Le cancer pancréatique, pour lequel le rapport incidence/décès est de 0,99, est à ce jour le cancer dont le pronostic est le plus sombre. Les facteurs de risque associés à la forme sporadique de ...l’adénocarcinome pancréatique sont très mal connus et seuls 5 à 10 % des patients atteints sont en mesure de recevoir un traitement curatif (chirurgie associée à la radiothérapie ou chimiothérapie périopératoire) avec un taux de survie faible à cinq ans (10 à 20 % selon les séries). En effet, plus de 90 % des patients présentent au moment du diagnostic une tumeur inextirpable ou des métastases. Il est donc impératif de mieux comprendre l’étiologie du cancer pancréatique afin d’identifier de nouveaux marqueurs de pronostic et de nouvelles cibles thérapeutiques. De nombreuses équipes se sont intéressées à l’identification des altérations génétiques associées au cancer pancréatique et au rôle de ces altérations dans son développement. Cette revue fait le point des connaissances actuelles sur les anomalies génétiques associées aux deux lésions pancréatiques à potentiel malin que sont les néoplasies intraépithéliales pancréatiques (PanIN) et les tumeurs intracanalaires papillaires et mucineuses du pancréas (TIPMP). La plupart des anomalies génétiques détectées sont communes à ces deux situations précancéreuses et diffèrent peu de celles observées dans les adénocarcinomes pancréatiques. Certaines d’entre elles pourraient être la base de cibles thérapeutiques permettant d’éviter la transformation maligne.
Pancreatic adenocarcinoma, with an incidence/death ratio of 0.99, has the worst prognosis of all cancers. Risk factors associated with the sporadic form of pancreatic adenocarcinoma are unknown and curative treatment (surgery associated with radiotherapy or chemotherapy) is possible in less than 10% with a low 5-year survival rate (10–20%). The tumor discovered at diagnosis is not resectable or has already produced metastases in more than 90% of patients. Thus, a better understanding of the etiology of pancreatic cancer is paramount to identify new prognostic markers and new therapeutic targets. There are numerous data on the identification of genetic alterations associated with pancreatic cancer and their role in the development of cancer. This review focuses on existing knowledge of genetic alterations associated with two pancreatic lesions that can potentially evolve into pancreatic adenocarcinoma, Pancreatic Intraepithelial Neoplasias (PanIN) and Intraductal Papillary Mucinous Neoplasms (IPMN). They share most of their typical genetic alterations, whose panel is close to that found in pancreatic adenocarcinoma. Some of these alterations could lead to therapeutic targets that would prevent degeneration of PanIN and IPMN towards cancer.
Thrombin-stimulated endothelial cells produce platelet-activating factor (PAF) in a dose-dependent manner: the activation of a Ca super(2+)-dependent lyso-PAF acetyltransferase is the rate-limiting ...step in this process. The present study shows that acetyltransferase activation and consequent PAF production induced by thrombin in human endothelial cells are markedly inhibited in Na super(+)-free media or after addition of the amiloride analog 5-(N-ethyl-N-isopropyl)amiloride, suggesting that a Na super(+)/H super(+) antiport system is present in endothelial cells and plays a prominent role in thrombin-induced PAF synthesis.
Evaluer l’impact sur la survie, le mode de récidive et les données anatomopathologiques du traitement du cancer de la tête du pancréas résécable selon deux attitudes : chirurgie d’emblée (CE)
versus ...radiochimiothérapie néoadjuvante (RCT).
Il s’agit d’une étude rétrospective en intention de traiter. De 1997 à 2006, 174 patients ayant un adénocarcinome canalaire résécable de la tête du pancréas ont été pris en charge dans deux centres référents ayant une stratégie différente. 85 patients ont eu une CE et 89 patients une RCT. Les patients ayant une RCT étaient tous réévalués avant chirurgie.
67 patients (79 %) ont été réséqués dans le groupe CE. Dans le groupe RCT, 33 patients n’ont pas eu de laparotomie après réévaluation en raison d’une évolution métastatique (42 %) ou locorégionale (36 %) ou en cas de contre-indications opératoires ; 38 patients (43 %) ont été réséqués. Les médianes de survie en intention de traiter dans le groupe CE

Une majorité de patients évolue pendant la RCT et ne bénéficie pas d’une résection curative. Les médianes de survie en intention de traiter sont comparables, mais la survie à long terme des patients réséqués après RCT est inférieure à celle des patients réséqués d’emblée. Cependant, l’intérêt d’une RCT doit être considéré car elle améliore le contrôle local.
A full-custom 32-b ECL (emitter coupled logic) microprocessor which uses a 1.0- mu m-drawn single-poly technology with four layers of aluminum and one layer of gold interconnect is described. The ...15.4-mm*12.6-mm die contains 468-k bipolar transistors (f/sub t/=13 GHz) and 206-k resistors. The chip implements a subset of an existing RISC (reduced instruction set computer) architecture. The machine has five pipeline stages, each one clock cycle long. A 128-b bus is used for incoming data; a 64-b bus is used for outgoing data. The chip uses a single-phase differential clock. The clock distribution network consists of three levels of H-trees. The 2-kB instruction and data caches use a 16-B line, contain byte parity, and are direct-mapped. The chip contains 30 internal master bias circuits, whose outputs are wire-ORed together. The logic portions of the chip primarily use a single-ended 575-mV swing.< >
Platelet activation is accompanied by an increase of cytosolic free Ca2+ concentration, Ca2+i, (due to both extracellular Ca2+ influx and Ca2+ movements from the dense tubular system) and an Na+ ...influx associated with H+ extrusion. The latter event is attributable to the activation of Na+/H+ exchange, which requires Na+ in the extracellular medium and is inhibited by amiloride and its analogs. The present study was carried out to determine whether a link exists between Ca2+ transients (measured by the quin2 method and the 45CaCl2 technique) and Na+/H+ exchange activation (studied with the pH-sensitive intracellular probe, 6-carboxyfluorescein) during platelet stimulation. Washed human platelets, stimulated with thrombin and arachidonic acid, showed: (1) a large and rapid Ca2+i rise, mostly due to a Ca2+ influx through the plasma membrane; (2) a marked intracellular alkalinization. Both phenomena were markedly inhibited in the absence of extracellular Na+ or in the presence of an amiloride analog (EIPA). Monensin, a cation exchanger which elicits Na+ influx and alkalinization, and NH4Cl, which induces alkalinization only, were able to evoke an increase in Ca2+i, mostly as an influx from the extracellular medium. Our results suggest that Ca2+ influx induced by thrombin and arachidonic acid in human platelets is strictly dependent on Na+/H+-exchange activation.
