makes a comparison with experiences similar to those conducted in other territories.
