Background/Aim
The aim of this in vitro study was to investigate the impact fracture resistance of anterior teeth that had been fractured and restored with modified partial‐coverage ceramics using ...the edge‐up technique.
Material and methods
A total of 178 extracted human mandibular central and lateral incisors were included in this study. The incisors were randomly divided into two groups. Group 1 (n = 89) teeth were cleaved and restored with modified partial‐coverage ceramics using the edge‐up technique; Group 2 (n = 89) consisted of intact teeth that served as control. Impact strength was tested in a modified impact testing machine (pendulum type), and the severity of crown fracture after fracture resistance test was registered.
Results
Impact strength was insignificantly lower in restored teeth (median 5.39 and range 0.81–14.12 kJ m−2) than in intact teeth (median 5.45 and range 0.31–16.47 kJ m−2), although no statistically significant differences were observed. Regarding the severity of crown fracture, restored teeth showed a lower rate of severe fractures (20.22%) compared to intact teeth (25.84%), but no statistically significant differences were observed.
Conclusions
Restoration of cleaved mandibular incisors with modified partial‐coverage ceramics using the edge‐up technique shows fracture resistance and severity of crown fracture following traumatism similar to intact teeth. In this way, this technique can be used successfully for the treatment of fractured anterior teeth.
It’s introduced Lean techniques in a Clinical Laboratory to improve the operability and the efficiency in continuous processes of analysis, failsafe systems, analysis of areas of value pursuit of ...zero defects and reduction of waste, and it promote continuous improvement in presented difficulties in adapting to the changing needs of the healthcare environment. Whereas it is necessary to incorporate certification and accreditation, note that the adaptability of the clinical laboratory to the changing needs of physicians in obtaining analytical information is reduced. The application of an agile methodology on analytical systems can provide a line of work that allows the incorporation of planning short work cycles on equips quickly with operational autonomy on the basis of demand and respecting the accreditation requirements and flexibility to ensure adequate performance as the intercomparison of results from the different units analytics, analytical quality and turnaround times. Between 2012 and 2014, a process of analysis and improvement was applied to circuits, a 5 s system, transportation of samples, inventory of reactive and samples, motion of personal and samples, reductions of waiting and delays, overproduction, over processing, and defects of results and reports. At last it seems necessary to apply the Agile methodology to adapt to the evolving necessities in time and the different origins of the samples. It’s have used modular systems where the modules of this study are programmed with immunoassay techniques and it has reduced the operative modules depending on the required activity, ensuring the goals of turnaround times, analytic quality, service, health care continuity, and keeping up with the ISO 15189 accreditation requirements. The results of applying the concept of Lean-Agile to a modular system allows us to reduce the associated costs to the seasonal variation of the health care demand and to adapt the system to the changes on the pattern of analytic demand. The savings obtained are two laboratory technicians for a month and those derived from deactivating a complete modular section consisting of three analysts. Average turnaround time (TAT) of 99.5% from the analyzed parameters has been kept equal to the one obtained in common conditions with a p < 0.01. The laboratory has maintained the quality goals measured in analytical variability. Prior to that, it most objectified the inter-comparability of the results among the used channels.
Resumen
Introducción
El objetivo de este estudio es comprobar la evolución de las especificaciones de la prestación analítica (EPA) utilizadas en programas de garantía externa de la calidad (EQA) y ...el papel de un programa de categoría 1 en la vigilancia de la estandarización de la prestación de los laboratorios clínicos en España.
Métodos
Se ha revisado la bibliografía sobre tipos de especificaciones de la calidad usados en programas de otros países y se ha comprobado su evolución; se ha comparado el posible impacto de distintas EPA empleadas en ocho países en la toma de decisiones clínicas con tres ejemplos de magnitudes: sodio, tirotropina (TSH) y tiempo de tromboplastina parcial activado (TTPA).
Resultados
Se ha evidenciado la estandarización entre métodos analíticos comprobando si los resultados medios se desvían respecto al valor de referencia certificado del control dentro de las EPA derivadas de la variación biológica (VB). Las EPA usadas en EQA han evolucionado desde el estado del arte hacia la VB. Si se aplican los resultados que se aceptarían con algunas EPA se podrían producir decisiones clínicas erróneas.
Conclusiónes
En España, solo 2 de las 18 magnitudes biológicas estudiadas se pueden considerar bien estandarizadas. Sería necesaria una colaboración más estrecha entre los laboratorios y proveedores de sistemas analíticos para resolver las discrepancias.
Purpose
The aim was to investigate the accuracy of quantitative computed tomography bone mineral density (BMD) measurements in mandibles, comparing measured BMD with calibrated BMD.
Materials and ...Methods
Seventy mandibles from adult cadavers were used. Twenty tomographic cuts were made in each mandible. In each tomographic cut, a region of interest was located, and the bone density was measured in Hounsfield unit (HU). A polymethyl methacrylate phantom containing four inserts of different predetermined densities (hydroxyapatite 100, 200, 500, and 700 mg/cm3) was used to calculate calibrated bone density. Correlation between measured and calibrated bone densities was calculated.
Results
Mean total correlation between measured and calibrated BMD in the 20 sagittal tomography cuts showed almost perfect positive correlation (r = 0.998, p < .001). However, when average BMD measurements in HU were compared, the measured total BMD (in the 20 sagittal tomography cuts studied) was 54.99 ± 421.59, whereas the total calibrated BMD was 49.28 ± 364.95, with statistically significant difference (p = .001).
Conclusions
There are discrepancies between measured and calibrated BMD; in this sense, a calibrated bone phantom with a predetermined mineral density should be used to determine the exact BMD before dental implants surgery.

