To sustain and extend its discovery potential, the Large Hadron Collider (LHC) will undergo a major upgrade in the coming years, referred to as High Luminosity LHC (HLLHC), aimed to increase its ...instantaneous luminosity, 5 times larger than the designed limit, and, consequently leading to high levels of radiation, with the goal to collect 10 times larger the original designed integrated luminosity. The drift tube chambers (DT) of CMS muon detector system is built to proficiently measure and trigger on muons in the harsh radiation environment expected during the HL-LHC era. Ageing studies are performed at the CERNs gamma ray irradiation facility (GIF++) by measuring the muon hit efficiency of these detectors at various LHC operation conditions. One such irradiation campaign was started in October 2017, when a spare MB2 chamber moved inside the bunker and irradiated at lower acceleration factors. Two out of twelve layers of the DT chamber were operated while being irradiated with the radioactive source and then their muon hit efficiency was calculated in coincidence with other ten layers which were kept on the standby. The chamber absorbed an integrated dose equivalent to two times the expected integrated luminosity of the HL-LHC. Investigation on the outgassing of cell materials and of the gas components used at the GIF++ are underway and strategies to mitigate the aging effects are also being developed. The effect of radiation on the performance of DT chamber and its impact on the overall muon reconstruction efficiency expected during the HL-LHC are presented.
Objectives The purpose of this study was to compare sulfur hexafluoride microbubble (SonoVue)-enhanced myocardial contrast echocardiography (MCE) with single-photon emission computed tomography ...(SPECT) relative to coronary angiography (CA) for assessment of coronary artery disease (CAD). Background Small-scale studies have shown that myocardial perfusion assessed by SonoVue-enhanced MCE is a viable alternative to SPECT for CAD assessment. However, large multicenter studies are lacking. Methods Patients referred for myocardial ischemia testing at 34 centers underwent rest/vasodilator SonoVue-enhanced flash-replenishment MCE, standard99m Tc-labeled electrocardiography-gated SPECT, and quantitative CA within 1 month. Myocardial ischemia assessments by 3 independent, blinded readers for MCE and 3 readers for SPECT were collapsed into 1 diagnosis per patient per technique and were compared to CA (reference standard) read by 1 independent blinded reader. Results Of 628 enrolled patients who received SonoVue (71% males; mean age: 64 years; >1 cardiovascular CV risk factor in 99% of patients) 516 patients underwent all 3 examinations, of whom 161 (31.2%) had ≥70% stenosis (131 had single-vessel disease SVD; 30 had multivessel disease), and 310 (60.1%) had ≥50% stenosis. Higher sensitivity was obtained with MCE than with SPECT (75.2% vs. 49.1%, respectively; p < 0.0001), although specificity was lower (52.4% vs. 80.6%, respectively; p < 0.0001) for ≥70% stenosis. Similar findings were obtained for patients with ≥50% stenosis. Sensitivity levels for detection of SVD and proximal disease for ≥70% stenosis were higher for MCE (72.5% vs. 42.7%, respectively; p < 0.0001; 80% vs. 58%, respectively; p = 0.005, respectively). Conclusions SonoVue-enhanced MCE demonstrated superior sensitivity but lower specificity for detection of CAD compared to SPECT in a population with a high incidence of CV risk factors and intermediate-high prevalence of CAD. This study compared the cost-effectiveness of dobutamine-atropine stress echocardiography (DASE) and electrocardiographic exercise testing (EET) implemented in emergency department accelerated ...diagnostic protocols for the early stratification of low-risk patients presenting with acute chest pain (ACP). One hundred ninety-nine patients with ACP, nondiagnostic electrocardiographic results, and negative biomarker results were randomized to DASE (n = 110) or EET (n = 89) <6 hours after emergency department presentation. Patients with negative risk assessment results were immediately discharged and followed for 2 months. Ninety patients (82%) in the DASE arm and 78 (88%) in the EET arm were discharged after the diagnosis of nonischemic ACP. The mean lengths of stay in the hospital were 23 ± 12 and 31 ± 23 hours in the DASE and EET arms, respectively (p = 0.01). No 2-month follow-up events occurred in DASE patients, and the event rate was significantly higher in EET patients (0% vs 11%, p = 0.004). The DASE strategy showed lower costs compared with the EET strategy at 1-month ($1,026 ± $250 vs $1,329 ± $1,288, p = 0.03) and 2-month ($1,029 ± 253 vs $1,684 ± $2,149, p = 0.005) follow-up. In conclusion, early DASE in emergency department triage of low-risk patients with ACP is safe and reduces costs of care compared to EET.
The CMS drift tubes (DT) muon detector, built for withstanding the LHC expected integrated and instantaneous luminosities, will be used also in the High Luminosity LHC (HL-LHC) at a 5 times larger ...instantaneous luminosity and, consequently, much higher levels of radiation, reaching about 10 times the LHC integrated luminosity. Initial irradiation tests of a spare DT chamber at the CERN gamma irradiation facility (GIF++), at large (\(\sim\)O(100)) acceleration factor, showed ageing effects resulting in a degradation of the DT cell performance. However, full CMS simulations have shown almost no impact in the muon reconstruction efficiency over the full barrel acceptance and for the full integrated luminosity. A second spare DT chamber was moved inside the GIF++ bunker in October 2017. The chamber was being irradiated at lower acceleration factors, and only 2 out of the 12 layers of the chamber were switched at working voltage when the radioactive source was active, being the other layers in standby. In this way the other non-aged layers are used as reference and as a precise and unbiased telescope of muon tracks for the efficiency computation of the aged layers of the chamber, when set at working voltage for measurements. An integrated dose equivalent to two times the expected integrated luminosity of the HL-LHC run has been absorbed by this second spare DT chamber and the final impact on the muon reconstruction efficiency is under study. Direct inspection of some extracted aged anode wires presented a melted resistive deposition of materials. Investigation on the outgassing of cell materials and of the gas components used at the GIF++ are underway. Strategies to mitigate the ageing effects are also being developed. From the long irradiation measurements of the second spare DT chamber, the effects of radiation in the performance of the DTs expected during the HL-LHC run will be presented.