Inherited variability in host immune responses influences susceptibility and outcome of Influenza A virus (IAV) infection, but these factors remain largely unknown. Components of the innate immune ...response may be crucial in the first days of the infection. The collectins surfactant protein (SP)-A1, -A2, and -D and mannose-binding lectin (MBL) neutralize IAV infectivity, although only SP-A2 can establish an efficient neutralization of poorly glycosylated pandemic IAV strains.
We studied the role of polymorphic variants at the genes of MBL (MBL2), SP-A1 (SFTPA1), SP-A2 (SFTPA2), and SP-D (SFTPD) in 93 patients with H1N1 pandemic 2009 (H1N1pdm) infection.
Multivariate analysis showed that two frequent SFTPA2 missense alleles (rs1965708-C and rs1059046-A) and the SFTPA2 haplotype 1A(0) were associated with a need for mechanical ventilation, acute respiratory failure, and acute respiratory distress syndrome. The SFTPA2 haplotype 1A(1) was a protective variant. Kaplan-Meier analysis and Cox regression also showed that diplotypes not containing the 1A(1) haplotype were associated with a significantly shorter time to ICU admission in hospitalized patients. In addition, rs1965708-C (P = 0.0007), rs1059046-A (P = 0.0007), and haplotype 1A(0) (P = 0.0004) were associated, in a dose-dependent fashion, with lower PaO2/FiO2 ratio, whereas haplotype 1A(1) was associated with a higher PaO2/FiO2 ratio (P = 0.001).
Our data suggest an effect of genetic variants of SFTPA2 on the severity of H1N1pdm infection and could pave the way for a potential treatment with haplotype-specific (1A(1)) SP-A2 for future IAV pandemics.

Se presenta el caso de una paciente atendida en Bolivia, en la Clínica del Colaborador de La Paz, que presentaba una tumoración en el pie derecho que había sido resecada en 2 oportunidades, con ...recidiva in situ casi inmediata y evolución no satisfactoria. Para resecar la tumoración, identificada como un tumor desmoide extraabdominal tanto por biopsias anteriores y por la clínica y los complementarios, se realizó una amputación de tipo Syme. Ésta dejó un muñón que, después de la rehabilitación, resultó útil para la marcha y las funciones habituales de la paciente, como la reincorporación a su duro trabajo anterior.

Introducción: en el cumplimiento de una misión internacionalista en la Clínica del Colaborador, ubicada en La Paz, Bolivia, se presentó una niña de 12 años de edad, portadora de una gran tumoración ...en la espalda, acompañada de otras deformidades, que no había sido tratada por carecer de recursos económicos. Los médicos que la habían visto antes habían expresado que al realizar una cirugía, único modo de resolver la situación, no se garantizaba la vida de la menor y también por el hecho de residir en una región intrincada del país. El objetivo de esta comunicación es mostrar un caso poco común en la práctica diaria y el resultado de su tratamiento. Descripción: el tratamiento realizado fue eminentemente quirúrgico, apoyado en un tratamiento higiénico-dietético adecuado y la posterior rehabilitación. Los resultados fueron excelentes; finalmente pudo restituirse a la paciente a su lugar de residencia, su escuela y la sociedad. Conclusiones: la tumoración gigante de la espalda, acompañada de otras deformidades en la paciente, se resolvieron después de un tratamiento adecuado.
The objective of the present study was to examine the evolution of the analytical performance specifications (APS) used in External Quality Assurance (EQA) schemes, as well as the efficacy of a ...category 1 EQA scheme in monitoring the harmonization of clinical laboratory results in Spain.
A review of the literature on the types of quality specifications used in schemes in other countries and their evolution was performed. In addition, a comparative analysis of the potential impact that different APS from eight countries had on clinical decision-making was made based on three measurands: sodium, thyroid-stimulating hormone (TSH), and activated partial thromboplastin time (aPTT).
Harmonization of analytical methods was demonstrated by assessing whether average results deviated from the certified reference value of control materials within the APS derived from biological variation (BV). The APS used in EQA have evolved from state-of-the-art models to BV. Poor clinical decision-making would occur if the results accepted by some APS were applied.
In Spain, only 2 of the 18 measurands studied are considered to be well harmonized. Closer collaboration between laboratories and analytical system providers would be required to resolve discrepancies.

El objetivo de este trabajo fue realizar actualización acerca de la vida y obra del Dr. Manuel Martínez Curbelo. Este universitario brillante e infatigable investigador llegó a ser el anestesiólogo cubano con más aportes a la anestesiología mundial en la centuria pasada. Los anestesiólogos cubanos deben transmitir a las nuevas generaciones de anestesiólogos cubanos el conocimiento acerca de la vida y la obra de este médico como ejemplo a seguir. Su constante dedicación ante la especialidad es un digno ejemplo a imitar por las futuras generaciones de anestesiólogos cubanos.