This study assessed the effects on quality of life (QoL) of dobutamine-atropine stress echocardiography (DASE) and electrocardiogram exercise testing (EET) accelerated diagnostic protocols for early ...stratification of low-risk patients with acute chest pain (ACP). A total of 290 patients with ACP, a nondiagnostic electrocardiogram, and negative biomarkers were randomly assigned to an accelerated diagnostic protocol (DASE, n = 110, or EET, n = 89) or usual care (n = 91) and followed up for 2 months. QoL was assessed at discharge and 2-month follow-up using the Nottingham Health Profile questionnaire. Baseline and 2-month follow-up answers to the Nottingham Health Profile questionnaire were available for 207 patients (71%; 55 in the usual-care, 77 in the DASE, and 75 in the ETT arm). At predischarge, patients in the usual-care arm reported higher impairment in the physical mobility and pain dimensions compared with the DASE and EET arms (p = 0.019 and p = 0.023, respectively). At 2-month follow-up, QoL improved in all groups; however, patients in the usual-care arm had significantly worse scores than patients managed using accelerated diagnostic protocols in the physical mobility, pain, social isolation, emotional reactions, and energy level dimensions (p = 0.014, p = 0.002, p = 0.04, p = 0.01, and p = 0.003, respectively). In conclusion, low-risk patients with ACP had non-negligible impairment of QoL in the acute phase. Emergency department ADPs with early DASE and EET reduced QoL impairment at both baseline and 2-month follow-up.
Due to their importance for the control of meat quality in broiler chickens, the present study aimed at identifying the factors associated with the occurrence of myopathies and characterizing the ...meat properties when affected by myopathies. To this aim, a total of 768 broiler chickens were reared until slaughter (46 d) to evaluate the effect of genotype, gender, and feeding regime (ad libitum vs. restricted rate, 80% from 13 to 21 d of age) on performance and meat quality. Standard broilers were heavier (3,270 vs. 3,139 g; P < 0.001) and showed lower feed conversion (1.56 vs. 1.61; P < 0.001) than the high-yield broilers. Males showed higher final live weight (3,492 vs. 2,845 g) and lower feed conversion (1.54 vs. 1.63) than females (P < 0.001). Feed restriction decreased final live weight (3,194 vs. 3,142 g; P < 0.01) and feed conversion (1.60 vs. 1.57; P < 0.01) compared to ad libitum feeding. At gross examination, feed restriction tended to increase white-striped breasts (69.5 vs. 79.5%; P < 0.10), whereas females showed less wooden breasts than males (8.0 vs. 16.3%; P < 0.05). White-striped fillets had higher pHu (5.87 vs. 5.83), and lower a* (−0.81 vs. −0.59) and b* color indexes (13.7 vs. 14.5) (P < 0.05), whereas wooden breast fillets exhibited higher cooking losses (25.6 vs. 22.1%) and AK-shear force (4.23 vs. 2.84 kg/g) compared with normal fillets (P < 0.001). At histological examination, 3.1% of pectoralis major were normal, 26.6% mildly degenerated, 45.3% moderately degenerated, and 25.0% severely degenerated. In conclusion, genotype had a moderate effect on growth without modifying myopathy occurrence. In contrast, gender and feed restriction affected performance, meat quality, and breast abnormalities.

To evaluate muscle fiber degeneration (MFD) associated with white striping and wooden breast, pectoralis major of 192 broilers differing for genotype (standard vs. high breast yield), gender, and ...feeding regime (ad libitum vs. restricted rate 80% from 13 to 21 d of age) were sampled at 14, 21, 28, 35, and 46 d of age for histological analyses by hematoxylin and eosin (H&E) staining to evaluate tissue morphology, Masson's trichrome to identify collagen presence, and Oil red and Nile blue for lipid presence. Microvessels (diameter ≤15 μm), nuclei positive to anti-cleaved lamin A and monoclonal proliferating cell nuclear antigen (PCNA) antisera were counted to assess apoptotic and regenerative processes, respectively. Significant differences were found according to feeding system, age, and their interactions. The frequency of chickens with MFD was higher with ad libitum than restricted feeding (75.0% vs. 62.5%; P = 0.01) and increased with age (18.8%, 28.1%, 75.1%, 96.9%, and 96.9% at 14, 21, 28, 35, and 46 d). However, at 14 d a similar frequency (18.8%) was found in all broilers; at 21 d, MFD occurred more in broilers fed ad libitum than in those under restriction (50.0% vs. 6.3%; P < 0.01); at 28 d differences were reduced (87.5% vs. 62.5%; P = 0.10) to disappear by 35 (100% and 93.8%) and 46 d (96.9% and 96.9%). The number of microvessels decreased with age (20.7 to 9.46; P < 0.001) and the number of nuclei positive to the anti-cleaved lamin A antibody increased. At histology, MFD at 46 d corresponded to loss of typical cross striations, massive necrotic process, degenerating fibers surrounded by inflammatory cells, scattered fibers in an abundant collagen-rich connective tissue, numerous adipose cells; necrotic fibers showed a high percentage of apoptotic nuclei, and regenerating fibers appeared positive to anti-PCNA antibody. In conclusion, MFD soon occurred after 2 wk of growth and increased dramatically within 28 d. Early feed restriction reduced MFD as long as animals were restricted, but no residual effect was recorded after re-alimentation.
